In 1937, Thomas Hale Ham proved when incubated in acidified serum, the erythrocyte of PNH are hemolyzed through activation of the alternative pathway of complement but normal erythrocytes are resistant to this reaction, which thereafter has been applied to the diagnosis of PNH. The most distinctive feature of PNH is an intrinsic erythrocyte abnormality and reduces the life span of the erythrocyte, in this respect the disorder resembles many other hemolytic anemias. But whereas most of hemolytic anemia is hereditary, PNH is acquired not family disease and nearly always coexist with a population of normal red cells. Ross & Dacie demonstrated that PNH erythrocytes generally consist of two populations, which are respectively sensitive(pnh m or n) or insensitive (PNH 1) to complement in the complement lysis sensitivity test. The proportion of PNH m erythrocyte varies from patient to patient and is very closely releated to the intensity and frequency of hemoglobinuria. The complication of PNH case is very diverse such as hemolysis, bone marrow hypoplasia, thrombosis, hemorrhage ai)d infection, and the patient lives 5 to 10 more years after the diagnosis but some lives more than that. PNH is presumed to break out due to lack or reduction of DAF, MIRL on the PNH cell membrane which is GPI anchor protein resulted from the defect at GPI anchor by somatic mutation. Key words : PNH, DAF, MIRL, GPI anchor, MAC I. 서론에대하여적혈구가높은민감성을가지며 1) 혈소판및백 발작성야간혈색소뇨증 (paroxysmal nocturnal hemoglobinuria: PNH) 은보체에의해서일어나는용혈반응 혈구의이상도동반되는 2, 3) 후천성클론성질환이다 4, 5, 6) PNH 에대하여비교적정확한기술은 1882 년 Strubings 에의한것으로수면과혈색소뇨가관계있음을밝? 1 l 4 Q d

PNH 는만성용혈성빈혈로서활성화된보 체의용혈반옹에적혈구가비정상적인민감성을나타 내며 l), 이로인해혈전형성, 감염, 골수형성부전, 심한 빈혈, 용혈등에의한임상증상을나타내고, 평균생존 기간은진단후 5-10 년정도이나 8) 더장기간생존한 예도있는비교적희귀한질병으로알려져있다 9) 이질환의원인으로는 somatic mutation 에의해 5) GPI(glycosyl phosphatidylinositol) anchor 에결함이생 겨 10) GPI anchor protein 인 DAF(decay accelerating factor, CD55)11, 12) MIRL(membrane inhibitor of reactive lysis, CD59)13) 등이 PNH 세포의막에결핍되거 나감소되었기때문일것으로추정된다 14-16) 저자들은최근가톨릭대학교의과대학부속강남성 모병원에입원한 33 세의여자환자에게서발작성야간 혈색소뇨증을경험하였기에문헌고찰과함께보고하는 바이다. 환자 : 정 00, 여자 32 세 n. 증례 상기환자는 18 년전경북대학교병원에서재생불량 성빈혈로진단받고 androgen 으로치료받았으며, 4 년 전부터갈색의소변이나타나성남의내과의원에서 검사결과 Hb : 8.8 g/dl, WBC: 3.4 x 10 9 / ~, platelet : 140 x 10 9 / ~, total bilirubin : 0.6 mg/dl, LDH : 1384 IU / ~, urine hemosiderin : + 로나왔고, 약 9 개월간치 료를받았으며보다정확한진단과치료를위해본원 내과를거쳐입원하였다. 본병원에서의검사소견은다음의 Table 1 과같으며, 혈액학적검사소견상특이한점은빈혈과범혈구감 소증, 호중구의감소, 약간의망상적혈구수치의상승, 그리고골수검사상 erythroid hyperplasia 가있었다. 생 T able 1. 3 T배a ν화학적검사소견에서는심한 LDH 의상숭과함께 bili- 변형된방법으로하였다. rubin 이약간증가되었고, 대변잠혈반웅이양성이었으 며, 기타검사상특이한점은없었다. 본병원에내원 2. Sucrose hemolysis test 전인 1994 년 11 월 15 일에갈색소변이나타났으며이 때의색조는오전에는진하고오후에는약해졌다고한 다. Sucrose hemoly~is test 는 Hartmann 19 ) 과 Williams 18 ) 의방법으로하였다. m. 실험재료및방법 m. 실험성적 1. Ham s test 1. Ham s test Ham s test 는 Dacie 17 ) 와 Williams 18 ) 에의해최근에 Ham s test 의결과는다음과같았다 (Table 2). V. 고찰 PNH 적혈구는, 혈액형이같은정상인의혈청을 ph 6.5--7.0 으로산성화시켜 37t 에서함께가온했을때 용혈이일어나는데이것이 Ham s test 의원리이다 4) 이시험에서 Mg++ 0.

4 V. 고찰 PNH 적혈구는, 혈액형이같은정상인의혈청을 ph 으로산성화시켜 37t 에서함께가온했을때 용혈이일어나는데이것이 Ham s test 의원리이다 4) 이시험에서 Mg mol/ R 을가했을때용혈이 증가되며, 가하지않았을때에는비정상 PNH 세포모 두가용혈되지않고일부만용혈되며 20), 산성화는그 기전이불확실하나이두가지요인을적정화했을때 이시혐의. 특이성과민감성이증가된다 14) 위시험에서 위음성은 impotent serum 의사용, 지나친산성화, PNH 세포의수가적을때등이고 2l), 위양성반응은 HEMPAS (hereditary erythroblastic multinuclearity with a positive acidified serum lysis test) 에서일어나며, 이질환은 선천성질환이며용혈은 IgM 항체에의해일어나는데 이항체는정상인의혈청에존재한다. 이환자의혈청이철결핍성빈혈이있을경우에 PNH의병발을의심 에는 HEMPAS 에대한항체가없으므로 PNH 환자와 는달리자신의혈청으로는용혈이일어나지않으며 sucrose hemolysis test 에음성으로나오므로위의두 가지시험으로 PNH 와구분이가능하다 14) Sucrose he 야한다. 재생불량성빈혈의경과중적어도한번이 molysis test 는등장성의 sucrose 용액을사용하여보체 의홉착을촉진시키며, PNH 세포는보체에대한높은 민감성으로인해서용혈이일어나나정상적인세포는 류되어산성상태로되면서보체가부경로를 24) 통해활 성화되어적혈구가파괴되기때문이다 25) 그러나혈색 소뇨의증상은환자의약 25 % 정도에서만나타난다 18) 국내에서는고등 26) 이관찰한 14 예의 PNH 환자중혈 색소뇨에의한갈색의소변을주증상으로하여내원한 환자는단 1 예였다. PNH 환자에게서주된사망의원인 은혈전, 감염, 심한빈혈, 용혈등이다 25, 27) 이등 28) 은 총 21 예의 PNH 환자에서합병증으로혈전색전증이 5 예 (42.0 %), 감염 4 예 (33.0 %), 급성신부전증 3 예 (25. 0%) 를관찰하였고사망한 3 예에서는혈전증 2 예, 감 염 1 예가발생하였음을보고한바있다. 재생불량성빈혈의경과중에특별한원인이없이용 혈성빈혈이계속될때, 혈색소뇨가있을때, 골수검사 상골수기능저하가있던없던간에범혈구감소증이나 일부혈구들의감소를보이는경우, 특별한원인이없 해야하며, 이밖에반복되는복통및배뇨통을수반할 경우, 정맥혈전증이반복될경우그리고설명할수없 는수혈반응이나타날경우에도 PNH 의발생을의심해 상은 Ham s test 를해볼필요가있다 29) PNH 환자에게서적혈구가 Ham s test 에항상용혈 을일으키지는않는다. 이것은단지일정량의 PNH 적 용혈이일어나지않는다 22) 이시험에서 leukemia, my- 혈구만이보체에민감성을가짐을나타낸다. 이러한 elosclerosis 등이위양성반응이나타날수있으나용혈민감성이있는적혈구의비융은환자에따라다양하 은 10 % 이하이고, 이들은 Ham s test 에는음성이며, 며, 이비율과용혈의정도와는비례한다 30) 보체에의 PNH 세포는본시험에서 % 의다양한용혈정한용혈시험에서민감성이있는군과없는군에대한 도를나타내나 5% 이하로나올수도있으므로주의연구는 Rosse 와 Dacie 등에의해이루어졌는데 l), 두 를요한다 17). DAF 와 MIRL 에대한항체를이용한군으로구성되어있는것으로나타났으며, 어떤경우 flow cytometry 에의한검사법과 sucrose hemolysis test 에는세군으로이루어진것으로나타났단 ) Type 를상호비교했을때, 23, 465 예에서 sucrose hemolysis 1 은정상적인민감성을갖는세포를말하며, Type II test 는 36 명이양성으로나왔으나 flow cytometry 에서는중간정도의민감성을가지며정상세포보다 3--5 배 는모두음성으로나와 0.2 % 의위양성률을나타내었더민감하며, Type m 은매우민감성을갖는세포로 다. 이러한결과로볼때 sucrose hemolysis test 는높은정상적인세포보다 배더보체의용혈작용에 특이성을가지는 screening test 로서높은가치가있음민감성을갖는다. Type II, m 의세포는정상적인세포 을알수있다 23) 보다더많은보체가막의표면에부착하고 33), Type m PNH 환자에서야간에수면중체내에서용혈이일어에는여기에다보체 C3--9 가더많이부착하는것으 나는이유는호홉의감소로폐포내에이산화탄소가저로나타났는데 34, 35) 이로인해 Type III 이가장심한용

PNH 세포전은아직밝혀지지않고있다 14) Homologous restrict- 에서 GPI anchor 단백질들이세포가노화되어가면서 ion factor(hrf) 또는 C8 binding protein(c8bp) 은 그숫자가감소되는것으로나타났다 37) 따라서 PNH MAC 의활성을조절하는것으로보고되었으며, C8 이나 적혈구는비정상적으로적은숫자의 GPI anchor 단백 C9 또는둘모두의결합을저해한다 (5) 그러나 HRF/ 질들을가지고순환혈액중으로들어와노화가진행됨 C8bp 의산성화시킨혈청에의한 PNH 적혈구의용혈 에따라급속히 DAF와 MIRL 등이감소되면서보체 반응에서의역할에대해연구된것은아직없다 15) 에의해혈관내에서용혈이일어나는것으로여겨진다 15) DAF가결여된 PNH 적혈구에정제한 DAF를부착시 많은생화학적변화가 PNH 세포에서보고되었는데, 켰을때, DAF 는부분적으로 C3 convertase 의활성증대 세포표면의단백질들이없거나감소되어있었다. PNH 를차단하여산성화된혈청에서용혈을부분적으로차 적혈구에서 DAF, MIRL I6 ), acetylcholinesterase, PNH 단하였고, MIRL 이결여된 PNH 적혈구에정제한 granulocyte 에서 alkaline phosphatase 38 ) 등이감소되거 MIRL 을부착시켰을때, MIRL 은 MAC 의활성을차단 나완전히결여된것으로보고되었으며, PNH 세포의하고또한 C3 convertase 의활성을저해시켜산성화된 표면에서결여된분자들은다음의 Table 4 와같다. 혈청에의한용혈을저해하였다. 정상적혈구에서 DAF Table 4. DAF 와 MIRL 둘다의기능을항체로막았을때에는보체의 부경로와 MAC 의활성을조절하는능력이상실되었고, 산성화시키지않은혈청에서도용혈이일어나는것으 로나타났다. 위의결과로 DAF 와 MIRL 은상호협동 적으로 acidified serum lysis test 에서감수성올조절하 며, 두단백질중 MIRL 이더중요한역할을함을알 PNH 세포에서용혈이일어나는원인은대량의 C3 수있다 15 애 ) 최근에는이러한사실에근거하여 DAF 와 가세포막에부착하기때문이며, 또한 PNH 세포막에 MIRL 에대한항체를이용하여 PNH 를진단하는방법 서의 convertase 의높은효율성때문이다 39). DAF 는보이시도되고있다 46- 뼈 ) 위의도표에서도알수있듯이 체조절단백으로, 고전적및부경로를통한보체의 PNH 세포에서많은단백질들이결핍되거나감소된것 활성증가를 C3 convertase 의형성올저해함으로서보으로나타났는데, 이들의공통점은 glycosyl phospha- 체의용혈작용으로부터정상세포를보호하며 15, (0), PNH tidyl-inositol(gpi) 부분을통해막표면에고정되는 세포에서는현저히감소되어있거나완전히결여되어있단백질들이다. 다 11, 16) 처음에는활성화된보체에대한민감성이 DAF GPI 는 phosphatidylinositol, N - glucosamine, three 가없어서생기는것으로여겼으나 ll4l), 곧다른요인 mannose residus, ethanolamine 으로구성되어있으며,

6 최근에 PNH의주원인은 anchor의생합성과정의결함때문인것으로발표되고있다 10, 36, ). GPI의생합성 과정에서중간물질을전사하는 PIG - A(phosphatidylinositol glycan class A) 가밝혀졌고 52), Takeda 등은 53) 1 명의환자에게서 somatic mutation 에의해이 PIG-A 유전자의기능이상실되었음을알아내었다. Miyata 등 은 49) 15 명의 PNH 환자의 granulocyte 에서 PIG-A 유전 자에관련된이상을밝혀내었는데, 3 명에게서크기의 이상으로인해 PIG-A 의기능이완전히상실되었고, 1 명에서는 PIG-A 에의한전사가매우낮았으며, 나 머지 11 명에게서는크기와전사는정상이었으나만들 어진전사체의일부에서기능이없는것으로나타났 고, 2 명에게서는 somatic mutation 이발생하였음을증 명하였다. GPI 의생합성과정에는 10 개또는그이상 의유전자가관련되어있으며 54), 지금까지연구한 26 명 의 PNH 환자모두에게서결함의원인은 GPI anchor 에 있음을보고하고있다 49, 53-57) PNH 에서 GPI anchor 부 분이근본적인결함이라면이들이결여되었을때나타 나는모든임상증상들을설명할수있어야하나그렇 지못하다. 따라서더많은연구가진행되어야 PNH 의 원인을정확히알아낼수있을것으로여겨진다. 혈소 판에의한혈전형성은부분적으로설명이가능하다. 그러나조혈과정중의결함과백혈병으로발전되는 것등에서 GPI 의결함과어떤연관성이있는지모르 고있다.
