1 Lymphatic system of the Head and Neck 123-> right lymphatic duct 2) 오른쪽상반신을제외한그밖의모든영역에서오는림프 thoracic duct 에모인다. ; 이표는참고만하세요. ^^; 1. 조직에서스며나온림프의순환경로 여러개의수입림프관 (afferent lymphatic vessel) 림프절 (lymph node) 림프절의수출림프관 (efferent lymphatic vessel) 1) 몸의우상부에서오는림프 1 머리목에서오는림프 -> Jugular trunk 2 팔다리에서오는림프 -> Subclavian trunk 3 가슴에서오는것 -> Right bronchomediastinal trunk thoracic duct 1) 창자쪽에서오는림프가 celiac trunk 와 intestinal trunk 에모이고이것이양쪽의 lumbar trunk 에이르러 thoracic duct 의시작부위인 cisterna chyli 에합류한다. 2) cisterna chyli( 가슴림프팽대관 ) 은첫번째허리뼈와 12 번쨰등뼈부위에위치한다. 3) thoracic duct 는 cisterna chyli 에서시작하여 5 번째의등뼈를기준으로 5 번째아래에서는오른쪽측면을타고가고위에서는오른쪽측면을타고간다.( 아래 course of thoracic duct 그림참고 ) 4) 일곱번째목뼈높이에서활모양으로휘어진다. 5) thoracic duct 는왼쪽부분의머리, 목, 왼팔, 오른다리, 왼다리, 왼쪽가슴, 배벽, 배공간, 골반내장에서오는림프를받는다. 6) thoracic duc( 가슴림프관 ) 의길이는 38-45cm 이고오른림프관보다길다.

superficial and deep cervical lymph nodes 2) deep cervical nodes 1 deep cervical nodes( 깊은목림프절 ) 은 internal jugular v. 을따라형성된림프절의집단 2 deep cervical nodes 은 upper와 lower 부분으로나뉜다.

4 superficial and deep cervical lymph nodes 2) deep cervical nodes 1 deep cervical nodes( 깊은목림프절 ) 은 internal jugular v. 을따라형성된림프절의집단 2 deep cervical nodes 은 upper와 lower 부분으로나뉜다. -upper 부분 jugulodigastric node 라하고 tonsillar지역에있는림프관를받는다. -lower 부분 juguloomohyoid node 라하고 tongue로부터의림프관를받는다. 3 deep cervical nodes은결국직접또는주변부위의림프절무리를통해머리와목에서오는림프배출을받는다. deep cervical nodes에서나온림프관은 right & left jugular trunk를형성하며, 이것은오른쪽에서오는 right lymphatic duct 와왼쪽에서는 thoracic duct로들어간다. 1) Superficial cervical nodes 1 Superficial lymph nodes부터림프들은 SCM의얕은면에있는 External jugular v. 을따라내려가는 Superficial cervical nodes 라에모이게된다. 즉 Superficial cervical nodes는 External jugular v. 을따라배열된림프절의집합체이다. 2 여기서모아진림프들은 lymphatic v. 을타고 deep cervical node 에보내어진다.

5 general arrangement of lymph nodes ; 앞에서배운림프노드의위치를확인하시면됩니다. posterior cervical node는임상적으로 CN XI인뇌신경이지나가므로이부분에있는림프절에종양및제거시 SCM, trapezius m. 이마비가올수있다. level system of nodal enlargement( 중요 ) ; 머리, 목부위에종양의림프절전이를평가하는데매우유용. ;fig 1. 의그림이랑참고하셔서보세요

8 1) Oral vestible은치아, 입술, 볼사이의좁은공간 2) Oral vestible은뒤쪽으로 second superior molar tooth가있는곳에서 parotid duct의개구부가있다. 3) 다수의 frenulum( 주름띠 ) ; 과도한움직임을막아주기위한띠 1frenulum of upper lip( 위입술주름띠 ) 2 sup. buccal frenulum( 위볼주름띠 ) 3 frenulum of lower lip( 아랫입술주름띠 ) 4inf. buccal frenulum( 아래볼주름띠 ) 4) Gingiva(gum, 잇몸 ) 구조도 Oral vestible에서관찰 oral cavity proper 1) Oral cavity proper은입안을말하며, 치아뒤에서 pharynx까지를말한다. 2) 앞쪽은치아가식립된치아궁으로둘러져있고뒤쪽은목구멍을통해인두와연결아래는혀가대부분차지한다. 3) Submandibular gland 와 sublingual gland의도관이개구한다. 이때혀뒤쪽의 sublingual caruncle는 Subma ndibular gland에 sublingual fold 는 sublingual gland에연결된다

9 teeth 4) Oral cavity 위쪽으로 hard palate와 soft palate가있으며 soft palate 앞쪽부분에는 palatine aponeurosis 가있어서이부분이말랑하고움직일수있다. 5) soft palate 뒤쪽에는 palatoglossal arch와 palatine tonsil, palatopharyngeal arch 구조가있다. 6) palatine tonsil은감기가걸리면염증이생긴다. 1) 차아의종류및기능 Incisors(cut), Canines(tear), Premolars and Molars(crush) 2) 치아뿌리는위턱과아래턱뼈에묻혀있으며치은에의해덮혀있다. 3) 치아발달은유치는태어나 6달부터시작하여총 20개이며영구치의겨우에는 6살부터시작하여총 32개이다. ;dental pulp 는결합조직으로채워져있으며 apical foramen 을통하여신경과혈관들이들어옮.

가나오는구멍이얼굴에 Infraorbital foramen에서나오고이신경의경우치통이얼굴통증까지동반하게됨.

10 innervation gums and teeth ; 치은과치아에부위별신경들을확인만하자. ( 복잡해서시험에안나옴이라고하심 ). ; 위의신경들이 innervation 그림. Infraorbital n. 의경우 incisor과 canines 쪽에분포. Infraorbital n. 가나오는구멍이얼굴에 Infraorbital foramen에서나오고이신경의경우치통이얼굴통증까지동반하게됨.( 위 아래그림은왼쪽그림의신경과연결하여확인만하세요 ) - 왼쪽은치은쪽에분포하는신경이고오른쪽은치아쪽분포신경 - maxillary 의경우에는 CN V₂에서 innervation, mandible 의경우 CN V₃에서 innervation 신경들이치아와치은에분포. - 위쪽의 palate 부분의 palatine n, 분포. 아래쪽의혀가있는부위는 lingual n. 가분포.

12 ; cheek에서는 superficial fascia에있는구조들을주로알아두시면됩니다. ; risorius m. 의경우입을다무는역할을하는데 risorius m. 이입꼬리에서뺨쪽으로있는변형이생기거나성형수술시보조개가생성. Plate ;SMAS 는성형수술시여러얼굴근육이연결되어얼굴주름을 펼수있다. -SMAS 에연결된근육 Frontal belly of occipito-frontalis m.(superiorly), Platysma m.(inferiorly), Zygomaticusmajor m.(anteriorly) ; 입천장은입안의천정을이루고, 앞쪽 2/3 부분은뼈로이루어 져 hard plate 라하고뒤쪽 1/3 은근육으로이루어져 soft plate 라하며, 입안과코안의경계를이룸.

단단입천장은정중부위에는 palatine raphe라는능선이있고, 이능선의앞끝에는 incisive papilla가있으며, incisive papilla는 incisive foramen 을덮고있다.

13 ; 신경과 artery 는위에표시한것위주로암기하시면될것같 아요. 특히 posterior septal branch 와 great palatine a. 는 kisselbach s artery 와연결해서보세요. hard plate 앞부분에는 transverse palatine fold 가있으며짐 승에서발달사람에서는퇴화어린시절관찰 - Hard plate ;ppt글위주로보시고부연설명만첨부할께요. 단단입천장은정중부위에는 palatine raphe라는능선이있고, 이능선의앞끝에는 incisive papilla가있으며, incisive papilla는 incisive foramen 을덮고있다. 말하고음식을먹는데기능함. 위의질병은 cleft plate로발음이새고음식물이코로들어갈수있음. 특히아래그림은언청이 (=cleft lip) 라고하며 cleft plate가얼굴쪽으로뻗어나온경우. ; 아래의그림은 nerve라구멍이랑다시한번확인하시라고넣은그림.

21 1 양쪽이 2개로침샘중가장크다. 2 Serous한성분으로구강내의 20% 의침을생성. 3 Parotid duct=stensen duct라고하는관을통해서배출. 4 위치는위의 ppt에서확인 5 Parotid gland에는얼굴근육에분포하는얼굴신경이지나감. 이신경에의해 gland가얕은층부분과깊은층부위로나뉨. 6 Parotid gland의위에는 accessory Parotid gland가있을수있다. 2) Parotid fascia (1) 목부분의 5개의층 1 Skin 2 Subcutaneous layer : platysma 존재 3 Superficial layer of cervical fascia 4 Infrahyoid m. 5 Great vessels: common carotid a.,ext. jugular vessels 들이존재. (2) superficial layer는 parotid gland의 margin에서두개로갈라지는데한쪽은샘의겉을싸고또다른한쪽은깊은면을싼다. 이렇게갈라져샘을싸는막을 Parotid fascia 라한다. (3) Superficial layer의경계위 zygomatic arch, 앞의 massmass fascia, 뒤는 SCM (4) Stylomandibular ligament는 Parotid fascia 가두꺼워진부분.

23 5) Submandibular gland 4) Structures passing through parotid gland 1 Submandibular gland는대부분턱밑삼각속에위치하고 digastric m. 위에위치 2 혼합샘으로구강내에 65-70% 의침을생산 3 Submandibular duct(wharton duct) 을통해서침이구강으로공급 4 Mylohyoid m. 와 hyoglossus m. 사이구조물들은확인 5 Submandibular gland는 Mylohyoid m. 의위아래로있음.

4 Sublingual gland는구강내침의 3-5% 를생산. ; Mylohyoid m. 와 hyoglossus m.

24 에위치. 3 Sublingual gland의분비관 -major sublingual duct는 sublingual caruncle으로열림. -minor sublingual duct는 Rivinus duct로연림. Rivinus duct의경우앞서두샘의 duct와달리관이얇고가늘면서여러개의관구조로되어있음. 4 Sublingual gland는구강내침의 3-5% 를생산. ; Mylohyoid m. 와 hyoglossus m. 사이 vena comitans vein 이 12 번뇌신경 위에서지나가는것을보여주는그림임. 6) Sublingual gland 1 Sublingual gland는침샘침샘중가장작고, 좌우 2개가있으며혼합선이다. 2 Sublingual gland은압안바닥에 submandibular gland 앞쪽
