1 = 증례보고 = 대한안과학회지 2013 년제 54 권제 7 호 J Korean Ophthalmol Soc 2013;54(7): pissn: eissn: 국소아밀로이드증 (Amyloidosis) 이동반된안와점막연관림프조직형림프종 (MALT lymphoma) 1 예 유인정 우경인 김윤덕 성균관대학교의과대학삼성서울병원안과학교실 목적 : 안와내에발생한국소아밀로이드증이동반된점막연관림프조직형림프종증례 1 예를경험하였기에이를보고하고자한다. 증례요약 : 61 세남자가내원 5 개월전부터시작된복시를주소로내원하였다. 안과검사상 2 mm 의좌안안구돌출과좌측및상하주시시좌안안구운동제한이관찰되었다. 자기공명영상에서좌측안와하부에조영이잘되는경계가불분명한침윤성종괴가확인되어좌안절개생검을시행하였다. 조직병리검사상혈관과간질에무정형의호산성섬유소물질의침착과소림프구침윤이관찰되었다. 면역조직화학염색상소림프구는 CD20 및 κ-light chain 에양성을보였으며, 호산성섬유소물질은 κ-light chain 에양성을보이고 Congo red 염색에서이색성및녹색복굴절이있음을확인하였다. 결론 : 안와내에발생한국소아밀로이드증이동반된점막연관림프조직형림프종은전세계적으로매우드물고국내에서는지금까지보고된예가없어이를보고하는바이다. < 대한안과학회지 2013;54(7): > 아밀로이드증은세포외간질에비용해성단백질성섬유가침착되는질환으로안와내침범은흔하지않은것으로알려졌다. 또한안와내에발생한림프종에서국소아밀로이드증이동반되는경우는더욱드물어세계적으로도소수의증례만이알려졌으며, 국내에서는아직보고된바가없다. 이에저자들은안와내에발생한국소아밀로이드증이동반된점막연관림프조직형림프종 (MALT lymphoma) 증례 1예를경험하였기에이를보고하고자한다. 증례보고 63세남자가내원 5개월전부터시작된복시를주소로타병원방문하여시행한자기공명영상검사에서좌측안와내종괴가발견되어본원에의뢰되었다. 과거력상고혈압외당뇨, 갑상선질환의병력은없었으며가족력도특이사항을보이지않았다. Hertel 안구돌출계검사에서우안 12 mm, 좌안 14 mm (bar 100 mm) 로좌안에 2 mm의안구돌출이관찰되었고좌측안와하연에경계가명확하지않은종괴가촉진되었다. 상대구심동공장애는관찰되지않았다 (Fig. 1). 외부에서시행한자기공명영상에서좌측안와하부의원추안팎에걸쳐있는경계가불분명한침윤성종괴가관찰되었다. 종괴는 T1 강조영상에서외안근과동등한신호강도를보이고 T2 강조영상에서는부분적으로높은신호강도를보였으며, 조영증강영상에서종괴의균질한조영증강이관찰되었다 (Fig. 2). 전산화단층촬영을통해종괴내부에석회화가존재함을확인하였다 (Fig. 3). 진단을위해국소마취하에좌안아래결막구석을통해절개생검을시행하였다. 종괴는주변조직과구별이잘되지않는양상이었다. 조직병리검사에서종괴는소림프구침윤과함께혈관및간질에무정형의호산성섬유소물질의침착을보였다. 면역조직화학염색상소림프구는 CD 20 및 κ-light chain에양성이었고, 무정형호산성섬유소물질또한 κ-light chain에양성을보였다. 무정형호산성섬유소물질은 Congo red 염색에서이색성을보였고편광현미경상녹색복굴절이있음을확인하여, 아밀로이드증을동 1109

대부분의 안와 림 프종은 B세포 기원의 비호지킨성 림프종이며 이 중 MALT Figure 3.

MALT lymphoma는 서구의 안 와 및 안부속기에 발생하는 림프증식 질환 중 50-78%, 일 2 본과 우리나라에서는 약 80-90%을 차지한다. 면역 조직 반한 MALT lymphoma로 진단되었다(Fig. 4).

(C) T1-weighted postcontrast image demonstrates a well-enhancing soft tissue mass. 상 MALT lymphoma의 재발 증거는 보이지 않았고 아밀로 이드증만이 확인되었다. 고 찰 안와에 원발성으로 발생하는 림프종은 매우 드물고 모든 1 악성 림프종의 약 1% 정도로 알려졌다. 대부분의 안와 림 프종은 B세포 기원의 비호지킨성 림프종이며 이 중 MALT Figure 3. MALT lymphoma는 서구의 안 와 및 안부속기에 발생하는 림프증식 질환 중 50-78%, 일 2 본과 우리나라에서는 약 80-90%을 차지한다. 면역 조직 반한 MALT lymphoma로 진단되었다(Fig. 4). 헬리코박터 화학 염색상 CD5와 CD10에 음성이며, CD20 및 CD79a에 및 클라미디아 감염에 대한 중합효소연쇄반응 결과는 음성 양성을 보인다. 3 이었다. 골수생검을 포함한 전신검진 상 안와 외 악성 림프 아밀로이드증은 여러 원인에 의해 불용성의 무정형 초자 종이나 전신 아밀로이드증의 증거는 관찰되지 않았으나, 전 질 단백질성 섬유가 전신 및 국소적으로 세포 외 간질에 침 립선암이 발견되었으며 비뇨기과 의뢰되어 호르몬 치료를 착하는 질환이다. 아밀로이드증은 침범범위에 따라 전신성 시행하였다. 좌측 안와 병변에 대하여 30 Gy의 국소 방사 과 국소성으로 나뉘고, 원인에 따라 일차성, 이차성 그리고 선 치료를 시행하였으나, 좌안 안구돌출은 5 mm로 증가하 유전성으로 분류한다. 아밀로이드 단백질의 특성에 따라 였으며 방사선 치료 후 8개월째 시행한 전산화단층촬영에 AA (amyloid associated), AL (amyloid light chain) 및 서 종양의 크기는 감소하지 않았다. 이에 대하여 국소마취 ATTR (transthyretin) 등으로 나누기도 한다. 일차성 아밀 하에 좌안 절개생검을 추가로 시행하였으며, 조직병리검사 로이드증(대부분 AL 아밀로이드증)은 다발성 골수종과 같 1110

의 침착으로 하나 이상의 장기나 조직에 아밀로이드 물질 구운동장애를 동반한 안와 종괴 환자에서 조직검사를 통하 이 침범하는 것이며, 이차성 아밀로이드증(대부분 AA 아밀 여 MALT lymphoma와 아밀로이드증을 진단하였다.

마지막으로 유전성 아밀로이드증은 가장 드문 형 내에 발생한 병리생태학적 기전은 크게 두 가지로 생각해 태로써 ATTR 등 유전자 변이에 의해 발생한다. 그러나 최 볼 수 있다.

4 - 대한안과학회지 2013 년제 54 권제 7 호 - 종에따른 AL (amyloid light chain) 단백질이증가로아밀로이드증이발생하였을것이라추정된다. 안와내국한된 MALT lymphoma의치료는방사선치료가현재까지가장효과적이고탁월한것으로알려졌다. 한편국소아밀로이드증이단독으로발생한경우증상이없는경우에는경과관찰을하거나, 외과적인절제로임상증상의개선을가져올수있다고알려졌다. 최근에는국소아밀로이드증에서도방사선치료가도움이된다는보고들이있다. 6,15 Khaira et al 15 은아밀로이드증이동반된 MALT lymphoma 1예에서 34 Gy 방사선치료후안구돌출과안구운동장애가호전되었음을보고하였다. 그러나치료후에도지속적으로안와종괴가촉진되어추가적인조직검사를시행하였으며, 림프종의증거없이아밀로이드증만이남아있음을확인하였다. 15 본증례의경우방사선치료에대한반응이좋지않았고안구돌출도진행되어조직검사를추가로시행하였고, 그결과앞의증례와같이 MALT lymphoma의재발증거는보이지않았으며아밀로이드증만이관찰되었다. 이는국소아밀로이드증이동반된 MALT lymphoma의경우치료후림프종이호전되더라도아밀로이드증이남아있는경우영상검사만으로는치료효과를판단하기에제한이있어추가적인조직검사가필요할수있음을시사한다. 추후더많은증례및장기간의경과관찰을통해국소아밀로이드증이동반된 MALT lymphoma의치료및예후에대한추가적인연구들이필요할것이다. Case summary: A 61-year-old male visited our clinic with diplopia, which had developed approximately 5 months earlier. Magnetic resonance imaging of the orbit showed an ill-defined well-enhanced mass in the left inferior orbit. Incisional biopsy of the orbital mass was performed. Histopathological examination revealed diffuse deposits of an amorphous, eosinophilic substance in the extracellular matrix and vessels with lymphocytes infiltration. Lymphocytes were positive for the immunohistochemical stain against the CD20 and κ-light chain antigens. The amorphous material stained positive for κ-light chain antigen, and Congo red staining showed birefringence. Conclusions: We report a rare case of primary orbital MALT lymphoma associated with localized amyloidosis.
