Methods: We conducted a retrospective study of 32 consecutive eyes of 26 patients with a visually significant cataract and moderate corneal astigmatism (higher than 1.25 diopter [D] and lower than 4.5 D) undergoing cataract surgery with implantation of the aspheric Tecnis ZCT toric IOL (Abbott Medical Optics Inc.) and the Zeiss AT TORBI toric IOL (Carl Zeiss Meditec AG). Phacoemulsification was performed by the same experienced surgeon using 2.2 mm temporal incision. Visual, refractive and aberrometric changes were evaluated during a 3-month follow-up. Power vector analysis of Cartesian astigmatism (J0) and oblique astigmatism (J45) was performed. Results: At the 3-month follow-up, corrected distance visual acuity (CDVA) and residual astigmatism showed no statistically significant differences between groups (p = and p = 0.364, respectively). Pre- and postoperative J0 were 0.71 ± 0.84 and 0.05 ± 0.39 in the Tecnis Toric group and, 0.88 ± 1.27 and ± 0.16 in the AT TORBI group, respectively, which showed statistically significant differences (p = and p = 0.032, respectively). Pre- and post-operative differences of J0 and J45 were not statistically significant (p = and p = 0.603, respectively). No eye had IOL rotation 10. Ocular aberrometry values were statistically significantly differenct between the groups, except for spherical aberration, which was higher in the AT TORBI group (p = ). Conclusions: Both IOLs showed good postoperative uncorrected distance visual acuity, CDVA and refractive results in this study. Rotational stability was excellent for both IOLs until the 3-month follow-up. 2 - 양순원외 : 두가지토릭인공수정체임상결과비교 - 막윤부나각막에이완절개를하는방법을고안해왔다. 4,5 이러한방법들은결과예측이힘들고부정확하다는단점이제기되어왔고, 개인의상처치유반응및술자의술기에의존하기때문에교정할수있는난시량에한계가있다. 6 또한과교정이나저교정, 각막천공및상처벌어짐등도고려되어야할문제점이다. 레이저를이용한굴절수술로난시를교정하는방법은비교적정확한교정효과를보이지만각막절편관련합병증이나눈부심, 달무리같은문제와이차수술로인한환자의불편감등의단점이있다. 6-8 난시교정인공수정체를이용한난시교정은결과에대한예측도가높으며가역적이라는장점외에도불규칙난시를유발하지않으므로뛰어난시력의질을보여줄것으로기대되어왔다. 난시교정인공수정체는 1992년부터임상적으로사용되어왔으며, 초창기에는약 20% 의환자에서 30도이상의렌즈회전이관찰되었다. 9 실제로난시교정용인공수정체의축이목표축으로부터 1도씩벗어날때마다인공수정체의난시교정효과가약 3% 씩감소한다고알려져있으며, 축이목표치에서 30도이상차이가날경우, 오히려난시량이더증가할수있다 또한초창기난시교정용인공수정체는수술시절개창이커서각막난시유발및정확한도수결정에어려움이있었다. 최근에는미세절개창백내장수술을시행하는것이가능해져서수술후난시유도가적고, 12 인공수정체의디자인및재질이발전하여그안정성및예측도가많이개선되었다. 따라서난시교정용인공수정체삽입이가장효과적이고, 안정적으로각막난시를줄일수있는방법으로보고되고있다 테크니스난시교정용인공수정체 (TECNIS ZCT Toric IOL; Abbott Medical Optics Inc., Santa Ana, CA, USA) 는일체형 C loop의지지대를가진형태로소수성아크릴재질이며 μm의음성구면수차를가지고있다. 또한인공수정체의앞면에난시교정효과를가지고있는비구면렌즈이다. 자이스 AT TORBI 난시교정용인공수정체 (AT TORBI 709M Toric IOL; Carl Zeiss Meditec AG, Jena, Germany) 는일체형판형지지대를가진형태로친수성아크릴에소수성코팅을한재질이며난시교정효과가인공수정체의전후면모두에있는특징이있고구면수차가 0 μm이다. 테크니스난시교정용인공수정체및자이스 AT TORBI 난시교정용인공수정체의경우국외여러논문에서그효과와안정성이보고되었으나, 국내에서는아직까지보고된바가없다. 이에본연구에서는테크니스난시교정용인공수정체및자이스 AT TORBI 난시교정용인공수정체의임상효과를비교하여분석해보고자한다. 대상과방법 본연구는 2013년 4월부터 2015년 5월까지가톨릭대학교서울성모병원에서수정체유화술및인공수정체후낭삽입술을시행받은 26명중 32안을대상으로의무기록을후향적으로분석하였으며, 가톨릭중앙의료원연구윤리심의위원회 (Institutional Review Board, IRB) 로부터과제번호 KC15RISI0380로심의승인을받았다. 1.00D 이상 4.50D 이하의규칙각막난시를가진환자들을대상으로하였으며, 불규칙각막난시, 각막혼탁, 홍채이상, 시력에영향을끼칠수있는안구질환등이있을경우와수술중후낭파열등의합병증이발생한예는본연구에서제외하였다. 수술전검사수술전검사로는환자병력청취, 나안시력, 최대교정시력, 현성굴절검사, 세극등현미경검사, 안저검사, 안축장, 각막곡률측정검사및각막지형도검사 (Pentacam, Oculus GmBH, Wetzlar, Germany) 를시행하였다. 안축장은 IOLMaster (Carl Zeiss Meditec Inc., Dublin, CA, USA) 로측정하였고측정되지않는경우 A-scan (Axis II, Quantel Medical, Cedex, France) 을이용하였다. Pentacam에서각막전후면을고려한 total cornea refractive power map (TCRP) 의 3 mm 지역의난시축과난시량을바탕으로인터넷-기반의난시교정용인공수정체도수계산기 (TECNIS Toric Calculator; AT TORBI 709M IOL Calculator; zeiss.com/zcalc) 를통해목표굴절치는정시에가장가까운근사치로 Haigis 공식을이용하여난시교정용인공수정체를결정하였다. AT TORBI 709M IOL Calculator 사용시기록하게되어있는전방깊이는 IOLMaster (Carl Zeiss Meditec Inc., Dublin, CA, USA) 측정값을대입하여계산하였다. 수술후유도되는난시 (surgically induced astigmatism, SIA) 값은 0.25D로산정하였다. 수술방법수술 3일전부터 0.5% moxifloxacine (Vigamox, Alcon, TX, USA) 안약을하루에 4회씩점안하였다. 술전에 Mydrin-P (Santen, Osaka, Japan) 를 10분간격으로 3회점안하여충분히산동시킨후 4% lidocaine을이용하여점안마취를시행하였다. 수술전 Surgery Guidance SG5000 (SensoMotoric Instruments GmbH, Teltow, Germany) 을측정하여환자의홍채를인식한후수술직전 TCRP의난시축을입력하고누운상태에서 SMI guide하에난시축을표시하였다 (Fig. 1). 모든수술은숙련된한술자에의하여이루어졌으며각막이측에 2.2 mm의미세각막절개를하고, 201

3 - 대한안과학회지 2016 년제 57 권제 2 호 - Infinity Vision System (Alcon Laboratories Inc., Fort Worth, TX, USA) 과 OZil Torsional Handpiece (Alcon Laboratories Inc., Fort Worth, TX, USA) 를이용하여수정체유화술을시행하였다. 인공수정체를후낭에삽입한후, 후낭내에서완전히펴지기전에관류및흡입을시행하여인공수정체뒤에남아있는점탄물질을제거하고인공수정체를회전시켜미리표시해둔표식에따라인공수정체의축을위치시켰다. 그후에인공수정체를조심스럽게후방쪽으로두드려주어전낭절개면이렌즈의광학부를덮을수있게위치시키고전방의점탄물질을제거후 SMI를이용하여인공수정체의난시축을다시확인하고수술을종료하였다. 수술중후낭파열등의합병증및수술후합병증은발생하지않았다. 수술후검사수술후 1일, 1주일, 1달그리고 3개월에나안시력, 최대교정시력, 현성굴절검사, 세극등현미경검사를시행하였다. 수술후 3개월째수술한눈을산동한후세극등현미경앞에서정면을보도록한후역조명을비추어전안부사진을촬영하고 SMI로인공수정체면에표시되어있는기준표지자 (reference mark) 의축을측정하여수술시의도하였던삽입축과인공수정체가실제로삽입된축의차이를분석하였다. 수술후 3개월째 Wavescan (VISX, Abbott Laboratories Inc., Santa Ana, CA, USA) 을이용하여 6 mm 동공에서전체고위수차 (total higher order aberration), 구면수차 (spherical aberration), 코마수차 (coma) 및트레포일수차 (trefoil) 값을측정하였다. 난시분석방법 Power vector 방법을이용해서 Cartesian astigmatism (J0), oblique astigmatism (J45) 으로난시분석을시행하였다. 자동굴절검사값을다음과같은수식에대입하여계산하였다 (C는 cylinder 값을, α는 cylinder 축을나타낸다 ). 4 - 양순원외 : 두가지토릭인공수정체임상결과비교 - 를사용하였으며모든수치는평균 ± 표준편차의형식으로나타내었다. p<0.05인경우통계학적으로유의한값으로간주하였다. 결과 본연구는 1.0D 이상및 4.5D 이하의규칙각막난시를동반한백내장으로난시교정용인공수정체를삽입한 26명의 32안을대상으로하였다. 환자군의연령은 21-80세까지분포하였으며, 테크니스군은 14명의 18안, 자이스 AT TORBI 군은 12명의 14안이었다. Table 1은환자의수술전특징및사용한인공수정체를기술한표로써수술전나안시력, 교정시력, 구면치, 굴절난시, 각막난시및각막구면수차에서통계적으로유의한차이는없었다. 시력및굴절테크니스난시교정용인공수정체를넣은군과자이스 AT TORBI를넣은군모두에서수술 3개월째나안시력, 최대교정시력, 구면치, 굴절난시및구면대응치가수술전보다통계적으로유의하게증가하였다 (Table 2, 3). 나안시력은테크니스군중 94.5%, AT TORBI 군중 91.7% 에서 0.3 logmar 시력 ( 스넬렌시력표 20/40 이상 ) 보다더좋은시 력수치를보였고테크니스군중 72.2%, AT TORBI 군중 75.0% 에서 0.0 logmar 시력 ( 스넬렌시력표 20/20 이상 ) 보다더좋은시력수치를보였다. 최대교정시력은두군모두에서 0.10 logmar 시력 ( 스넬렌시력표 20/25 이상 ) 보다더좋은수치를보였다. 수술후구면렌즈디옵터가 ±0.5D 범위내로측정된수는테크니스군에서 16안 (89%), AT TORBI 군에서 12안 (86%) 이었고, 수술후 ±1.0D 범위내로측정된수는두군모두 100% 였다. 현성굴절검사상나타난굴절난시가 <0.50D로측정된수는테크니스군에서 16안 (89%), AT TORBI 군에서 12안 (86%) 이었으며, <1.00D 로측정된수는테크니스군에서 17안 (94%), AT TORBI군에서 13안 (93%) 이었다. 수술후계산한구면렌즈대응치는수술전의도했던것과비교하여볼때그차이가 ±0.5D 범위내로측정된수는테크니스군에서 16안 (89%), AT TORBI 군에서 12안 (86%) 이었고, ±1.0D 범위내로측정된수는두군모두 100% 였다. 두군을비교하였을때수술후 1일, 1주, 1개월및 3개월째두군의굴절난시및구면대응치는통계적으로유의한차이를보이지않았다 (Fig. 2). 난시분석수술후 3개월째시행한현성굴절검사결과를토대로파워벡터분석을시행하였다 (Table 4). 테크니스군에서 J0 Table 2. AT TORBI 군에서 J0는수술전 0.88 ± 1.27D에서수술후 ± 0.16D 로감소하였고역시통계학적으로유의한결과를보였다 (p=0.032). J45는테크니스군과 AT TORBI 군모두에서수술전후통계학적으로유의한차이를보이지않았다. J0의경우 ±0.5D 범위내로측정된수는테크니스군에서 18안 (100%), AT TORBI 군에서 13안 (93%) 이었고, J45의경우모두 ±0.5D 범위내로측정되었다. 두군의수술전후 J0 변화량은통계적으로유의한차이를보이지않았다. 인공수정체축회전에대한안정성수술후 3개월째각인공수정체축회전의평균값은테 크니스군의경우 3.2 ± 2.2 (0-8 ), AT TORBI 군의경우 2.4 ± 2.0 (0-8 ) 로나타났고통계적으로유의한차이는없었다 (p>0.05). 두군모두에서 10도이상회전한경우는없었다. 고위수차분석수술후 3개월째두군의고위수차를비교분석하였다 (Table 5). 두군의전체총고위수차 (root mean squre, RMS), 코마수차및트레포일수차는두군간통계적으로유의한차이를보이지않았다. 구면수차가테크니스군에서 0.03 ± 0.04 μm (-0.04~0.06 μm), AT TORBI 군에서 0.18 ± 0.06 μm (0.041~0.57 μm) 로측정되었고 AT TORBI 군에서통계학적으로유의하게높은수치를보였다 (p=0.008). 204

6 - 양순원외 : 두가지토릭인공수정체임상결과비교 - 고찰 난시교정용인공수정체 (Toric IOL) 를이용하여백내장수술을시행하는것은환자의백내장및각막난시를함께교정하여술후임상결과를잘예측할수있는방법으로잘알려져있다. 7 복잡한절차나술기를요구하지않고, 추가적인시술로인해합병증이발생할가능성도없어효과적인난시교정방법이라할수있다. 본연구에서는서로다른디자인과구면수차를보이는테크니스난시교정용인공수정체와 AT TORBI 난시교정용인공수정체를삽입한후의임상결과에대해비교하였다. 국내에서는 Acrysof 난시교정용인공수정체의임상결과를연구한국내논문은있었으나, 21,22 아직까지두가지난시교정인공수정체를직접적으로비교한국내논문은없었다. 수술전난시축결정을위해본연구에서는 Surgery Guidance SG5000 (SensoMotoric Instruments GmbH, Teltow, Germany) 을측정하여환자의홍채를인식한후수술직전 TCRP의난시축을입력하고누운상태에서 Surgery Guidance SG5000 (SensoMotoric Instruments GmbH, Teltow, Germany) guide하에난시축을표시하였다. 이는실시간으로눈의이동을추적하고, 이를통해난시교정용인공수정체가올바른위치에놓일수있도록하는정확도를높여줄수있는것으로보고되어있다. Pentacam에서각막전후면을고려한 TCRP의 3 mm 지역의난시축과난시양을바탕으로인공수정체도수계산을시행한후, 수술을시행하였다. 다수의연구에서각막후면난시를고려하지않은경우부정확한결과를초래할수있다고보고하였으며, 특히각막전면난시만계산하여수술을시행할경우직난시를가진환자는과교정을, 도난시를가진환자는저교정을보일수있다고하였다 또한 Pentacam의경우각막전체또는후면난시량에대한예측도가우수하며, 특히 TCRP 방법은각막의 Profile에대한예측의정확도가좋은것으로보고된바있다. 26 이러한이유로본연구에서는테크니스 toric calculator 및자이스 toric calculator를통한인공수정체의도수계산시에둘다각막전후면난시모두를고려하기위해 TCRP 3 mm 지역의난시축과양을대입하여계산하였고, 전면난시만을고려한 IOLMaster의난시값은배제하였다. 실제로본연구에서자이스 toric calculator 계산시 IOLMaster의난시값을대입한경우및 Pentacam의 TCRP 3 mm 지역의난시값을대입한경우를비교해보았을때, IOLMaster의난시값으로계산한경우후면난시로인한보정효과가없어 D 범위로직난시를가진환자에서과교정을, 도난시를가진환자 에서저교정을일으킬수있는렌즈도수가계산되었다. 테크니스 Toric IOL 및 AT TORBI Toric IOL은국외유수의논문에서그안전성, 시력교정효과, 인공수정체회전에대한안정성및개인의만족도등에있어서좋은결과를보여왔다. 14,18,27-33 본연구에서도두가지인공수정체모두시력교정효과가훌륭한것으로나타났다. 테크니스군중 94.5%, AT TORBI 군중 91.7% 에서나안시력으로 0.3 logmar 시력 ( 스넬렌시력표 20/40 이상 ) 보다더좋은시력수치를보였고, 테크니스군중 72.2%, AT TORBI 군중 75.0% 에서 0.0 logmar 시력 ( 스넬렌시력표 20/20 이상 ) 보다더좋은시력수치를보였다. 본연구에서수술후구면렌즈디옵터가 ±0.5D 범위내로측정된수는테크니스군에서 16안 (89%), AT TORBI 군에서 12안 (86%) 이었고, ±1.0D 범위내로측정된수는두군모두 100% 로우수한결과를보였다. 굴절난시가 <0.50D로측정된수는테크니스군에서 16안 (89%), AT TORBI 군에서 12안 (86%) 이었으며, <1.00D로측정된수는테크니스군에서 17안 (94%), AT TORBI 군에서 13안 (93%) 으로우수한난시교정효과를보였다. Toric IOL을삽입한후, 난시값의변화를보고한연구들은다양하게있었다. 14,16,21,29,34,35 Sheppard et al 35 은테크니스 Toric IOL을이용한연구에서술전굴절난시값이 ± 1.07D에서술후 ± 0.54D로통계학적으로유의하게평균 65% 감소하였음을보고하였고 (p<0.05), Bascaran et al 29 은 AT TORBI 709M IOL로시행한연구에따르면굴절난시가술전 ± 1.72D에서술후 ± 0.53으로통계학적으로유의하게평균 80% 감소했음을보고하였다 (p=0.00). 본연구에서는현성굴절검사상난시값이테크니스군에서는평균 76.89%, AT TORBI 군에서는평균 86.96% 감소하여우수한난시교정효과를보였으며, 굴절난시값의변화는두군간에차이는통계학적으로유의한결과를보이지않았다. 두렌즈에서구면렌즈대응치및굴절난시의감소효과는수술후 1일에서부터 1주, 1개월및 3개월까지유의한변화를보이지않았으며, 두렌즈간비교하였을때도각관찰시기에구면렌즈대응치및굴절난시에서유의한차이를보이지않았다. 난시교정용인공수정체삽입후난시변화량을파워벡터분석을통해나타낸연구들에서도본연구와비슷한경향을보였는데, Sheppard et al 35 은 J0가수술후통계학적으로유의하게감소하였으며 (p=0.002), J45는수술전후통계학적으로유의한차이를보이지않았다고보고하였다 (p=0.703). 본연구에서도테크니스군과 AT TORBI 군모두에서 J0는수술후통계학적으로유의하게감소하였고 (p=0.029, p=0.032), J45는수술전후통계학적으로유의한차이를보이지않은것으로나타났다 (p=0.472, p=0.750). 205

7 - 대한안과학회지 2016 년제 57 권제 2 호 - J45의경우수술전후벡터값의크기가거의 0에가깝게매우작은것이그원인으로생각된다. 난시교정인공수정체삽입후인공수정체축회전의경우 Waltz et al 27 은테크니스 Toric IOL을삽입한 156안에서술후 3개월째평균 1.79 ± 2.12 의축회전을보였고, 그중 92.9% 에서 5 이내, 98.7% 에서 10 이내의축회전을보였다고보고하였다. Bascaran et al 29 은 AT TORBI 709M IOL 을삽입한군에서술후평균 4.42 ± 4.31 (0-16) 의축회전을보였고, 그중 74.4% 에서 5 이내, 86.0% 에서 10 이내의축회전을보였다고보고하였다. 본연구에서는술후 3개월째각인공수정체축회전의평균값은테크니스군의경우 3.2 ± 2.2 (0-10 ), AT TORBI 군의경우 2.4 ± 2.0 (0-8 ) 로나타났고, 두군모두에서 10 이상회전한경우는없었다. 그리고이는최근에시행된기존연구들의결과와비슷한경향을보임을알수있었다. 수술후안구고위수차분석은 VISX Customvue Wavescan (Abbott Laboratories Inc., Santa Ana, CA, USA) 을이용하여시행하였다. 본연구에서는수술후측정한총고위수차값 (RMS), coma 및 trefoil 수차에서두난시교정인공수정체간에통계학적으로유의한차이를보이지않았다. 그러나안구구면수차는 AT TORBI Toric 인공수정체삽입군에서테크니스인공수정체삽입군보다통계적으로높은수치를보였다 (p=0.047). 수술전 Pentacam으로측정한각막구면수차값은테크니스군에서 0.32 ± 0.37D, AT TORBI 군에서 0.36 ± 0.31D로통계적으로유의한차이를보이지않았다. 테크니스 Toric 인공수정체는 μm의음성구면수차를가지고있고, AT TORBI Toric 인공수정체는 0의구면수차를가지고있으므로안구구면수차가 AT TORBI 삽입군에서높은것은두인공수정체가렌즈고유의비구면특징을잘구현하고있다고해석할수있다. 결론적으로테크니스난시교정용인공수정체와자이스 AT TORBI 난시교정용인공수정체는두렌즈모두우수한난시교정효과및시력개선효과를보였다. J Cataract Refract Surg 2013;39:41-7. = 국문초록 = 두가지토릭인공수정체삽입후임상결과분석 목적 : TECNIS Toric 인공수정체 (Abbott Medical Optics Inc., Santa Ana, CA, USA) 와 AT TORBI Toric 인공수정체 (Carl Zeiss Meditec AG, Jena, Germany) 삽입술을시행한환자들의임상결과및고위수차를비교하고자하였다. 대상과방법 : 본연구는 1.25D 이상및 4.5D 이하의각막난시를동반한백내장으로난시교정용인공수정체를삽입한 26 명의 32 안을대상으로후향적으로고찰하였다. 백내장수술은동일한술자에의해 2.2 mm 이측절개로시행하였다. 수술후 3 개월까지나안및교정시력, 굴절력, 잔여난시, 인공수정체축회전및고위수차를관찰하였다. 난시는 Cartesian astigmatism (J0) 및 oblique astigmatism (J45) 을이용한 power vector 분석을이용하였다. 결과 : 수술 3 개월째최대교정시력및잔여난시는두군간에통계적으로유의한차이는없었다 (p>0.05). TECNIS Toric 삽입군에서 J0 는수술전 0.71 ± 0.84D 에서수술후 0.05 ± 0.39D 로유의하게감소하였고 (p=0.029), AT TORBI Toric 삽입군에서 J0 는수술전 0.88 ± 1.27D 에서수술후 0.02 ± 0.16D 로유의하게감소하였다 (p=0.032). 고위수차분석에서구면수차를제외하고는두군간에통계적으로유의한차이는없었으며, AT TORBI Toric 삽입군에서높은구면수차를보였다 (p=0.047). 결론 : 난시를동반한백내장환자에서두가지난시교정용인공수정체삽입술은우수한난시교정및시력교정효과를보였으며결과는 3 개월까지안정적이었다. < 대한안과학회지 2016;57(2): > 207
