2 1. 초기용량 : 성인 1일 2회, 1회 100mg ACE inhibitor나 ARB를복용하고있지않은환자또는이전에상기약물들을저용량으로복용하고있던환자, 중증신장애환자 (egfr<30ml/ 분 /1.73m 2 ), 중등증간장애환자 (Child-Pugh B) 는초기용량 1일 2회, 1회 50mg으로시작 용법용량 2. 유지용량 : 성인 1일 2회, 1회 200mg 내약성에따라매 2~4주간격으로두배씩증량하여목표유지용량인 1일 2회, 1회 200mg 도달내약성에문제가있는경우 ( 증상이있는저혈압, 고칼륨혈증, 신기능장애등 ), 병용약물의조정또는일시적인감량고려 금 기 *Valsartan 성분을포함하고있으므로 ARB와병용해서는안됨 * 혈관부종의위험성이있으므로 ACE inhibitor와병용하지않음 ACE inhibitor 투여중단 36시간이후에이약투여시작 1. ACE inhibitor 또는 ARB 투여시혈관부종의병력이있는환자 4. 유전성또는특발성혈관부종환자 5 임부또는임신하고있을가능성이있는여성및수유부 6 중증의간장애환자, 간경화증또는담도폐쇄, 담즙정체환자 7 당뇨병이나중등증 ~ 중증의신장애환자 (egfr<60ml/ 분 /1.73m 2 ) 에서알리스키렌과의병용투여 8 원발고알도스테론증환자 1. 매우흔하게 : 고칼륨혈증, 저혈압, 신기능장애 이상반응 2. 흔하게 : 빈혈, 저칼륨혈증, 저혈당, 어지러움, 두통, 실신, 현기증, 기립성저혈압, 기침, 설사, 구역, 위염, 신부전, 피로, 무력증 3. 흔하지않게 : 과민증, 체위성어지러움, 가려움증, 홍반, 혈관부종 1. 임부 1) 임신가능성이있는여성 : 이약의마지막투여후 1주일까지피임해야한다. 2) 임부 : 임부에게이약이투여된경험은없으나, 레닌-안지오텐신-알도스테론계 (RAAS) 에작용하는다른약물과마찬가지로태아에대한위험을무시할수없다. 임신중또는임신을계획하고있는여성에게이약을투여해서는안되며투약중임신이확인되는경우즉시투여를중지한다. 1Valsartan: 임신 2~3기에 ACE inhibitor에노출된임부에서태아및신생아의저혈압, 고칼륨혈증, 신생아두개골발육부전, 요감소그리고 / 혹은무뇨증, 가역적또는비가역적인신부전, 자궁내성장지체및사망을포함하여태아및신생아손상과관련성이있었다. 태아신기능감소때문인임부 / 수유부것으로추측되는양수과소증이보고되었고, 양수과소증은태아사지구축, 두개안면기형및폐발육부전과연관성이있었다. 후향적자료에의하면, 임신 1기에 ACE inhibitor를사용하는것은출생결함의잠재적위험과연관이있었다. 또한발사르탄에의한것인지명확하지는않지만미숙, 자궁내발육지연, 동맥관열림증이나타났다는보고가있다. 임부가우발적으로발사르탄을복용했을때, 자연유산, 양수과소증, 신생아신기능이상이나타났다는보고가있다. 임신 2기이후로임부가이약에노출되었을경우, 태아의신장기능과두개골초음파검사가권장된다. 또한, 자궁내에서이약에노출된경험이있는신생아의경우충분한배뇨, 고칼륨혈증, 혈 압을면밀히조사하여야한다. 2

8 Background Apalutamide 는 Androgen 수용체의경쟁적저해제로현재전립선암치료약물로개발중이다. 전이위험이높은비전 이성거세저항성전립선암환자에서 Apalutamide 의효과를평가하였다. Methods 비전이성거세저항전립선암과 10개월이내전립선특이항원 (PSA) 수치가두배로나타난남성을대상으로이중맹검, 위약대조, 3상연구를진행하였다. 환자는 Apalutamide 투여군 (240mg/day) 또는위약군을 2:1 비율로무작위배정하였다. 모든환자들은안드로겐차단요법 (androgen-deprivation therapy, ADT) 을지속하였다. 일차종결점은무진행생존기간 (metastasis-free survival) 이며, 무작위배정이후원격전이가발견되거나사망까지의기간으로정의하였다. Results 전체 1,207명이 Apalutamide군 806명, 위약군 401명으로무작위배정되었다. 378건의사건이발생한이후계획된주요분석이시행되었으며무진행생존기간중앙값은 Apalutamide군 40.5개월, 위약군 16.2개월이었다 (HR for metastasis or death, 0.28; 95% CI, 0.23 to 0.35; P<0.001). 증상이나타난전이까지의기간은위약군보다 Apalutamide군에서유의하게길게나타났다 (HR, 0.45; 95% CI, 0.32 to 0.63; P<0.001). 임상연구처방을중단하게된이상사례는 Apalutamide군에서 10.6%, 위약군에서 7.0% 였다. 다음이상반응은 Apalutamide군에서위약군보다더높은비율로발생하였다 : 발진 (23.8% vs. 5.5%), 갑상선기능저하 (8.1% vs. 2.0%), 골절 (11.7% vs. 6.5%).
