36 Session 1 _ Face Lift I March 29 Room A Anatomical study of SMAS and it s clinical significance SMAS 의해부학적연구와그임상의중요성 SMAS 的解剖学研究和其临床的重要性 Lin Cui Department of Anatomy, Yanbian Medical University, China Objective : To investigate the anatomical characteristics of the superficial musculoaponeurotic system (SMAS) and to provide clinical guideness when performing extensive SMAS dissection during the rhytidectomy, and.to protect the facial nerve from injury. Materials and Methods : An anatomical study was performed on 30 temporoparietal regions from 10 fixed adult cadavers and five fresh cadavers. A step-by-step dissection from the superficial layer to the deep layer was involved; all the measurement data were analyzed, and the mean and standard deviation were calculated and expressed in centimeters. Results : A step-by-step dissection of the face from the superficial to deep layer, listing the Skin, subcutaneous fat, superficial fascia and deep fascia. SMAS locates in the superficial fascia layer, investing the zygomaticus muscles in the zygomatic area. From the upward and downward, SMAS continues with the frontalis muscle and the platysma. The five major branches of facial nerves in parotid masseter fascia run in the deep surface of SMAS. Conclusion : SMAS locates within the superficial fascia of facial and neck, it is a fascia like structure continuing with or investing the superficial musculus risorius in the face. Dissection beneath SMAS should be with cautious to protect the facial nerves. 34

Beautiful face is a work of art, facial beautification could be regarded as a complex systematic engineering. Plastic surgeons act as not only operators, but also engineering designers in this project. Doctors should make integral plan before carrying out any operation, facial analysis and beautifying plan should follow the 4D principle- 3 dimensional space + time. Human face contour is a three-dimensional structure, involves not only length-width proportional relationship, but also depth (protrusion) consideration. We believe that there presence five basic contour lines on the face which is divided into two regions and four facial planes, they can reflect the basic characteristics of face profile. 38 Session 1 _ Face Lift I March 29 Room A Simplified purse-strings suture combined with autologous fat filling rhytidectomy 주름살제거를위한자가지방을결합한간단한 purse-strings suture 简易荷包缝合联合自体脂肪填充除皱 Jin Guangshan, Piao Yongnan,Hong Shengri, Jin Jinji Yanji city Jin Guangshan Plastic Surgery Hospital, China Objective : To observe the clinical curative effect of simple purse string suture combined with autologous fat filling method to treat nasolabial groove lines and glabellas lines and neck wrinkle rhytidectomy. Methods : Stripping inner and outer or upper and lower side of nasolabial groove lines and glabellas lines and neck wrinkle subcutaneous and then injection of autologous fat particles filling, performing the purse-strings suture. Result : For 127 cases in this group, including the nasolabial groove lines 75cases, the glabellas lines 26cases, the neck wrinkle 17cases, the nasolabial folds 6 cases, the other 3 cases. Followed up for 6 months to 2 years, the significant curative effect cases 109 (86%), the effective cases 18 (14%) and No complications cases occurred. Conclusion: Simplified purse string suture combined with autologous fat filling method is easy to operate, have the curative effect and anti-wrinkle function as well as an application range is wide and a safe and effective rhytidectomy. 39 Session 1 _ Face Lift I Management of malar fat pad and buccal fat pad during cervicofacial rhytidectomy 경부안면의주름살제거시눈밑뺨지방패드와볼살지방패드의처치 颈面部的除皱时处理眼下面颊脂肪垫和面颊脂肪垫 March 29 Room A Wang Zhijun, Wang Na, Shi Heng, Xiao Mingming BRAVOU Aesthetic Plastic Hospital, Chengdu, China Second, after suspended tightening of the SMAS-platysma flap, suspend the supra-lateral part of the malar fat pad in a vertical direction whose location is 1~2 cm infra-lateral to the lateral canthus and fix it on the deep temporal fascia. Third, the fat injection procedure was performed began at infra-orbital rim and proceeded downward sequentially in deep plane. About 1000 patients acquired this operation, all of them were satisfied with the short-term outcome, and the long-term result is under evaluating. The malar fat pad and buccal fat pad play significant role in facial soft tissue aging, the aging changes occurred on them are in substantial agreement which composed of sagging and volume depletion. The stigmata of aging caused by malar fat pad were that deepen of the nasolabial fold, facial contour turned disgraceful due to the descend of malar prominence, arisen and aggrevated of the midcheek groove. These appearance are the evident sign of aging and it is difficult to treat by surgery. The aging changes that buccal fat pad lead were the formation of jowls which located infra-lateral to commissure. The size and location of jowls will be changed according to patient position. Another aging feature related to buccal fat pad is the midcheek hollow located at premasseter area. During the past years, aging of buccal fat pad did not draw great attention, the simple intra-oral excision method was just an assistant way to facial contour alteration. I began to routinely correct the sagging of malar fat pad and buccal fat pad during cervicofacial rhytidectomy since Additionally, I did fat injection procedure for volume lifting in midface. Here I described the main procedure as follow. First, undermine the sub-smas layer until anterior border of the massteric muscle and expose the buccal fat pad which will be fixed on the hollow area below zygomatic arch or accessory parotid gland using 3-0 polyester thread. 37

40 Session 2 _ Rhinoplasty March 29 Room A Corrective methods for the dissatisfied appearance of nasal tip after augmentation rhinoplasty 융비술후불만족스런코끝교정 矫正隆鼻手术后不满意的鼻尖 Jianming Wu SHANGHAI THE EAST BEAUTY PLASTIC SURGERY HOSPITAL, China Objective : To study the causes of dissatisfied nasal tip and operative methods after augmentation rhinoplasty Methods : The dissatisfied appearances of nasal tip based on clinical manifestations included anterior extruded tip obtuse tip droopy tip nostril valgus and dissymmetry The relevant correction methods for different variety such as changing and/or reshaping prosthesis resculpturing cartilage support cartilage graft and transferring skin flap were offered and the results of the treatment were evaluated in 116 cases Results : Follow-up varied from 1 month to 5 years and all cases obtained satisfactory effects except 3 cases experiencing relative long time of nasal ecchymoses Conclusion : on the basis of the skilled technique of sculpting prosthesis and the proper operative methods in nasal tip plasty the results continue to be consistent and pleasing 38

41 Session 2 _ Rhinoplasty Application to cosmetic surgery of upper lip frenulum resection 상순소대절제에의한비주및비공성형 利用切开舌小带的鼻椎及鼻孔整形 March 29 Room A Nakamura Tomomi Meikai University School of Dentistry form function growth Laboratory oral pediatrics Pediatric Dentistry specialist Upper lip frenulum is found attached near the alveolar crest immediately after birth, and it moves upward as the high alveolar diameter increases. Morphological abnormalities of the upper lip frenulum often result in hypertrophy, spreading out in a fanlike form at the outermost edge of lips. In case of hypertrophic upper lip frenulum adhering to the high alveolar portion, it suppresses the movement of the lips and leads to physiological dysfunction. In another case, if it makes the transition to the incisive papilla on the palate side, malposition of the teeth and diastema are caused and surgical operation becomes necessary. Upper lip frenulum resection and translocation are dental surgeries conventionally performed to improve the closing lip and teeth alignment. In recent years, CO2 laser became more accessible, and it has helped reduce the duration and downtime of surgery. While there have been many reports of impacts on lip closure by upper lip frenulum resection, various operative methods of laser application, and of field survey, this paper addresses aesthetic improvements of the lips, form and height of the nose. [Case 1] Patient : 9-year-old male Chief complaint : Tooth is not growing. Family history, medical history : N/A History of present illness : Maxillary left central incisor did not grow. The patient was under observation in a local hospital for one year. There was a bulging of the department gum but no indication to erupt, so he was referred to our hospital. Present condition : Eruption of the maxillary right central incisor and gingival swelling were observed in the left maxillary central incisor. X-ray inspection indicated opacity image of a tooth just below bulge, and root formation of about one-half was observed. The formation is about the same as that of left central incisor teeth. Treatment and assessment : Upper lip frenulum resection and fenestration were performed with infiltration anesthesia. No recurrence after surgery. 39
