Expression of the Thyroid Transcription Factor-1 in Carcinomas of Salivary Gland Type and Pulmonary Adenocarcinomas Jong Sun Choi, Jeong-Wook Seo 1, Jin Won Seo and Joungho Han Department of Pathology, Samsung Medical Center, Sungkyunkwan University School of Medicine and 1 Seoul National University College of Medicine, Seoul, Korea Background : Carcinomas of salivary gland type (CSGT) in the low respiratory tract are rare and often difficult to differentiate from pulmonary adenocarcinomas, which are more common and more aggressive than CSGT, especially in the biopsy specimens. Thyroid transcription factor-1 (TTF-1) is a tissue-specific transcription factor expressed in the thyroid gland and the lung. The purpose of this study is to elucidate the diagnostic utility of TTF-1 in discerning CSGT from adenocarcinomas in the low respiratory tract. Methods : Immunohistochemical expression of TTF-1 was investigated in 14 CSGT (9 mucoepidermoid carcinomas and 5 adenoid cystic carcinomas) and 19 adenocarcinomas (5 well differentiated, 12 moderately differentiated and 2 poorly differentiated). Results : Normal alveolar cells as well as bronchial epithelial cells showed the immunoreactivity for TTF-1, but not in the bronchial glands. In tumors, immunoreactivity for TTF-1 was identified in 17 out of 19 cases of adenocarcinomas (89%), but in none of 14 cases of CSGT. In adenocarcinomas, the immunoreactivity for TTF-1 was strongly correlated with the degree of differentiation (p<0.001). Conclusions : These results suggest that TTF-1 can be a valuable marker in distinguishing CSGT from adenocarcinomas of the low respiratory tract. Key Words : Thyroid Transcription Factor-1- Carcinomas of Salivary-Gand Type- Mucoepidermoid Carcinoma- Adenoid Cystic Carcinoma- Adenocarcinoma 하부호흡기의침샘형암종은드문종양으로, 침샘형암종중샘낭암종은전체원발성폐종양의 0.2%, 점막표피모양암종은전체폐암종의 % 를차지한다. 1,2 침샘형암종은대부분천천히자라는저등급의악성종양들로, 기관지의장점액샘에서기원한다는점에서통상적인폐샘암종과구별되며, 폐샘암종에비하여예후가좋다. 2 그러나생검과같은작은검체에서폐샘암종과의조직학적감별이종종어려울수있다. 그러므로이두종양을쉽게감별할수있는면역표지자가있다면매우유용할것이다. Thyroid transcription factor-1 (TTF-1) 은 38 kda의핵단백질로갑상샘에서갑상샘특이유전자의전사를돕는물질로알려졌다. 3 TTF-1은갑상샘의소포세포에서갑상샘글로불린 (thyroglobulin) 과 thyroperoxidase 유전자의전사를활성화시키는것외에도발달중인뇌와같은신경외배엽에서도발현된다. 4 또한 인간태아의폐에서는발달중인기도의상피세포의핵에서발현되며, 출생이후에는제 2형폐포세포와기관지상피세포의일부에서발현되어계면활성단백 A, B, C 및 10 kda 클라라세포유전자의촉진제를활성화시키는역할을하는것으로알려졌다. 4,5 이후갑상샘과폐에생기는종양들의 TTF-1 발현이연구되면서특히, 폐샘암종에서 TTF-1의발현율이높다는것이알려지게되었다. 6 그러나폐및기관에생기는침샘형암종의 TTF-1 발현여부에대해서는현재까지보고된바가없다. 본연구에서는침샘형암종과폐샘암종의 TTF-1 발현을조사하여 TTF-1이두종양의감별진단에유용한지알아보고자하였다. 또한, 이종양들의임상및병리학적소견들과 TTF-1 발현과의연관성에대해서도조사하였다. 180

2 침샘형암종과폐샘암종의 Thyroid Transcription Factor-1 발현 181 재료와방법재료 1995년 1월부터 2002년 12월까지침샘형암종과폐샘암종으로각각진단되어외과적절제를시행한예중수술전방사선요법이나약물요법등의보조적치료를받지않았고, 파라핀블록의보존상태가양호하고환자의임상적자료검색이가능한침샘형암종 14예와폐샘암종 19예를대상으로하였다. 침샘형암종 14예중 9예는점막표피모양암종, 5예는샘낭암종이었다. 폐샘암종 19예에는고분화 5예, 중분화 12예, 저분화 2예가포함되었다. 대조군은각환자의정상폐조직으로하였다. 방법 임상소견및조직학적검색환자의입원병록지와외래병록지를통해진단당시환자의연령, 성별, 종양의병기등을조사하였고, 종양의병기는 American joint committee on cancer의기준을따랐다. 7 병리조직학적검색은 10% 중성포르말린에 12시간이상고정후일련의과정을거쳐파라핀에포매된조직을이용하였다. 4 m 두께로박절한후헤마톡실린과에오신염색을시행한유리슬라이드를모두검색하여종양의크기, 조직학적등급, 조직학적분화도, 조직학적유형, 림프절전이여부등을조사하였다. 점막표피모양암종의조직학적등급은 Colby 등 2 의기준에의해저등급과고등급으로나누었다. 샘낭암종의조직학적유형은체모양 (cribriform), 세관성 (tubular), 고형성 (solid) 유형으로나누었고폐샘암종의조직학적유형은세계보건기구의기준을따랐다. 8 폐샘암종은분화정도에따라고분화, 중분화, 저분화로세분하였다. 면역조직화학염색병리조직학적검색을통하여비종양성폐조직이충분히포함되고종양을대변할수있는가장대표적인파라핀포매블록을선택하여 4 m 두께로박절하였다. 면역조직화학염색은통상적인 streptavidin-biotin complex 방법을따랐으며, DAKO Tech MateTM 1000 자동염색기를이용하였다. 일차항체는 TTF-1 에대한단클론항체 8G7G3/1 (DAKO, 1:50, Glostrup, Denmark) 를사용하였다. 구체적인염색과정은다음과같다. 4 m 두께로파라핀포매조직을박절하고자일렌으로탈파라핀과정을거친후에틸알코올에서단계적으로함수하고, 0.1 M citrate buffer (ph 6.0) 에담근채 800W microwave에 5분씩 2 회처치하였다. 내인성과산화효소의활성을억제하기위해 0.3% hydrogen peroxide-methanol에 10분간처리후증류수에세척하였다. 그후일차항체와 30분간반응시킨후 biotin이부착된이차항체에 30분동안부치시키고수세하였다. 다음 streptoavidin 에 30분동안부치시킨후 10분간수세하였다. DAB (3,3 -diaminobenzidine tetrachloride) 를 10분간도포하여발색하고 hematoxylin으로대조염색한후봉입하였다. TTF-1 염색의판독은현미경 100배시야에서 1,000개의종양세포중세포질이아닌핵이염색된세포의백분율을계산하며그값이 5% 미만일경우음성으로, 5% 이상일경우양성으로판정하였다. 양성의경우, 다시 5% 이상 50% 미만을약양성 (+), 50% 이상의경우를강양성 (++) 으로구분하였다. 양성인경우염색강도의차이는거의없어염색강도별구분은하지않았다. 통계학적분석통계학적처리는 SPSS 10.0 통계프로그램을이용하여 Chisquare test 및 t-test 검정을사용하였고 p값이 0.05 미만인경우를통계학적으로의미있는것으로판정하였다. 결과임상및병리조직학적소견침샘형암종환자의연령분포는점막표피모양암종의경우 9세에서 69세까지로평균 40세, 샘낭암종은 30세에서 48세까지로평균 41세였다. 샘암종환자의연령은 37세에서 78세까지로평균 56세였다. 남녀비는점막표피모양암종의경우 5:4, 샘낭암종의경우는 3:2였으며, 샘암종은 10:9였다. 점막표피모양암종의크기는평균 2.5 cm였고샘낭암종은평균 4.0 cm, 샘암종은평균 4.4 cm 였다. 점막표피모양암종은조직학적등급에따라저등급이 5예, 고등급이 4예였다. 샘낭암종은조직학적유형에따라체모양 2예, 세관성 2예, 고형성 1예가있었으며, 폐샘암종의경우 18예가혼합성아형 (mixed subtype) 이었고, 나머지 1예는유두모양 (papillary) 유형이었다. 면역조직화학적소견 TTF-1은정상폐포상피세포와세기관지상피세포, 일부기관지상피세포의핵에서발현되었으나, 기관지샘과기관지상피세포에는발현되지않았다 (Fig. 1A). 종양에서의 TTF-1 발현은침샘형암종과폐샘암종에서통계적으로유의한차이를보였다 (p<0.001) (Table 1). 폐샘암종 19예중 17예 (89%) 에서 TTF- 1이양성이었고, 양성인 17예중 10예 (59%) 는약양성을, 7예 (41%) 는강양성이었다 (Fig. 1B). 이와대조적으로점막표피모양암종 9예와샘낭암종 5예모두 TTF-1이음성이었다 (Fig. 1C, D). 샘암종에서임상및병리학적소견에따른 TTF-1 발현은 Table 2와같다. 샘암종의분화도에따라 TTF-1 발현이달라져, 고분화또는중분화샘암종은모두 TTF-1 양성이었으며, 저분화샘암종 2예는모두 TTF-1 음성이었다. 또한고분화샘암종

TTF-1 음성군의평균크기는 9.0 cm, 약양성군은 4.1 cm, 강양성군은 3.5 cm으로종양의크기가작을수록 TTF-1 발현이의미있게높았다 (p=0.027). 그러나종양의크기를 3 cm 기준으로나누었을때는 TTF-1 발현의차이가없었다. 샘암종의조직학적유형에따른 TTF-1 발현의차이는없었으나혼합성 18예중 2예는종양의일부분에서비점액성세기관지폐포암종과같은모양 을보였으며, 특히이부위에서 TTF-1 발현이현저했다. 그외연령이나성별, 종양의병기, 림프절전이여부에따른 TTF-1 발현의차이는없었다. 고찰점막표피모양암종은 Stewart 등 9 에의해침샘에서처음기술

Immunoreactivity of TTF-1 in pulmonary adenocarcinomas according to the clinicopathologic features No. of cases Positive cases Grade of positivity p-value Age (yr) > Sex Male Female Size (cm) 3 cm >3 cm Differentiation Well Moderately Poorly <0.001 Subtypes Mixed Papillary Lymph node Metastasis No metastasis Stage I II III No., numbers; -, 0-<5%; +, 5-<50%; ++, %. 되었고하부호흡기에서는 Sementana 등 10 과 Liewbow 11 에의해처음기술되었다. 원발성하부호흡기점막표피모양암종은폐암의 % 정도를차지하며, 현재의침샘형종양에해당하는과거 기관지샘종 ' 이라불렸던병변들의 1-5% 를차지한다. 12 Mayo Clinic에서폐암으로치료받은 10,134명의환자중점막표피모양암종환자는 12명이었다. 12 샘낭암종역시하부호흡기침샘형종양중에서는가장흔하지만전체침샘형종양의 6% 를차지하며원발성폐종양의 0.2% 정도이다. 1 우리나라에서는원발성하부호흡기샘낭암종이모두 14예, 점막표피모양암종이 4 예정도가증례보고형식을빌어산발적으로보고된바있다. 13,14 이종양들은대부분예후가좋은종양들로예후가나쁜다른폐암들과반드시구별되어야한다. 특히조직학적으로유사한폐샘암종과의감별해야하는데, 작은생검조직에서는감별하는것이때로쉽지않다. 이럴때도움이되는면역표지자로침샘형 암종의근육상피세포에발현하는 S-100 단백등이있기는하지만, 일부폐샘암종에도발현되는등비특이적이라실제적으로는크게도움이되지못한다. 2 TTF-1은처음에갑상샘에서갑상샘특이유전자의전사를조절하는핵단백질로알려졌으나, 갑상샘외에도폐와발달중인뇌에서발현됨이알려졌다. 4 이런발견은곧이단백질이폐에서무슨역할을하는가에대한질문을야기시켰고, 계면활성단백질 A, B, C 및클라라세포유전자의촉진제를활성화시킴으로써세기관지폐포의상피세포로의분화를유지하는데관여하는것이밝혀졌다. 더구나 TTF-1이없는생쥐에서원위폐포, 세기관지, 간엽성폐조직이형성되지않아 TTF-1이폐의발달과분화과정에중요한역할을한다는것이알려졌다. 15 또한 Minoo 등 16 은 TTF-1이기능을못할경우추출된배아폐조직이미분화되고과형성된상피조직덩어리로남게된다는사실을

5 184 최종순 서정욱 서진원외 1 인 밝혔다. 이러한관찰들을바탕으로정상폐에서의 TTF-1의주기능은폐의모양형성에관여하고폐에특이한유전자를활성화시키는것으로볼수있다. 17 TTF-1은종양에서도발현되는데거의갑상샘종양과폐종양에국한되어발현된다. 따라서원발성폐암과다른장기에서폐로전이한전이암에서의 TTF-1 발현이다른데, 특히폐샘암종에서 TTF-1 발현율은보고자에따라 26.7% 18 에서 96% 19 까지비교적높게보고되어원발성폐샘암종과전이성샘암종과의감별에 TTF-1이매우유용하다는것이알려졌다. 6,20,21 또한 TTF-1은원발성폐샘암종과악성중피종과의감별에도유용하다고한다. 22 원발성폐암의경우에도조직학적유형에따라 TTF-1의발현율이달라샘암종의 72.5%, 편평세포암종의 10%, 대세포암종의 25.8%, 소세포암종의 94.1% 에서발현된다. 18 최근폐에원발성으로생긴양성혼합종양 1예에서샘성분의안쪽층을이루는세포에 TTF-1이양성이라는보고가있다. 23 그러나하부호흡기에원발성으로생기는침샘형암종의 TTF-1 발현여부는지금까지조사된바없다. 본연구에서 TTF-1은점막표피모양암종이나샘낭암종의어떤예에서도발현되지않았고샘암종의 89% 에서발현되었다. 침샘형종양은그기원을기관지샘으로생각하고있고, 이에비해샘암종은일부는기관지샘에서기원하기도하지만대부분세기관지폐포또는기관지의상피세포에서기원한다고알려져있다. 24 본연구에의하면정상폐조직에서폐포및세기관지, 일부기관지의상피세포에 TTF-1 이발현된것에비해기관지샘에는발현되지않았으므로, 종양에서도그기원세포에따라 TTF-1 발현이다를수있으리라생각된다. 두종양간의 TTF-1 발현의차이는침샘형암종과폐샘암종의감별에매우큰도움이될수있으며, 점막표피모양암종은물론샘낭암종의경우특히작은생검검체에서체모양이나세관모양을보이는폐샘암종과의구별이어려울수있는데, TTF-1은이러한경우유용하리라본다. 고등급점막표피모양암종은샘암종외에도샘편평세포암종과조직학적으로구별하는것이매우힘들다. 25 고등급점막표피모양암종의경우주로용종성으로기관지에생기는점, 기관지상피내암을볼수없는점, 편평진주를볼수없는점등으로구별하기도한다. 2 Bejarano 등 6 에의하면샘편평세포암종역시 TTF-1이발현되므로두종양의구별이힘들때 TTF-1 이유용할수있다. 본연구에서샘암종의경우분화가좋을수록 TTF-1 발현율이통계적으로유의하게높았다 (p<0.001). Nakamura 등 19 도 TTF-1은분화가좋은폐샘암종에서더잘발현된다고하였다. 따라서 TTF-1은정상폐뿐아니라종양에서도그모양을형성하는데관여하리라생각된다. 폐샘암종의조직학적유형에따른 TTF-1 발현의차이는증례수가적어의미가없었으나, 세기관지폐포암종과같은모양을보이는부위에서 TTF-1 발현이현저한것은흥미있는소견이었다. 앞선보고에의하면세기관지폐포암종, 특히비점액성의경우 TTF-1 양성률이높다고하였는데, 26 TTF-1이정상조직에서폐포나세기관지에서잘발현 되는사실을감안하면비점액성세기관지폐포암종을형성하는데 TTF-1이관여할것으로사료된다. 폐샘암종의크기와 TTF-1 발현과의관계를살펴보면, Pelosi 등 27 은제 I병기의샘암종에서 3 cm 이하일경우 TTF-1 발현이의미있게높다고하였으나, 본연구에서는 3 cm 이하군과초과군사이에통계학적으로의미있는차이가없었다. 그러나 TTF-1 음성군, 약양성군, 강양성군각각의종양의평균크기는차이를보여 (p=0.027) 종양의크기가작을수록 TTF-1 발현이높은것을확인할수있었다. 결론적으로 TTF-1은하부호흡기에생긴침샘형암종에서전혀발현되지않았고폐샘암종에서는높은발현율을보여두종양을감별하는데매우유용하리라생각된다. 참고문헌 1.
