But there are a few researches about intraneural ganglion in foot and ankle. We studied 7cases of intraneural ganglion. We are going to prove the synovial theory by indentifying articular branch of intraneural ganglion. Materials and Methods: From August 2003 to May 2011, we evaluated 7 ouf of 8 patients diagnosed as a intraneural ganglion in foot and ankle. The gender ratio were 4 male and 3 female, and the mean age at the time of surgery was 52.9 years. Clinically, we checked pre and post operative symptom, muscle tone and whether loss of muscle tone and sensation exists. We analyzed surgical records and preoperative MRI and compared those with intra-operative finding. Results: In MRI analysis of 7cases, the connection around the joints were confirmed, and 1 case was confirmed in the retrospective analysis of MRI. Intraneural ganglions occurred in medial plantar nerve 3 cases, lateral plantar nerve 1 case, superficial peroneal nerve 1 case and sural nerve 1 case. We could not found recurrence during the follow up periods. Most patients relieved pain after operation, but recovery of sensation was unsatisfactory. We could find some cases pathological finding of the nerve intraoperatively, and clinical result of that cases was poor. Conclusion: Intraneural ganglion can occur in various parts in foot and ankle. We concluded that the intranneural ganglion originated from joint by identifying the artichlar branch of ganglion. Due to its small size, it is difficult to find articular branch in operation field. But we do our best to find and remove articular branch. Currently, considering the small amount of research in foot and ankle, more research about articular brach is needed. 2 최장석 김광희 곽지훈 박홍기 이신우 경에서는보고된바를확인할수없었다. 주로남성에서호발하며, 연령대는 9~74세로다양하다고알려져있다. 3-5) 발생기전은최근까지논쟁의여지가있으나관절과연결되는신경관절분지에의해발생하는활액가설이주목받고있고, 주로슬관절주위를중심으로연구가진행되고있다. 6-8) 족부족관절에서신경내결절종은드물며신경관절분지에대한연구는아직미미한상태이다. 저자는족부족관절에서신경내결절종으로진단받은 7예의임상적소견, 방사선소견, 수술기록지등을분석하여질환의특징및신경관절분지 (articular branch) 의중요성을고찰하였다. 1. 대상 대상및방법 2003년 8월부터 2011년 5월까지가천의대길병원에서족부족관절에발생한신경내결절종으로진단및수술을받은 8예중 7예를대상으로하였다. 1예는천비골신경의안쪽발등피부신경 (medial dorsal cutaneous nerve) 에발생하였으나, 크기가너무작고자기공명영상의부적합한촬영기법으로관절분지의추적관찰이불가능하여제외하였다. 남자 4예, 여자 3예였고, 수술당시평균연령은 52.9세였다. 6예에서수술소견및조직검사에서신경내결절종으로, 나머지 1예는자기공명영상으로확인되었으며, 추시기간은평균 31.1개월이었다. 2. 방법및분석 임상적소견으로수술전후주증상의변화및감각소 실과회복여부를확인하였고, 술전자기공명영상및신경전도검사를시행하였다. 재발유무를확인하고자술후 6개월에초음파검사, 신경병증의회복여부를판단하기위해신경전도검사를병행하였다. 자기공명영상은 3-T (Siemens Magnetom Verio, SIEMENS, PA, USA) 과 1.5-T (Siemens Magnetom Vision, SIEMENS) 가사용되었다. T1, T2 강조영상및시상면상지방포화양자밀도영상, 축상면상경사에코영상을얻었으며, 절편두께는관상면상 mm, 시상면상 mm, 축상면상 mm로측정하였다. 각신경내결절종및신경관절분지는 Scout view를통하여관상면, 시상면, 축상면상에서확인하였다. 분석은한명의근골격계영상의학과의사와두명의족부족관절부의사가담당하였다. 환자의술전자기공명영상을 6예에서전향적분석을하였고 1예는후향적분석을하였다. 수술소견및수술기록지를참조하여신경내결절종과신경분지를영상소견과비교분석하였다. 3. 증례 1) 증례 1 38세남자는약 2년전부터시작된우하지원위부전외측에두개의종물및족부배측의저린감으로내원하였다. 이학적검사상종물은족관절면상방약 5 cm와 3 cm에존재하고난형이며티넬증후는없었다. 건측에비해표피비골신경분포의감각저하가있었다. 자기공명영상에서표피비골신경의주행을따라균일하게명확한경계의낭종 2개가세로로있으며 T1영상에서저신호강도, T2영상에서고신호강도를보였다 (Fig. 1A). 3 그리고두낭종사이도전반적으로굵기가증가되어있었으며, 축성면과시상면으로추적관찰해보면족관절로진행하는신경관절분지를확인할수있었다 (Fig. 1B-D). 신경전도검사에서건측에비해표피비골신경의전도속도가감소되어있었다. 수술소견상병변은피하층의표피비골신경을따라존재하며신경외피가얇아져일부파열된사이로점액물질을싸고있는낭종벽이풍선처럼팽창되어있었고, 원위부에서는중간발등피부신경 (intermediate dorsal cutaneous nerve) 이신전지대에유착되었다. 중간발등피부신경의주행을따라절개를족관절원위부까지진행하였으나신경관절분지확인은실패하였다. 술후 6개월에시행한신경전도검사상술전에비해향상되었고, 초음파검사상신경의굵기는정상측에비해약간증가되어있으나재발소견은없었다. 술후 52개월에재발증후는없었으며, 티넬증후는음성이나감각회복은불완전하였다. 2) 증례 2 64세여자는약 20년전부터우측외측족관절부에종물이있었으며, 우족관절통증및족부외측부감각이상을주소로내원하였다. 이학적검사상종물이촉지되었으며, 티넬증후는양성이었다. 건측에비해비복신경지배부위의감각저하가관찰되었다. 자기공명영상에서비복신경내에약 4.3 cm의조영증강이있는낭성종물이 2개가아령모양으로길게관찰되었으며, T1영상에서저신호강도, T2영상에서고신호강도를보였다. 2개의신경가지중주분지에서거종관절의후관절면과연결된신경관절분지를확인하였다 (Fig. 2A, B). 신경전도검사에서건측에비해비복신경의전도속도가 감소되어있었다. 병변은 cm 크기로증가되어있고, 거종관절의후관절면과연결된하얀빛깔의약 1 mm 직경이하의관형태를확인하였으며, 내부는비어있었다. 조직학적성상을확인하기위해조직검사를의뢰하였으나검체가작아결과를얻지는못하였다. 술후 6개월신경전도검사상술전에비해향상되었고, 초음파검사상신경의굵기는정상부위에비해증가되어있으나재발소견은없었다. 술후 12개월에감각은약간회복되었으나건측에비해떨어졌으며족관절통증은소실되었다. 3) 증례 3 51세남자는약 3개월전부터시작된우족저부통증및우 1,2,3족지저린감을주소로내원하였다. 이학적검사상족근관에서종물은촉지되지않았으나, 티넬증후는양성이었다. 후경골신경의내족저신경지배부위의감각저하및무지외전근의위축이관찰되었다. 자기공명영상에서후경골신경의내족저신경에 cm의종물이있으며 T1영상에서저신호강도, T2 영상에서고신호강도를보였고, 족근구에관절액이차있는소견을보였다. 병변부내족저신경의관절분지가족근구와연결되어있음을확인할수있었다 (Fig. 3A-C). 신경전도검사에서내족저신경의전도속도가감소되었다. 수술소견상낭종은내족저신경의기시부부터시작되었으며, 병변부는변색및경화되었고후경골신경및외족저신경은정상이었다. 현미경하에서자기공명영상에서보이는내족저신경의분지가족근구를통하여거골하관절과연결되는직경약 1 mm 이하의선상형태의분지를확인할수있었다. 조직학적성상을확인하기위해신경관 A B C Figure 2. (D) Light micrograph shows a fibrous connective tissue, a portion of cyst wall (arrow)(h & E stain, 100). 절연결부의조직검사를의뢰하였고, 검사결과상섬유조직성막을가지고있으며, 내부는점액성물질로이루어진낭성구조물을형태를관찰할수있었다 (Fig. 3D). 술후 6 개월에시행한신경전도검사상호전되었고초음파검사상신경의굵기는커져있으나재발소견은없었다. 술후 24개월에무지외전근의위축및족저부통증및감각저하의회복정도는만족스럽지못하였다. 4) 증례 4 62세여자는약 2년전부터시작된우족저부통증과내측족저부저린감을주소로개인병원에서족근관부의결절종으로절제생검을시행받은병력이있는환자로증상이재발하여내원하였다. 신체검사상족근관부에서종물은촉지되지않았으며, 티넬증후는양성이었다. 후경골신경의내족저신경분포부위의감각저하가관찰되었고, 무지외전근의위축은없었다. 자기공명영상에서후경골신경의내족저신경에 cm 크기의종물이있었고, T1영상에서저신호강도, T2영상에서고신호강도를보이는신경내결절종이있다. 내족저신경의 2개의관절분지가거골하관절의재거돌기주위로진행하여거골하관절과연결되어있었다. 고해상도 3T의 T2 강조영상에서, 거골하관절과연결되는신경관절분지를재거돌기전방및후방에서확인할수있 었다 (Fig. 4). 신경전도검사에서내족저신경에전도속도가감소하는소견을보였다. 수술소견상병변은내족저신경의근위부에서원위부로진행하였으며, 병변부신경의변색이관찰되었고, 외족저신경및후경골신경은정상이었다. 내족저신경의원위부에서재거돌기로향하는관절분지를확인하였다. 술후 6 개월초음파소견상내족저신경의팽윤은지속되었다. 술후 12개월에족저부통증은호전되었으나감각은부분적으로회복되었다. 5) 증례 5 60세남자는약 10년전좌족근관증후군진단하에개인병원에서수술적치료를시행하였으나재발하여내원하였다. 주증상은족저부통증및저린감이었다. 신체검사상족근관부위에서종물은촉지되지않았으며티넬증후는양성이었다. 좌족저부의후경골신경의내외족저신경지배부위의감각저하가있었고무지외전근은정상이었다. 후향적자기공명영상검토에서후경골신경과그내외족저신경에종물이위치하고있으며, 내족저신경의거골하관절의족근구와연결되는신경관절분지를확인할수있었다 (Fig. 5). 수술소견상병변은후경골신경의일부, 분지기시부및내외족저신경의원위부로진행함이관찰되었다. 당시에는신경관절분지에대한탐색을하지않았

A B C 다. 술후 6년경과하였으며통증은없으며족부감각이회복되어건측과비슷하고재발증후는없었다. 6) 증례 6 68세남자는약 3년전좌측발바닥통증이있었으며최근수면을방해할정도로악화되어내원하였다. 신체검사상족근관에종물은촉지되지않았으며, 티넬증후는양성이었다. 내족저신경분포부위에감각소실을보였으며 족무지외전근의위축은없었다. Figure 5. C 액성 물질로 이루어진 낭성 구조물을 형태를 보이며, 섬유 인대조직성 막을 가지고 있었다(Fig. 6D). 술 후 6개월에 초음파 소견상 내 족저 신경의 팽윤은 지속되었다. 술 후 12개월에 통증은 소실되었고 감각 회복은 불완전하였다. 7) 증례 7 51세 남자는 6개월 전부터 좌 족부 외측부에 감각 감소 를 주소로 내원하였으며 이러한 증상은 과거에 두 차례 반 복되었고 그 후 호전되었으나 최근에는 1개월 정도 증상 호 전이 없어 내원하였다. 신체 검사상 제4, 5족지를 포함한 족부 외측에 감각이 감소하였다. 자기 공명 영상에서 후경 골 신경의 외 족저 신경 내에 T1영상에서 저신호 강도, T2 영상에서 고신호 강도의 약 7 cm 길이의 낭종이 있으며 외 족저 신경의 신경관절 분지가 족근구로 연결됨을 확인하였 다(Fig. 7A, B). 술 전 신경 전도 검사상 정상 소견을 보였 다. 삼 주간 압박 스타킹을 착용한 후 증상이 호전되어 재 시행한 자기 공명 영상 소견상 외 족저 신경내 결절종은 사 라졌다(Fig. 7C). 추시 3개월 후 감각 회복은 불완전한 상 태이며 현재 외래 추시 중이다. 고 찰 결절종은 점액성 용액을 함유하고 교원성 섬유막을 가진 흔한 양성 종양으로서 주로 수배부와 족배부에 발생하지만,
7 신경내결절종은말초신경내에발생하는양성연부조직종양으로주로비골두주위외측슬와신경이나비골신경에발생하는것으로보고되고있으며, 발생빈도가드문것으로되어있다. 9) 신경내결절종은최초로 1810년 Beauchene 이척골신경에서보고하였고, Nucci 등 4) 과 Uretani 등, 10) Evans 등, 11) Coleman 등 3) 이각각 60예, 16예, 22예, 40예의신경내결절종의발생을보고하였으나이는모두총비골신경과그분지에서발생한경우였다. 국내문헌고찰에서도 Ahn 등 12) 이 1972년처음발표한이후현재까지총 5 예의국내보고역시총비골신경과그분지에서발생한경우였다. 경골신경에발생한신경내결절종은매우드문것으로 1967년 Friedlander 13) 에의해처음발표한후 6례정도가보고되었고, 국내에서는 Yoo 등 14) 이족관절부위에서후경골신경과그의분지인내측족저신경내에발생한신경내결절종을보고하였고, Suh 등 15) 은슬관절에서발생한경골신경내결절종에대하여보고하였다. 족부족관절에발생한신경내결절종은대부분후경골신경의분지인내족저신경에서발생하였음을보고하였으나저자는후경골신경과그분지를포함한예와여러표피신경에도발생함을확인할수있었다. 발생기전은아직논란의여지가많으나현재두가지의가설이대두되고있다. 첫번째가설은퇴행성이론으로 Ledderhose 16) 에의해제안되었으며, 관절주위의조직이자극에의해형성되는과정에서낭종이형성되고, 이때정상적인방추상세포가검액 (the detect liquid) 상공포를함유하는구상세포로변형된다는이론이다. Jesen과 Brooks 는낭종이이소성활액조직의배아잔존물에서부터형성된다고하였고, Gurdjian은낭종의형성은외상후발생하는신경내혈종의흡수에서부터발생한다고하였으며, 환자의약 25% 에서외상과같은물리적요인이있다는사실은퇴행성이론을뒷받침한다 ) 두번째가설은활액가설 (synovial theory) 로신경내결절종은인접해있는관절에서유래하였으며신경내결절종과인접관절은신경관절분지 (articular branch) 라고하는통로를통해서로교통하며, 이통로가일방통로로작용하여관절액이신경내결절종으로이동할수있게되고, 관절압력이높아지면, 신경내결절종의크기가증가된다고하였다. 따라서신경관절분지의해부학적및병리학적인증거에기초하여관절혹은관절주위에서발생된진성결절종으로간주한다. Robert 등 21) 에의하면활액가설을뒷받침하는중요한요소로신경내결절종과경비관절과연결하는비골신경의신경관절분지의존재를들수있다. 실제로보고된증례의약 40% 에서발견된신경관절분지는일종의통로로작용하고, 17,19) 이곳 을통하여신경내로의성장을조장한다. 다른증거는신경내결절종이조직학적으로내피형세포로이루어졌다는것이다. 21) 발생기전에는논란이많지만최근에는활액가설이받아들여지고있다. Spinner 등 6,8) 은상경비관절과신경내결절종이관절분지로연결되어있다는것을임상적, 수술적, 영상학적, 조직학적증거로제시하여신경내결절종의원인은이웃하는관절과연결되는관절분지임을증명하였다. 현재척골신경, 경골신경및정중신경과주관절, 상경비관절, 요주상관절각각의관계및관절분지에대한연구는활발하게진행되고있다. 족부족관절의관절에대한신경지배에대한연구는최근에 Morin, Lippert, Gardner, Champetier 등에의하여행해졌다 ) 족관절을지배하는신경은족관절을통과하는모든신경이해당하며, 신경지배범위도매우다양하다. 23) 후경골신경은 3~5개의관절분지를가지며, 내과의전후면및관절면의내측전체를지배한다. 23,24) 심부비골신경은관절면부위에서 3~5개의관절분지를갖고, 23,24) 관절낭, 전하방경비인대, 전방거비인대의전방을지배한다. 복재신경은두개의짧은분지가내과의전측에분포한다. 비복신경은 3개의관절분지를가지며, 족관절후방까지도달한다. 표재성비골신경은 2개의관절분지가불규칙하게족관절을지배하며, 23) 부심비골신경 (acessory deep peronial nerve) 도관절분지를갖는다. 거골족근관절 (talotarsal joint) 는후경골신경, 내측족저신경, 심비골신경, 비복신경, 부심비골신경으로부터지배를받는다. 후거골하관절은후경골신경의내측관절분지와비복신경의외측 2개의관절분지에지배를받으며, 족근동은심부비골신경의관절분지에지배를받는다. 족근구는후경골신경의관절분지에지배를받는다. 이러한관절분지가신경내결절종의발생에기여한다. 신경관절분지의확인은수술시야에서직접확인및술전방사선검사로가능하다. 수술적소견에서 Seddon 27) 은실제신경관절분지는매우작고, 심부에위치하며, 외관상으로정상적으로보인다고하였다. 또한잘알려지지않아관절분지를확인하기쉽지않음을보고하였다. Spinner 등 8,28) 은신경관절분지는절제하거나흡인하기전에는낭종의형태로보이지않고, 박리하여횡단면을관찰하여야비로소원통형에관절액이차있는소견을보여수술시야에서확인함이쉽지않다고하였다. 저자도확대경을착용한상태에서관절분지를확인하기쉽지않았다. 또한신경관절분지로의심되는부위도크기가작아절단면의확인및생리식염수의투여가쉽지않았다. 방사선적진단방법으로고해상도의얇은간격으로촬영하여다중단면재구성이
8 최장석 김광희 곽지훈 박홍기 이신우 가능한자기공명영상촬영기법과자기공명관절조영술, gadolinium 관절조영술을포함한특별한영상촬영은진단에매우도움이되며, 초음파, CT 관절조영술등으로가능하다. 29) 저자도내족저신경에발생한증례 4에서술전에시행한자기공명영상소견에서거골하관절과연결된관절분지부위를확인한후다시그부위를고해상도의다중단면재구성이가능한 T2 프로톤밀도강조영상을재촬영하고초음파검사를병용하여 2개의관절분지를확실하게확인할수있었다. 신경내결절종에있어관절분지의중요성은병태생리를이해하는측면이외에치료결과에영향을미쳐재발과도연관이있는것으로알려져있다. Spinner 등 30) 은신경내결절종은신생물이아님에도예후를예측하기어려워재발률이 30% 에이른다고하였으며, 저자도본연구중 2 례에서재발한경우를접하였고, 관절분지가적절히제거되지않으면재발가능성이높아장기간추시가필요하다고생각한다. 따라서저자는수술시야에서가능하면일차적으로관절분지를찾아제거하고자하였고, 불가능한경우는병변신경과주위관절을충분히박리하여신경관절분지에의한연결을차단하고자노력하였다. 신경변성을동반한예와자연소실된예를제외하고는추시기간내재발은없었으며, 술후시행한전기전도검사와통증양상은호전소견을보였으나, 감각회복은불완전하였다. 또한신경변성을동반한예 ( 증례 3) 는술후임상적결과가불량하여조기에수술적치료를권하고싶다. 증례 7에서신경내결절종의소실은기존의이론으로는설명이불가능하며, 이에대한연구및추후지속적인관찰이요망된다. 족부족관절에서신경내결절종에대한증례가많지않고, 관절분지에대한연구가부족하다. 이와같은한계에도불구하고, 신경내결절종의발병기전인활액가설은자기공명영상분석과수술실소견으로충분한근거를얻을수있었다. 결론족부족관절에서신경내결절종은표피신경을포함한다양한신경에서발생할수있다. 신경관절분지의존재는신경내결절종이관절과의교통을통해발생함을설명해줄수있다. 그러나족부족관절에서신경관절분지의존재및질환의진행에대한연구는미미한상태로, 좀더많은연구가필요하다.
