However, the role of nestin in retinal vascular development is not well established. Thus, we investigated the expression of nestin in developmental mouse retina and identified which retinal cells are related to the expression of nestin during the retinal vascular development. Methods: Eyes were enucleated from C57BL/6 mice on postnatal day (P) 4, P8, P12, P16 and P26. Immunofluorescence was used to evaluate nestin expression in relation to endothelial cells (isolectin B4), pericytes (neural/glial antigen 2) and astrocytes (glial fibrillary acidic protein). Results: Nestin was strongly expressed from the ganglion cell layer to retinoblast layer at P4. At P8, P12 and P16, the expression of nestin was observed from the upper border of the ganglion cell layer, and vertically penetrating to outer nuclear layer. At P26, the expression of nestin was decreased and confined to the ganglion cell layer and inner nuclear layer. Interestingly, there was strong vascular shape expression of nestin at all stages. The superficial, deep and intermediate vascular plexus was completely merged with nestin expression at P4, P8, P12 and P16. In addition, the nestin expression merged with pericytes but not with astrocytes. Conclusions: Nestin was expressed in endothelial cells and pericytes during retinal vascular development in the retina. These results suggest that nestin could play an important role in developmental angiogenesis via interplay with endothelial cells and pericytes. 2 - 대한안과학회지 2016 년제 57 권제 3 호 - 혈관신생 (angiogenesis) 은기존의혈관으로부터새로운혈관이만들어지는과정으로, 정상적인기관의발달뿐만아니라상처치유과정이나다양한신생혈관성질환, 악성종양의신생혈관형성에도관여한다. 1 안구내신생혈관으로인해발생하는나이관련황반변성, 당뇨망막병증, 그리고미숙아망막병증은가장중요한실명의원인질환으로꼽히고있으며, 특히선진국에서나이관련황반변성과당뇨망막병증은각각실명한노인환자의 20.9%, 2.7% 에서원인이되는질환으로알려져있다. 2 우리나라에서시행된역학조사에따르면 40세이상인구에서초기및후기나이관련황반변성은각각 5.1% 및 0.3%, 당뇨를가진 40세이상인구에서당뇨망막병증은 13.4% 의유병률을가지는것으로조사된바있으며, 최근들어유병률이더증가하고있는만큼임상적인중요성도함께커지고있다고할수있다. 3-5 따라서정상적인발달과정에서의혈관신생을이해하는것은신생혈관성질환의발생과정을이해하고궁극적으로는적절한치료법을찾아이로인한실명의위험을낮추는데매우중요하다고할수있겠다. 망막혈관의발생과정은크게 1차혈관형성 (primary vasculogenesis) 과 2차혈관신생으로나뉜다. 혈관형성은혈구모세포 (angioblast) 로부터혈관이새로생성되는과정을뜻하며, 얕은혈관얼기 (superficial vascular plexus) 는 1차혈관형성을통해시신경유두에서부터망막의주변부를향해방사형으로자라나가는혈관들로만들어진다고알려져있다. 혈관신생은이미존재하는혈관으로부터새로운혈관이생겨나는과정을뜻하며, 중간및깊은혈관얼기 (intermediate and deep vascular plexus) 는얕은혈관얼기에서혈관신생을통해수직으로자라들어가는혈관들로부터생겨나는것으로이해된다. 6 이러한망막혈관의발생과정은포유류의망막혈관발생과정에서비교적유사한형태로일어나고있으며, 적어도생쥐에서는시간적, 그리고공간적으로일정한과정을취하고있다. 7,8 이러한사실은망막혈관의발생과정에서특정한신생혈관유도기전이작용하고있다는것을강하게암시하며, 혈관내피세포 (vascular endothelial cell), 혈관주위세포 (pericyte), 성상교세포 (astrocyte) 등의세포가긴밀하게관련되어있는것으로생각되고있다. 9 제6형중간섬유로분류되는네스틴 (nestin) 은중추신경계에서신경계줄기세포의표지자로알려져있으며, 10,11 신경계외에다른조직의줄기세포에서도발현된다 최근에는네스틴이혈관신생과정의표지자로서주목받고있는데, 췌장, 황체, 태반등다양한기관뿐만아니라다양한종류의암의혈관신생과정에서혈관내피세포로부터네스틴이발현되는것으로알려져있다. 15,16 이러한사실은네스틴이혈관신생과정에서중요한역할을담당하고있음을암시하 고있지만, 망막의혈관신생과정에서의네스틴의역할에대해서는명확히알려진바가없다. 본연구에서는망막혈관발생과정에서의네스틴의발현에대해알아보았으며, 특히어떤종류의망막세포에서네스틴을발현하고있는지를 isolectin B4 (IB4), neural/glial antigen 2 (NG2), glial fibrillary acidic protein (GFAP) 에대한면역형광염색을통해분석하였다. 대상과방법 대상외견상외안부질환이없는 C57BL/6 생쥐 (Central Lab. Animal, Korea) 를대상으로하였으며, 안과및시과학연구회 (Association for Research in Vision and Ophthalmology) 에서규정하고있는동물사용에대한지침을준수하였다. C57BL/6 생쥐는 12시간의낮 / 밤주기와약 23 의상온에서돌보았으며, 출생후 4일, 8일, 12일, 16일그리고 26일에각각 5마리의 C57BL/6 생쥐의안구를적출하였다. 조직의고정및표본제작생쥐를희생시킨후, 안구를적출하여 4% 파라포르말린용액에서 4시간동안고정시켰다. 고정된안구를파라핀포매한후각막의중심부에서부터시신경을가로지르는방향으로잘라블록을제작하였다. 안구블록을 4 μm 두께의절편으로만들어유리슬라이드에표본제작하였다. 면역형광염색위에서만든조직절편슬라이드중가능한시신경단면을포함하는슬라이드를골라실험에사용하였다. 조직절편슬라이드를 60 오븐에서 2시간동안반응시켜파라핀을녹인후자일렌 (xylene) 에 5분씩 4회, 100%-100%-95%- 80%-70% 에탄올에순서대로 5분씩반응시켜서파라핀을제거하고 5분간흐르는증류수에세척하였다. 이후 ph 6.0의 0.01 M 구연산나트륨완충용액 (sodium citrate buffer solution) 에담가고압멸균기 (autoclave) 를이용하여 10분간 120 로반응시키고 30분간 4 에서냉각시켰다. 슬라이드를인산완충식염수 (phosphate buffer solution) 로 5분간 2 회세척한뒤 0.2% Triton X-100 용액으로상온에서 15분간처리하였다. 다시인산완충식염수로 5분간 5회세척후차단용액 (blocking solution) 으로상온에서 1시간처리하고, 각슬라이드마다정해놓은 1차항체를인산완충식염수로희석시킨용액에 4 에서하룻밤동안반응시켰다. 이후다시인산완충식염수로 10분간 5회세척한후정해놓은 2 차항체를인산완충식염수로희석시킨용액에상온에서 2 500

3 - 김진수외 : 망막혈관발생에서네스틴발현분석 - 시간동안반응시켰다. 다시인산완충식염수로 10분간 2회세척후 4,6-diamidino-2-phenylindole (DAPI, Sigma, St. Louis, MO, USA) 을이용하여세포내핵의대조염색을시행하였다. 항체 1차항체로서생쥐항-네스틴 IgG1 단일클론항체 (1:1,000, Merk Millipore, Temecula, CA, USA), 항-IB4 항체 (1:200, Alexa Fluor 488 conjugated, Life Technologies, Camarillo, CA, USA), 토끼항-NG2 다클론항체 (1:1,000, Abcam, Cambridge, MA, USA), 쥐 IgG2 항-GFAP 단일클론항체 (1:1,000, Novex, Life technologies, USA) 가사용되었다. 2차항체로서는알렉사플루오르 594 염소항-생쥐 항체 (Alexa Fluor 594 goat anti-mouse, 1:500, Life technologies), 알렉사플루오르 488 당나귀항-토끼항체 (Alexa Fluor 488 donkey anti-rabbit, 1:500, Life technologies), 알렉사플루오르 647 염소항-쥐항체 (Alexa Fluor 647 goat anti-rat, 1:500, Life technologies) 가사용되었다. 이미지분석면역형광염색을시행한조직절편슬라이드를형광현미경 (Nikon Eclipse 80i, Nikon, Tokyo, Japan) 을이용하여관찰하였으며, 이미지는 NIS-Elements Microscope Imaging Software ver (Nikon, Tokyo, Japan) 을이용하여얻었다. 4 - 대한안과학회지 2016년 제 57 권 제 3 호 - 결 생쥐의 망막혈관 발생 과정에서 네스틴의 발현 생후 4일, 네스틴은 신경절세포층(ganglion cell layer)부 과 정상 생쥐 망막혈관 발생 생후 4일, 시신경유두에서부터 뻗어나가기 시작한 얕은 터 망막모세포층(retinoblast layer)에 걸쳐 강하게 발현되는 혈관 얼기는 중간 주변부 망막까지 뻗어 있는 양상이었으 적으로 약해져 있는 양상이며, 신경절세포층의 위경계선에 며(Fig. 1A), 생후 8일에는 먼 주변부 망막에 도달하는 모 서 시작하여 외과립층(outer nuclear layer)까지 수직으로 뻗 습이었다(Fig. 1B). 생후 12일에는 얕은 혈관 얼기에서 분 어 있었다(Fig. 2B). 이후 생후 12일, 16일을 거치며 점점 지하여 망막 깊은 쪽으로 자라 들어가는 혈관이 관찰되었 수직으로 뻗어 있는 네스틴의 발현은 줄어드는 양상이었으 으며, 생후 16일 및 26일에는 중간 혈관 얼기 및 깊은 혈관 며(Fig. 2C, D), 생후 26일에는 네스틴의 발현이 감소되어 얼기가 형성되어 있는 것이 관찰되었다(Fig. 1C). 이상의 신경절세포층과 내과립층(inner nuclear layer)에 국한되어 생쥐 망막혈관 발생과정을 종합하여 볼 때, 시신경유두에 있었다(Fig. 2E). 또한 모든 망막 발생 단계에서 망막혈관 서 주변부 망막으로 뻗어나가는 방사형의 얕은 혈관 얼기 과 유사한 모양으로 발현되는 네스틴이 관찰되었는데, 세 를 만드는 일차 혈관형성 과정은 생후 8일까지 일어났으며, 포질 내에 응집된 형태로 발현되고 있었다. 모습이었다(Fig. 2A). 생후 8일에는 네스틴의 발현이 전체 이후에는 수직으로 망막의 내과립층까지 뻗어 들어가는 이 차 혈관신생과정이 일어났다. 5 - 김진수 외 : 망막혈관 발생에서 네스틴 발현 분석 - 생쥐 망막혈관 발생 과정에서 IB4, NG2, GFAP의 발현과 네스틴 발현과의 관계 IB4의 발현은 생후 4일에는 얕은 혈관 얼기가 위치한 신 고 찰 망막혈관의 정상적인 발달 및 유지는 정상적인 망막 기 경절세포층의 위경계선에 국한되어 있었으며(Fig. 2A), 생 능의 유지에 필수적이다. 선진국의 경우, 노인층에서 실명 후 8일 이후에는 중간 및 깊은 혈관 얼기에 해당하는 내과 의 원인이 되는 많은 질환이 비정상적인 혈관의 발생에 기 립층의 위경계선 및 아래경계선 바로 바깥쪽에서 관찰되었 인한다. 인구구조가 점점 고령화되어 가는 우리나라 역시 다(Fig. 2B-E). 생후 4일부터 생후 16일까지 IB4의 발현은 선진국의 이러한 흐름에 따를 것이 예상되므로, 망막혈관 네스틴의 발현과 완전히 어우러지는 양상이었으며(Fig. 의 정상적인 발생에 대한 이해는 나이관련황반변성, 당뇨 2A-D), 생후 26일에는 네스틴의 발현과 부분적으로 어우러 망막병증, 미숙아망막병증 등 안구의 신생혈관성 질환을 지는 양상이었다(Fig. 2E). 이해하고 나아가 새로운 치료법을 찾는데 필수적이라고 할 NG2의 발현은 생후 8일까지 얕은 혈관 얼기가 위치한 2 수 있다. 신경절세포층의 위경계선에 국한되어 있었다(Fig. 3A, B). 생쥐의 망막혈관의 발생 과정은 시간적, 공간적으로 일 생후 12일째 이후에는 중간 및 깊은 혈관 얼기의 위치에 해 정한 과정을 취하고 있다는 것이 많은 연구를 통해 알려져 당하는 부위에서 NG2의 발현이 관찰되었으며(Fig. 3C-E), 있다.7,8 본 연구에서 생쥐 망막의 얕은 혈관 얼기는 생후 8 모든 단계에서 NG2의 발현은 네스틴의 발현과 부분적으로 일이 되어서야 망막의 주변부까지 뻗어나가는 것이 확인되 어우러지는 양상이었다. GFAP의 발현은 생후 4일부터 26 었다. 이것은 생쥐의 얕은 혈관 얼기의 형성이 출생 직후 일까지 본 연구에서 관찰한 모든 생쥐의 망막혈관 발달단 시신경유두에서 시작되어 생후 첫 1주에 걸쳐 완성된다는 계에서 신경절세포층에 국한되어 있는 모습이었으며 네스 Dorrell et al8,17의 보고와 일치하는 결과이다. 또한 생후 8 틴의 발현과는 어우러지지 않았다(Fig. 4). 일부터 얕은 혈관 얼기에서 망막을 수직으로 침투하는 양 A B D E C Figure 3. IB4와 NG2는 본 연구에서 망막혈관의 발생과정 중 관찰된 네스틴의 각각 혈관내피세포 및 혈관주위세포의 표지자로 알려져 있 발현은 크게 두 종류로 나눠볼 수 있는데, 첫 번째는 망막 으므로, 네스틴은 이들 세포에서 발현되고 있을 가능성이 을 수직으로 관통하는 방향으로 뻗어 있는 양상의 다발 형 있다. 네스틴이 혈관내피세포에서 발현된다는 점은 앞에서 태의 발현, 두 번째는 세포질 내에서 응집되어 있는 형태의 도 언급했듯이 많은 선행연구에서 보고된 사실이나, 발현이다. 전자의 경우 비교적 초기인 생후 4일부터 망막의 관주위세포에서의 네스틴의 발현에 대해서는 잘 알려져 있 전층에 걸쳐서 강하게 발현되었으며, 생후 8일에는 신경절 지 않다. Wohl et al19은 생쥐의 망막에서 네스틴과 NG2를 15,18 혈 세포층의 위경계선에서 시작하여 외과립층까지 수직으로 함께 발현하는 미세아교세포(microglia)를 보고하였으며, 뻗어 있는 양상이었다. 이러한 형태의 네스틴의 발현은 시 Lee et al18은 전자현미경을 통하여 네스틴을 부분적으로 발 간이 지나면서 그 세기 및 범위가 점점 줄어들어서 생후 26 현하고 있는 혈관주위세포를 보고하였다. 특히 Lee et al18 일에는 신경절층과 내과립층에 국한되어 약하게 발현되었 은 전형적으로 네스틴을 전구세포의 표지자의 관점에서 발 다. 이는 네스틴이 생쥐의 망막혈관 발생 과정에서 방사아 달 중인 망막 조직에 접근하여 네스틴 발현 양상을 교세포(radial glial cell)로부터 새로 생성된 신경세포의 이 Bromodeoxyuridine (BrdU)와 함께 분석하여 망막에서 증 동에 관여하며, 시간이 지날수록 발현이 줄어들어 뮬러세 식 및 이동하는 신경전구세포의 표지자로 네스틴을 사용할 A B D E C Figure 4. 7 - 김진수외 : 망막혈관발생에서네스틴발현분석 - 수있음을제시하였다. 하지만면역조직화학염색및면역전자현미경을이용하여혈관내피세포와혈관주위세포에서네스틴의발현을관찰하였으나, 단지네스틴은전구세포의표지자의관점에서혈관내피세포및혈관주위세포의전구세포에서네스틴이발현된다고보고하였다. 본연구에서는네스틴의발현을전구세포의표지자로서의관점보다는망막혈관발생에서의역할에초점을맞추어혈관내피세포및혈관주위세포가생쥐의망막혈관발생과정에서밀접하게연관되어있음을제시하였다. 제6형중간섬유인네스틴은중추신경계에서신경계줄기세포의표지자로처음알려졌지만, 10,11 이후신경계외의다른조직의줄기세포에서도발현되는것이보고되고있으며, 최근에는다양한종류의암에서신생혈관의표지자로주목받고있다. 15,16 네스틴은짧은 N-말단과상대적으로긴 C- 말단을가지고있는데, C-말단은비멘틴 (vimentin), 데스민 (desmin), 인터넥신 (internexin) 과같은다른중간섬유들과상호작용하여이종이합체 (heterodimer) 를이룬다. 20,21 최근 Liang et al 22 은네스틴을통해혈관내피세포의세포골격재배치가일어나는기전을제시하였는데, 혈관내피성장인자 (vascular endothelial growth factor, VEGF) 의유도로혈관내피세포의극화 (polarization) 및사상위족 (filopodia) 의형성이일어난다고설명하였다. 혈관주위세포역시네스틴의발현과관계가있다는본연구결과로미루어볼때혈관주위세포의이동및세포골격의재배치과정에서도네스틴이관여하고있을가능성이있으며, 더나아가네스틴이두세포사이에서중간자의역할을담당하고있을가능성도생각해볼수있다. 이에대한검증및조절인자를밝히는후속연구가필요할것으로생각되며, 네스틴을통한혈관내피세포및혈관주위세포의조절과정은잠재적으로신생혈관성질환을조절하는약물개발에좋은표적이될것으로기대된다. 결론적으로생쥐의망막혈관발생과정에서의네스틴의발현은크게두가지의형태를보이고있었는데, 망막을수직으로관통하는다발형태의발현과망막혈관과유사한형태를보이는세포질내의발현을보였다. 후자에해당하는네스틴의발현은전체망막혈관발달단계에서혈관내피세포및혈관주위세포와밀접한연관이있었으며, 성상교세포와는관련이없었다. 이러한결과는네스틴이혈관내피세포와혈관주위세포사이에서중간자역할을함으로써혈관발생과정에서중요한역할을담당할수있음을시사한다. Am J Physiol Cell Physiol 2015;308:C = 국문초록 = 생쥐의망막혈관발생과정에서혈관내피세포및혈관주위세포의네스틴발현분석 목적 : 신경계줄기세포표지자인네스틴은망막발생과정에서뮬러세포에서발현되는것으로알려져있다. 하지만망막혈관발달에서네스틴의역할은잘알려져있지않다. 본연구에서는생쥐망막혈관발생과정에서네스틴의발현에대해분석하고, 어떠한세포가네스틴의발현과연관되어있는지를확인하고자하였다. 대상과방법 : C57BL/6 생쥐의안구를생후 4 일, 8 일, 12 일, 16 일, 26 일째적출하였다. 면역화학염색법을이용하여망막발생과정에서네스틴의발현과혈관내피세포 ( 이소렉틴 B4 염색 ), 혈관주위세포 ( 신경 - 아교세포항원 2 염색 ), 그리고성상교세포 ( 아교섬유산성단백질염색 ) 의연관성을확인하였다. 결과 : 네스틴은생후 4 일째신경절세포층부터망막모세포층에걸쳐강하게발현되었으며, 생후 8 일부터 16 일까지는신경절세포층의위경계선에서외과립층까지수직으로뻗어있는양상이었다. 생후 26 일에는네스틴의발현이감소되어신경절세포층과내과립층에국한되어있었다. 또한모든망막발생단계에서망막혈관과유사한모양으로발현되는네스틴이관찰되었는데, 특히생후 4 일부터 16 일까지는네스틴의발현과망막혈관이완전히어우러지는양상이었다. 또한네스틴의발현은혈관주위세포와부분적으로어우러졌으나성상교세포와는어우러지지않았다. 결론 : 생쥐의망막혈관발달과정에서혈관내피세포와혈관주위세포에서발현되는네스틴은혈관내피세포와혈관주위세포사이에서중간자역할을함으로써혈관발생과정에서중요한역할을할수있음을시사한다. < 대한안과학회지 2016;57(3): > 506
