Inappropriate expressions of growth factors and oncogenes may influence the biological and clinical phenotype of tumor. But there are still controversy regarding their clinical attributions in head and neck cancer. The authors evaluated the prognostic significance of p53 protein, EGF and c-erbb-2 expression in the squamous cell carcinoma of head and neck to determine their relationship with the clinicopathologic parameters. Materials and MethodsParaffin embedded tissue specimen from 64 cases of squamous cell carcinomas of head and neck were studied by immunohistochemical staining. Resultp53 protein, EGF and c-erbb-2 were expressed 53.1%, 29.7% and 57.8%, respectively, in 64 cases of head and neck cancer. The positive expression of p53 protein was associated closely with T-stage and clinical stage. This expression was statistically significant p0.05. There were no significant relationships between the reactivity of EGF, c-erbb-2 and clinical parameters. ConclusionThe expressions of p53 protein, EGF and c-erbb-2 could be related to oncogenesis in squamous cell carcinoma of the head and neck. And, the expression of p53 protein can be used as a prognostic factor in head and neck squamous cell carcinoma. CA, USA)은 1 50으로 희석하 전이 및 악성도 등과 관련 있을 것으로 여겨지는 p53단백, 여 반응시켰다. 발색은 3-acetyl-9-ethyl carbazole EGF, c-erbb-2의 면역조직화학적 발현양상을 임상적 특 (AEC, Immunotech Co., CA, USA)를 이용하였으며, 과염 징과 비교 검토하여, 악성도와 관련한 예후 인자로서의 가 색을 방지하기 위하여 발색시작 5분 경과 시점으로부터는 능성을 알아보고자 본 연구를 시행하였다. 수시로 현미경을 통한 감시를 하여 적정한 시점에서 발색을 마쳤다. 그후 Mayer s hematoxylin으로 대조염색을 하고 대상 및 방법 universal crystal mount(biomeda co., USA)로 봉입하여 관찰하였다. 대 상 1995년 1월부터 1999년 6월까지 두경부 편평세포암종 면역 조직화학적 염색의 결과 판정 으로 수술을 받았던 환자의 조직표본 중 보관상태가 양호한 염색결과의 평가는 주관성을 배제하기 위해 1인의 병리의 64예를 대상으로 하여 면역조직화학적 염색을 시행하였다. 에 의해 동일한 기준으로 일관성을 가지고 관찰하여 정하였 환자의 남녀 비는 54 10으로 남자가 많았으며, 연령별 분 다. p53단백(fig. 1)이나 EGF(Fig. 2)는 세포의 핵에 염색 포는 27세부터 85세로 평균연령은 60.8세였다. 발생부위별 된 경우를 양성으로, c-erbb-2(fig. 3)는 세포막이나 세포 로는 후두암 19예, 설암 11예, 편도암 8예, 구강저암 7예, 질에 염색된 경우를 양성으로 판정하였다. 광학현미경에 나 하인두암 7예, 상악암 5예, 기타암종 7예였다. 병리조직학적 타난 종양 부위를 정하고 100배 시야에서 종양세포 중 양 분화도는 고분화암이 48예, 중등도분화암이 11예, 저분화 성으로 염색된 세포의 수를 세었다. 그러나 종양세포의 5% 암이 5예였다. T-병기, 경부 림프절 전이를 이용한 임상적 미만에서 나타난 경우는 음성으로 판정하였다. 병기는 미국 암협회분류(AJC classification, 1997)에 따라 분류하였다. T-병기는 T1 10예, T2 26예, T3 15예, T4 통계학적 검증 13예 이었으며, 림프절 병기는 N0 34예, N1 14예, N2 12 p53단백, EGF, c-erbb-2의 양성 발현율과 원발종양의 예, N3 4예 이었으며, 임상적 병기는 제 1 기 9예, 제 2 기 13 크기, 경부 림프절 전이유무, 병리조직학적 분화도, 임상적 병 예, 제3기 18예, 제4기 24예 이었다. 기 등 예후 관련 요소들과 발현율과의 상관성은 Chi-square test를 통해 시행하였으며 유의수준은 p<0.05로 정하였다. 방 법 환자의 임상기록과 병리기록으로부터 나이, 성별, 원발부 위의 구분과 종양의 크기, 림프절 전이, 병리조직학적 분화 도 등을 후향적으로 분석하였고 조직표본 재료는 hematoxylineosin 염색표본을 검토하여 조직학적 아형과 등급을 정 한 다음 종양조직이 50%이상 포함되고 파라핀 포매 조직 의 보관상태가 양호한 부위를 선택하여 면역조직화학적 검 사를 시행하였다. 면역조직화학적 염색 일반 광학현미경 검경 후 선택된 파라핀 블록(block)을 찾 아 각각 4 μm로 연속 박절하여 탈 파라핀과 함수 과정을 거 친 후 항원성 복구가 요구되는 항체인 경우 saline sodium citrate 용액에 담구어 PlecoTM laboratory microwave oven Fig. 1. T-기에 따른 양성 발현 율은 T1, T2를 조기 군으로, 1%), 음성 반응은 30예(46.9%)였다. EGF는 19예(29.7%) T3,T4를 진행 군으로 나누어 비교하였다. p53단백의 경우 에서 양성 발현을 보였으나 염색성이 낮게 나타났으며, 45 는 조기 병기인 경우 41.7%의 양성 발현율이었고 진행된 예(70.3%)는 음성 반응이었다. c-erbb-2는 37예(57.8%) 병기인 경우 67.9%의 발현율로서 진행된 병기에서 발현율 가 양성 발현을 보였고 27예(42.2%)가 음성 반응을 나타 이 높았으며 이는 통계학적인 유의성이 있었다(p<0.05). 냈다. 그러나 c-erbb-2의 경우 47.2%와 71.4%로 진행 군에서 발현율이 증가하였고, EGF의 경우 병기가 증가할수록 발 원발부위와 발현율의 비교 p53단백에 대한 발현은 구강저암과 기타암에서 42.9%로 현율이 감소하였으나 이들은 통계적인 유의성은 없었다 (Table 2). 가장 낮았고 편도암에서 62.5%로 가장 높았다.
