It shows various cytologic and histologic features, so making an exact diagnosis is difficult. We report here on two cases of epithelial-myoepithelial carcinoma arising from the parotid gland and we compare the cytologic findings of the aspirated samples with the histologic findings of the tumors. We think the finding of mixed pattern of large, clear myoepithelia and small epithelia is the most valuable finding in the diagnosis of EMC. (Korean J Cytopathol 2008;19(2): ) Key Words : Epithelial-myoepithelial carcinoma, Aspiration cytology 책임저자 : 박미자주소 : ( ) 대전시서구둔산동을지대학교부속병원병리과전화 : 팩스 : address : pmj@eulji.ac.kr 서 론 증 례 상피-근육상피암종은침샘사이관의상피와근육상피가증식성변화를한종양으로침샘상피종양의약 1% 를차지한다. 1 상피세포와근상피세포의다양한증식소견으로상피-근육상피암종의흡인세포소견도다양하게기술되어있다. 2-5 국내에서는남등 6 과김등 7 이턱밑샘과귀밑샘에서흡인된세포도말소견을보고한바있다. 저자들은최근서로다른세침흡인세포소견과조직소견을보이는상피-근육상피암종 2예을경험하고각증례들의세포도말소견을조직소견과비교하였다. 임상소견증례 1 : 환자는 50세여자로 3년전부터서서히자라는좌측귀밑샘의무통성종양을주소로내원하였으며흡인세포검사상여러형태샘종으로진단받고수술받았다. 증례 2 : 환자는 76세남자로약 10년전부터서서히자라는좌측귀밑샘부위종양을주소로내원하였다. 최근종양의크기가증가하고통증이있어반복적인세침흡인세포검사후저등급악성종양으로진단받고수술받았다. Several cuboidal, ductlike cells surround the lumens and are surrounded themselves by thick outer mantles of clear myoepithelial cells.(h&e) 고 찰 다. 세포질은 아주 적거나 거의 관찰되지 않았다. 적은 수 의 상피세포들이 군집을 이루어 관찰되었으며 고배율 현미 상피-근육상피암종은 주로 귀밑샘에서 발생하는 저등급 경 소견 상 중심부에 호산성 물질을 갖는 가는 관 형태로 악성 종양으로 약 50%에서 재발 가능성이 있다고 보고되 관찰되었다(Fig. 5). 었다.8,9 상피-근육상피암종은 침샘의 사이관의 상피세포와 근육상피세포의 증식성 변화를 보이는 종양인데 근상피세 병리소견 포가 증식하면서 상피세포 종양와 간질세포 종양 양쪽으로 분화되는 형태를 모두 보일 수 있으므로 그의 소견은 매우 증례 1의 절제된 종양은 cm으로 피막에 의 다양하다. 본 증례들에서도 증례 1에서는 상피-근육상피암 해 잘 둘러싸인 회백색의 고형종양이었다. 증례 2의 종양 종의 흔한 세포학적 소견인 기둥형태의 두 종류의 세포증 은 cm 으로 절단면 상 부분적으로 낭성 병변과 식이 주로 관찰되었으나 증례 2에서는 방추세포의 증식이 회백색의 고형병변으로 이루어져 있었다. 현미경소견 상 주 소견으로 전체 종양의 약 5 10%만이 입방형 상피세포 증례 1의 종양은 주로 가지를 이루는 기둥배열을 이루고 와 방추형 근육상피세포의 증식이 같이 관찰되었다. 세포 있었다. 기둥구조는 내측에는 입방형 상피로, 외측은 투명 도말 소견도 증례 1에서는 입방형 상피세포와 공포형성이 한 세포질을 가지는 근육상피로 이루어져 있었다(Fig. 6). 뚜렷한 투명세포들이 혼재되어 있는 삼차원의 세포군집형 면역조직화학 염색 상 내측의 입방형 세포는 cytokeratin 성이 주 소견이었으나 증례 2에서는 특별한 구조 형성없이 에 양성, S-100 단백에 음성이었으며 외측의 투명세포는 방추세포들만이 주로 도말되는 양상이었고 아주 적은 부분 S-100 단백에 양성, cytokeratin에 음성이었다(Fig. 7). 증 에서만 상피세포과 근육상피세포가 같이 존재하는 소견이 례 2의 종양의 대부분은 방추세포의 증식으로 이루어져 있 었다. 상피-근육상피암종의 조직 소견과 세포 소견이 다양 었으며 전체 종양의 약 5 10%정도에서 상피세포와 근육 한 것은 근육상피의 분화능력과 관련있다고 생각되는데 증 상피세포의 증식이 기둥배열로 관찰되었다. 방추세포 증식 례 1의 근육상피는 근육상피세포종이나 여러형태 샘종에 은 사이사이에 섬유성 물질과 얽힌 듯이 관찰되었으며 일 서 주로 관찰되는 형질세포양 근육상피 대신 세포질 내 공 부에서 낭성 변화와 함께 신경집종에서 관찰되는 핵의 울 포형성이 뚜렷한 투명세포로 관찰되었으며 증례 2에서는 타리배열도 관찰되었다(Fig. 8). 세포들의 비정형성이나 유 신경집종과 같은 방추세포로 관찰되었다. 세포도말 소견과 사분열은 거의 없었다. 조직 소견을 비교해 보았을 때, 증례 1의 세포도말에서 관 The Korean Journal of Cytopathology Vol 19 No 2 /

Ductal epithelial cells arranged in tubules are surrounded by spindle myoepithelial cells (H&E). 찰되는공포를형성하는세포들은조직소견상공포를함유하는증식성근육상피들이삼차원적으로도말되는소견으로생각되었으며주위나핵세포들은 Arora 등 10 이언급한바와같이근육상피세포인투명세포의세포질이약하여흡인과정과세포도말과정에서세포막의파열에의해나핵세포로관찰되었다고생각하였다. 증례 2에서도말표본상세포충실성이적은것은종양의대부분을차지하는근육상피들에서분비된근육원섬유들이섬유성병변을유발하여흡인되기어려웠기때문으로생각하였다. 위에기술한바와같이조직소견의다양성때문에상피- 근육상피암종은다른종양과의감별진단이쉽지않아각저자마다몇가지의미있는소견들이기술하였다 그중두종류의세포가도말된다는점과투명세포가관찰된다는점, 호산성의구상물질은저자들이가장많이언급한 176 대한세포병리학회지제19권제2호 / 2008

5 HK Lee et al. : FNAC of Epithelial-Myoepithelial Carcinoma of Parotid Gland 소견으로진단적가치가있다고생각하였다. 그러나이러한소견들은여러형태샘종이나샘낭암종에서도관찰되는소견이므로각소견들의조합을고려하여진단해야할것으로생각하였다. 증례 1에서두종류의세포가도말된다는점에서세포성여러형태샘종을, 많은호산성의구상물질이관찰된다는점에서샘낭암종과의감별진단이중요하다고생각하였으나여러형태샘종보다많은세포가도말되며편평상피가거의관찰되지않는점, 밀집된삼차원의세포군집이관찰되는점, 호산성의구상물질과함께투명세포들이관찰되는점들이감별진단에도움을주었다. 또한구상호산성물질이자주관찰되어샘낭암종과의감별이어려웠으나 Klijanienko 등 4 이기술하였던손가락모양의구조없이투명세포가뚜렷한점, 상피와근육상피가밀집되어관찰된다는점, 타원형세포들의핵의염색질이진하지않아감별하였다. 다형성저등급샘암종에서도호산성구상물질이관찰되지만상피-근육상피암종에서는호산성의뚜렷한간질중심이관찰되는것과달리가는수지상간질중심을보이면서투명세포가드물다는것이다른소견이었다. 증례 2에서는어떤형태학적구조를관찰하기어려워신경내분비종양과같은분화가나쁜종양들을감별해야한다고생각하였으나재검하니적은양이지만중심부에호산성물질을가지는작은상피군집들이관찰되어종양성분이두종류임을시사하는소견이있었다 (Fig. 5). 이에저자들은상피-근육상피암종의진단을위해두종류의세포가도말되어야하며근육상피세포의증식양상에따라도말되는세포의양과형태가다양하다고생각하였다. 참고문헌 1.
