4 - Abstract - Purpose: To analyze the clinical results of the total knee arthroplasty(tka) without patellar resurfacing. Material and Method: TKA without resurfacing of the patella (Nexgen, Zimmer USA) were performed between Feb., 1995 and Feb., 2000, and were analyzed 89 cases which were followed up more than 36 months. The follow up period ranged from 36 to 70 months. The diagnosis included degenerative joint disease in 83 cases (93%), rheumatoid arthritis in 5 cases (6%) and avascular necrosis of medial femoral condyle in 1 case (1%). The clinical results were evaluated according to 'The Knee Society Clinical Rating System'. Results: The mean preoperative knee score was 34 points (range, 5-63 points) according to the knee society clinical rating system. and the mean preoperative functional score was 38 points (range, points). The mean postoperative knee score was 91 points (range, points) and the mean postoperative functional score was 89 points (range, points) There were no patellar dislocations or subluxations and also no fracture of the patella with normal patellar tracking. The current study indicate that patellar complications associated with unresurfacing was not found in our study but we experienced 2 cases of persistent pain (2 cases) which was resolved in postoperative 6 months. Conclusion: On the basis of our findings, there was little evidence to support - i -

5 a recommendation for routine patellar resurfacing in total knee arthroplasty because overall results of the nonresurfaced patella total knee arthroplasties were satisfactory. Furthermore the complications including patellar dislocation or subluxation, rupture of the patellar tendon, fracture of the patella could also be avoid by nonresurfaced patella. 6 - 국문초록 - 목적 : 인공슬관절전치환술시슬개골의표면대치술을시행하지않은예에대하여추 시결과를분석하고자한다. 대상및방법 : 1995 년 2 월부터 2000 년 2 월까지본교실에서슬개골표면대치술을 시행하지않은슬관절전치환술을시행받은환자중 3 년이상추시가가능하였던 89 례 (51 명 ) 에대하여 The Knee Society Clinical Rating System 에의하여평가하 였다. 결과 : 슬개건표면대치술을시행하지않아도정상적인슬개골 tracking 과연부조직 균형을갖추었으며슬전평균 knee score 는 34 점 (5-63 점 ) 이었으며술전평균기능점 수 38 점 (10-65 점 ) 이었고, 술후평균슬관절점수는 91 점 (65~98 점 ) 이었으며술후 평균기능점수는 89 점 ( 점 ) 이었다. 슬개골표면대치술후발생할수있는 슬개골골절또는슬개골탈구및아탈구와슬개건파열등의합병증은없었다. 술후 6 개월까지슬관절동통을호소하는 2 례가있었으나보존적치료로 6 개월이후에는동 통이소실되었다. 결론 : 슬관절대치술시슬개골표면대치술을시행하지않을경우임상적으로만족스 러운결과를얻을수있었으며또한슬개골표면대치술을시행하여발생할수있는 여러가지합병증을예방할수있을것으로사료되었다. 색인단어 : 슬개골표면대치술, 인공슬관절전대치술, 슬관절 - iii -

9 슬관절대치술후슬개골의합병증의발생빈도가 1% 에서 50% 까지다양한변화가있다. 이러한합병증으로는골괴사 21,27), 골절 10,14,16,17,30), 탈구 6), malalignment 2), tracking abnormality 24), loosening of the component 18,23,28) 등과또한동통등이있을수있다. 이들합병증에대한원인규명과치료에대하여많은연구가있으며슬개골에대한대치술과미대치술에대한논란도많은것이사실이다 1,3,9,25,210). 이에저자는슬관절전대치술시슬개골표면대치술을시행하지않은 51명 (89예) 에대하여 36개월이상추시한결과를보고하고자한다

10 1995년 2월부터 2000년 2월까지본원에서슬관절전치환술을시행한환자중슬개골표면대치술을시행하지않은 108례중 36개월이상추시가가능하였던 89례 (51 명 ) 에서그결과를분석하였다. 사용한기구는전예에서 Nexgen(Zimmer, U.S.A.) 이었다. 남자가 6명여자가 45명이었으며우측이 38례, 좌측이 57례이었으며퇴행성관절염이 83례 (93%), 류마티스관절염 5례 (6%) 무혈성괴사 1례 (1%) 이었으며수술당시의평균연령은 68세 (59-79세) 이었다. 상기환자전예에서슬개골의인공관절표면대치술을시행하지않았으며평균추시기간은 42개월 (36-70개월) 이었으며 The Knee Society Clinical Rating System 19,20) 에의하여술전, 술후평가를시행하였다. The Knee Society Clinical Rating System 의경우결과는슬관절자체에대한점수 (knee score) 와보행및계단을오를수있는능력에대한점수, 즉기능에대한점수 (functional score) 로나누었다. 동통, 안정성, 운동범위등 3가지주요판단기준에따라결정되었으며굴곡구축, 신전지연 (extension lag) 및 misalignment는점수에서제외했다. 슬관절점수 100점은잘정열되어있고동통은없으며 125 의운동범위를보이고전후방과내외측의불안정성이없을경우에해당하였다. 기능점수의평가항목은보행거리, 계단오르기등으로평가하였으며보조기구사용시감점하였고기능점수 100점은거리제한없이보행할수있으며정상적으로계단을오르내릴수있는경우에해당한다.(Table 1) 수술시슬개골의연골상태를정상, 경도, 중등도, 중도로구분하였으며경도는 연골상태가정상혹은연화된경우중등도는미세섬유화와미란이 25% 이하, 중도는골극형성및연골이소실되어뼈가 25% 이상노출된경우로분류하였다. 89례중슬개골이정상에서경도가 18례, 중등도 26례, 중도 45례이었으며수술시중도인경우 - 2 -

11 골극제거및연골의 shaving 을시행하였고연골이소실된부위에대하여특별한조 작은시행하지않았으며중등도인경우연골의 shaving 만시행하였다

12 슬개골의관절표면대치술을시행하지않은 89례에서수술후추시할당시슬개건파열, 슬개골탈수, 혹은아탈구, 슬개골골절등은없었다. 수술후전체적인합병증은슬개골주위동통 2례, 감염 1례, 대퇴골과상부골절 2례등 5례 (6%) 에서있었다. 슬개골주위동통은슬개건염의증상으로보존적치료후술후 6개월이후동통의소실이있었으며수술 1년후황색포도상구균에의한감염이 1례있었으나대퇴인공물 (femoral prosthesis) 주위의 curettage와연부조직 debridement 후항생제투여로치료되었다. 낙상으로인한대퇴과상부골절이 2례있었으나골수강내고정술을시행후 4개월내골유합을얻을수있었다. Knee society clinical rating system에의한판정결과술전 knee score의경우 69-60점 2례 (2%), 59-50점 8례 (10%), 49-40점 18례 (20%), 39-30점 25례 (28%), 29-20점 22례 (25%), 19-10점 9례 (10%), 9-0점 5례 (6%) 이었으며술전평균슬관절점수는 34점 (5-63점) 이었으며술후에는 점이 39례 (44%), 89-80점 41례 (46%), 79-70점 7례 (8%), 69-60점 2례 (2%) 이었다. 술후평균슬관절점수는 91점 (65-98점) 이었다. 술전기능점수는 69-60점 2례 (2%) 59-50점 3례 (3%) 49-40점 42례 (46%) 39-30점 35례 (39%) 29-20점 5례 (6%) 19-10점 2례 (2%) 이었으며술전평균기능점수는 38점 (10-65점) 이었으며술후기능점수는 점 45례 (51%) 89-80점 37례 (41%) 79-70점 6례 (7%) 69-60점 1례 (1%) 이었으며술후평균기능점수는 89례 (63-100점) 이었다.(table 2) 슬개대퇴관절을 Merchant 촬영에의하여관찰하였으며슬개골의이상위치 (malposition) 나탈구또는아탈구의 소견은없었으며수술후 1 년, 3 년그리고 5 년의 Merchant 방사선촬영결과특별한 - 4 -

13 슬개골의골변화는발견할수없었다.( 사진 1) 동통술전 Knee society clinical rating system에의하면동통점수가 50점만점에서수술전 0점이었으나수술후 47점이었으며 73례 (82%) 에서동통이없었고, 13례 (15%) 에서경도의동통, 3(3%) 에서중등도의통증을호소하였며심한동통의예는없었다. 운동범위술전평균굴곡구축은 10 (0-35 ) 이었으며평균굴곡은 120 ( ) 이었다. 술후평균굴곡구축은 2 (0-5 ) 이었으며평균굴곡은 118 ( ) 이었다. 보행술전 12례는보행이불가능하였으며 5례는 1 block 이하보행가능, 18례는 1-5 block의보행가능하였으며 1례는 5-10 block의보행이가능하였다. 술후동통이나운동범위의제한없이 84(94.4%) 에서보행이가능하였으며 10 block이상이 3례 (3.3%), 5-10 block이 2례 (2.3%) 이었다. 2례 (2%) 에서계속적인동통을호소하였으나 6개월후동통의소실이있었다. 슬개골재표면술 (Resurfacing) 를시행하지않은전례에서정상적인슬개골 tracking과연부조직균형을갖추었다고사료되었다

14 인공슬관절전치환술후슬개골의합병증으로는골절 14,16,17,30), 대퇴슬개불안정 7,15), 슬개대퇴부동통 10), 슬관절신전기전의파열 12,26), 슬개부품파손 18,20,23,28), 연부조직 충돌현상 5) 등이있다. 슬개골골절의경우너무적게혹은많은슬개골의절제가있 었을경우골절의가능성이있으며 Reuben 등은슬개골의두께가 15mm이하인경우골절의위험성이있다고보고하였다. 또한수술중슬개골의혈액공급의차단으로무혈성괴사로슬개골골절의빈도가증가할수있다. 또한 correct patellar tracking이중요하다. maltracking으로 shear type의슬개골골절이올수있다. 슬개골 component의큰 central lugs가슬개골골절에기여할수있으므로최근골절을줄이기위하여작은 peripheral fixation lugs를사용한다. 인공슬관절대치술후슬개골대퇴골불안정성 8,15) 은 1-20% 이발생하는것으로발표되었으며대부분수술기법의잘못에기인한다. 대부분슬개대퇴골탈구보다는아탈구가많으며수술적치료를 요하는것이대부분이다. 정상적인인공물로증상이있는슬개골아탈구의경우 lateral retinacular release로충분할수있다. 사두고근의 imbalance로인한재발성슬개골아탈구-탈구에대하여 proximal patellar realignment가좋다고보고하였다 8). Whiteside는슬개건의내축을내측으로전이와함께 distal realignment를선호하였다. Distal alignment시술후창상감염의증가와슬개건의파열이보고되었다 12,26). 슬개대퇴불안정 8,15) 이상당한관절대치물의배열이상인경우관절대치물재치환술이좋은해결방법이다. 신전기전의파열은슬관절전치환술후의합병증이다. 빈도는높지않다고보고되어있지만슬개건파열이사두고근파열보다는흔하다. 슬개건파열시봉합술이불량한임상결과와 persistent extensor lag을가져온다. 만성슬개골파열의경우동종슬개골이식술로치료할수있다 12). 그러나슬개골표면대치술을시행하지않은경우이러한합병증의발생을줄일수있을것이다. 15 failure 23) 는대퇴부품을지나치게큰것을사용한경우, 슬개부품의두께가증가된경우, 슬관절굴곡의증가, 환자의활동정도및비만등이요인으로생각되며슬관절표면대치술을시행하지않을경우이러한것들을걱정할필요가없을것으로사료된다. 2001년 Barrack 7) 는슬개골표면대치술을시행한경우와시행하지않은경우슬관절점수및동통에차이가없었으며특히슬관절동통은슬개골표면대치술을시행했느냐여부보다는구조물의도안이나수술기법등에달려있다고발표하였다. 슬개골표면대치술을시행하지않은경우, 슬개골대퇴관절이계속적인동통의해결방 법으로슬개골표면대치술이필요하다고하나 22) 슬개골표면대치술을시행하였다고 반드시슬개골대퇴관절의동통이해결된다고예측할수는없다고보고하였다 1,7). 이러한여러가지합병증은슬개골표면대치술을시행하지않은경우문제가자연히해결될수있다. 저자의경우슬개골로인한합병증이없었으며이는표면대치술을시행하지않을경우이러한여러가지합병증을예방할수있을것으로사료되었고슬개대퇴관절동통이일시적인것이었으며임상적추시결과또한만족스러웠다

16 저자들은슬관절전치환술의슬개골표면대치술을시행하지않은 89례에대한추시결과다음의결과를얻었다. 첫째, 슬개골표면대치술을시행하지않아도 The Knee Society Clinical Rating System에의하면술전평균슬관절점수는 34점에서술후 91점으로호전을보였으며기능점수에서는술전평균 38점에서술후 89점으로호전 을보였다. 둘째, 슬개골표면대치술을시행하지않으므로슬개골골절, 슬개골탈구 및아탈구, 슬개건파열및슬개골삽입물의해리등의합병증을예방할수있었다. 셋째, 슬개골주위의만성동통은술후 6개월에소실될수있었다. 넷째, 슬개골표면대치술을시행하지않아도슬개대퇴관절의 tracking 및연부조직균형은잘유지되었다
