Methods: The medical records of patients who underwent phacoemulsification and IOL implantation in the posterior chamber by a surgeon (H.J.C) were reviewed (68 patients, 97 eyes). IOL was selected based on preoperative corneal spherical aberration measured by corneal topography (ATLAS 9000, Carl Zeiss). The target postoperative total ocular spherical aberration was set to zero (0) and one of the following lenses was chosen: Acrysof SA60AT (n = 25), Acrysof IQ (n = 36) or Tecnis ZCB00 (n = 36). The Wavescan aberrometer and the corneal topography were obtained postoperatively. Absolute prediction errors of postoperative total ocular spherical aberration were analyzed. Results: Preoperative corneal spherical aberration was μm; total postoperative ocular spherical aberration was μm (Acrysof SA60AT: μm, Tecnis ZCB00: μm, Acrysof IQ: μm). Postoperative corneal spherical aberration was μm, which was not significantly different from the preoperative value (p = 0.199). Postoperative ocular spherical aberration was μm; μm (Acrysof SA60AT), μm (Acrysof IQ), and μm (ZCB00). The reducing amounts of spherical aberration were μm (Acrysof IQ) and μm (ZCB00). The overall absolute prediction error was μm. The absolute prediction error of the Acrysof SA60AT group was μm, Tecnis ZCB00 group was μm and Acrysof IQ group was μm. There was no significant difference among the 3 groups (p = 0.089). Conclusions: Aspheric IOLs can efficiently reduce total ocular spherical aberrations according to preoperative corneal spherical aberrations. 2 - 대한안과학회지 2014 년제 55 권제 6 호 - 시키려는노력도지속되고있으며, 7 최근에는다초점인공수정체등을통한조절력상실의극복도소기의성과를거두고있다 년대초반각막굴절교정수술에서먼저시도된파면 (Wavefront) 분석을이용한맞춤수술의시도는술후시력에큰향상을보이지는않았으나, 대비감도나빛번짐 (glare), 술후수차분석등시력의질적인개선을도모할수있는가능성이제시되었다 이어서백내장수술에도파면분석을이용한시력예후개선의개념이도입되었고, 12,14-17 특히고위수차 (High-order aberration) 와시력의질 (Quality of vision) 의관계의중요성이강조되면서술후고위수차를줄이기위한노력이지속되고있다. 18 특히고위수차가운데에서도 Zernike 다항식의 4차항 (Z4,0) 에해당하는구면수차는비교적쉽게수술전후에얻고목표수차값에맞는비구면인공수정체를선택하여교정할수있으며, 저위수차인 Defocus (Z2,0) 상쇄에의한초점심도의변화, 19,20 대비감도나빛번짐등시력의질에중요한역할을하고있다. Tecnis ZCB00 (Advanced Medical Optics (AMO), Santa Ana, CA, USA) 인공수정체는각막구면수차를극복하기위해전방비구면렌즈로구현되었으며후낭혼탁의발생을억제하기위한 360도방향에서각진모서리형태의후면을갖고있다. 각막구면수차를극복하기위해 6-mm optical zone에서 μm의구면수차를도입하고있으며, 이는먼저출시된 3-piece IOL인 Tecnis Z9000/2/3과는달리일체형소수성아크릴제제이다. Acrysof IQ (SN60WF; Alcon, Ft Worth, TEX, USA) 의경우자외선과청색광에대한필터처리가되어있는일체형아크릴인공수정체이다. Acrysof IQ 렌즈의특징은후방의비구면성으로구면수차를줄이도록설계되었다는점이며, 6-mm optical zone에서 μm의구면수차를도입한다. Salvatore et al 21 은상기두비구면인공수정체를이용한백내장수술 2개월후경과를비교하였으며, 술후고위수차, 시유발전위, 망막민감도에는차이가없었지만대비민감도의경우 Tecnis ZCB00를사용한군에서더나은결과를보였다. Assaf and Kotb 22 의경우수술전각막파면분석후비구면또는구면렌즈 (Sensar AR40e, AMO) 를선택하여사용한백내장수술에서수술후비구면렌즈군에서유의한구면수차감소와대비민감도의우위를발표하였다. 그러나이런중요성및해외발표현황에비하여, 국내에출간된한국인에서의각막구면수차를이용한맞춤백내장수술의논문 ( 특히 Tecnis ZCB00를이용한경우 ) 은많지않은상태이다. Kim et al 23 의경우비구면인공수정체로 HOYA AF-1 isert와 HOYA AF-1 imics ( 이상 HOYA, Tokyo, Japan), 구 면인공수정체로 Akreos MI60 (Bausch & Lomb, Inc., Rochester, NY, USA) 를선택하여술후 6개월에 Akreos MI60군에서유의하게구면수차가비구면인공수정체보다높음을보고하였다. Lee et al 24 의경우본연구에서사용한인공수정체인 Acrysof IQ와 Tecnis ZCB00, 그리고또다른비구면인공수정체인 envista MX60 (Bausch & Lomb, Rochester, NY) 의임상결과를비교하였고, 수술후대등한고위수차결과를보였다. 본연구에서는수술전각막구면수차분석후 Tecnis ZCB00와 Acrysof IQ의비구면인공수정체또는구면인공수정체 (Acrysof SA60AT, Alcon) 를선택하였다. 그리고이를이용한백내장수술후원거리시력및구면수차를측정하여비교, 분석하였다. 대상과방법 2011년 2월부터 2012년 10월까지서울대학교병원안과에서한명의술자 (H.J.C) 에게수정체초음파유화술및낭내후방인공수정체삽입술을시행받은환자의의무기록을후향적으로분석하였다. 각막이식술을받았거나, 동공크기에영향을줄수있는홍채질환이있는경우, 각막또는안내수술의기왕력이있는경우, 각막굴절수술을받은경우는제외하였다. 또한수술중원형전낭절개가불완전하였거나 ( 방사상열개가발생하였거나 optic zone을모두덮지못한경우포함 ), 전낭절개가너무크게되어중심이탈의우려가큰경우, 섬모체소대이상이발견된경우, 후낭파열이있었던경우, 인공수정체를낭내 (in-the-bag) 로삽입하지못한경우역시배제하였다. 수술후세극등현미경검사에서원형전낭절개또는인공수정체의중심이탈이관찰된경우도제외하였다. 수술전에초음파및 IOL Master (Carl Zeiss, Dublin, CA, USA) 를이용하여계측한술전인자를이용하여 SRK/T Formula 를통해삽입할인공수정체도수를구하였다. ATLAS 9000 각막지형도 (Carl Zeiss) 를이용하여 6 mm의 optical zone에서각막구면수차를확인하였으며, 수술전각막수차와인공수정체의구면수차를더하여계산한수술후예상구면수차가가장 0 μm에가까운인공수정체를다음중에서선택하였다 : Acrysof SA60AT, Acrysof IQ, Tecnis ZCB00. 수술전각막구면수차가 μm 미만인경우, Acrysof SA60AT, μm 이상 μm 미만인경우, Acrysof IQ, μm 이상인경우, ZCB00 인공수정체를선택하는것을원칙으로했으나, 다음의경우에는각막구면수차가 μm 이상이어도 Acrysof SA60AT를사용하였다 : 각막지형도에서부정난시가있는경우, 헤르페스 810

3 - 이진호외 : 구면수차교정을동반한백내장수술 - 각막염등각막의염증성질환을앓은병력이있는경우 ( 현성감염의경우에는배제 ), 반대안의각막구면수차가커서 Tecnis ZCB00를삽입하여양안의인공수정체의색을맞추기위한경우 ( μm보다크지않은경우에한하여 ), 모든검사과정과분석은 Declaration of Helsinki에의거하여실행되었다. 백내장수술은 Nadbath s method를이용한안면신경마비후 0.5% proparacaine hydrochloride 점안액을통한점안마취후시행하였다. 이측방향각막으로접근하여각막절개도를이용하여 2.75 mm 길이의투명윤부절개창을만들고점탄물질을전방내에채워넣은후, 26G needle 및미세집게를이용한원형전낭절개후평형생리식염수 (balanced salt solution, BSS, Alcon) 를이용하여기질수화 (stromal hydration) 를시행하였다. 초음파유화기 (Infinity, Alcon) 를이용하여초음파수정체유화술을시행한다음, 해당인공수정체를후낭에삽입하고전방에남은점탄물질을관류및흡입장치로제거하였다. 수술전모든환자들에서최대교정시력검사, 세극등현미경검사, 안저검사, 자동굴절검사및 ATLAS 9000 각막지형도검사를시행하였다. 백내장으로인한매체혼탁및빛의산란으로인한측정오류및오차를감안하여수술전에는안구구면수차는측정하지않고, 각막지형도를이용하여각막구면수차만측정하였다. 수술 3개월후최대교정시력, 박명시 (mesopic condition) 조건하에서 Wavescan 수차계 (Wavescan Wavefront system, VISX, Santa Clara, CA, USA) 및 ATLAS 9000 각막지형도를얻어안구및각막구면수차를다시분석하였다. 수술전각막구면수차에인공수정체의구면수차를더하여계산한수술후예상안구구면수차와수술후실측된안구구면수차를비교하기위해예상구면수차와실측구면수차의차이의절대값을절대오차로정의하고, 각군별로절대오차를계산, 이를비교하였다. 각군에서수술전후의구면수차는대응표본 t-검정을이용하여비교하였으며, 세군간의절대오차는 One way ANOVA 검정을이용하여분석후, Bonferroni법을통하여사후분석을시행하였다. 통계분석은 SPSS 18.0 (SPSS Inc., Chicago, IL, USA) 를이용하였다. p<0.05인경우통계적으로유의하다고판단하였다. 결과 전체 68명 97안중에서 Acrysof SA60AT군은 18명 25안, Tecnis ZCB00군은 24명 36안, Acrysof IQ군은 26명 36안이었다. 세군의수술시행시연령 (p=0.083) 및성별비율 (p=0.272) 은유의한차이가없었다. 기본적인인구학적정보및수술전후주요수치는 Table 1에요약하였다. 본연구에서양안수술을받은 32명중에서 29명은양안에동일한인공수정체를삽입하였다. 각군의수술전최대교정시력은 Acrysof SA60AT 군 0.67 ± 0.59, Acrysof IQ 0.60 ± 0.34, Tecnis ZCB ± 0.54 (logmar) 였으며 (p=0.777), 수술후최대교정시력은 Acrysof SA60AT 군 0.11 ± 0.18, Acrysof IQ 0.12 ± 0.16, Tecnis ZCB ± 0.48 (logmar) 로세군간에유의한차이는없었다 (p=0.080). 수술전전체집단의 6-mm optical zone 각막구면수차는 ± μm였다. 인공수정체군별로세부분석시각각 ± μm (Acrysof SA60AT), ± μm (Acrysof IQ), ± μm (Tecnis ZCB00) 이었으며인공수정체의구면수차교정량을고려한수술후예상안구구면수차는 μm (Acrysof SA60AT), μ m (Acrysof IQ), μm (Tecnis ZCB00) 이었다 (Fig. 1). 수술후전체집단의안구구면수차는 ± μm 로측정되었다. Acrysof SA60AT군에서는 ± μm, Acrysof IQ 군에서는 ± μm, Tecnis ZCB00군에서는 ± μm로측정되었다. 전체군의수술전각막구면수차및수술후안구구면수차의분포를 Fig. 2에요약하였다. 수술후전체집단의각막구면수차는 ± μm였다. (B) Distribution of postoperative data. 에서는 ± μm로 측정되었다. 전체 환자군에서는 수 전 각막 구면수차와 수술 후 안구 구면수차를 비교하였을 술 전후 각막 구면수차에서 유의한 변화는 없었다(p=0.199). 때 각각 ± μm (Acrysof IQ), ± μm 인공수정체 군별로 비교 시에도 수술 전후에 유의한 변화는 (Tecnis ZCB00)의 구면수차를 상쇄하는 효과를 보였다(두 나타나지 않았다(p=0.424, 0.694, 0.190: Acrysof SA60AT, 군 모두 p<0.001). Acrysof IQ 인공수정체를 사용한 환자의 Acrysof IQ, Tecnis ZCB00 순). Acrysof SA60AT를 제외한 두 비구면 인공수정체는 수술 812 수술 전후 각막지형도 및 수술 후 Wavescan 수차계 측정결 과를 Fig. 1에 제시하였다.

Note the central gray area that the absolute errors are less than 0.1 μm. 수술후전체집단의절대오차는 ± μm이었고, Acrysof SA60AT 군의절대오차는 ± μm, Tecnis ZCB00군은 ± μm, Acrysof IQ군은 ± μm였다. 세군간의절대오차는유의한차이가없었다 (p=0.089, one way ANOVA). 수술후예상안구구면수차와실측안구구면수차를 Fig. 3에산점도로표기하였다. 전체환자중 53.6% 에서는절대오차가 μm 이내였으며, 75.3% 에서는 μm 이내로확인되었다. 고 각막및수정체광학에대한연구가진행되면서, 파면분석을이용한고위수차의분석및이에대한수술적교정이시도되었다. 이러한시도는먼저각막굴절교정수술에서시도되었고, 이후백내장수술에도점차확대되었다. 고위수차중에서도구면수차를비구면인공수정체를이용한수술로감소시킨후에시력의질을개선하는시도들이보고되었다. 이미 Guirao et al 25 은각막구면수차가노화와함께증가함을밝혔으며, Uchio et al 26 이구면수차에따른인공수정체선택이빛번짐등시력의질에영향을줄수있는가능성을제시하였다. Yamaguchi et al 27 은수술후구면수차가대비민감도와유의하게음의상관관계를가짐을발표하였다. Beiko 28 는 Tecnis 인공수정체를이용한수술시 μm로구면수차를남기는경우가목표값을 0 μm로설정할때보다수술후대비감도가더우월하다고보고하였다. 그러나 Wang and Koch 29 는구면수차의수술후목표를 0 μm로설정하고수술시시력의질이개선됨을보고하였다. 또한보편적으로많은연구에서수술후목표구 찰 면수차를 0으로설정하고있으므로, 본연구에서역시수술후목표값을 0 μm로설정후적절한인공수정체를채택하기로결정하였다. 연구방법에서기술한대로수술전각막구면수차에따라인공수정체를선택하는것을원칙으로하였다. 그러나각막지형도에서부정난시가있는경우, 헤르페스각막염등각막의염증성질환을앓은병력이있는경우에는비구면인공수정체의효과가좋지않을것이예상되므로, 구면인공수정체를삽입하였을때각막수차의변화가없다는것을확인하기위하여각막구면수차에상관없이구면인공수정체를삽입하였다. 또한, 구면인공수정체를선택해반대안의각막구면수차가커서 Tecnis ZCB00를삽입하여 Acrysof IQ를사용하면양안인공수정체색이달라지는경우등에서는각막구면수차가 μm 이상인경우에도 Acrysof SA60AT 를사용하였기때문에수술전 Acrysof SA60AT 군의평균각막구면수차가 μm로 μm 보다크게측정되었다. 술후최대교정시력은수술전에비하여세인공수정체사용군에서모두향상되었다. 비구면인공수정체사용시 Acrysof IQ 삽입군에서 ± μm, Tecnis ZCB00 삽입군에서 ± μm의수술전각막구면수차보정효과를보였다. 또한 91.8% 의수술안에서수술후전체안구구면수차가 0.15 μm 이내로측정되었다. Acrysof IQ의경우기존에해외에보고된수술후임상결과가많은편이나, Tecnis ZCB00 를사용한연구는많지않은편이다. 이에기존연구중 Tecnis ZCB00는 Z9000/2/3 등 3-piece IOL을사용한경우와비교하였다. Tecnis 3-piece IOL의경우도인공수정체의구면수차는 μm로 Tecnis ZCB00와같다. 그러나기존연구에서비구면인공수정체의수차교정량은명확히제시되지않고, 수술전후의각막수차및수술후전체안구구면수차만을보고한경우가대부분이어서수술후안구구면수차를비교하였다. 먼저 Awwad et al 30 은 Acrysof IQ삽입군에서수술후안구구면수차가 0.09 ± 0.04 μm로구면인공수정체인 Acrysof SN60AT (Alcon Inc.) 에비해유의하게낮음을보고하였다. 수술후안구구면수차가본연구의 Acrysof IQ삽입군에서더낮게측정된것은본연구에서수술전에각막구면수차가 μm보다큰군은주로 Tecnis ZCB00삽입군으로선택되었고, 그보다작은군에서만 Acrysof IQ삽입군에포함되었기때문으로생각한다. Packer et al 4 은본연구와비슷하게수술전각막구면수차가 μm 미만인경우구면렌즈인 SofPort Advanced Optics (AO) (LI61AO; Bausch & Lomb, San Dimas, CA, USA), μm 이상 μm 미만인경우 Acrysof IQ, μm 이상인경 813

6 - 대한안과학회지 2014 년제 55 권제 6 호 - 우 Tecnis Z9003 인공수정체를선택하였다. 수술후안구구면수차를전체군은 ± μm, Acrysof IQ삽입군 ± μm, Tecnis Z9000 삽입군 ± μm로보고하였다. Caporossi et al 31 은 5-mm pupil에서측정한수술후구면수차를 Tecnis Z9000군 0.05 ± μm, Acrysof IQ군 ± μm로보고하였다. 개개인의차이 ( 특히기존서양연구와인종간차이 ), 검사조건및장비, 수술법의다양성때문에직접적인비교는어려우나, 기존연구와구면수차감소에대한동향은본연구에서도비슷하게확인되었다. Kasper et al 32 은 Tecnis ZCB00군에서의수술후구면수차를 μm로보고하였으나, 표준편차는제시하지않았다. Denoyer et al 15 의경우 Tecnis ZCB00군에서수술후구면수차를 ± 0.06 μm로보고하였다. 기존에여러가지연구들에대한메타분석은아직까지는없는실정이다. 본연구의목적인맞춤수술의결과평가의일환으로절대오차를계산하였다. 수술후전체집단의절대오차는 ± μm이었다. Packer et al 4 은절대오차를전체 ± μm, Acrysof IQ군 ± μm, Tecnis ZCB00군 ± μm로보고하였다. 본연구와마찬가지로 Acrysof IQ군과 Tecnis ZCB00군의절대오차는유의한차이가없었다. 예측오차를계산할때다음과같은점을염두에두어야한다 : 수술후동공의크기, 인공수정체의기울어짐또는이탈, 수술로유도된각막구면수차. 기본적으로수술후안구구면수차의예측은수술전 6-mm optical zone의각막구면수차 Z (4,0) 에각인공수정체의알려진 Z (4,0) 를더하여단순계산하였다. 그러나, 수술후실제빛이통과하는틈새는수술시시행한원형전낭절개크기의영향이크고이는미리예측할수없이개인차가큰부분이므로오차에큰영향을끼치지만, 실제로어느정도의오차가이때문에발생하는지현실적으로파악하기어렵다. 동공크기의경우는본연구에서박명시조건에서 Wavescan 수차계를촬영했으므로실제로구면수차가환자의시력의질에큰영향을주는시간대의동공상태에서의안구구면수차와그에따른교정량계산이잘반영되었다고해석할수있다. 본연구에서는수술시인공수정체이탈을유발할만한원형전낭절개의문제가있거나실제로수술후세극등현미경검사에서뚜렷한중심이탈이발견된경우는제외하였다. 그러나 Scheimpflug photography와같은정밀한측정장비를사용하지못한점은본연구의단점으로지적할수있다. 다만 Tecnis ZCB00 33 나 Acrysof IQ 34 의중심이탈및기울어짐에관련된연구에서이인공수정체들은 uncomplicated cataract surgery 시구면인공수정체와비교하여유의하게문제되는중심이탈을보이지않았고, 본연구의환자군도 centration이나 tilt에문제가발생한경우는제외된 uncomplicated surgery로이루어졌으므로기존에보고된중심이탈의정도가유의하게크지않을것으로예상된다. 또한 Piers et al 35 은기울어짐또는이탈에의한비구면렌즈의기능저하를고려할때, 0.8 mm 이탈이나 10 기울임, 15 회전까지의범위는예민하게느끼기어렵다고보고하였다. 이에본연구에서인공수정체의기울임이나이탈이수술후구면수차에중대한영향을미치지는않을것으로판단된다. 수술로인한각막구면수차의변화역시어느정도일어날것으로예상되며, 이는예측오차에어느정도영향을주는지알아보기위해수술후각막구면수차를함께측정하였다. Guirao et al 36 은이미평균적으로 3.5 mm 투명각막절개와일초점인공수정체삽입술을시행했을경우수술로유도되는 6-mm optical zone 각막구면수차 (surgically induced spherical aberration) 를 0.03 ± 0.17 μm로보고하였다. 본연구에서는수술후전체각막구면수차는 ± μm 로, 수술전각막구면수차와유의한차이는없었고 (p=0.199), 각인공수정체군마다따로비교한경우에도모두유의한변화가없었다. 각환자에서수술전후각막구면수차의차이를계산해보면 0.01 ± 0.07 μm로 Guirao et al 36 이보고한값보다적었다. 이는 3.5 mm가아닌 2.75 mm 절개를시행하였기때문에각막주는영향이적었다고생각할수있다. 본연구의제한점으로는일괄적으로같은수술전후의각막지형도검사와, 수술후 Wavescan 파면분석을시행하였으므로개인별로다른동공의크기에대한고려가불가능했다는점, 수술 3개월후경과관찰을하였으므로충분히오랜시간후의평가가되지않았다는점, 실제로환자가주관적으로느끼는시력의질을평가하기위한설문조사나대비민감도, 빛번짐검사등을시행하지못한점을들수있다. 다만, 기존에구면수차를포함한고위수차교정시에시력의질향상에대한기존보고를참고할때, 26,27,36 본연구의환자들도시력의질에도움을얻었음을간접적으로추론할수있다. 추가적인제한점으로, 환자의 angle of Kappa가큰경우에는인공수정체와원형전낭절개의중심에대한오차가클수있으나, 실제로연구설계시이에대한점이고려되지못한점도꼽을수있다. 결론적으로수술전각막구면수차를고려하여구면또는비구면인공수정체를개인마다맞추어선택적으로이용한백내장수술은효과적으로안구구면수차를감소시킬수있으며, 이는백내장치료에서시력의질향상에이바지할수있을것으로생각한다. 그러나백내장수술에서사용 814

Invest Ophthalmol Vis Sci 2004; 45: = 국문초록 = 각막구면수차의교정을동반한백내장수술의임상결과분석 목적 : 수술전각막지형도로알아낸구면수차를고려하여비구면혹은구면인공수정체를선택하여백내장수술을시행하였을때수술전후구면수차의변화를분석하고자한다. 대상과방법 : 2011 년 2 월부터 2012 년 10 월까지한명의술자에게수정체초음파유화술및낭내후방인공수정체삽입술을시행받은 68 명 97 안을분석하였다. 수술전 ATLAS 9000 각막지형도를이용하여각막구면수차를분석하였으며수술후목표구면수차를 0 μm 로설정하고다음중적절한인공수정체를선택하였다 : Acrysof SA60AT (n=25), Acrysof IQ (n=36), Tecnis R ZCB00 (n=36). 수술 3 개월후 WaveScan 수차계및 ATLAS 9000 각막지형도를이용해안구및각막구면수차를다시측정하였고수술전에예측한수술후안구구면수차와실측된안구구면수차의절대오차를계산하였다. 결과 : 수술전전체평균각막구면수차는 μm 이었고수술후전체평균각막구면수차는 μm 였다 (p=0.199). 수술후안구구면수차는 μm 였다. 각그룹의술후안구구면수차는각각 μm, μm, μm 로사용된두비구면인공수정체는각각 μm (Acrysof IQ), μm (Tecnis R ZCB00) 의구면수차를상쇄하는효과를보였다. 세군간의절대오차는유의한차이가없었다 (p=0.089). 결론 : 수술전각막구면수차를고려한인공수정체선택으로구면수차를효과적으로감소시킬수있다. < 대한안과학회지 2014;55(6): > 816
