It is common in patients with diabetes having a high body mass index (BMI), but it also occurs in patients with increased proteinuria. Recently, we treated a 28-year-old woman and a 15-year-old boy with proteinuria and a high BMI (woman: 35 kg/m 2 ; boy: 27.7 kg/m 2 ). At that time, they were diagnosed with obesity-related glomerulopathy based on the laboratory, urinary, and kidney biopsy findings. After treatment with an angiotensin-converting enzyme inhibitor and weight loss, the proteinuria was sustained in the latter, while it improved in the former. We believe that these cases suggest an association between obesity and glomerulopathy. (Korean J Med 76:S148-S153, 2009) Key Words: Obesity; Proteinuria; Glomerulosclerosis 서론지난 20여년간비만환자의수는폭발적으로증가하고있으며, 비만환자의일부에서단백뇨가발생한다고알려져왔다. 1974년 Weisinger 등이과도한비만 (obesity) 환자에서다량의단백뇨를동반한국소분절사구체경화증 (focal segmental glomerulosclerosis, FSGS) 증례를처음보고한이래 1), 이러한신장질환의형태학적인특징에대한많은증례보고가있어왔다 2-4). 1975년 Arthur H. Cohem은비만연관사구 체병증 (Obesity-related glomerulopathy, ORG) 이라는용어를처음으로사용하였다. 그는 ORG가조직학적으로사구체비대 (glomerular enlargement), 경한고세포증식 (mild hypercellularity), 사구체기저막 (glomerular basement membrane) 의다양한확장 (variable widening) 그리고 FSGS 를특징으로하며, 그중사구체비대 (glomerulomegaly) 를가장뚜렷한소견으로기술하였다 5). 이후신장조직검사를통해비만과관련된단백뇨의조직학적특징이 FSGS, 혈관간세포증식 (mesangial proliferation) 과비대 (hypertrophy) 그리고사구체비대 (glomerulomegaly) 로알려졌으며, 일부보고에서심한 FSGS를동 Received: Accepted: Correspondence to Hyun Chul Chung, M.D., Department of Internal Medicine, Ulsan University Hospital, Jeonha-dong, Dong-gu, Ulsan , Korea hcjungmd@uuh.ulsan.kr - S 148 -

증례 1 환자 : 김 근, 15세남자주소 : 3년전학교신체검사에서우연히발견된단백뇨현병력 : 학교신체검사에서단백뇨발견하고본원소아 과에서간헐적인약물치료하였으나단백뇨지속되는소견으로내원하였다.

사지에서함요부종은관찰되지않았고, BMI 지수는 27.7 kg/m 2 였다.

Two cases of obesity-related glomerulopathy - 반한경우에말기신부전으로진행할수있다고알려져왔다 2,3). 저자들은단백뇨를주소로내원한비만환자와과체중환자에서신장조직검사상비만과연관된사구체병증으로진단된흥미로운증례 2예를보고하고자한다. 증례 1 환자 : 김 근, 15세남자주소 : 3년전학교신체검사에서우연히발견된단백뇨현병력 : 학교신체검사에서단백뇨발견하고본원소아 과에서간헐적인약물치료하였으나단백뇨지속되는소견으로내원하였다. 과거력, 가족력, 사회력 : 특이소견없음. 이학적소견 : 입원당시혈압 110/70 mmhg, 맥박 70회 / 분, 호흡수 20회 / 분, 체온 36 였고, 전신상태는양호하였다. 폐음및심음은정상이었다. 사지에서함요부종은관찰되지않았고, BMI 지수는 27.7 kg/m 2 였다. 검사소견 : 말초혈액검사는백혈구 9,620/μL, 혈색소 16 g/dl, 혈소판 361,000/μL였고, 혈청생화학검사에서총단백 8.0 g/dl, 알부민 4.8 g/dl, 혈중요소질소 10.6 mg/dl, 크레아 A B C D Figure 1. The follow-up 24-hour urine protein and BMI. 티닌 1.28 mg/dl, 총콜레스테롤 161 mg/dl, 혈당은 96 mg/dl 였다. 요검사에서요단백 2+, 적혈구 1~3개 /HPF, 24시간소변단백량은 1,813 mg/day이었다. 혈청학적검사는 HBsAg, HCV-Ab, ANA, ANCA, 류마티스인자 (Rheumatoid factor) 모두음성이었다. 복부초음파소견 : 양측신장은우측 11.4 cm, 좌측 11.0 cm 으로정상소견을보였다. 신장조직검사 : 신장조직소견상광학현미경검사에서 10 개의사구체중에서 2개에서전엽성경화가관찰되었고, 사구체직경은 μm였다. 나머지사구체는정상적인형태를보였고, 기저막의비후소견도보이지않았다. 경화성사구체주위의신장세뇨관위축을제외하고는신장세뇨관과간질조직은비교적잘유지되어있었다 ( 그림 1). 전자현미경검사에서국소적인발돌기소실 (focal foot process effacement) 이관찰되었다 ( 그림 1). 면역형광조직소견에서항체침착은관찰되지않았다. 임상경과및치료 : 신장조직검사결과를바탕으로사구체경화증을동반한비만연관사구체병증으로진단하였고, 체중감량교육과 ACE 억제제를사용하였으며, 추적관찰중 BMI 증가와단백뇨증가소견을보였다 ( 그림 2). 이후지속적인 ACE 억제제와 ARB (angiotensin II receptor blocker) 사용과체중감소에대한교육중이다. 증례 2 환자 : 이 숙, 28세여자주소 : 부종없이수개월전부터발생한거품뇨현병력 : 환자는내원수개월전부터발생한거품뇨를주 소로외부병원에서치료를하였으나증상의호전없어본원으로의뢰되었다. 과거력, 가족력, 사회력 : 특이소견없음. 이학적소견 : 입원당시혈압 150/80 mmhg, 맥박 80회 / 분, 호흡수 20회 / 분, 체온 36.5 였고, 전신상태는양호하였다. 폐음및심음은정상이었다. 사지에서함요부종은관찰되지않았고, BMI 지수는 35 kg/m 2 였다. 검사소견 : 말초혈액검사는백혈구 10,600/μL, 혈색소 11.7 g/dl, 혈소판 462,000/μL였고, 혈청생화학검사에서총단백 8.1 g/dl, 알부민 4.2 g/dl, 혈중요소질소 10.0 mg/dl, 크레아티닌 0.79 mg/dl, 총콜레스테롤 178 mg/dl, 혈당은 113 mg/dl였다. 요검사는요단백 2+, 적혈구 5~10개 /HPF, 백혈구 5~10개 /HPF 였으며 24시간소변단백량은 1,086 mg/ day였다. 혈청학적검사는 HBsAg, HCV-Ab, ANA, ANCA, 류마티스인자 (Rheumatoid factor) 모두음성이었으며단백면역검사에서 C3, C4는정상범위였다. 복부초음파소견 : 양측신장은우측 10.7 cm, 좌측 10.5 cm 정상소견을보였다. 신장조직검사 : 광학현미경관찰에서 9개의사구체모두전엽성경화 (global sclerosis) 소견은보이지않았고사구체직경 (glomerular diameter) 은 270 μm로사구체비대 (glomerulomegaly) 소견과균일한기저막비후소견이관찰되었다 ( 그림 3). 전자현미경검사에서발돌기소실 (foot process effacement) 소견을보였고 ( 그림 3), 면역형광조직소견에서 IgG, IgA, IgM, C3, C4, C1q에대한항체는음성이었다. 임상경과및치료 : 신장조직검사결과를바탕으로비만과연관된사구체병증으로진단하에체중감량과 ACE 억제 - S 150 -

때로는신장증후군범위의단백뇨를보이기도하며, 단백뇨는사구체여과율 (glomerular filtration rate: GFR) 의감소가발생하기몇년전부터선행하여나타난다. 가장중요하며자주발견되는신장병리적인특징은 FSGS와사구체비대 (glomerulomegaly) 이 다 6-8).

Focal sclerotic mesangium and focal foot process effacement were noted (C: EM stain, 2,500). 제 (angiotensin converting enzyme inhibitor) 로치료를시작하였으며, 5개월후 24시간단백뇨 548 mg/day으로감소소견과알부민은정상소견을보였다. 고찰비만은당뇨, 고혈압과별개로만성신장질환의독립적위험인자이다. 일반적인임상증상은비만한환자에서서서히진행하는단백뇨를보이는것이다. 때로는신장증후군범위의단백뇨를보이기도하며, 단백뇨는사구체여과율 (glomerular filtration rate: GFR) 의감소가발생하기몇년전부터선행하여나타난다. 가장중요하며자주발견되는신장병리적인특징은 FSGS와사구체비대 (glomerulomegaly) 이 다 6-8). Kambham 등은 6,818건의신장조직검사를분석하여그중 71명에서 Body Mass Index (BMI) >30이며, 신장조직검사에서 1) obesity-associated FSGS with glomerulomegaly, 2) obesity-associated glomerulomegaly alone을보이는경우를 obesity-related glomerulopathy (ORG) 로정의하고, idiopathic FSGS (i-fsgs) 와는임상적, 조직학적으로다른새로운질환존재 (disease entity) 로제시하였다 6). 그는 ORG가비만에의해발생하는독립된합병증으로서그빈도가점차증가하고있으며, BMI 30~40인환자에서도 40 이상의과도한비만에서와임상적, 형태적으로차이가없는 ORG가발생한다고보고하였다. 또한 ORG가 i-fsgs에비해신장증후군과부종 - S 151 -

5 - The Korean Journal of Medicine: Vol. 76, Suppl. 1, 의발현비율이낮고콜레스테롤 (cholesterol) 수치가낮으며분절성경화 (segmental sclerosis) 와발세포발돌기융합 (podocyte foot process fusion) 의정도가낮다고보고하였다 6). 통계적으로유의한차이는 i-fsgs에비해나이가많고 (42.9 세 vs 32.6세 ) 정상혈청 albumin (3.9 vs 2.9 g/dl) 을보이는것이라고기술하였다 6). Praga 등도 15명의 obesity-associated FSGS 환자를 i-fsgs 와비교하여보고하였는데 Kambham 등의보고와같이신장증후군과부종이나타나지않고혈청 albumin (4.2 vs 2.5 g/dl) 과사구체직경 (glomerular diameter) 의차이 (256 vs 199 μm) 를보인다고보고하였다. 하지만 obesity-associated FSGS 15명에서신장생존율 (renal survival) 이 5년에 77%, 10년에 51% 로장기간의신장예후가불량한것으로보고하였다 7). 본증례 1의경우알부민은정상이고, BMI가증가하면서부종이없는신증후군범위의단백뇨소견을보였다. 조직검사소견상 FSGS 소견을보였고, 15세의젊은동양인남자에대한정상사구체크기가알려진바가없는것을고려한다면, 사구체비대여부를판정하기는어렵다. 증례 2의경우알부민정상범위였으며, 단백뇨가 1.8 g/day으로신증후군범위의단백뇨는없었다. 조직검사소견상사구체비대 (glomerulomegaly) 는보이지만 FSGS 소견은동반되지않았다. 비만연관사구체병증의병리기전으로혈역학적변화 (hemodynamic change) 10-13), 고지혈증 (hyperlipidemia) 14-16), Renin-angiotensin-aldosterone system (RAAS) 13,17,18) 의활성화, 인슐린저항성 19,20) 등을제안하고있다. 비만과연관된사구체병증은조직학적변화로가장흔하게사구체비대, 국소분절성사구체경화소견이관찰되고기저막비후, 국소간질조직증식, minimal foot process effacement 등을보인다. 미만성간질경화와기저막비후를보이는당뇨병성신장증과조직학적으로구분된다. 본증례 1, 2에서도전자현미경상발돌기소실 (foot process effacement) 소견을보였다. ORG는대부분부종과같은임상증상이없이건강검진에서우연히발견된단백뇨로내원하므로환자의일부에서초기부터신기능손상이있다. 현재까지알려진예후의가장강력한예측인자는초기의크레아티닌 (creatinine) 수치이며 6,7), 일부에서단백뇨의정도 6) 와관위축 (tubular atrophy) 과사이질섬유증 (interstitial fibrosis) 같은조직소견 7) 을나쁜예후인자로보고하였다. 본증례 1의경우초기크레아티닌 1.28 mg/dl이었고, ACE 억제제와 ARB 사용중에도 BMI가증가하면서크레아티닌 1.8 mg/dl로증가되었다가 BMI감소하면서크레아티닌 1.3 mg/dl로감소소견을보였다. 본증례 2의경우초기크레아티닌 0.79 mg/dl로이후체중증가 는없었고, ACE 억제제사용후단백뇨가감소하였고, 크레아티닌증가는없었다. 치료기간이짧은것을고려한다면이후지속적인추적관찰이필요하다. 치료에서가장중요한것은기저질환인비만을교정하는것이다. 대부분의연구에서지속적인체중감소는달성하기어려웠으나, 체중감소만으로도단백뇨의감소를보이며 6,8), 체중증가에의해단백뇨가다시증가하였다 8). ACE 억제제는비만환자에서의단백뇨감소에효과적이나 6), 체중감소를동반하지않고사용한경우 6개월내에서는단백뇨가감소하나 12개월에는다시증가하였다는보고도있다 7). 처음부터신장기능이상이있는경우 ACE 억제제의사용에도불구하고지속적으로신장기능이악화된다고보고하였다 7). 요 저자들은단백뇨를첫증상으로내원한비만과연관된사구체병증 2예를경험하였기에문헌고찰과함께이를보고하는바이다. 약 증심단어 : 비만 ; 단백뇨 ; 사구체경화증 REFERENCES 1) Weisinger JR, Kempson RL, Eldridge FL, Swenson RS.
