Our study was performed to evaluate the clinical characteristics and prognosis of TBN patients with proteinuria. ethods : We conducted a retrospective study on 231 kidney biopsies. A urine protein level more than 500 mg for the 2hour urine excretion was considered as significant proteinuria. We studied the clinical characteristics, the pathological findings and the response to treatment of these patients. Results : Ten ( males and females) of 1 cases of TBN had significant proteinuria (59%). The mean patient age was 35 years. Six patients had hypertension and 1 patient had nephrotic syndrome. Two patients had proteinuria only, and 8 patients had both hematuria and proteinuria. At the time of biopsy, the amount of urine protein was 1,881 mg per day, and all the patients except one showed normal renal function. The GB thickness ranged from 201 to 252 nm. Nine patients were treated with angiotensin receptor blocker, and 1 patient suffering from nephrotic syndrome was treated with prednisolone and cyclophosphamide. arked improvement of the proteinuria (59 mg per day) was observed in 8 patients during the followup period. Conclusions : Thin basement membrane nephropathy is one of the causes of proteinuria. 2 The Korean Journal of edicine : Vol. 3, No., 200 서론 Thin basement membrane nephropathy (TBN) 는임상증상으로는지속적인혈뇨와, 병리학적으로는전자현미경에서사구체기저막의비박화를보이나광학현미경이나면역형광현미경에서는특이한미세구조변화가없는상태로정의할수있다. TBN 은성인과소아에서혈뇨의흔한원인질환으로발생빈도는 0.8% 에서 11% 까지보고되고있으며, 가족력과연관되어나타난다 1, 2). 대부분의환자는신기능이정상이며경도의단백뇨를동반하기도하지만특별한치료가필요하지않는양성질환이다. 그러나 TBN 환자에서 2시간요단백량이 500 mg 이상의단백뇨를동반하거나또는신기능저하를초래한예가보고되었으며 3), TBN 이외에다른사구체질환과관련되어다량의단백뇨와신증후군이동반된다는보고가있어, TBN 은양성경과만을보이는것이아니라는것이알려져있다 ). 저자들은 TBN 으로진단된환자에게서단백뇨가동반된환자의임상적특성과치료경과에대해알아보고자하였다. 대상및방법 1. 대상 2002년 5월부터 2005년 월까지순천향대학교부속부천병원신장내과에서신생검을시행했던 231예중 TBN 으로진단된 1명을대상으로하였다. TBN 의진단기준으로 Dische 등 8) 의기준에따라전자현미경에서사구체기저막의두께가 330 nm 미만으로미만성비박화를보이고, 광학현미경에서경미한변화가있거나정상이며, 면역형광현미경에서면역글로불린 (Ig) 과보체 (C) 의침착이경도로있거나없는경우로하였고, Alport 증후군의특징적증상인청력소실과시력장애등의증상이동반된경우는제외하였다. 2. 방법모든환자는신생검당시혈압, 체중측정과말초혈액검사, 일반화학검사, 요검사, 2시간요단백검사및복부초음파를시행하였다. 사구체여과율은 Cockcroft Gault 공식을이용하여측정하였다. 2시간요단백량이 500 mg 이상을 Jafar 등 9) 의보고에따라단백뇨치료기준에서의미있는단백뇨로정의하였다. 신생검은 1 gauge Trucut needle을사용하여초음파주시하에경피적으로환자의좌측신장에서시행하였다. 광학현미경검사를위하여 formalin으로고정하여 paraffin 에넣어 μm 두께로잘라내고, hematoxylineosin, periodic acid Schiff, asson's trichrome 과 ethenamine sliver로염색하였다. 면역형광현미경검사를위하여검체를 liquid nitrogen으로동결하고전기냉동박절법으로절편하여, IgG, IgA, Ig, C 3, C, C 1q, fibrinogen 에대한 fluorescein isothiocyanateconjugated rabbit antihuman sera를이용하였다. 전자현미경검사를위하여 2.5% glutaraldehyde로고정하고, 0.1 m phosphate buffer 를이용하였으며, 1% osmium tetroxide 로재고정하여 0~90 nm 두께로절편하였다. 전자현미경에서기저막의두께는 lamina interna, lamina densa, lamina externa 를모두포함하여가장두꺼운부위와가장얇은부위를각각측정하여기저막두께의범위를표시하였다. 신생검당시환자의임상정보와추적결과에대해환자기록지를통한후향적방법으로분석하였다. 통계처리는 SPSS.0을이용하여결과는평균값 ± 표준편차로표시하였고, p<0.05 이하를통계적유의성이있는것으로판정하였다. 결과 2002년 5월부터 2005년 월까지순천향대학교부속부천병원신장내과에내원하여신생검을시행한 231예중 TBN 으로진단된환자는 1명이었고, 이중 2시간요단백량이 500 mg 이상인환자는 명 (1번번예 ) 이었다 ( 표 1). 전체환자의남녀비는 11:이었고, 연령은 19세에서 2세사이로이들의평균연령은 30.5±9.3세였다. 신생검당시주증상으로단백뇨 2명, 현미경적혈뇨와단백뇨모두동반된환자는 8명, 현미경적혈뇨 5명, 육안적혈뇨 2명이었다. 동반질환으로고혈압이 명, 신증후군이 1명이었다. 생검당시전체혈청알부민은.2±0. g/dl, 혈청크레아티닌은 0.9±0.3 mg/dl이었고, 사구체여과율은 1.1±30. ml/min이었다. 소변검사에서 2명에게서는혈뇨가없었고, 단백뇨는 dipstick 검사에서각 08

Clinical data at the time of renal biopsy Case No. Gender Age Present Albumin Creatinine GR Urine Urine 2hr urine BP (mmg) ation (g/dl) (mg/dl) (ml/min) RBC (/P) protein TP (mg/day) P P G G 130/0 0/80 /90 0/0 0/0 130/80 5/0 /90 0/0 1/0 0/0 1/5 130/ <½V 59 <1 29 < >½V <½V <½V >½V 30 <½V ,2 1,851 2,11, 1,513 1, 1, , P, protein;, hematuria; G, gross hematuria; GR, glomerular filtraton rate calculated by the CockcroftGault formula; V, visual field; TP, total protein 각 1+ 1명, 2+ 명, 3+ 3명이었다. 전체환자의평균 2시간요단백량은 1,18.±1,220.0 mg이었다 ( 표 1). 신조직의광학현미경에서보이는사구체는모두정상으로사구체의경화나그외세뇨관, 간질, 세동맥의이상소견은없었다. 면역형광현미경에서 5명 (3번, 번, 번, 12번, 1번예 ) 에서 Ig, 명 (번, 1번, 1번, 1번예 ) 에서 C 3 의미미한침착을보였다. 전자현미경에서사구체의메산지움내전자고밀도물질의침착이나상피세포의족돌기소실은관찰되지않았으며, 사구체기저막의평균두께는 TBN 전체환자에서최저 19.12± 2.3 nm, 최고 ±30.1 nm였고, 2시간요단백량이 500 mg 이상인환자에서는최저 200.5±23. nm, 최고 252.0±2.9 nm이었다 ( 표 2)( 그림 1A, 1B). 2시간요단백량이 500 mg 이상인 명의남녀비는 :, 평균연령은 35.0±. 세였다. 주증상으로현미경적혈뇨와단백뇨가같이동반되었으며 2명에서는혈뇨없이단백뇨만있었다. 고혈압과신증후군이동반된환자모두는이들에속하였다. 번예에서혈청알부민 2. g/dl로감소, 번예에서혈청크레아티닌 1. mg/dl 로증가하여있었고, 사구체여과율은 5번, 번, 8번예에서 각각.2 ml/min, 9.2 ml/min, 55.0 ml/min로저하되어있었다. 2시간요단백량의평균은 1,881.2±1,. mg이었으며, 2시간요단백량에따른각환자수는 5001,000 mg 2명, 1,0003,500 mg 명이었고, 1명에서는,00 mg으로신증후군범위였다 ( 표 1). 2시간요단백량이 500 mg 이상인환자 명중 2번예를제외한 9명을추적관찰하였다. 모든환자에게 angiotensin receptor blocker () 를투여하였고, 혈압조절을위해추가적으로 calcium channel blocker 를투여하였으며 (번, 번예 ), 신증후군을보인 번예는 prednisolone과 cyclophosphamide 를추가투여하였다. 추적기간 19.0±.1개월동안관찰한결과, 혈압은수축기 119.±11.8 mmg, 이완기 5.±9.8 mmg으로치료전수축기혈압 133.5±.3 mmg, 이완기혈압 8.0 ±12. mmg에비해저하되었고, 번예를제외한나머지 8명에서의혈압은정상범위로조절되었다. 추적환자들의혈청알부민과혈청크레아티닌은각각.± 0.3 g/dl, 1.0±0.2 mg/dl 로, 혈청크레아티닌 1.5 mg/dl 인 번예를제외하고모두정상범위였다. 사구체여과율은 1.1±25.3 ml/min로치료전사구체여 09

The level of serum creatinine was within the normal range before and after treatment in all the patients except for one patient (case ). The GR was increased in three patients (cases 5, and 8) after treatment, and their previous GRs were less than 90 ml/min. The 2hour urine total protein was decreased in all the patients except for 1 patient (case 8) after treatment. In case, the serum creatinine and 2hour urine total protein were decreased, and the GR was increased after treatment. 원인질환의하나로전자현미경에서사구체기저막의비박화가특징이며, 혈뇨이외에경도의단백뇨를동반할수있으나신기능은정상인양성질환이다. TBN 은 192년 Baehr 등 ) 에의해 "benign and curable form of hemorrhagic nephritis" 에서처음기술되었으며, 그후 congenital hereditary nephritis 11), familial benign essential hematuria 12) 로불리었고, 여기서 Rogers 등 12) 은재발성양성혈뇨와사구체기저막의비박화와의관련성을처음으로보고하였다. 그러나진행성인 Alport syndrome 과구별하기위해 Aarons 등 2) 과 Dische 등 ) 은 thin membrane nephropathy로하였다. 또한 thin glomerular basement syndrome 1), thin glomerular basement membrane nephropathy 5), thin basement membrane nephropathy 13), thin basement membrane disease 1) 로불리우기도한다. TBN의조직소견은광학현미경과면역형광현미경에서특이소견이없으며, 전자현미경에서는사구체기저막의비박화가특징이다. 사구체기저막두께는나이와성별에따라다양하게나타날수있으며방법과조직준비과정에서차이가날수있어, 두께에대한비박의 11

6 The Korean Journal of edicine : Vol. 3, No., 200 정의는보고자마다다르다. 국내에서 TBN 과관련된보고로는소아에게서는 198년조등 ) 이 1예를처음으로보고하였고, 성인에게서는 1993년박등 1) 이 2예를처음으로보고하였으며, 이후총 개의문헌이보고되어있다 120). TBN 에서단백뇨의동반은대부분 2시간요단백량이 500 mg 이하인경도의단백뇨로 1% 까지보고되고있지만다량의단백뇨는드물다 21). 고혈압의동반은성인에게서 31% 까지보고되고있다 22). 국내의보고에서신등 1) 은신생검을시행한 19명중 명에게서 TBN 으로진단되었고, 이중 명에게서 2시간요단백량이 500 mg 이상인단백뇨가있었으며, 최등 19) 은 20 명의 TBN 환자중 11명에게서단백뇨, 3명에게서고혈압이동반되었다. 본원에서는 2002년 5월부터 2005년 월까지신장내과에서신생검을시행한총 231명중 1 명이 TBN 으로진단되어.% 의발생빈도를보였고, TBN 환자 1명중 명에게서 2시간요단백량이 500 mg 이상의단백뇨, 명에게서고혈압이동반되었다 ( 표 1). TBN 의임상적경과는대체로양호한것으로알려져있으나, 단백뇨, 고혈압, 다른사구체질환과동반되어신부전으로진행한보고들이있다 3). Piel 등 3) 은사구체기저막의비박화가있는소아환자중 2+ 이상의단백뇨가있는 9명을추적관찰하여 1명에게서신기능손상을보였다고보고하였다. Dische 등 ) 은 1명의 TBN 환자중 명에게서단백뇨가있었고이중 3명은신부전으로진행된반면, 진단당시단백뇨가동반되지않았던환자에게서는고혈압또는만성신부전을발견할수없었다고하였다. 또한단백뇨가동반된 TBN 환자에서고혈압이동반된 명의환자중 1명이신부전의악화로인해혈액투석을하게된예가있었다고보고하였다. Gandhi 등 ) 은단백뇨가동반된 TBN 의증례에서신기능이점진적으로악화된예를보고하여 TBN 이양성질환만이아님을보였다. 국내에서는 TBN 의단백뇨동반과연관되어신부전으로진행된보고는없다. 신등 1) 은 명에게서 2시간요단백량이 500 mg 이상의단백뇨가동반되었지만이에대한추적관찰은없었으며, 최등 19) 은 11명의단백뇨가동반된 TBN 환자중다른신질환과동반된환자는 명으로 명에게서 IgA 신병증, 1명에서미세변화신증후군, 두가지모두동반된경우가 1명이었으나신부전으로진행한환자는없었다고보고 하였다. 한편, 무증상의혈뇨와단백뇨는 TBN 과관계없이장기적으로신질환진행의지표이다. 1999년박등 23) 은성인남녀에서 2시간요단백량이 0 mg 이상인무증상의단백뇨환자들과무증상의혈뇨와단백뇨가동시에있는환자들을평균.2±.년간장기추적조사한결과, 각각.1%, 8.% 에서만성신부전으로진행함을보고하였다. Iseki 등 2) 은,1명을 1년간추적조사했을때요검사상 1+ 의단백뇨가있었던환자군이단백뇨가없었던군에비해남자 1.93, 여자 2.2의 odd ratio 를보임으로단백뇨가말기신질환으로진행하는강력하고독립적인예측인자임을밝혔다. 따라서단백뇨는신부전진행의악화인자이며, TBN 에서의미있는단백뇨가동반된환자는적극적인치료가필요하다. Nogueira 등 ) 은 2시간요단백량이 g인 TBN 환자 8명중 3명에게서순수한 TBN, 5명에게서초점성분절성사구체경화증등의다른사구체병변이동반되었는데, 이들에게 steroid 치료로단백뇨의감소를보였고, 최등 20) 은원발성막성신병증이동반된 1예에게서단백뇨동반이있었으나약물치료로단백뇨가소실되었음을보고하였다. 한편, Jafar 등 9) 은비당뇨병성신질환의진행을늦추는단백뇨치료에서최대의단백뇨감소효과를보이는 2시간요단백량은 500 mg 이상일때라고하였고, Ruggenenti 등 25) 은당뇨병성단백뇨에서 ACE inhibitor 투여로말기신질환의진행을늦추었다. 저자들은 1명의 TBN 환자에서단백뇨가동반된환자의신부전진행을늦추기위한적극적치료로 2시간요단백량이 500 mg 이상인환자 명중추적되지않은 1명 (2번예 ) 을제외한모든환자에게 angiotensin receptor blocker () 를투여하였고, 130/80 mmg이하로혈압조절을하기위해다른고혈압약제를추가하였다 (번, 번예 ). 평균 19.0±.1개월간의추적검사결과, 치료전신기능저하를보였던 번예에서혈청크레아티닌의감소와사구체여과율의증가를보였고, 2시간요단백량은 59.2±28.0 mg으로 8번예를제외한모두에서단백뇨감소를보였다 ( 표 3)( 그림 2). 8번예의경우외래관찰기간중, 환자임의의일시적방문중단과투약중지로인한결과일것으로판단하였고, 추적관찰함에도불구하고지속적인단백뇨증가소견을보인다면재신생검을고려하기로하였다. 12
