The identification of a metastasis from a second primary lung tumor is clinically important for patients with pulmonary metastases of an extrathoracic origin. lthough the cytologic features of metastatic tumors involving the lung have been extensively described, making the cytologic diagnosis is usually not easy in the absence of clinical information. However, the immunohistochemical staining for many tumor markers and the different expressions of cytokeratin 7 and 20 are very useful in the diagnosis. This review presents the cytomorphological spectrum of metastatic tumors along with the immunohistochemical findings. (Korean J Cytopathol 2008;19(1):16-26) Key Words : Lung, Metastasis, Cytology, Immunohistochemistry 서론 폐는대부분의악성종양의전이장소이며최근암발병율이높아짐에따라폐전이의빈도도증가하고있는추세이다. 기존의악성종양을가진환자에서폐에종괴가생긴경우는첫번째로전이를먼저의심해야하지만원발성폐암과비종양성병변도아울러감별진단에생각하여야할것이다. 원발성폐암과전이성종양의치료는판이하게다르므로그감별진단은임상적으로무척중요하다. 악성종양이폐로전이되는양상은매우다양하며원발종양의위치에따라특징적인소견을보일수있다 (Table 1). 1 가장흔하게볼수있는방사선학적소견은주로는한개또는 여러개의분리된결절이나간질성침윤 (interstitial infiltrate) 이며드물게기관지내병변과함께무기폐나간질성폐렴을동반하는양상을보이기도한다. 2 이들의감별진단에흔히사용되는방법은세포학적검사이며전이성종양의절반가량이세포학적검사에서발견되는것으로알려져있다. 3,4 세포학적검사로는객담검사, 기관지세척, 흉수및세침흡인검사를들수있다. 세포학적검사로원발성폐암과전이성종양을감별하는것은대부분의경우쉽지않기때문에확실히전이성병변을증명할수있을때까지는원발성폐암의가능성을배제해서는안된다. 이를위해서는임상적양상및기존종양의조직학적소견에대한충분한검토와비교가매우중요하지 16 대한세포병리학회지제 19 권제 1 호 / 2008

C-225 세포들도 관찰된다 (Fig. 2). 종양세포들은 대체적으로 핵 는 유방암의 98% 정도에 양성이어서 매우 예민한 항체이 /세포질 비율이 높고, 핵소체가 뚜렷하게 보이는 거품핵이 나 폐선암종의 74%, 난소암종의 80%에서 양성이므로 그 나 과염색상의 핵을 갖고 있다 (Fig. 2). 소엽형 (lobular 변별력이 떨어지고 이 경우 C 19-9과 CE의 염색이 함 carcinoma)인 경우는 핵이 작고 세포질내 점액성 공포가 께 이루어지면 그 변별력이 높아진다 (Fig. 3, Table 5).15,16 핵을 밀어 마치 반지세포처럼 보이는 경우도 있으며 드물 최근에는 cytokeratin 7 양성/cytokeratin 20 음성인 선암 게 소세포암종과 같은 세포소견을 보이기도 한다. 특히 성 종의 감별 시 유방암에 비교적 특이적인 mammaglobin이 인 여자의 악성 흉수의 원인으로 유방암과 폐암이 제일 흔 감별에 도움을 줄 수 있다는 보고가 있다.17 하며 이들의 감별이 중요하다. 이 두 암종 모두 cytokeratin 7 양성/cytokeratin 20 음성이며 ER, PR이 양성인 경우 유 3) 전립선암 방암으로 진단하는데 도움을 받을 수 있다 (Fig. 2C & D). 폐에 전이된 전립선암은 그 빈도가 드물고 다양한 조직 그러나 ER은 항체의 클론에 따라 폐선암종에서도 17 학적 소견을 보이기 때문에 원발성 폐선암종, 신경내분비 80%까지 양성인 것으로 보고되어 있기 때문에 ER 양성만 암종 및 다른 전이성 암종과의 감별이 어렵다.18 미세소엽 으로는 유방암으로 확진할 수 없다.13,14 따라서 ER이 양성 형태를 이루는 분화가 좋은 전형적인 전립선암의 세포들은 C D Fig. 2. Most Predictive Multiple-marker Phenotypes for Each Primary Site16 핵/세포질 비율이 높고 핵소체가 또렷한 양상을 보이며 대 Upper GI 로 진단에 어려움이 없다 (Fig. 4). 그러나 분화도가 나빠지 Tract 면 세포의 비정형성으로 인해 원발성 선암종과의 감별이 CE+ C19-9+ C125+ 어려워진다. 또한 분화도가 나쁜 전립선암종이 전이된 경 Colon reast Lung Ovary CE+ C225+ C225+ C125+ C225- CECE+ CEC125- C125- C19-9- C19-9- 부분의 경우 prostate-specific antigen (PS)에 양성이므 우에서 신경내분비성 분화도를 보일수 있는데 이때 대부분 Sensitivity 의 경우 세포들은 괴사물질과 오래된 혈액이 뒤섞여 더러 Specificity 운 도말배경에 작고 개개로 흩어지는 양상을 보인다.19,20 PPV NPV % Correct PPV=positive predictive value, NPV=negative predictive value, % Correct=percentage of correct prediction of primary site. 종양세포들은 작고 둥글거나 난원형의 핵을 갖고 있으며 핵/세포질 비율이 놓고 핵소체는 뚜렷하지 않다. 핵염색질 은 골고루 분포되어 있고 핵이 서로 붙어 찌부러지거나 뭉 개지는 양상을 보여 마치 원발성 폐소세포암종을 보는 듯 하다. C 주 나쁜 경우에는 음성일 수 있으니 유의해야 한다.19,20 아 비정형성이 심해져 핵의 과염색과 큰 핵소체 등이 자주 관 울러 전립선암은 TTF-1 음성이며 cytokeratin 7과 20에 대 찰되며 간혹 육종 모양을 보이기도 한다. 신장암은 cytok- 부분 음성이다.5 eratin 20과 TTF-1에 모두 음성이다.5 반면에 cytokeratin 7은 투명세포유형의 신장암에서는 음성이나 혐색소성유형 4) 신장암 과 유두모양유형의 신장암에서는 양성이고, vimentin은 신장암은 특히 전이성 병변이 첫 임상 증상으로 나타나 이와 반대의 양상을 보이며 폐선암종에서는 38 47%에서 는 경우가 약 1/3을 차지하기 때문에 세포검사에서 전이성 만 양성이다 최근에 CD10과 RCC Ma가 신장암에 특 병변을 인지하는 것은 환자의 진단과 치료에 큰 역할을 차 이적인 표지자로 각광 받고 있다 지한다.2 세포도말 슬라이드는 대부분이 혈액성 또는 괴사 성 배경을 갖고 있으며 세포들은 밀착된 군집이나 개개의 5) 간세포암 세포들로 흩어지는 양상을 보인다 (Fig. 5 & ). 전이성 폐에 전이된 간세포암은 원발성 간암과 유사하게 기둥 종양을 신장암으로 인지하는데 가장 중요한 소견은 세포질 모양 (trabecular/sinusoidal), 세엽 또는 다층으로 배열되 에 풍부한 지질성 공포를 갖고 있는 전형적인 투명세포이 어 있다 (Fig. 6). 세포는 정다각형이며 핵은 중앙에 위치 다 (Fig. 5C & D). 종양세포의 핵은 크고 염색질은 옅으며 하고 세포질은 다양하게 함유하고 있다 (Fig. 6). 분화가 한 개 또는 여러 개의 핵소체를 갖고 있다. 종종 핵내 봉입 좋은 경우는 핵내 세포질 봉입체를 볼 수 있으며 간혹 유리 체를 관찰할 수 있다. 그러나 분화도가 나쁜 경우는 핵의 질 체와 담즙색소를 관찰할 수 있어 간세포암으로 진단하 The Korean Journal of Cytopathology Vol 19 No 1 /

(D) Tissue section of clear cell renal cell carcinoma from a lung specimen (H&E). 기 어렵지 않으나 분화가 좋지 않은 경우는 선암종과의 감 나타나는 소견이기에 이 둘의 감별이 간혹 어려울 수 있 별이 종종 어렵다.27 Cytokeratin 7은 종양의 80%에서 음 다.32 또한 이 두 종양 모두 TTF-1 양성, cytokeratin 7 양 성이고 cytokeratin 20에는 대부분이 음성이며, 아울러 성, cytokeratin 20 음성 및 galectin-3 양성으로 유사한 면 TTF-1이 음성인 점은 원발성 선암종과의 감별에 유용하 역조직화학적 양상을 보이기 때문에 그 감별은 더욱 어려 다.28,29 또한 HepPar1은 강도의 차이는 있지만 모든 간세 울 수 있고 이 경우 갑상샘 유두암종에만 양성 반응을 보이 포암종에서 양성이기 때문에 폐의 원발성 암종이나 다른 는 thyroglobulin만이 도움을 줄 수 있다.33 사종체는 이 두 전이성 암종과의 구분에 매우 유용하다.30 종양 외에도 난소 종양, 자궁내막 또는 자궁관 암종에서도 관찰 될 수 있으므로 감별진단에 이러한 종양의 전이도 포 6) 갑상샘 유두암종 함시켜야 하며 이 때는 환자의 병력과 임상 소견이 중요한 갑상샘 유두암종의 원위 전이는 약 8 30%까지 보고되 정보가 될 수 있다. 대부분의 경우는 폐 전이가 발생하기 어 있으며 특히 그 전이 부위로 폐가 제일 흔하다.31 갑상샘 전에 골반 종괴나 복수, 복강 내 전이가 발견된다. 유두암종의 특징인 유두상 형태, 핵내봉입체 및 사종체는 비점액성 세기관지폐포암의 경우와는 달리 점액성 세기 폐의 전이성 병변에서 갑상선 기원을 밝히는데 도움을 주 관지폐포암은 점액을 분비하는 모든 전이성 선암종과의 감 지만 이러한 소견은 폐의 비점액성 세기관지폐포암에서도 별진단 시 포함시켜야 할 종양이다. 대장암, 유방암, 췌장 22 대한세포병리학회지 제19권 제1호 / 2008

(H&E). 암, 위암, 신장암및전립선암등이점액성세기관지폐포암과유사한세포학적양상을보일뿐아니라조직학적으로도폐포관을따라전이하는 lepidic 양상 을보여형태학적배열도점액성세기관지폐포암을닮았다. 34 또한점액성세기관지폐포암은전형적인폐선암종과는달리 TTF-1에음성이고 cytokeratin 20에양성이어서세포진단에혼선을야기할수있다. 13,33 2. 편평세포분화암종의감별폐의원발성편평세포암종과의감별을요하는전이성암종으로대표적으로두경부암종과자궁경부암종을들수있다. 특히두경부암종의환자들은종종심한흡연자이기때문에이차적인원발성폐암종의발생위험이높다. 또한편평세포암종은아니지만그러한분화를보이거나비슷한세포양상을보이는요로상피암종을들수있다. 또한어느유형의전이성암종이더라도변성이된경우는세포질이호산성을띨수있기때문에단지이것만을근거로편평세포암종으로진단해서는안된다. 1) 두경부암종전이성두경부암종과폐의원발성편평세포암종의감별은세포학적으로감별이불가능하며면역조직화학검사도도움이되지않는다. 폐의병변이전이성을시사하는경우는두경부암종의임상병기가 3 또는 4기로높고, 폐병변이 발견된당시두경부부위에재발이된경우, 폐병변이말초부위에있고여러개인경우와두경부종양발생후 3년이내에폐병변이발견된경우이다. 35 2) 요로상피암종전이성요로상피암은폐에전이되는암종의약 3% 를차지하며대부분의경우는분화도가나쁘기때문에세포학적검사로그원발성병변을쉽게진단하기어려운유형이다. 36 대부분의경우별다른특징이없는세포들로이루어져있거나방추형, 기둥형또는입방형의세포들로이루어져있다 (Fig. 7). 37 Renshaw와 Madge 36 는 cercariform cell로지칭하는핵이한쪽으로치우쳐있고둥글거나넙적한꼬리를갖는긴세포질이요로상피암에특징적이라고기술하고있다 (Fig. 7). 그러나이러한세포들은분화가나쁜악성상피종양에서도관찰되는것으로알려져있어꼭요로상피암에특이적이라고단정지을수는없다. 38 세포질은편평상피또는샘상피의분화를보여세포질이짙거나세포질내공포를관찰할수있다. 또한 Renshaw 등 39 은흉수에서관찰되는전이된요로상피암종의세포질에호산성봉입물이종종관찰되는것을보고하였으며이는아마도변성된세포질내미세섬유로생각된다. 요로상피암종은 cytokeratin 7에대부분양성이며 50% 에서 cytokeratin 20 에양성이다. 40 또한전이성요로상피암종의 60 80% 에서요로상피암종에매우특이적인 uroplakin III에양성이다. 41, 42 p63은요로상피암종과편평세포암종둘다에서양 The Korean Journal of Cytopathology Vol 19 No 1 /

(Papanicolaou stain). 성이기때문에감별에도움이되지않는다. 5,43 3. 소세포암종과의감별폐의원발성소세포암종과의감별로는폐외소세포암종, Merkel 세포암, 분화나쁜편평세포암과전립선암및림프종을들수있다. 44 폐의원발성소세포암종과폐외소세포암종은세포학적으로감별이불가능하며둘다 TTF-1 에양성이고다른신경내분비표지자도유사한염색양상을보이기때문에감별이어렵다. Merkel 세포암은 cytokeratin 7 음성, TTF-1 음성및 cytokeratin 20이양성이어서원발성폐소세포암종과의감별이용이하며 neuron-specific enolase는둘다에서양성이다. 28 분화가나쁜편평세포암종과의감별에는 p63이도움을주며림프종과의감별에는 leukocyte common antigen 검사가유용하다 기타전이성종양드물지만흑색종, 육종및림프종이폐에전이된다. 2 흑색종은악성종양의모든유형을닮을수있는천의얼굴을갖는게특징이며폐의전이성흑색종은멜라닌색소를함유하지않는종양세포들로이루어지는경우가많기때문에원발성병변에대한임상적정보가없는상태에서는진단이어려울수있다. 그러나종양세포가아주큰핵소체를갖고있으며핵내세포질봉입체를보이면흑색종을염두에두어야하며만일갈색의멜라닌색소를함유하고있으 면확실히진단이가능하다. HM-45, Melan- 및 S-100 단백의면역조직화학염색결과가도움을준다. 전이성육종은전이성암종보다그빈도가떨어지지만폐의원발성육종보다는빈번하기때문에세포검사에서육종의소견을관찰하였을때그가능성을꼭생각하여야하며또한육종성암종도감별에포함시켜야한다. 육종세포들은밀착도가강한군집을형성하는암종에비해느슨한형태를형성하며개개의세포들은암종이나육종모두에서관찰되나육종의경우비정형성이더심하다. 45 림프종역시어느유형이던지폐를침범할수있고특히소세포림프종인경우는양성림프구나폐의소세포암종과의감별을요한다. 결 론 폐의전이성종양을진단하는데세포학적검사가유용하게사용될수있으나정확도를증가시키기위해서는임상소견, 원발성암의조직및세포슬라이드의재조사가꼭필요하며면역조직화학검사가보조적으로유용하게사용될수있다. 참고문헌 1.
