Background : Matrix metalloproteinase (MMP)-2 and MMP-9 degrade type IV collagen and are antagonized by the tissue inhibitors of metalloproteinase (TIMP)-2 and TIMP-1, respectively. Methods : We studied by immunohistochemistry the expressions of MMP-2, MMP-9, TIMP- 1 and TIMP-2 in 72 cases of adenocarcinoma of the gallbladder. Results : The MMP-2, MMP- 9 and TIMP-1 expressions were significantly higher in well/moderately differentiated adenocarcinomas than in poorly differentiated adenocarcinomas, in adenocarcinomas that had invaded the lamina propria/proper muscle than in those that had invaded the perimuscular connective tissue or beyond the serosa, and in adenocarcinomas with fungating growth than in those with infiltrative growth. The TIMP-2 expression showed a similar pattern without statistical significance. Regarding the status of lymph node metastasis, the MMP-2 expression was significantly higher in cases without lymph node metastasis. The MMP-2 and MMP-9 expressions were significantly related to those of TIMP-2 and TIMP-1, respectively, with regard to depth of invasion, differentiation, and growth patterns of the adenocarcinomas. Conclusions : MMP-2, MMP-9, TIMP-1 and TIMP-2 are suggested to play important roles in the progression to early invasion of adenocarcinomas, in which the function of MMP-2 is inhibited by TIMP-2. Key Words : Gallbladder Neoplasms-Matrix Metalloproteinases-Tissue Inhibitor of Metalloproteinases-Immunohistochemistry 일반적으로종양의발생은최초의유전적변형에서시작하여점차적으로국소침윤과전이를할수있는악성종양으로진행한다. 이는종양세포가기저막을뚫고침윤을시작하여기질을통하여이동한후기질내혈관의내벽을뚫고종양색전을형성함으로써이루어진다. 이런종양세포의침윤과전이과정은종양세포와주변기질간의상호작용에의해이루어지며, 여기에는기저막과세포외기질을분해하는단백분해효소가작용한다. 1-4 이단백분해효소는 matrix metalloproteinases (MMPs), serine proteases, cystein proteases와 aspartic proteases로크게나눌수있으며, 이중 MMPs는가장강력하고중요한단백 분해효소이다. 1 지금까지약 20종의 MMPs가발견되었는데, 이중 MMP-2 와 MMP-9 은기저막을형성하는제4형아교질섬유를분해하는효소로서구강, 5 식도, 6 위, 7 대장, 8,9 간내담도, 10 췌장, 요도, 14 방광, 15 난소 16,17 종양의국소침윤이나전이과정과관련한많은연구보고가있다. MMPs의조절은크게유전자전사조절과전사후조절로나눌수있다. 유전자전사조절은매우복잡한과정을거쳐 MMPs의생산량을조절할수있으며, MMPs의활성을조절하는데는전사후조절이더중요하게작용한다. 2 대부분용해성 MMPs는비활성형으로분비되어 N-말단전구영역이분리된 1

2 2 배종엽 최진섭 정현철외 2 인 다음활성화되는데여러단백분해효소들이이전구효소의활성화에관여한다. 활성형 MMPs는주로 tissue inhibitor of metalloproteinases (TIMPs) 에의해억제된다. 1,2,4 TIMPs는지금까지 4군이알려져있고이중 TIMP-1은 MMP-9을, TIMP- 2는 MMP-2 를각각대항함으로써췌장, 13 폐, 18 간 19 종양에서종양의침윤이나예후인자들과연관성을보였다는보고가있다. 결국이들물질들의상호작용에의한효과가종양에서침윤과전이및환자의예후와밀접한관련이있을것으로기대된다. 담낭암종은담도암중에서는가장흔하고서구에서보다우리나라를비롯한동아시아에더흔하다. 최근종양세포와기질의상호작용에관여하는단백분해효소들이여러종양의진행에관련되어있음이활발히연구되고있으나담낭암종에서는아직체계적인연구가없다. 따라서본연구에서저자들은담낭샘암종에서 MMP-2, MMP-9, TIMP-1 및 TIMP-2에대한면역조직화학적검색을통하여이들효소들의담낭암종의진행에기여하는역할을알아보고자하였다. 재료와방법재료 을도포하여비특이적결합을억제하였다. 일차항체인 MMP- 2, MMP-9, TIMP-1, TIMP-2 (Oncogene, Boston, MA, U.S.A.) 를각각 1:50으로희석하여 4 에서밤새반응시켰다. 이차항체를 12분간처리하고다시 Tris buffer에서 10분간처리한후 streptavidin-biotin universal detection system (Immunotech, Marseille, France) 를적용하였다. Tris buffer 에 10분간수세하고 DAB chromogen kit (Immunotech) 로발색한다음증류수로수세하고 Mayer's hematoxylin으로대조염색을하여각항원을발현하는세포와부위를조사하였다. 판독은종양세포의 10% 미만에서약양성이거나염색이되지않는예를음성으로하였다. 종양세포의 10% 이상에서염색된예를양성으로하였다. 통계분석 SPSS 10.0 프로그램 (SPSS Inc., Chicago, U.S.A.) 을이용하여각변수들간의유의성을 Pearson Chi-square test로검정하여 p 값이 0.05 미만인경우를유의하다고판정하였다. 결과 1986년부터 2000년까지연세대학교신촌세브란스병원외과에서담낭절제술을받은원발성담낭샘암종 72예를대상으로하였다. 병리조직학적검색종양의대표적인부분을포함하는파라핀포매조직을 4 m 두께로박절하여 HE 염색을시행하여조직학적소견을관찰하였다. 침윤성샘암종을 World Health Organization의조직학적분화도기준 20 에따라고도, 중등도, 저분화암종으로나누었다. 종양의국소침윤정도에따라고유점막층또는근층까지침범한경우와그이상침범한예로나누었다. 또암종의성장양상에따라돌출형, 침윤형, 혼재형으로나누었다. 같이절제된림프절에서종양의전이여부를확인하여음성과양성으로나누었다. 면역조직화학염색 10% 중성포르말린용액에고정하고파라핀포매한조직을 4 m 두께로절편을만들어 xylene으로탈파라핀하고고농도알코올에서저농도의알코올을거쳐증류수로함수시켰다. 항원의노출을위하여 citric acid buffer에서극초단파를이용하여 1 분 30초간끓였다. 내인성 peroxidase의활성을억제하기위하여 3% 과산화수소수로 15분간처리하였다. 다시증류수로 2분간세척하고 Tris buffer로 10분간처리하였다. 정상염소혈청 임상및병리조직학적소견환자의평균연령은 60.8세였고, 남자가 28명, 여자가 44명으로남녀비는 1:1.6이었다. 조직학적분화도에따라고도또는중등도분화암종이 51예, 저분화암종이 21예였다. 침윤깊이에따라고유점막층또는근층까지침범한예가 20예, 그이상침범한예가 52예였다. 암종의성장양상에따라돌출형이 18예, 침윤형이 44예, 혼재형이 10예였다. 샘암종중림프절이확인된예는 41예였고, 이중음성이 23예, 양성이 18예였다. MMP-2에대한면역조직화학염색담낭샘암종 72예중 27예 (37.5%) 는 MMP-2에음성이었으며, 45예 (62.5%) 는양성이었다 (Fig. 1)(Table 1). 침윤정도에따른발현을살펴보면, 고유점막층또는근층까지침윤한경우가 20예이었는데그중 1예 (5.0%) 는음성이었고, 19예 (95.0%) 는양성이었다. 그이상침윤한샘암종의경우는 52예였으며그중 26예 (50.0%) 는음성이었고, 26예 (50.0%) 는양성이었으며고유점막층이나근층까지침윤한예가그이상침윤한예보다발현율이높았다 (p<0.05). 분화도에따라고도또는중등도분화암종 51예중 12예 (23.5%) 는음성이었고 39 예 (76.5%) 는양성이었으며, 저분화암종 21예중 15예 (71.4%) 가음성이었고, 6예 (28.6%) 가양성으로분화가좋은종양에서더높은발현율을보였다 (p<0.05). 분화가나쁜암종에서는일

TIMP-2 에대한면역조직화학염색 MMP-2 와 TIMP-2 의발현간의관계 담낭샘암종 72예중 48예 (66.7%) 는음성이었고, 24예 (33.3%) 는양성이었다 (Fig. 4)(Table 4). 침윤정도에따라, 고유점막층또는근층까지침윤한예가 20예였는데그중 5예 (25.0%) 는음성이었고, 15예 (75.0%) 는양성이었다. 그이상침윤한예는 52예였으며그중 43예 (82.7%) 는음성이었고, 9 예 (17.3%) 는양성으로고유점막층이나근층까지침윤한예가 담낭샘암종 72예중에서 MMP-2 가음성인예는 27예였고, 이중 24예 (88.9%) 가 TIMP-2도음성이었다. MMP-2가양성인 45예중 21예 (46.7%) 가 TIMP-2 도양성으로이들두효소의발현에유의한상관관계를보였다 (p<0.05). 두효소의발현양상은침윤깊이, 분화도, 성장양상에서유의한차이를보였다 (p<0.05)(table 5). 특히침윤깊이와성장

7 담낭샘암종에서 MMPs, TIMPs 의발현 7 양상은 MMP-2 가양성이고 TIMP-2 가음성인예에서두효소가모두양성인예보다침윤성성장이증가하여, MMP-2와 TIMP-2 가모두음성인예에서근층이상침윤한예와침윤형의비율이가장높았다. 분화도는 MMP-2 가음성인예에서뚜렷이나빠지는양상이었다. 림프절전이는이들효소의발현양상과상관관계가없었다. MMP-9과 TIMP-1의발현간의관계담낭샘암종 72예중 MMP-9 이음성인예는 34예였고, 이중 30예 (88.2%) 는 TIMP-1도음성이었다. MMP-9이양성인 38예중 28예 (73.7%) 가 TIMP-1 도양성으로이들두효소의발현에유의한상관관계를보였다 (p<0.05). 담낭샘암종에서이두효소의발현양상은침윤깊이, 분화도, 성장양상과유의한차이를보였다 (p<0.05)(table 6). 이들효소의발현과림프절전이와는유의한상관관계가없었다. 고찰본연구에서 MMP-2, MMP-9, TIMP-1 및 TIMP-2는초기침윤암종에서발현이증가하다가본격적인심한침윤을보이는예에서는다시발현이감소하였다. 이러한소견으로이들효소들은침윤성암종으로진행하는과정에서초기에주로작용할것으로추정된다. 조직학적분화도에따른이들효소들의발현이분화가나쁜경우에발현율이낮은결과를보였다. 이는폐샘암종 18 에서 MMP-2 가, 대장암종 8 에서 MMP-2와 MMP-9 이종양의분화가좋은경우가나쁜경우보다더강하게발현되는것과같은결과이다. Sakamoto 등 21 도간세포암종에서 MMP-9 은분화가좋은작은암종에서더강하게발현되었다고보고하였다. 또침윤성샘암종에서샘구조를잘형성하지않는부위에서는발현이거의안되었으나, 샘구조를형성하는부위에서는샘구조내에서침윤전방부에해당하는세포들에서다시발현되었다. 이사실은이들효소들이샘구조를형성하지않고침윤이진행되는과정중에는발현이일시적으로억제되었다가새로운샘구조의침윤전방부를형성할때다시발현이증가되는것으로이해할수있다. 그리고분화가나쁜샘암종에서는주변기질로침윤하는과정에 MMP-2 나 MMP-9 외에다른기질분해효소들이작용할것으로기대된다. Brinckerhoff 등 22 의고찰에의하면 MMP-2 와 MMP-9 이주로기저막의파괴에관여하는것과는달리제 1형또는제 3형아교질을분해하는효소인 membrane type 1-MMP (MT1-MMP), MMP-1, MMP-13 등이일부상피암종과간엽기원육종에서침윤전방부의종양세포와기질에서높게발현됨을보고하고있어최근이들간엽교원질분해효소에대한관심이고조되고있다. 림프절전이와의관계에있어서 MMP-2, MMP-9, TIMP-1 이전이가없는군에서발현율이높았다. 이결과는이들효소의발현이림프절전이를억제하기보다는이들효소가림프절전이가있기전샘암종진행초기에발현되기때문으로보는것이타당하겠다. 한편다른장기의종양에서 MMPs와 TIMPs에관한연구들은다소다양한결과를보고하였는데, 췌장암종에서는 MMP-2 의 mrna 및면역조직화학적발현이침윤성과섬유증식과관계가있다고보고되었다. 12 종양의병기나조직학적분화도와관련하여난소종양에서진행암종일수록, 분화가나쁠수록 MMP-2, MMP-9, TIMP-2의면역조직화학적발현율이높았음이보고되어있다. 16 이행상피종양에대해 Nakanishi 등 14 과 Papathoma 등 15 은각각 MMP-2의 m-rna 및면역조직화학적발현과, MMP-9의면역조직화학적발현이침윤정도와조직학적등급에비례해서증가함을보고하여본연구결과와상이하였다. 또한같은담관계종양이지만간내담도암종에서 MMPs의면역조직화학적발현율이주위로의침윤이심할수록높았다는 19 결과와도달라담낭암종과간내담도암종의진행에있어분자생물학적과정은서로다를수있음을시사한다. TIMPs는 MMPs에대한대항작용으로종양진행을억제하거나이와관련된임상병리학적지표들과연관이있을것으로기대되었으나, 여러장기종양에서의연구들에의하면더다양하고복잡한결과들이보고되었다. 간내담도암종에서침윤이심할수록 TIMPs 의면역조직화학적발현율이높았고, 19 간세포암종에서 TIMP-2 mrna는조직학적등급이높을수록발현율이높았다는보고 23 가있다. 비소세포성폐암종에서 TIMP-1 와 TIMP-2 의면역조직화학적발현은높은병기일수록높았다. 18 췌장암종에서 Gong 등 13 의연구에의하면 TIMP-1 mrna 발현은피막침윤과림프절전이와상관관계를보였으나, Yamamoto 등 24 의연구에서는 TIMPs 의면역조직화학적발현은췌장암종의임상병리적지표와는관계가없음을보고하였다. 난소종양에서면역조직화학적및 mrna 발현에관한연구에서 MMP-2, MMP-9, TIMP-2의과발현과 TIMP-1의하향조절이종양의진행과관련있었다. 16 본연구에서는 TIMP-1 과 TIMP-2 의발현이각각 MMP-9 와 MMP-2 의발현과대체로병행하는결과를보였는데, 이는이들효소들이작용할때상호작용을하여, 담낭샘암종의초기진행에관여한다는사실을말해준다. MMP-2와 TIMP-2가같은종양에서유사한정도의발현을보이는것은증가된 MMPs 의활성을대항하기위한생체의노력으로도볼수있다. 4 이사실은본연구의 MMP-2 와 TIMP-2 의발현양상과병리학적소견과의비교분석에서 MMP-2 만발현되는군이 MMP-2 와 TIMP-2 모두발현되는군보다근층이상침윤한예와침윤성성장을보인예가뚜렷이증가하는양상을보여, TIMP-2 의발현이담낭샘암종초기에 MMP-2 의작용을억제하는것으로이해할수있다. 그리고 TIMPs의주된생물학적작용은 MMPs를억제하는것이지만단순히억제하는기능외에활성

8 8 배종엽 최진섭 정현철외 2 인 화에도관여한다는것이최근알려졌다. MMP-2가활성화되는방법으로는 prommp-2가 MT1-MMP와결합하여 N-말단영역을분리하는과정에 TIMP-2 가같이결합한다는사실 1,2,4 이 MMP-2 와 TIMP-2 의발현이병행하는이유에대한또다른설명이될수있겠다. TIMP-1 의경우는성장인자와많은구조적동질성을보여, 4 최초의 TIMPs가 MMPs의기질분해작용을억제함으로써종양진행을억제할것이라는기대와는달리최근종양의성장과진행을촉진하는작용등다양한기능을가지고있음이밝혀지고있다. 1,2,4 그이외에각각효소들의복잡한작용과상호작용이존재하여각각효소들의절대적인양보다서로의상대적인양에의해생물학적효과를나타낸다. 즉고농도의 TIMP-2는 prommp-2의활성화를억제하며, MMP- 9/TIMP-1의비율이 1보다크면 MMP-9는오히려활성화된다. 2 MMPs와 TIMPs의효소활성의정량적인차이가결국이들효소의작용을반영하므로효소활성치에관한정량적비교를하면좀더정확한생물학적효과를이해할수있을것이다. 결론적으로, 본연구에서 MMP-2, MMP-9, TIMP-1 및 TIMP-2 는담낭암종의진행에있어침윤성성장초기에주로작용하며종양의분화가나쁘고침윤이아주심한종양에서는더이상의침윤에는별로관여하지않음을알수있었다. 그리고 MMP-2와 TIMP-2, MMP-9과 TIMP-1은서로상관관계를가지며동시에발현하는양상을보여, 이들효소는담낭샘암종의초기진행에각각독립적으로작용하기보다는상호작용이이루어짐을알수있었다. 특히 TIMP-2 는담낭샘암종의초기진행에 MMP-2 의작용을억제함을알수있었다. 이들효소들의자세한상호작용결과를알기위해서는각각효소와그대항효소의활성치에관한정량적연구가더필요할것으로생각한다. 참고문헌 1.
