A 21-year-old woman presented with a mass of the left breast. Breast ultrasonography showed a cm sized, well-circumscribed mass in left subareolar region. The mass was excised. Grossly, the mass was well circumscribed and the cut surfaces were grayish-white in color and elastic in consistency. Microscopically, the breast lesion showed the histological features of fibroadenoma. There were areas of extensive squamous metaplasia seen as isolated clusters or involving duct. But some metaplastic clusters surrounded by fibrous stroma were compressed and distorted and separation of cells into isolated clusters mimicked invasive carcinoma. Co-expression of CK 5/6, high molecular weight CK and p63 in areas of squamous metaplasia and normal myoepithelial cells supported myoepithelial cell origin of metaplastic cells. (J Korean Surg Soc 2009;76:56-60) Key Words: Breast, Fibroadenoma, Squamous metaplasia 중심단어 : 유방, 섬유샘종, 편평세포화생 서 섬유샘종은유방에서발생하는가장흔한양성종양으로주로젊은여성에서발생한다. 조직학적으로상피성분과간질성분으로구성되어있고. 두성분의비율과환자의연령에따라다양한모습을보인다.(1) 섬유샘종의간질성분의세포충실성은다양하고, 상피성분은아포크린화생, 편평상피화생, 관상피증식과암종등이발생할수있다.(2) 저자들은 21세젊은여성에게서발생한섬유샘종에서광범 책임저자 : 문우성, 전북전주시덕진구금암동산 , 전북대학교의학전문대학원병리학교실 Tel: , Fax: mws@chonbuk.ac.kr 접수일 :2008 년 5 월 21 일, 게재승인일 :2008 년 7 월 29 일 론 위한편평상피화생을보이며, 가성침윤소견을보여편평세포암종과감별이필요한 1예를경험하였기에보고하고자한다. 증례 21세여자환자가 7일전에우연히발견한왼쪽유방의종괴를주소로내원하였다. 진찰소견에서왼쪽유방에경계가좋고잘움직이는단단한종괴가만져졌으나종괴상방의피부에오목형성 (dimpling) 이나유두분비물은관찰되지않았다. 액와부나빗장위림프절종대는관찰되지않았다. 과거력에서염증성병변이나진단적시술, 외상을받은적은없었다. 유방초음파에서왼쪽유방의유륜하부에 cm 크기의경계가좋은종괴가관찰되었다 (Fig. 1). 환자는종괴절제술을시행받았다. 육안소견에서종괴는 56

조직소 견에서 종괴는 간질성분과 상피성분으로 구성된 섬유상피 성 종양으로 간질성분의 세포밀도가 높아 섬유샘종 소견을 보였으며 간질의 과성장이나 괴사는 관찰되지 않았다.

성 세포질을 가진 세포로 결합체(intercellular bridge)가 관찰 되었다. 간질 내에 고립된 군집형의 편평상피화생은 경계 Fig. 2.

2 Eun Jung Cha, et al Extensive Squamosu Metaplasia Mimicking Squamous Cell Carcinoma in Fibroadenoma 57 주변 조직과의 경계가 좋았으며 크기는 cm였다. 종괴 의 절단면은 회백색을 띠며 단단한 충실성 종괴로, 뚜렷한 틈새는 관찰되지 않았고, 출혈이나 괴사는 없었다. 조직소 견에서 종괴는 간질성분과 상피성분으로 구성된 섬유상피 성 종양으로 간질성분의 세포밀도가 높아 섬유샘종 소견을 보였으며 간질의 과성장이나 괴사는 관찰되지 않았다. 또 한 주로 소관내형의 성장을 보이고 간질의 점액성 변화가 자주 관찰되었다. 반면 상피성분은 광범위하게 편평상피화 생을 보이며, 관 내나 간질 내에 고립된 군집의 형태로 관찰 되었다(Fig. 2A). 편평상피화생은 작고 둥근 핵과 밝은 호산 Fig. 1. 4 Eun Jung Cha, et al:extensive Squamosu Metaplasia Mimicking Squamous Cell Carcinoma in Fibroadenoma 59 형유사분열상은없었다 (Fig. 2C). 편평상피화생은관상피세포와근상피세포에서분명히이행되는부분이관찰되었으며 (Fig. 2D), 염증세포는극히일부에서소수가관찰되었다. 일부편평상피화생에서이상성석회화 (dystrophic calcification) 와작은낭성변화가관찰되었다. 면역조직화학염색에서 p63 (Fig. 3A), cytokeratin 5/6 (Fig. 3B), high molecular weight cytokeratin (HMWCK) (Fig. 3C) 은편평상피화생과근상피세포에서양성이었고 epithelial membrane antigen (EMA) 는편평상피화생과관상피세포에서양성이었으며 CD10과 smooth muscle actin (SMA) 은근상피세포에서만양성이었다. Ki-67 항체에대한양성률은 5% 미만이었고 (Fig. 3D), p53은편평상피화생에서 1% 미만의세포에서국소적으로양성소견을보여음성으로간주하였다 (Table 1). 고찰 유방에서발생하는섬유상피성종양에는섬유샘종과엽상종양이있다. 대부분의섬유상피성종양은양성이지만엽상종양은침윤성성장과간질과성장, 흔한유사분열상, 괴사와심한간질세포다형성등을기준으로양성, 경계성, 악성으로나뉜다. 엽상종양은 40 50대에주로발생하며간질성분은세포밀도가높고낭성공간내로나뭇잎모양으로성장하는특징적인조직소견을보인다.(3) 전형적인조직소견을보이는섬유샘종과엽상종양의구분은쉬우나간질의세포밀도가높은섬유샘종이나비전형적인조직소견을보이는엽상종양은구분이어렵다. 유방에발생하는편평상피화생과편평세포암종은드문조직학적소견으로편평상피화생은양성또는악성종양의일부분에서관찰된다.(2,4) 양성종양에서관찰되는편평상피화생은주로염증이나고름집주변, 관내유두종, 여성형유방증, 섬유샘종, 엽상종양및유두종증에서보고되었으며,(3,5-10) 대부분에서일부국한된곳에서만발생하며, 다발성이나광범위하게관찰되는경우는매우드물다. 유방질환의진단적방법인세침흡인검사에서편평상피세포가관찰되면유방에발생한종괴가양성종양의편평상피화생, 악성종양의편평상피화생, 유상피낭종, 원발성편평세포암종, 피부에서발생한편평세포암종과전이성편평세포암종의가능성을생각해볼수있다. 도말된슬라이드에서침윤성암종소견이나편평상피세포의세포학적이형성정도가암종에합당하면진단할수있지만편평상피세포가세포학적으로양성소견을보이거나흡인된편평상피세포의양 이많은경우에는다른진단적술기를통해확진해야할것으로보인다.(11,12) Soderstrom과 Toikkanen이보고한예에서는대부분이결합체로연결된편평상피세포로구성이되었으나일부에서괴사된부분과침윤으로보이는소견이관찰되어, 편평세포암종과감별이필요하였다. Soderstrom 과 Toikkanen은괴사와가성침윤부분이관찰되나, 세포학적으로악성으로볼수있는이형성이관찰되지않아편평세포화생으로결론지었다. 편평세포암종은유방의원발성악성암종의 1% 미만으로발생하며, 보고되었던유방의편평상피암종의대부분은크기가크고낭성변화와괴사를동반하며평균생존기간이 8.3 개월에불과한매우악성의경과를보인다. 또한발생연령은 58 64세로일반적인유방암종에비해늦은나이에발생한다.(13) 본예에서는간질내에고립된군집형의편평상피화생은경계가좋거나불규칙하며일부에서는섬유성간질에의해눌리거나비틀림으로인해낱개의세포로떨어져침윤성암종을시사하는소견이관찰되나종양의크기가작고주변의간질에결합조직증식증 (desmoplasia) 의소견이나상피내암병변이관찰되지않으며, 환자의나이가 21세로상피성암종이발생하기에는너무젊고, Ki-67 항체에대한양성률이 5% 미만으로증식정도가낮다는점, 종양세포에서 P53의유의한발현이관찰되지않은점으로편평세포암종보다는광범위한편평상피화생을동반한섬유샘종으로진단하였다.(14) 편평상피화생의기원은분명하지않다. Salm(8,9) 은섬유샘종에서발생한예에서상피-버팀질전환을제시하였다. 이후에 Reddick 등 (15) 은유두종에발생한편평상피화생에서면역조직화학염색과전자현미경을통해서편평상피화생이 S-100 단백과 muscle specific actin에양성이며, 근상피세포의미세구조가관찰되어근상피세포에서발생한것이라주장하었다. 또한 Raju(16) 도섬유샘종에발생한편평상피화생에서면역조직화학염색을통해근상피세포에서발생한것이라주장하였다. 그러나 Mills(17) 는뚜렷한병변이없는유방에서발생한편평상피화생에서 low molecular weight cytokeratin (CAM 5.2) 과 EMA에양성을보여상피에서발생한것이라주장하였다. 지금까지근상피세포면역학적표지자로 p63. Smooth muscle actin, CD10, CK 5/6, HMWCK, S-100 protein 등이쓰여지고있으나, 근상피세포에특이적인면역학적표지자는없다. 본예에서는편평상피화생이관내에서뚜렷한이행부위를보이며, 근상피세포의면역학적표지자인 p63과기저세포형시토케라틴 (basal

Vol. 76, No. 1 cell type CK) 인 CK5/6과 HMWCK에양성인점과면역조직화학염색의결과가편평상피화생을일으킨부위와근상피세포에일치되는점을보아서근상피세포기원임을뒷받침하고있다. 환자의유방병변의조직학적소견으로간질내편평상피화생군집의경계가불규칙하고낱개의세포로침윤하는듯하고, 유사분열상이관찰되는곳에서세포학적이형성이존재하여환자를지속적으로추적관찰한결과 2년간의기간중에재발이나전이소견은관찰되지않았다.
