To investigate this method as a model for photodynamic therapy (PDT), we cultures the oral squamous carcinoma cell line YD-10B on fibroblast-containing collagen gels at the air-liquid interface and assessed the efficacy of PDT using a photosensitiser 5-ALA-hexenyl ester. PDT effect on YD-10B organotypic raft cultures after twenty-four hours was determined. The number of residual cells was significantly reduced in comparison to control at all four different conditions. PCNA immunostaining and TUNEL assay revealed that PDT inhibited cell proliferation and increased apoptosis. These findings suggest that the organotypic raft culture model provides an effective and rapid laboratory method of investigating strategies for PDT on oral precancerous lesions and squamous cell carcinomas. Key words: Photodynamic therapy, Organotypic raft culture, Oral squamous carcinoma I. 서론 광역학치료 (Photodynamic Therapy, PDT) 는암치료분야에서현재가장촉망받는기술로알려져있다. 광역학치료에의한세포내작용기전은바닥상태의광민감제를특정파장의빛으로활성화시켜단일항상태나삼중항상태로되고여기서바닥상태로될때방출되는에너지가산소와반응하여활성산소 (O 2 ) 를생성시키거나(Type Ⅱ 반응) 광민 * Correspondence : Sang-Gun Ahn or Jung-Hoon Yoon, Department of Pathology, School of Dentistry, Chosun University, , 375 Seosuk-dong, Dong-gu, Gwangju South Korea Tel: , Fax: , ahnsg@chosun.ac.kr or jhyoon@chosun.ac.kr * This work was supported by the Korea Research Foundation Grant funded by the Korean Government (MOEHRD, Basic Research Promotion Fund) (KRF ). - 감제가전자전달계에관여하는경우자유라디칼이형성되 1-3) 어 (Type Ⅰ반응) 세포파괴가일어난다. 광역학치료는 1903 년피부암치료에처음사용된이래광민감제와광원의개발로 1992 년캐나다에서방광암, 식도암에대한치료방법으로처음승인되었고, 유럽에서폐암과식도암, 1994 년일본에서자궁경부암, 식도암, 폐암, 위암등에대한치료방법으로인정받았으며, 1995년미국 FDA 에서암치료로인정되어현재폐, 식도, 방광, 피부암의치료에이용되고있으며점차위암, 대장암등의소화기계암과유방암에도적용되어비교적좋은치료결과를보이고있다 4-6). 그러나광역학치료는주로진행성암이나재발성암의절제등고식적치료에적용되었으나, 최근들어점차표재성조기암의일차적치료로각광받고있다 7). 이는구강전암병소나구강암이광역학치료의좋은모델임을시사한다.

2 특히, 구강주위조직은그기능및심미적중요성으로인해이영역에서발생한암의치료에많은제약을받고있다 8). 구강암수술에따른기능적, 미용적결함, 언어장애는환자로하여금수술후의사회적응에많은제약을초래하고있다. 이러한현실에서정상조직은보존하고병변부위만을선택적이며효과적으로제거할수있는광역학치료는구강암환자에서치료후환자의삶의질을높일수있을것으로기대된다 7,8). 현재구강암및전암병소의예방및치료를위해다양한방법을이용하여연구하고있다. 특히, 구강암의광역학치료에대한실험으로는주로단층세포배양법 (monolayer cell culture) 을시행하는데, 정상구강조직및구강암조직의행동양상과환경을재현하지못하는한계가있다. 단층에서자란세포들은침전되어있고, 이물질표면위에형성 9) 되며, 층을형성하지못하기때문이다. 이러한한계점을극복하기위해누드마우스등동물을실험에이용할수있지만전임상예비실험에동물을사용하기에는시간, 경제적소모가많다. 현재다양한암세포에대한생화학적조작의효과를평가하기위해 organotypic culture 를점차많이사용하고있는추세이다 9,10). 이기술은공기 -액체경계면에서섬유모세포를포함한 collagen gel 상에세포를배양하는방법이다. 이는 in vivo 와유사한환경을만들고상피세포의형태와비슷하게분화되고중첩화된다 11-15). 따라서이연구에서는구강암및전암병소의잠재적인치료방법의하나인광역학치료모델로서 organotypic culture 의효용성을검토하고, 이모델에서의광역학치료효과와단층배양에서의광역학치료효과를비교해보고자한다. II. 연구재료및방법 함께 10% FBS가포함된배지를이용하였고 5% CO 2 배양기에서배양하였다. YD-10B 세포의삼차원배양을위해제 1형교원질을이용하여이를중화완충용액 (2.2% NaHCO 3, 0.05N NaOH, and200mmol/l HEPES) 과 10배의 DMEM/F12 농축배지를 8:1:1 의비율로혼합하고, 진피세포로는 swiss 3T3 섬유모세포를이용하였다. 섬유모세포는제 1 형교원질에 /ml의농도가되도록혼합하여 gel matrix 에섬유모세포가균일하게분포하게하여반지름 12 mm 의 0.2 um pore size membrane 으로제작된 millicell에 casting 한후 37 배양기에서 12시간이상중합하였다. 이후 YD-10B 세포를 6-well plate 에내부에위치한 millicell의각well 당 개의세포가되도록분주하고, 바깥쪽은 DMEM과 F12가 3:1 비율로혼합되고 100 IU/ml penicillin, 100 ug/ml streptomycin, 0.6 mg/ml L-glutamine 과함께 10% FBS가포함된배지 3ml를넣어주어 5%CO 2,37 배양기에서배양하였다. 배지는 2~3 일간격으로교환하고,5일후에는암세포상층부의배지를제거하여종양세포가대기에노출되도록하여 1주간추가배양하였다. 2. 삼차원배양구강암의광역학치료 YD-10B 세포의삼차원배양후배지에광민감제로는 5-ALA-hexynel ester를각각5와 10 um을첨가하여 4 시간배양한뒤 light doses 는 613 ~ 645 nm 사이의 wavelength 와 635 nm peak 를보이는 red lightemitting diode(led)(philips Luxeon Lumileds, San Jose, CA, U.S.A.) 를이용하여 5와 10 J/cm 2 에너지로조사하였다. 이는 Delta Ohm DO 9721 quantum photoradiometer and thermometer data logger(model DO9721, Padua, Italy) 15 로측정시35mW/cm 2 a 에해당하였다. 1. 구강암세포의삼차원배양 구강암세포주는 67세남자의혀에발생한중등도분화를보인편평세포암종에서기원한 YD-10B 세포로한국세포주은행에서구입하여이용하였다. 배지는 DMEM 과 F12가 3:1 비율로혼합되고 100 IU/ml penicillin, 100 ug/ml streptomycin, 0.6 mg/ml L-glutamine 과 3. 병리조직, 면역조직화학및 TUNEL 분석 진피유사체 (dermal equevalent) 와함께배양된삼차원구강암조직은 PDT 시행 2일후 10% 중성포르말린에 24 시간고정하고통법으로파라핀에포매하고, 병리조직학적검색을위해4μM 씩박절한조직을 hematoxylin-eosin (H-E) 으로염색하였다. 면역조직화학염색은 anti-pcna 354

3 antibody(dako) 를사용한 avidin-biotin peroxidase complex(abc) method 로시행하였다. 면역반응은 diaminobenzidine(dab) 로관찰하였으며, Mayer 씨 hematoxylin 으로대조염색하였다. PCNA 에양성인세포의수를 200 시야에서가장많은 5 부위의평균을구하였다. TUNEL 분석은 ApopTag Plus Peroxidase In SituApoptosis Detection Kit(Intergen, Purchase, NY) 을이용하여수행하였다. 슬라이드에서파라핀을제거하고 37 에서 15분간 20 μg/ml 의proteinase K 로처리하였다. 내인성과산화효소를차단하기위해 3% 과산화수소에담근후 terminal deoxynucleotidyl transferase(tdt) 를포함한 37 반응완충액에서 1시간동안 incubation 하였다. 과산화효소와결합한anti-dioxygenin antibody와30분간 incubation 한후반응산물을 2 mm hydrogen peroxide을함유한 0.03% 3'-3 diaminobenzidine tetrahydrochloride (DAB) 용액으로발색시켰다. 0.5% methyl green 으로대조염색을하였다. TUNEL 양성인세포의수를 200 시야에서가장많은 5 부위의평균을구하였다. 4. 통계분석 각그룹의평균값을구하고평균±SD 로표현하였다. 모든통계적계산은 Microsoft Excel을사용하여산출하였다. 통계학적유의성은 p>0.05 로설정하였다. III. 연구결과 1. 삼차원배양(oraganotypic raft culture) 구강암조직 YD-10B 세포를 1주간삼차원배양한결과 3-4층의중층편평상피를이루었고, 세포학적악성도를보이나기저막으로침윤성성장을보이지않는상피내암 (carcinoma in situ) 의소견을보였다 (Fig. 1A). 2. 삼차원배양구강암의광역학치료후병리학적소견 삼차원배양한구강암조직에 ALA-hx PDT 를시행한결과대조군에비해세포질은호산성으로변하거나핵주위로투명한공포(clear halo) 를형성하는세포사의소견이관찰되었다 (Fig. 2A). 또한 LED energy 가높을수록, 그리고 ALA-hx 농도가높은경우세포사가증가하여, 잔존하는핵의수는감소하였다 (Fig. 1B). 3. 삼차원배양구강암의광역학치료후 PCNA 발현의면역조직화학적평가삼차원배양구강암의광역학치료후세포의증식능을확인한결과대조군에비해 PCNA 양성세포수가감소하였고, LEDenergy 가높을수록, 그리고 ALA-hx 농도가높을수록감소하였다 (Fig. 2). 4. 삼차원배양구강암의광역학치료후 TUNEL assay 평가 삼차원배양구강암의광역학치료후세포사의형태를확인한결과대조군에비해TUNEL 양성인 apoptotic cell 의수가증가하였고, LEDenergy 가높을수록, 그리고 ALAhx 농도가높을수록증가하는양상을보였다 (Fig. 3). IV. 고찰 구강조직의삼차원배양은 1979 년 Bell 등 11) 이collagen lattice 를이용한배양상피재조합삼차원배양법을처음소개한후이에대한본격적인연구가이루어지기시작하였다. 처음에는 dead de-epidermized dermis (DED) 를이용하여상피하방에결합조직을제공하였으나 12), 최근에는실험의간편성과재현성을위해제1형교원질과섬유모세포로구성된진피유사체 (dermal equivalent) 를이용한방법이선호되고있다 13). 최근까지실험방법의많은개량을통해거의정상상피와유사한형태학적, 생리-화학적특성을갖는상피의재생이가능하게되었고, 이를이용하여상피역학, 생화학적물질에의한상피세포의 14) 분화- 증식유도, 종양형성기전연구, 상피조직내의바이러스증식및역학연구, 배양상피의외과적수복과약리학적효과와기전연구 15) 등의수많은분야에응용되고있다. 광역학치료의항암효과는크게 4가지기전에의해서일어나는것으로알려져있다. 첫째, 활성산소의직접효과로세포내구성조직인미토콘드리아, 세포막등의물리화학적인손상을유도함으로서종양세포의괴사를일으킨 355

5 다. 둘째, 혈관에미치는효과로혈관에손상을일으킴으로서혈류속도의감소및혈행정지를유발하여종양세포의산소결핍과종양괴사를유도한다. 셋째, 면역학적인효과로 T-세포와대식세포의활성화를유발하여다양한염증매개체의분비와종양세포의직접적인탐식작용을일으켜서종양의퇴행에기여한다. 넷째, apoptosis 의유도로종양세포의사멸을일으킨다 16,17). 이러한항암효과를모두충족하는실험모델로는한계점이있기는하지만누드마우스등동물을실험에이용할수있지만전임상예비실험에동물을사용하기에는시간, 경제적소모가많다 9.10). 따라서본연구에서는 organotypic culture 를통하여단층배양과는달리좀더생체조직에근접한구강암조직에대한광역학치료모델을확립하고자하였다. 본연구에서도 YD-10B 세포기원암종조직과유사한중층의조직을얻을수있었다. 조직학적으로하부결합조직으로의침습이없는상피내암의소견을보였다. 이는더많은섬유모세포의추가나또는장기간배양을통해침습형편평세포암종을유도할수있을것으로기대된다. 이모델을기초로구강전암병소조직에 ALA-hx PDT 를시행한결과대조군에비해세포질은호산성으로변하거나핵주위로투명한공포(clear halo) 를형성하는세포사의소견이관찰되어, 이는생체조직에서보는세포사에근접함을확인하였다. 또한 LEDenergy 가높을수록, 그리고 ALA-hx 농도가높은경우세포사가증가하여, 잔존하는핵의수는감소하여, 단층배양에서의기존의연구결과와일 치하였다 9,18). 삼차원배양구강암의광역학치료후세포의증식능을확인한결과대조군에비해 PCNA 양성세포수가감소하였고, LEDenergy 가높을수록, 그리고 ALA-hx 농도가높을수록감소하였다. 또한삼차원배양구강암의광역학치료후세포사의형태를확인한결과대조군에비해 TUNEL 양성인 apoptotic cell 의수가증가하였고, LED energy 가높을수록, 그리고 ALA-hx 농도가높을수록증가하는양상을보였다. 본실험실에서수행한동물모델을이용한광역학치료결과 PCNA 면역조직화학염색과 TUNEL assay가종양의증식능과세포사의유형을구분하는데도움이되고, 아울러광역학치료후증식능의감소와 apoptoaia의유도에의해종양억제가일어난다고보고한바있다 19). 또한본연구에서이용한 YD-10B 세포를이용한단층배양조건하의 ALA-hx PDT 결과미토콘드리아의 존성경로를통해 apoptosis 가유도된다는연구결과 18) 와일치하여구강암및전암병소에대한광역학치료모델로서 organotypic culture 법이효용성이있음을확인하였다. 그러나세포사이외의면역학적기전등다른기전에의한광역학치료의효용성과다른유형의광민감제나광원의효과에대한효능에대해서는추후보완연구가필요하다.
