The patient was a 35-year-old nulliparous woman who presented with a polypoid mass in the posterior wall of the lower vagina just above the hymenal ring. The tumor was relatively welldefined with an expansile margin and composed of stromal-type spindle cells with a myxoid stroma arranged in short fascicles and an irregular cord-like or reticular pattern. A few glandular structures lined by cuboidal cells with squamous metaplasia were also noted. Cellular atypia was not conspicuous and mitosis was not found. We examined this tumor immunohistochemically to identify the histogenesis. The coexpression of epithelial and mesenchymal markers in stromal-type spindle cells suggested a multipotential cell origin for this tumor. The patient has been well with no signs of recurrence during the 12 months after surgery. Key Words : Epithelioma; Spindle cell; Mixed tumor; Vaginal neoplasms 질의양성혼합종양은질외의다른기관에서발생하는양성혼합종양과는세포기원에있어서중요한차이점들을보이기때문에현재는방추세포상피종으로명명하도록권장되고있다. 1 이종양은 1953년처음보고된 2 이후현재까지영어권문헌에 50개 정도가 3-5 보고되어있으며국내문헌에는 Park 등 6 과 Kang 과 Yoon 7 에의해두예가보고된바있다. 저자들은 35세의미분만부여성의질에서발견된매우드문종양인방추세포상피종을경험하였기에문헌고찰과함께보고하고자한다. 증례 35세미분만부여성이한달전부터앉았다일어설때느껴지는회음부의불편감을주소로내원하였다. 내진시질의개구부직상방의후벽에서부드럽게촉지되는종괴가관찰되었으며촉진시압통은없었다. 환자의자궁크기는정상이었고양측자궁부속기에도촉지되는이상종물은없었으며질경상질종괴이외에상부질및자궁경부의특이소견도보이지않았다. 첫내원시시행한질및자궁경부의세포진검사결과는정상 이었고, 두번째내원시국소마취하에시행한단순절제술후외래치료의경과또한양호하였다. 질에서절제된종괴는육안적으로경계가분명한회백색의연부조직종양으로크기는 cm이었다. 현미경소견상종괴는경계가좋으나피막은가지지않는점막하종양이었으며종괴의상부는질의편평세포상피로덮여있었으나질상피와종양과의연결은없었다 (Fig. 1). 조직학적으로종양세포는크게기질양세포 (stromal-type cells) 들과상피세포들로구성되어있었는데종양의대부분을구성하고있는기질양세포들은고른염색질의핵과호산성의세포질을가지는난원형또는방추형세포들로다양한세포밀도를보이며배열되어있었으며, 세포밀도가낮은곳에서는심한섬유화가관찰되었다. 또한대부분의기질양세포들은서로교차하는작은다발로배열되어있었으며 (Fig. 2A), 점액모양의간질배경에서로불규칙한끈모양 (Fig. 2B) 또는망상형구조를보이는곳도있었다 (Fig. 2C). 그리고극히일부에서만관찰되었던상피세포들은점액을내강에함유하고있는선상구조로편평상피화생을동반한입방형의세포들로구성되어있었다 (Fig. 2D). 한편기질양세포및상피세포들모두에서이형성은없었 670

면역조직화학염색은 상피세포가 매우 국소적으로만 위치하여 주로 기질양 세포에서만 시행되었다.

The tumor is a well-circumscribed and unencapsulated mass located near, but unconnected to the vaginal epithelial surface. 면역조직화학염색은 상피세포가 매우 국소적으로만 위치하여 주로 기질양 세포에서만 시행되었다. 그 결과 pancytokeratin (1:500, Novocastra, Newcastle, UK), cytokeratin 7 (1:300, Dako, Glostrup, Denmark)과 epithelial membrane antigen (1:800, Neomarker, Fremont, CA, USA) 같은 상피성 표지 자에 모두 양성 반응을 보였으며, vimentin (1:2,000, Zymed, San Francisco, CA, USA), smooth muscle actin (1:1,000, Neomarker), CD10 (1:250, Novocastra), CD99 (1:1,000, Zymed), CD34 (1:5,000, Neomarker), bcl-2 (1:200, Dako), estrogen receptor (1:500, Dako) 및 progesterone receptor (1:500, Dako)에도 양성 반응을 보였고, desmin (1:1,000, Dako) 에는 국소적으로만 양성 반응을 보였고, cytokeratin 20 (1:500, Dako), S-100 protein (1:1,000, Neomarker)및 glial fibrillary acidic protein (1:2,000, Neomarker)에는 음성 반응을 보였다 (Fig. 3). 그리고 종양의 국소절제술 시행 후 12개월 경과된 현 재 재발이나 전이의 증거는 보이지 않고 있다. The tumor reveals a diffuse strong positive reaction for (A) pancytokeratin, (B) epithelial membrane antigen, (C) vimentin, (D) CD99, (E) CD34, (F) smooth muscle actin, (G) estrogen receptor, and (H) progesterone receptor. 고찰질에서발생하는방추세포상피종은매우드문종양으로 1953 년 Brown 2 이 33세여성에게서발생한형태학적으로타액선의양성혼합종양과매우흡사한종양이란의미로질의혼합상피성종양 (mixed epithelial tumor of the vagina) 이란제목으로처음보고한이래현재까지질의혼합종양 (mixed tumor of the vagina), 질의양성혼합종양 (benign mixed tumor of the vagina) 및질의양성뮐러관기원혼합종양 (benign Mu_llerian mixed tumor of the vagina) 등의이름으로명명되어왔다. 6 혼합종양이라는용어는타액선에서가장많이사용되고있는데이외에도유방, 피부, 음문, 외이도, 기관, 눈물샘및연부조직등에서자주사용되고있으며근육상피세포기원이라는공통점을가지고있다. 그러나질의경우는다른기관들의혼합종양과는확연한차이를보이는데첫째, 현미경적소견상의차이로종양을이루는세포들은대부분방추세포들이고형태학적으로상피세포의모양을보이는세포들은국소적으로만관찰된다. 둘째, 종양의기원에관한차이로질에는근육상피세포가존재하 지않는것으로알려져있으며, 실제로방추세포상피종의전자현미경적검사에서근육상피세포분화를의미하는근육원섬유 (myofibril), 치밀소체 (dense body), 세포흡수소포 (pinocytotic vesicle) 와치밀판 (basal lamina) 등은관찰되지않는다. 1 현재까지종양의기원에대해서는여러가지가설이있으나아직명확히밝혀진바는없다. 그런데그중요생식동기원상피세포 (urogenital sinus-derived epithelium) 의가능성이주목을받고있는데이를뒷받침하는이유는본종양의발생부위가질중에서도질의하부특히처녀막과인접한곳에빈번하다는것이다. 이는발생학적으로질의상위 2/3는뮐러관기원이고, 하위 1/3이요생식동기원으로서위치적으로일치한다는주장이다. 1,3,7 그러나본종양이형태학적으로거의대부분중간엽종양의모양을보이는기질양세포이고상피세포는아주드물게관찰되며면역조직화학염색에서도 smooth muscle actin과 cytokeratin이함께발현된다는점, 그리고 estrogen receptor와 progesterone receptor에양성인점도요생식동상피세포기원설에반하는이유이다. 그래서질의방추세포상피종은하나의다분화잠재성세포 (multipotential cell) 기원일가능성도제기

Differential diagnosis of a spindle cell dominant tumor of the vagina Gross Location Histology Epithelial marker (IHC) Spindle cell epithelioma WD Lower part No atypism Positive Leiomyoma WD Anywhere No atypism Negative Rhabdomyoma WD Anywhere No atypism Negative Solitary fibrous tumor ID No atypism Negative Aggressive angiomyxoma ID Atypism Negative MMMT Upper part Atypism Negative in spindle cell part MMRSS Upper part Atypism Negative in spindle cell part IHC, immunohistochemistry; WD, well defined; ID, ill defined; MMMT, malignant mu_llerian mixed tumor; MMRSS, malignant mixed tumor resembling synovial sarcoma. 되고있다. 8 본종양에서 estrogen과 progesterone의기능에대해서는아직명확하게밝혀진바가없으나, 본종양이가임기여성에게서호발하는점으로미루어보아종양발생에있어어떤중요한역할을할것으로추정되고있다. 5 현재까지보고된질의방추세포상피종은 20세에서 69세까지분포되어있으며평균연령은 30세였다. 4,5 이들은대부분증상없이우연히발견되었으나질출혈로내원한경우도있었다. 9 재발된경우는모두 3예로수술후재발까지의기간은각각 5개월, 3년, 그리고 8년이었으며 1,10 전이된예는없었다. 5 종양의육안소견은보고된모든예에서같은소견으로피막이없는경계가좋은종괴가팽창형성장을보이며종괴의상부는질상피로덮여있으나질상피와종양과의연결은없었다. 그러나현미경적으로는다소차이를보였는데종양의대부분이기질양방추세포로구성되었고상피세포성분이국소적으로만관찰되었다는점은공통된소견이나, 상피세포의성분인편평상피와선상구조는둘다나타나는경우와둘중각각한가지성분만이나타나는경우가있었다. 본증례의경우선세포의편평상피화생소견은보였으나기존의보고에서특징적인소견으로알려진투명한세포질을갖는섬모양의편평상피는관찰되지않았으며, 선상구조는극히드물게나타나진단에어려움이있었다. 이와같이뚜렷한상피세포성분이잘관찰되지않을때질의양성기질종양인평활근종, 횡문근종, 고립섬유종양 (solitary fibrous tumor) 및침습성혈관점액종 (aggressive angiomyxoma) 과의감별이필요하다. 평활근종과횡문근종의경우육안적으로는방추세포상피종과유사한소견을보이나 cytokeratin 및 epithelial membrane antigen과같은상피성표지자에음성반응을보이는면역조직화학적소견의도움을받을수있고, 고립섬유종양과침습성혈관점액종의경우는방추세포상피종과달리육안적으로경계가불분명하며상기상피성표지자에도음성인점으로감별할수있다. 그러나무엇보다도질에서대부분이방추형세포로이루어진종양을진단할때, 우선면밀한검토로상피세포성분의유무를살펴보는것이가장중요하며, 상피세포를발견하지못했을경우라도질의방추세포상피종을염두에두고 cytokeratin 및 epithelial membrane antigen과같은상피성표지자에대한면역 조직화학염색을시행하는것이도움이될것으로생각된다. 그외에감별해야할악성종양으로는악성 ( 뮐러관 ) 혼합종과활막육종을닮은질의악성종양 (malignant tumor resembling synovial sarcoma) 이있는데이둘은모두상피성및중간엽성성분을보이나종양의위치가질의상부에잘생기며세포의비정형성이뚜렷하고, 유사분열이빈번하나편평상피는관찰되지않는점등으로감별할수있다 (Table 1). 7,11 참고문헌 1.
