1 반측와위로시행한경피적신절석술 Percutaneous Nephrolithotomy in a Semi-Lateral Position Woo Jin Jeong, Hwang Gyun Jeon, Seung Chul Yang, Woong Kyu Han From the Department of Urology, Urological Science Institute, Yonsei University College of Medicine, Seoul, Korea Purpose: In the treatment of large-sized renal stones, percutaneous nephrolithotomy (PNL) is regarded as a minimally invasive surgery with an easy postoperative recovery. In previous procedures, which were done with the patient in a prone position, appropriate measures could not be taken in cases in which a cardiopulmonary emergency appeared. Given this background, we performed PNL in a manner to which the department of urology was accustomed. Then, we attempted to assess the treatment effect and safety of PNL on the basis of our clinical experiences. Materials and Methods: This study was conducted in 14 patients who underwent PNL in a semi-lateral position at our medical institution between April and October of The complete removal of renal stones, blood transfusion, the period of use of a catheter for nephrostomy, complications, and the need for additional procedures after the surgery were examined. Results: Six patients had stones in the pelvis, three had stones in the lower calyx, and five had multiple stones or pelvocalyceal stones. The mean stone size was 2.66 cm (range, cm). In all patients, the renal stone was completely removed and post-procedure complications were minimal except for blood transfusion in one patient. No additional procedures were required in any of the patients. Conclusions: PNL was performed in a semi-lateral position, which is a position to which the department of urology is accustomed. By use of this position, surgeons and anesthesiologists can reduce the surgical burden due to posture. Based on our clinical experiences, PNL in a semi-lateral position is an effective, safe modality. 대상 및 방법 신루 조성술 시행 시 환측 침대를 올려서 50도 이상의 각도 2008년에 4월부터 10월까지 신결석으로 본원에서 반측와 를 만들어 최대한 앙와위와 같이 내부 장기 및 신장의 움직 의 PNL을 시행한 환자 14명을 대상으로 하였다. 대상 환자 임이 최대한 적어지도록 한 후 초음파와 형광 투시경을 이 용하여 신루 조성술을 시행하였다 (Fig.

3 894 Korean Journal of Urology vol. 50, , September ). 신루가정확히확인이되면다시정상적인반측와위로바꾸고고식적인경피적신절석술의방법으로절석술을시행하였다. 신루조성시신장까지접근의용이성을확인하기위해조성된신루의수를조사하였으며치료효과및안정성을확인하기위해결석의완전제거여부, 수혈여부, 신루카테터유치기간, 수술시간, 합병증유무, 술후추가적술기의필요성유무에대하여조사하였다. Treatment results Parameters 결 14명의모든환자에서신루조성술시추가적인신루조성없이하나의신루를통해수술을진행되었다. 술후단순촬영에서잔석이관찰된환자는없었으며모든환자에서술후 extracorporeal shockwave lithotripsy (ESWL) 또는 ureterorenoscopy (URS) 등의추가적인술기가필요하지않았다. 평균수술시간은 103.2분 (52-210) 으로 210분이걸린 6.37 cm 신녹각석의경우를제외하면평균 95분이었다. 평균신루카테터유치기간은평균 3.85일이었으며가장짧은기간은 2일, 가장오랜기간유지한경우는 5일이었다. 수술중합병증으로전체신우및신배의일부를침범하였던 6.37 cm의신녹각석환자에서출혈이있어수혈을시행하였다. 이외의신우및신배파열, 주변장기손상등의추가적인합병증은없었다. 수술후경과관찰기간에도출혈, 감염그리고장마비등의합병증은보고되지않았다 (Table 2). Result Catheter indwelling period (day) 3.85 (2-5) PNL tract 1 session 14 (100%) Over 2 sessions 0 Operation time (min) (55-210) Transfusion Yes 1 (7%) No 13 (93%) Re-treatment Yes 0 No 14 (100%) Outcome Success 14 (100%) Residual stones 0 Complication Bleeding 1 (7%) Infection 0 Renal or adjacent organ injury 0 PNL: percutaneous nephrolithotomy 과 고찰지난 20년간 PNL과 ESWL 그리고 URS는상부요로결석에대한주요치료법으로사용되었다. 특히 Fernström과 Johansson이 1976년 nephrostomy tract을이용하여신결석을제거한이후 [13], PNL은개복수술을대신하여큰신결석을제거하는가장효과적인치료법으로발전되어왔다. 전통적으로 PNL은복와위를통해이루어져왔다. 복와위에서는신장이주변구조에의해지지되어있기에결석까지안정적으로신루를형성할수있고신우및신배까지혈관이없는 Brodel s plane을통해후방으로쉽게신루를형성할수있다는이점이있다 [14,15]. 외국에서 Segura 등이시행한조사에의하면 PNL을통해신결석에서 98.3% 그리고요관결석에서 88.2% 의치료효과를가졌다고보고하였다 [16]. 국내에서도 Park 등이 1987년에대한비뇨기과학회에발표한자료를보면 PNL을통해신우결석 96.5%, 신배결석 84.4%, 신우요관접합부및상부요관결석 76.8%, 신녹각석 100% 의치료효과를보고하여 [17], PNL이신결석을제거하는데유용한술기라는것을확인할수있었다. 그러나전통적인방법인복와위에서의 PNL은여러가지한계점을지니고있다. 심폐기능에문제가있는환자나비만환자에서복와위는수술과정에서심폐기능에문제가생겼을때적절한대처를하는데큰어려움을유발하며복와위자체가환자에심폐기능에무리를줄수있다 [18,19]. 또한확률은매우낮지만 PNL 시행도중주변장기에손상을입힐수있다. 특히대장손상이일어날수있는데복와위에서는앙와위보다대장이신장앞쪽으로위치할확률이높아투관침이신장을관통하여손상을줄가능성이있다 [20,21]. 이와같은복와의 PNL의한계점을극복하기위하여외국에서는자세에대해여러연구를진행하였다. 앙와위또는측와위를이용하여 PNL을시행하려는노력이있었다. 이를통하여수술중자세변경에의해유발되는문제점을줄이고수술을좀더용이하게진행할수있었다. 또한마취시에도자세에대한부담이적어좀더쉽게마취를진행할수있었으며심폐기능에문제가있거나비만환자에서도 PNL 술기를진행하는것이가능하게되었다. 대장의위치에따른손상가능성도낮출수있으므로합병증이발생할확률도낮출수있다 [20,21] 이러한점을참고하여본원에서는신장수술을시행할때가장흔히적용되는반측와위에서 PNL 시행이가능한지시도하였다. 그결과연구에포함된 14명모든환자에서신결석이성공적으로제거되었다. 반측와위에서 PNL을시행

4 Woo Jin Jeong, et al:percutaneous Nephrolithotomy in a Semi-Lateral Position 895 할때가질수있는가정큰장점은술자및마취과의사모두흔히접하는자세이기에심적으로편한상태에서수술할수있는점을들수있다. 본원에서반측와위에서 PNL을시행하기전신루를정확하고안전하게형성할수있는지가가장우려하였던문제였다. 따라서이러한점을해결하기위해반측와위에서다시수술부위측침대를올려각도를 50 o 이상유지함으로써콩팥및주변장기의움직임을최소화하였으며신우및신배로의접근을용이하게할수있었다. 복와위가아닌자세에서는신루형성시 Amplatz sheath를이용하여신루를확장하는과정에서신장이고정되지않고움직이게되어문제를유발할수있다. 따라서본원에서는단일과정으로결석까지천자바늘을관통하여유도철사를삽입후바로풍선확장기를통해신루를확장하는방법을통해신루확장과정에서생길수있는문제점을해결하였다. 마지막으로 PNL을시행하기전모든환자에서반드시 computerized tomography (CT) 를시행하였으며축면및관상면영상을모두확인하여가장적절한위치에서신루를형성할수있도록하였다. 반측와위 PNL에서우려하였던신루조성의어려움은없었으며 14명모든환자에서정확하게신루를형성할수있었다. 신루조성후반측와위로환자를고정한상태에서결석은모두제거되었으며결석의크기가 6.37 cm로측정되었던한명의환자에서만수혈이필요하였다. 이외의수술진행기간및입원기간동안추가적인합병증은발생하지않았다. Yoo와 Lee가발표한논문에서보고한초기경험 14%, 후기경험 9.5% 의주요합병증발생비율과비교하여볼때반측와위 PNL의주요합병증은 7% 로나은결과를보였다 [22]. 평균수술시간은 103.2분으로복와위에서보고된 112분보다짧은것이확인되었으며복와위초기경험에서평균 127분, 후기경험에서평균 97분소요된것을고려할때반측와의자세에서의수술시간도점차단축될것으로생각한다. 이러한결과를볼때반측와의 PNL은충분한치료효과및안정성을지니고있는것으로판단된다. 하지만본원에서진행한연구에서는몇가지제한점이존재한다. 우선단일술자가진행하여반측와위전복와위로수술을진행한환자군의자료부족으로직접적인비교를하지못하고다른술자가진행한환자군과의간접적인비교를하여정확하게자세변경에따른시간단축및치료효과판정이이루어질수없었다. 또한연구환자군의숫자가적어합병증및신결석위치에따른치료효과에대해추가적인자료가필요할것으로보인다. 측와위및반측와위가지닐수있는문제점으로가능성은낮지만아래쪽으로위치하는측순환기및호흡기관은복와위와마찬가지로문제를유발할수있으나이에대하여충분한연구가 이루어지지못한것도한계점이다. 마지막으로연구대상환자에서심각한심폐기능저하자및고도비만환자는없어이러한환자군에서반측와위 PNL이복와위에서수술을진행할때보다확실한이점을지니는지에대해확인할수없는한계점이있다. 따라서이러한한계점에대해추가적인연구가필요할것으로생각한다. 결 시행전반측와위에서수술의어려움은신루조성술시신루에대한안정성여부에있다고판단되었으나실제로시행한결과수술에적절한후측방신루를형성하는데어려운점은없었다. 수술과정중자세변경이좀더용이하여수술을시행하기때문에수술시간의단축이가능했으며환자회복에도유리하였다. 본원의경험을바탕으로반측와위경피적신절석술은수술의능률에있어보다효과적이며안정적으로시행이가능한술기로판단되었다. 론 REFERENCES 1.
