Instability of the patellofemoral joint can manifest as an acute dislocation, recurrent dislocation, habitual dislocation, and congenital dislocation. Patellar instability is associated with abnormalities in the anatomical structures around the knee, and a comprehensive analysis of the cause should be undertaken. Surgery can be performed to reconstruct any abnormal musculo-skeletal alignment. Considerable progress has been made in the treatment of patellar instability in recent years, and surgical methods include lateral release, soft tissue realignment procedure, transfer of the autologous tendon, trochleaplasty, and reconstruction of the medial patellofemoral ligament. Key words: patella, joint instability, joint dislocation, child, adolescent 서론 슬개골불안정성은급성탈구, 재발성탈구, 습관성탈구등으로정의할수있으며, 1) 이외에도선천성다발성결손이나증후군을동반한선천성탈구도있다. 2) 소아청소년에서의급성슬개골탈구는 10만명당 29-43명에서발생하여성인의 10만명당 명에비해 10배에달한다. 3) 최근연구에의하면 14세에서 18세의청소년에서 10만명당 147.7명에서발생한다. 4) 재발성탈구와습관성탈구는구별되어야한다. 5) 재발성탈구는주로외상에의해유발되어간헐적으로발생하고습관성탈구는일반적으로슬관절을구부릴때마다슬개골이탈구되며슬개골신전시자발적으로정복되는상태를말한다. 슬개골불안정성의수술적치료방법에는외측유리술, 연부조직재정렬수술, 자가인대이전술, 교정절골술, 활차성형술및 내측슬개-대퇴인대 (medial patellofemoral ligament, MPFL) 재건술등이있다. 과거에도슬개골불안정성의수술후좋은결과가보고된바있으나, 6-8) 최근 MPFL이슬개골에가장중요한수동적안정을제공하며슬개골탈구의약 80% 에서파열을보인다고알려진후 MPFL 재건술의중요성이부각되었다. 9) 성인에서시행한슬개골불안정에대한 MPFL 봉합술및재건술은임상점수와방사선각도에서우수한결과를보였다. 10) 성인환자의 MPFL 재건술시흔히대퇴골내측으로자가인대를골고정하여 MPFL 를복원한다. 소아청소년의경우대퇴골내측의부착부위가성장판에인접해있는어려움이있어내전근부위에고정하는술식이있으나 11) 성장판을피해골고정을시행한문헌도있다. 12) 본논문에서는소아및청소년슬개골불안정성의진단, 영상소견및다양한치료에대하여고찰해보고자한다. 2 202 Jung Woo Lee, et al. 본론 1. 진단재발성탈구는 2회이상의슬개골탈구를타인이관찰하거나환자가명확히표현한것으로정의내리며무릎이변형된것처럼보인후갑자기또는조작에의해정상적인모습으로회복된경험이있어야한다. 재발성슬개골탈구는여성에서호발 (70%) 하는것으로알려져있으며 14세이하의소아에서더흔하다. 13) 탈구가일어나지않을때가장두드러진증상은불안정성이다. 통증은탈구또는아탈구후에발생하지만성인과는달리주증상이아닌경우가많다. 슬개골재발성탈구가있는아동의전형적인병력은넘어진뒤무릎이 제자리를벗어난 것으로서무릎이제자리로돌아온뒤통증이발생하며종종붓는다. 최근발생한탈구후내원한경우슬개골주변동통및부종을동반하지만재발성탈구는대부분증상이없으며두가지이학적검사가진단에도움이된다. 첫번째는슬개골굴곡을시작하면서슬개골이특징적으로이동하는 J sign 이다. 이는슬관절완전신전상태에서굴곡할때슬개골이대퇴골활차의내부로들어왔다가나가는것으로서굴곡초기슬개골이외측으로이동했다가추가굴곡시내측으로이동한다. 두번째는페어뱅크 (Fairbank) 의불안검사로, 슬관절 30도굴곡상태에서슬개골을수동적으로외측으로이동시킬때명백한불안증상이일어난다. 14) 슬관절완전신전시슬개골과대퇴골외과간압력이없어지므로불안증상이감소한다. 슬개골이동불안검사로진단의정확성을더높일수있다. 먼저 1단계로슬관절신전상태에서 90도굴곡하며슬개골을외측으로압력을주고, 2단계에서는같은각도로슬관절을신전-굴곡하면서슬개골을내측으로압력을준다. 1단계에서불안증상이있고 2단계에서증상이사라지면검사양성으로, 마취상태에서보이는슬개골탈구와연관이있다. 15) 한편 Luhmann 등 16) 은슬개골불안정성환자에서수술전신전시염발음 (crepitus) 이 18% 에서발생하였으며이것의유무가슬개골연골손상의정도와관계있다고하였다. 흔히발생하는습관성탈구는굴곡시발생하고무릎신전시에는슬개골이정상위치이나무릎굴곡시마다슬개골이탈구되며신전시다시정복되는것을의미한다. 증상은보통 5세에서 8 세사이에나타나며무릎이불안정하고달리기및운동에어려움이있으며어린시기부터탈구가진행되어활차는주로평평하다. 반면무릎신전시발생하는습관성탈구는무릎이신전될때슬개골아탈구또는탈구되는것이특징이다. 슬개골굴곡이계속되면슬개골이정상위치로돌아가는데증상의심각도에따라그각도가다르다. 대퇴사두근단축이있어슬관절신전시고위슬개골을보일수있다. 11) 2. 병태생리및요인고위슬개골이있을때굴곡후반부에고위슬개골과대퇴골이만나며슬개골불안정성이생길수있다. 17) 대퇴골 -경골간회전변형은슬개골에외측으로향하는힘을가한다. 18) 대퇴전염각, 외반슬및경골결절외회전의조합은슬개건과대퇴사두근이이루는 Q 각에영향을준다. 재발성슬개골탈구에서대퇴사두근이슬개골의외측전위에영향을준다는것은널리알려져있다. 활차의이형성이있을경우대퇴골에의한슬개골의외측전위저항이감소되며, 19) 이는슬개골불안정성환자들의방사선사진에서도관찰된다 (Fig. 1). 20) 슬개골내측안정성을담당하는주요연부조직은 MPFL로서잦은슬개골탈구가있는환자에서일관되게이인대의파열을볼수있다. MPFL은내측지대내에다양한너비로존재하는관절외인대로서인대유리후슬개골의외측이동이 50% 증가하며인대봉합후안정성이회복되었다는보고가있다. 21) 사체연구에따르면 MPFL은외측이동제한의 41%-80% 를담당한다. 22) MPFL은같은길이 ( 등척성 ) 의구조물이아니다. 이는슬관절이완전히신전되었을때긴장되고슬개골이활차에들어있는슬관절굴곡시느슨해진다. 23) 소아청소년의 MPFL 손상은대퇴골부착부보다슬개골부착부 (81%) 에서흔하다는보고가있다. 24) 소아에서의슬개골불안정성은다운증후군, 25) 터너증후군 26) 등과연관된경우가있으며이러한경우 MPFL의기여도는아직명확히밝혀지지않았다. MPFL 외에도내측슬개-경골인대 (medial patella-tibial ligament, MPTL) 와내측슬개-연골판인대 (medial patella-meniscal ligament) 가사체연구에서 90도슬관절굴곡시외측이동의저항에 46% 를담당한다고알려졌다. 27) A B Figure 1. 3 203 Patellar Instability 3. 방사선적소견급성슬개골탈구환자의 30% 에서골연골골절이동반될수있다. 28) 골절이있을때주로슬개골의내측경계또는능선을포함하며드물게체중부하영역에서대퇴골외과의중간 3분의 1을포함한골절이발생한다. 골절은관절면의충돌로야기되고고위슬개골이없는환자및중간정도의활차형성장애가있는환자에서발생할확률이더크다. 체중부하방사선사진은슬개골불안정성에영향을줄수있는외반슬을보여줄수있고고위슬개골은측면방사선사진으로결정된다 (Fig. 2). 측면방사선사진에서슬개골길이 (length of patella, LP) 와슬개건길이 (length of patella tendon, LT) 는거의같아야하며, LT가 LP보다 20% 이상길다면 (LT/LP 1.2) 고위슬개골이있는것이다. 29) 슬관절 30도굴곡상태의측면방사선사진에서슬개골은과간절선의전방연결과원위대퇴성장판의중앙을연결한선사이에있어야한다. 30) 접선 (tangential) 슬개골촬영은슬개골이동, 슬개골경사및활차구조를정의하는데유용하다. 경도의슬관절굴곡시슬개골이고랑의상부에위치해있을때슬개골의불안정성을보는데가장유용하다. Merchant view는슬관절을 30도굴곡하여촬영한다. 일치각 (congruence angle) 은슬개대퇴탈구의확실한지표로, 31) 일치각이 16도이상일때외측아탈구로볼수있다. Laurin 등 32) 이기술한외측슬개-대퇴각 (lateral patellofemoral angle, LPFA) 은대퇴구 (femoral sulcus) 를잇는선과슬개골외측관절면 (lateral patellar facet) 을잇는선의각도이다. 정상슬개-대퇴관절에서 LPFA는외측으로열려있지만외측아탈구환자에서는 0도이거나내측으로열려있다. 방사선부하사진또한슬개골불안정성을평가하는데사용되는데, 불안정한슬개골은부하에의해 4 mm 이상움직인다고알 려져있다. 33) 활차이형성은측면방사선사진에서대퇴골외과의경계선과내과의경계선의분리가 5 mm 이내일때진단한다. 정확한측면방사선사진을얻기어렵기때문에, 컴퓨터단층촬영이필요하다. 경골결절에슬개건부착부의중심과활차고랑의중심사이의거리 (tibial tubercle to trochlear groove distance, TT- TG) 로슬개대퇴관절의관상면또는회전불일치를측정할수있다. 이거리가 20 mm보다클때심각한불균형이존재한다고추정할수있다. 34) 자기공명영상 (magnetic resonance imaging, MRI) 은슬개골불안정성에서중요한진단도구이며슬개골불안정성과관련된관절막, 인대및골연골손상을감지하는데매우민감한것으로나타났다. 35) 또한 MRI 영상은활차이형성, 슬개골높이및경골결절의위치를포함하여슬개골불안정성에기여할수있는해부학적형태를평가하는데사용될수있다. 36) 4. 비수술적치료슬개골탈구시초기에는편안함을위해무릎고정장치로고정해야한다. 처음며칠간환자는대퇴사두근을강화하기위해하지거상운동을시작하며통증이완화되면서내측광근에초점을맞춘강화운동이이루어진다. 무릎보조기또는슬개골안정화보조기가회복기간에유용할수있다. 슬개골재발성탈구의초기치료는보존적치료이다. 내측광근을강화하는것은대부분의치료지침의중요한요소이고환자의증상을줄일수있으며수술의결과에도긍정적인영향을미친다. 폐쇄형및개방형대퇴사두근강화운동모두권장된다. 높은강도의운동은관절연골에높은압력을유발할수있으므로피해야한다. 비수술적치료후장기추시결과에서스포츠기능을평가하는 Tegner activity level 37) 은 6.0으로의미있는호전을보이지않았으며 70% 의환자에서재탈구가일어났다는보고가있으나보존적치료후수술하게되는비율에대해서는밝혀진바가없다. 38) 단 Fithian 등 39) 은첫번째탈구이전에슬개골불안정성이있었던환자들의경우 49% 에서재발성탈구가발생하였으며외상으로인한 1회성탈구와구별하여야한다고하였다. 소아, 활차이형성등의위험인자가동반되어있는경우보존적치료시고려해야할것으로생각된다. A B 5. 수술적치료반복되는탈구의병력이있으면수술적치료가필요하다. 성장하는환아에서성장판이손상되면안되기때문에이에대한고려가필요하다. 슬개골높이뿐아니라 Q 각과회전문제를해결하는경골결절절골술과활차이형성을다루는활차절골술이있지만골격이미성숙할때는연부조직수술을고려해야한다. 회전절골술은심한염전의경우에고려될수있다.

4 204 Jung Woo Lee, et al. 1) 외측 유리술 4) 원위부 재정렬 수술 슬개골 경사나 아탈구가 있을 때 외측 유리술을 시행할 수 있다. 과도한 Q 각이 있으면 경골 결절에 슬개건이 외측으로 부착하기 재발성 탈구가 있는 어린 환자의 외측 유리술 후 장기 추시에서 때문에 원위부 재정렬 수술을 시행할 수 있다. TT-TG 측정 값이 40) 환자의 대다수는 수술에 만족한다는 보고가 있으나, 단독 유리 3) 20 mm보다 크거나 또는 슬개건 부착부의 중심과 후방십자인대 술만은 불충분한 경우가 많으며 추천되지 않는다. 또한 과도한 사이의 거리(TT to posterior cruciate ligament distance)가 24 mm 외측 지대 유리와 내측 봉합술은 의인성 내측 아탈구를 유발할 보다 클 때 교정이 필요한 부정정렬이 존재한다고 여겨진다.43) 성 41) 숙한 골격을 가진 환자에서는 경골 결절 절골술 및 내측 이전술 수 있으므로 피해야 한다. 인 Elmslie-Trillat 시술을 시행할 수 있다.7) Fulkerson 시술은 El2) 근위 연부조직 재정렬 수술 mslie-trillat 시술의 변형으로서 경골 결절 절골술 후 결절을 전 미성숙한 골격을 가진 소아청소년 환자에서는 슬개건의 기시부 방 및 내측으로 이전시킨다. 경골 결절의 성장판이 열려 있는 미 를 옮길 수 없기 때문에 수술법이 제한된다. Insall 등42)이 고안한 성숙한 골격을 가진 환자에서 절골술을 시행할 경우 성장 장애나 근위 연부조직 재정렬 수술은 슬개골에 가해지는 힘을 재정렬할 전반슬을 일으킬 수 있어 금기시된다. 이 경우 슬개건의 외측 부 수 있으며 41명 환자의 6개월에서 5년 사이의 추시에서 모든 환 분을 분리하여 내측 부분의 밑으로 통과시켜 근위 경골의 골막에 자에서 fair 이상의 좋은 결과를 보고했다. 그러나 대퇴골 외과의 봉합하는 Roux-Goldthwait 시술을 할 수 있다.6) Grammont 시술 결손이나 근육 결손을 동반한 경우 혹은 다운 증후군과 같이 인 은 슬개건과 골막 전체를 내측으로 이동하는 방법으로서 경골 결 대 이완증을 동반한 경우에 이 수술법만으로는 충분하지 않다. 절 성장판에 영향을 줄 수 있는 위험이 있다. 슬개건 부착부가 지 나치게 원위부로 고정될 경우에 슬개 하위증(patella baja)으로 통 3) 반건양건 고정술(semitendinosus tenodesis) 증을 유발할 수 있어 주의해야 한다. Joo 등44)은 외측 유리술, 근위 인대 이완증, 대퇴골 외과 결손, 내측 근육 결손을 동반한 슬개골 연부조직 재정렬술, 반건막근 건고정술, 슬개건 이전술을 동시에 탈구에서 반건양건은 정상인 경우가 많으며 반건양건 고정술을 시행하는 four-in-one 시술을 소개하여 좋은 결과를 얻은 바 있 근위부 재정렬 수술과 함께 시행하는 것이 추천된다. 이 시술은 다(Fig. 3). 슬개건의 힘의 방향을 회복시켜 주는 장점이 있다. Dewar-Galeazzi 시술은 외측 지대 유리, 내측 광근의 내측 전진 및 반건양건 5) 내측 슬개-대퇴 인대 재건술 슬개골 이전술을 포함한다. 반건양건 이전술은 대퇴사두근의 경 반복적인 슬개골 탈구에서 MPFL은 종종 약화되어 있거나 여러 골 부착에 작용하는 힘을 내측으로 재배열하며 42명의 환자 수술 부위에서 결손되어 있으며 최근 이에 대한 재건술이 보고되었 후 평균 5년 추시에서 1명에서 재발이 발생했고 80%에서 good 이 다.1) 소아청소년의 MPFL 부착부는 성장판의 근위부 및 원위부 8) 로 다양하나,45) 주로 성장판의 원위부에 위치해 있다고 알려져 있 상의 좋은 결과가 보고되었다. 다.46) Shea 등47)은 최근 사체 연구에서 MPFL 부착부가 7세 이하 의 경우 성장판의 원위부에, 7세 이상의 경우 성장판의 근위부에 A B C Figure 3. 5 205 Patellar Instability 부착함을보고하였다. 소아및청소년에서 MPFL 재건술을시행할시에는원위대퇴골의인대고정부위가성장판에가깝기때문에성장판에손상을가하지않아야하며슬개골의크기가작기때문에슬개골에인대를통과시킬때의인성골절을주의해야한다. 48) 또한원위대퇴골에고정할경우성장판, 49) 절흔 (notch) 과대퇴연골의손상을피하기위해대퇴부드릴의방향을원위부및전방으로 15-20도기울여야한다. 50) 이술식을사용하여 Nelitz 등 51) 은평균 12.6세환아의 2.6년추시에서재발없이 96% 에서만족한결과를보고하였다. 또한성인사체연구에서대퇴골에해부학적위치의재건이되지않을경우내측관절접촉압력이증가함이보고되어, 52) 수술시주의를요한다. MPFL은반건양건, 장경인대를이용하거나대퇴사두건으로 53) 재건할수있다. 슬개골이작을경우자가대퇴사두건사용시슬개골에손상없이수술을할수있는장점이있다 (Fig. 4). 최근 Alm 등 54) 은 30명의소아청소년환자에서반건양건과대퇴박근 (gracilis) 를절단하여대내전근 (adductor magnus) 부착부에걸고슬개골에고정하는 sling 수술을시행하여 2년추시에서 13% 에서재발함을보고하였다. Hohn과 Pandya 55) 는소아청소년환자에서동종건을사용하여 MPFL 재건술시행후 2년추시한결과 92% 에서안정을얻었다고보고하였다. 소아와성인을모두포함한문헌고찰에서 MPFL 재건후다양한합병증이 26% 에서발생하였고재수술은 4% 에서이루어졌다. 56) 최근 Sadigursky 등 57) 은평균 11세의소아청소년환자에서 MPFL과 MPTL을동시에재건후 12개월추시하여임상점수의호전을보였다고하였다. 6) 기타수술법슬개-대퇴불안정성이있는환자에서기계적축혹은회전변형이관련되었다는보고가있으며최근성인환자에서이를교정하기위해교정절골술을시행한보고가있다 ) Nelitz 등 46) 은평균 18.4세환자의 MPFL 재건술에서증가된대퇴염전각이재수술의이유가된다고보고하였다. 교정절골술을통한재정렬수술의적응증은아직명확하지않으며기계적축과원위대퇴골외측각의어느정도가교정이필 요한지는아직알려져있지않다. 61) 그러나원위대퇴골내측성장판임시고정술이나대퇴골원위부절골술을이용하여외반정렬을치료하는것이 Q 각호전에도움을줄수있다. 활차이형성에대한활차성형술은적응증이명확하지않으며골격이미성숙한환자의치료법은아닌것으고알려졌다. 62) Pesenti 등 63) 은평균 13.8 세의소아청소년환자의수술에서활차의구조적이상이있어도 MPFL 단독재건술만으로도재발률이증가하지않았다고하였다. 반면최근 Balcarek 등 64) 은평균 19세환자의문헌고찰에서인대수술과활차성형술을동시에시행했을때 MPFL 단독재건술보다재발률이낮다고보고하였다. 또한 Longo 등 65) 은평균 22세환자의문헌고찰에서 Dejour와 Le Coultre 66) 및 von Knoch 등 67) 이발표한활차성형술의결과가이전보다안정성및기능에서개선되었다고하였다. 활차성형술의적용시연령을고려한접근이필요하다. 6. 예후수술후장기예후는다양하다. Jaquith와 Parikh 68) 는소아청소년슬개골재발성탈구환자의수술시성장판이열려있는경우 43% 에서, 성장판이닫혀있는경우 22% 에서재발한다고보고하였다. 평균 25세환자의 14년의추시결과에서수술한경우 20% 에서재발성불안정성및 75% 에서슬개-대퇴관절의관절염을보였다. 69) 소아및청소년환자의인대재정렬수술후 5년추시에서 Lysholm score는 69.3 점및 International Knee Documentation Committee score는 65.6점으로보고되었으며어린환자와수술전증상의지속기간이짧은환자에서더좋은 Tegner activity level 을보였다. 70) 평균 14세환자의 MPFL 재건후 10 년추시결과에서활차이형성이있다고하더라도 Crosby/Insall criteria는 90% 에서 good to excellent의좋은기능결과를보였으며슬개-대퇴관절관절염도 87% 에서 none to mild로좋은결과를보였다. 71) Krych 등 72) 은성인을포함한평균 17.5세의운동선수의 MPFL 재건술후 47개월추시에서 85% 의환자가 8.1개월후운동에복귀하였으나수술후 6개월까지근력의약화를보였다고보고하였다. 특히경골결절이전술을동반한환자에서운동에복귀가늦었다. 6 206 Jung Woo Lee, et al. 결론 MPFL의역할에대한연구와새로운재건기술의개발은슬개골불안정성환자의수술기법및예후호전에크게기여하였다. 소아청소년슬개골불안정성환자의수술시성장판및성인에비해작은슬개골의크기를고려하여야한다. 한가지의시술만으로치료하기보다는환자의상태에맞게다양한술식을고려하여야하며연부조직술식뿐만아니라골격에대한수술도고려되어야한다.
