This study investigated AST/ALT ratio according to alcohol drinking and predictive power of AST/ALT ratio for heavy drinking in Korean men Methods: In 2015, 830 men with no history of disease or medication affecting liver function values were selected. Using 14 g of alcohol as the standard glass, odd ratios (ORs) of AST/ALT ratio over 1 among the drinking group were compared with the non-drinking group by multiple logistic analyses. Screening of heavy drinkers was conducted according to both NIAAA and Korean guidelines. Using AST/ALT ratio >1, sensitivity, specificity, positive predictive values, negative predictive values, positive likelihood ratios, negative likelihood ratios and odds ratios were investigated. Results: After correcting for age, body mass index, exercising, and smoking history, ORs (95% confidence interval) of AST/ALT >1 were ( ) in <8 glasses group, ( ) in the 14 glasses group, and ( ) in the >14 glasses group. When AST/ALT >1 in the drinking group, sensitivity, specificity, positive predictive values, negative predictive values, likelihood ratios (LR)+, LR- and OR according to NIAAA guidelines, were 57.2%, 52.7%, 23.2%, 76.8%, 1.21 ( ), 0.81 ( ), and 1.49 ( ), respectively. By Korean guidelines, these values were 55.2%, 55.3%, 33.7%, 66.3%, 1.24 ( ), 0.84 ( ), and 1.40 ( ), respectively. Conclusions: Healthy male adults showed a positive correlation between risk of AST/ALT >1 and drinking amount compared to the non-drinking group. Use of the AST/ALT >1 in combination with other blood markers to predict excessive drinking is advisable. Association between Alcohol Drinking and the Ratio of Aspartate Aminotransferase to Alanine Aminotransferase in Korean Adult Male 65 서 론 성인남성의적절음주량은 National Institute on Alcohol Abuse and Alcoholism (NIAAA) 1) 기준에따르면주당음주량이 14표준잔이하일때, 한국의적정음주가이드라인 2) 에서는 8표준잔이하일때로정의하고있다. 과음은위험음주라고도하며, 주당적정음주량을넘어서는음주를의미한다. 과음은교통사고위험을포함한상해, 고혈압, 뇌졸중, 폭력, 암, 자살등의위험을증가시킬뿐아니라, 3) 간염바이러스와함께간질환을일으키는매우흔한원인중에하나로, 비정상적인간기능검사와연관이있다. 알코올성간질환은지방간, 간염, 간경변증및간세포암등을포함한다양한범주의간손상을의미한다. 그중알코올성지방간은만성음주자의약 80-90% 에서발생하는가장흔한초기병변으로대개무증상이고금주를할경우호전된다. 그러나지속적인과다음주시일부환자들은알코올성간염을거쳐섬유화되어결국간경변증으로진행하게된다. 4) 심한과음을하는사람들에게는몇몇혈액검사가매우유용하게사용되는데, 여기에는 gamma-glutamyl transpeptidase (GGT), aspartate transaminase (AST)/alanine aminotransferase (ALT) ratio, mean corpuscular volume (MCV), carbohydrate deficient transferrin (CDT) 이있고, 이는만성음주자를찾는데유용하게사용되는마커이다. 5) 특히 AST, ALT는간세포손상및간독성을일으키는원인들을감별하는데있어매우유용한지표이고, 무증상외래환자를대상으로과음으로인한간손상을선별하는데유용한혈액검사로이용된다. American Gastroenterological Association에따르면무증상의 1-4% 는간기능검사에서비정상적인수치를보이고있었다고하였으며따라서일차의료인으로써간수치상승의의미를아는것은매우중요하다. 6) 대부분의급성간세포질환의경우대개 ALT보다 AST 수치가낮게측정되나, 알코올성간질환의경우 AST/ALT 비가 1 이상이고, 7) 특히 AST/ALT 비가 2 이상시에는 alcoholic liver disease를의심해볼수있다. 8) 알코올성간질환을일으키는최소알코올섭취량에대한명확한기준은없지만, 유럽의대규모코호트연구에서하루 30 g 이상음주시간경변증의위험이유의하게증가한다고하였다. 9) 그러나건강한한국인에서음주상태와 AST/ALT 비사이의관련성에대한연구를찾기가어렵다. 또한과음은공공의건강에중요한영향을끼치는문제로 cut down, annoyed, guilty, eye-opener (CAGE) 와 alcohol use disorders identification test (AUDIT) 같은설문지들이음주의정도를결정하는데임상에서활용되고있지만, 그외에음주문제를가진환자를찾는효과적이면서과민한선별 검사는부족한실정이다. 게다가 GGT, MCV, CDT와같은음주지표와는다르게선별검사로써의 AST/ALT 비의유용성에대한연구는찾아보기어렵다. 이에본연구는무증상의한국남성을대상으로음주량구간에따른 AST/ALT 비가 1을초과하는비율과과음을선별하는데있어 AST/ALT 비가 1을초과할때과음에대한양성예측도와음성예측도를알아보고자하였다. 1. 연구대상 방 법 2015년 1월부터 12월까지충남대학교병원종합검진센터에서검진을받은남성 1,500명을대상으로하였다. 그중간기능수치에영향을줄수있는간질환과혈색소증, 알파1-안티트립신결핍증, 윌슨병, 셀리악병, 용혈증, 근육병증, 자가면역성간염및갑상선항진증, 제2형당뇨, 심장질환의과거력이있거나, 10) 약물복용력이있는 670명은연구에서제외되어총 830명의성인남성이연구에포함되었다. 그중비음주자는 167명, 음주자는 663명이었다. 본연구는충남대학교병원생명윤리위원회의심사를통해승인을받았다 (IRB No.: CNUH ). 2. 연구방법 본연구는후향적인연구로서, 연구대상에대한기본적인데이터는건강검진시에행해진문진표로부터얻었다. 나이, 키, 몸무게를신체검진및계측자료로부터구하였고, 체질량지수 (body mass index, BMI) 는신체계측시측정한키와체중을이용하여계산하였다. 또한흡연상태 ( 비흡연, 과거흡연, 현재흡연 ), 흡연량, 한국인의신체활동지침을바탕으로주당 150분이상중등도강도의유산소운동의시행여부를문진표에기록되어있는것을바탕으로파악하였다. 11) 음주에관한항목에는주당음주횟수, 일회음주량을포함하였으며, 일회음주량은 NIAAA 1) 에서제시한가이드라인을이용하여, 1표준잔을알코올 14 g으로하였고, 주당음주량은주당음주횟수와일회음주량을곱하여계산하였다. NIAAA 1) 는주당음주량이 14표준잔을초과할때, 한국의적정음주가이드라인은 8표준잔을초과할때과음으로정의함을고려하여, 2) 음주군의주당음주량을 8잔이하, 8잔초과 14잔이하, 14잔초과로나누었다. AST, ALT 값은건강검진혈액검사결과를이용하였고, 이는표준화된검사실장비인 Toshiba 2000 FR (Toshiba Medical System

3 66 Korean J Health Promot Vol. 17, No. 2, 2017 Corporation, OTAWARA, Japan) 을통해산출되었다. 3. 통계비음주군, 8잔이하음주군, 8잔-14잔음주군, 14잔초과음주군 4그룹의일반적특성, 신체계측, AST, ALT, AST/ALT값과 AST/ALT 비가 1을초과하는비율을연속형변수의경우분산분석, 범주형변수의경우카이제곱분석을이용하여분석하였다. 비음주군을기준으로음주량증가에따른 AST/ALT 비가 1을초과하는교차비를알아보기위해 AST/ALT 비에영향을미칠수있는연령, BMI, 운동여부, 흡연량을보정하여로지스틱회귀분석을시행하였다. 또한 AST/ALT 비가 1을초과하는경우 NIAAA와한국기준의과음의선별검사로서타당한지확인하기위해민감도, 특이도, 양성예측도, 음성예측도, 양성우도비, 음성우도비, 교차비를조사하였다. 또한 ROC 곡선분석을통해성인남성에서과음을예측하기위한적절한 AST/ALT 비의절단점및 area under the receiver operative characteristic curve, 민감도, 특이도, Youden index 값을확인하였다. 통계분석에는 IBM SPSS for window version 18 (SPSS Inc., Chicago, IL, USA) 과 MedCalc Statistical Software ver (MedCalc Software, Mariakerke, Belgium) 을사용하였으며, 유의수준은 P<0.05 로하였다. 결과 1. 연구대상의특성비교 비음주군, 8잔이하, 8잔초과 14잔이하, 14잔초과음주군의인구통계학적특성을비교한결과연령, 체중, 체질량지수, 허리둘레, 흡연상태는통계적으로유의미한차이 (P=0.000) 가있는것으로확인되었으나, 운동상태의경우각집단별로통계적으로유의한차이를보이지않았다. 주당음주횟수, 일회음주량, 주당음주량과 AST, ALT, AST/ALT 비는각집단별차이가통계적으로유의하였다 (P<0.05). 주당음주량에따른 AST/ALT 비가 1을초과하는비율에대해살펴보면비음주군 43.1%, 8잔이하군에서 48.2%, 8잔초과 14잔이하군에서 51.2%, 14잔초과군에서 57.2% 로 AST/ALT 비가 1을초과하는비율이높았고, 음주군의음주량이증가할수록비율역시증가하는경향을보였으며이는통계적으로유의하였다 (P=0.048) (Table 1). Association between Alcohol Drinking and the Ratio of Aspartate Aminotransferase to Alanine Aminotransferase in Korean Adult Male 주당음주량에따른 AST/ALT >1 에대한교차비 AST/ALT비에영향을줄수있는연령, 체질량지수 (BMI), 흡연량, WHO 권고운동여부를보정한후로지스틱회귀분석을실시하여비음주군과비교하여음주량구간에따라 AST/ALT 비가 1을초과하는교차비 (95% confidence interval) 를살펴보면, 8잔이하음주군의경우교차비가 ( ), 8잔초과 14잔이하음주군의경우 ( ), 14잔초과음주군의경우 ( ) 으로모든음주군에서교차비가증가하였고, 그값이통계적으로유의하였으며, P for trend 는 0.000이다 (Table 2). 3. AST/ALT >1 의과음선별에대한민감도, 특이도, 양성예측도, 음성예측도, 양성우도비, 음성우도비, 교차비 비음주군중 AST/ALT 비가 1을초과하는사람은 95명이었고, 1 이하인사람은 72명이었다. AST/ALT 비가 1을초과하면서 NIAAA guideline 기준의과음과적절음주에해당하는사람은각각 95명, 314명이었고, Korean guidline 기준의경우각각 138명, 271명이었다. 또한 AST/ALT 비가 1 이하인경우 NIAAA guideline 기준의적절음주, 과음에해당하는사람은각각 71명, 350명이었고, Korean guidline 기준의경우각각 112명, 309명이었다 (Table 3). AST/ALT 비가 1을초과할때 NIAAA guideline 기준의과음을선별하는데있어서민감도, 특이도는 57.2%, 52.7% 였으며, 양성예측도, 음성예측도는 23.2%, 83.1% 였고, 양성우도비, 음성우도비, 교차비는 1.21 ( ), 0.81 ( ), 1.49 ( ) 를보였다. Korean guideline 기준의과음을선별하는데는민감도, 특이도가 55.2%, 53.3% 였으며, 양성예측도, 음성예측도는 33.7%, 73.4% 였고, 양성우도비, 음성우도비, 교차비는 1.18 ( ), 0.84 ( ), 1.40 ( ) 을보였다 (Table 4). NIAAA guideline 기준에따른 AST/ALT 비의 area under the receiver operating characteristic curve (AUROC) 는 0.597이었고, 유의확률이 0.001로통계적으로유의하였다. 절단점은 0.90으로민감도는 70.48%, 특이도는 48.80% 였으며, Youden index는 0.19였다. Proper cut off value calculated by receiver operating characteristic (ROC) curves analysis 으로통계적으로유의하지않았다. 절단점은 0.88로민감도는 69.49%, 특이도는 44.91% 였으며, Youden index 는 0.14이었다 (Table 5). 고 찰 본연구는무증상의한국남성을대상으로음주량구간에따른 AST/ALT 비가 1을초과하는비율과과음을선별하는데있어 AST/ALT 비가 1을초과할때의과음예측도를알아보고자하였다. 상승한 AST 값을보이는환자에서 ALT에대한 AST의우위는알코올성간질환과관련이있음은이전부터연구되어져왔고, 12) 이는간의저하된 ALT 활성도, 13) 간내 Pyridoxal 5 -phosphate 부족, 14) 과음하는사람에서 mitochondria 손상으로인한 mitochondrial aspartate 의활성도증가와관련되어있다. 15) 본연구에서는비음주군과비교하여 14잔초과음주군에서의평균 AST 값이비음주군과 14 잔이하음주군보다유의하게높았고, 이는미국인을대상으로실시한이전연구에서음주자의경우하루음주량이증가할수록 AST 값이증가하였다는결과와유사하다. 5) 또한비음주군과비교하여음주군에서 AST/ALT 비가 1 을초과하는비율이높았고그값이통계적으로유의하였다. 또한이는알코올성간질환의경우 AST/ALT 비가 1 이상임을밝혔던논문과유사한결과로생각되며, 7) 알코올성간경화환자들에서 AST/ALT 비가증가할수록환자의병이상당히진행되었음을나타낼뿐아니라, 간경화가있는환자들의평균 AST/ALT 비가일반적으로 1 보다약간크다고한이전연구와비슷한결과이다. 16) 그러나비음주군을기준으로음주량증가에따른 AST/ALT 비가 1을초과하는교차비가음주량이증가할수록증가하였던본연구의결과는알코올성간경화합병증으로입원한환자와알코올금단증상증후군을치료하기위 해입원한환자를대상으로 AST/ALT 비를비교한 Nyblom 등 17) 의연구결과와는차이가있다. Nyblom 등 17) 은알코올금단증상증후군치료를위해입원한환자들에서 AST/ALT 비는알코올소비전체기간및양과관련성이없다고하였다. 알코올중독에빠진사람들이나음주문제를가진사람들은자신의음주문제를왜곡하거나축소하기위한수단으로 부정 이라는방어기제를사용한다. 부정은알코올의존환자들이흔히보이는특징적인사고의왜곡으로자신의알코올소비량, 음주문제가있었던기간, 알코올이자신의생활에끼친영향을축소하거나무시한다. 18) 따라서결과해석시알코올금단증상증후군이있는환자들의알코올소비에관한자가리포트식데이터를 100% 신뢰할수없는제한점이있다는것을고려해야한다. 음주구간에따라 AST/ALT 비가 2를초과하는비율은표로제시하지않았지만, 이역시비음주군과비교하여음주량이증가할수록 AST/ALT 비가 2를초과하는비율이높아지는경향을보였다. 그러나결과를해석하기에는건강한성인군에서 AST/ALT 비가 2를초과하는인원의수가 10명미만으로매우적어분석에제한점이있다는한계가있었고, 이는향후모집단의수를늘린대규모연구를통해극복할수있을것으로본다. 본연구에서음주군의 AST/ALT 비가 1을초과할때 NIAAA guideline 기준의과음에대해민감도 57.2%, 특이도 52.7% 와양성예측도 23.2%, 음성예측도 83.1% 를보였고, Korean guideline 기준의과음에대해서는민감도 55.2%, 특이도 53.3% 와양성예측도 33.7%, 음성예측도 73.4% 를보였다. 이와같은본연구의결과는이전연구에서논의되었던적이없었던내용으로써의의가있다. 그러나미국과음기준과한국과음기준을적용하였을때음성예측도를제외한민감도, 특이도, 양성예측도는모두낮았다. 따라서 AST/ALT 비의 1 초과를과음에대한선별검사로이용할때 AST/ALT 비를단독으로사용하기에는제한이있고, MCV, GGT, CDT 와같은다른혈액마커들의조합을통해임상에서활용하여야겠다. 한국성인남성에서과음을예측할수있는적절한 AST/ALT 비의절단점을알기위해 ROC 분석을하였다. NIAAA guideline 의과음기준을적용하였을때 AST/ALT 비의절단점이 0.90, Korean guideline 기준의과음에대해 서는 0.88이었고, 각각이상이면과음을할수있다고판단할수있겠다. 그러나낮은 AUC 값을고려하면이를실제임상에적용하기에는한계가있다. 알코올성간경화합병증으로입원한환자군에서입원후금주하면서 AST/ALT 비가크게감소한것으로볼때혈액검사전에음주를자제한기간이길수록직접적인알코올의독성효과는작아진다. 17) 이러한영향이 AST/ALT 비에

Association between Alcohol Drinking and the Ratio of Aspartate Aminotransferase to Alanine Aminotransferase in Korean Adult Male 69 도반영된다는점을고려하여과음을선별하는데 AST/ALT 비를활용하기위해서는건강검진문진시에마지막음주일을확인하는것이좋을것으로생각된다. 본연구결과 AST/ALT 비가 1을초과할때미국의과음기준보다한국의과음기준에대한양성예측도는더높고, 음성예측도는더낮았다. 한국과음기준을적용하였을때 AST/ALT 비가 1을초과할경우실제로과음일확률이미국과음기준을적용하였을때보다높았으나, 실제큰차이는없는것으로생각된다. 본연구에서양성우도비는과음기준에해당하는사람중에 AST/ALT 비가 1을초과하는사람일확률을과음기준에해당하지않는사람중에 AST/ALT 비가 1을초과하는사람일확률로나눈값이다. 음성우도비는과음기준에해당하는사람중에 AST/ALT 비가 1 이하의사람일확률을과음기준에해당하지않는사람중에 AST/ALT 비가 1 이하의사람일확률로나눈값이다. 따라서양성우도비는 1 보다클수록, 음성우도비는 1보다작을수록 AST/ALT 비가 1을초과할때과음으로판단하는데더타당하다고할수있다. 본연구에서한국과음기준에대해 AST/ALT 비가 1을초과하는양성우도비는 1.18로 1보다컸으며, 음성우도비는 0.84로 1보다작았다. 또한미국과음기준에대한양성우도비는 1.21, 음성우도비는 0.81이었다. 양성우도비와음성우도비측면에서 AST/ALT >1은한국과음기준과미국과음기준모두에대해타당성을가졌다고볼수있다. 본연구에서 AST/ALT 비가 1을초과할때미국과음기준과한국과음기준의과음을예측함에있어서의교차비 (odds ratio) 는 AST/ALT 비가 1을초과할때과음군을비과음군으로나눈 odds를 AST/ALT 비가 1 이하일때과음군을비과음군으로나눈 odds로나눈값으로표현되었고, 각각 1.49 ( ), 1.40 ( ) 이었다. 이를통해 AST/ALT 비가 1을초과할때미국과음기준과한국과음기준간교차비의차이는없는것을알수있다. 본연구는몇가지의제한점이있다. 첫째무증상의건강한성인남성을대상으로 AST/ALT 비를과음지표로서의사용가능성을보여준의미가있었으나, 음주행태를장기간관찰하여얻은결과가아니며단면연구라는것이큰제한점이다. 둘째남자만을대상으로시행되었다는점이고, 셋째대전의일개건강검진센터수검자들만을대상으로하였다는점이다. 건강검진을받는사람들은받지않는사람과비교하여건강에더관심이많고적절음주를포함한건강관리에더철저할수있다. 향후여자를포함한대규모의연구및전향적연구가필요하다. 이러한제한점에도불구하고본연구는 AST/ALT 비를 CDT, GGT 외에임상에서활용할수있는음주지표로고안될수있고이를바탕으로진료중해당환자에대한음 주문제에대해서상담및개입이가능할것을시사한점에서의의가있다. 또한 AST/ALT 비에대한기존의대다수의연구들은과음을하여실제로알코올의존이있는환자들을대상으로하였으며, 한국인을대상으로음주량과 AST/ALT 비를비교한연구가적은상황에서본연구는무증상의한국남성을대상으로 AST/ALT 비를과음의선별검사로써의유용성에대해알아본데에그의미가있다. 요약 연구배경 : 과음은간질환을일으키는흔한원인중에하나로간기능검사의변화를일으킨다. AST/ALT 비는과음시증가하는경향이있는것으로알려져있다. 본연구는한국남성에서음주량에따른 AST/ALT 비와 AST/ALT 비의과음예측도를살펴보고자하였다. 방법 : 2015년충남대학교병원종합검진센터에서검진을받은남성중, 간기능수치에영향을주는질병력및약물복용력이없는환자 830명을대상으로하였다. 음주량은알코올 14 g을표준잔으로하였고, 주당 8잔이하, 8잔초과 14잔이하, 14잔초과음주군으로나누어 AST/ALT 비가 1을초과하는교차비를로지스틱회귀분석을통해혼란변수들을보정하여비음주군과비교하였다. 또한 AST/ALT 비가 1을초과하는경우 NIAAA와한국기준의과음을선별하는데있어서민감도, 특이도, 양성예측도, 음성예측도, 양성우도비, 음성우도비, 교차비를조사하였다. 결과 : AST/ALT 비에영향을줄수있는연령, BMI, 운동여부, 흡연량을보정한후 AST/ALT 비가 1을초과하는 odds ratio (95% confidence interval) 는음주군에서 8잔이하음주군에서 ( ), 8잔초과 14잔이하에서 ( ), 14잔초과의경우 ( ) 이었다. 음주군에서 AST/ALT 비가 1을초과할때 NIAAA guidelines 기준의과음을선별하는데있어민감도, 특이도, 양성예측도, 음성예측도, 양성우도비, 음성우도비, 교차비는 57.2%, 52.7%, 23.2%, 76.8%, 1.21 ( ), 0.81 ( ), 1.49 ( ) 를보였다. Korean guidelines 기준의경우각각 55.2%, 55.3%, 33.7%, 66.3%, 1.24 ( ), 0.84 ( ), 1.40 ( ) 을보였다. 결론 : 무증상의건강한성인남성에서비음주군과비교하여음주군에서음주량이증가함에따라 AST/ALT 비가 1을초과할교차비가증가하였고, AST/ALT 비가 1을초과할때과음을예측하는데있어다른혈액마커와함께임상에서활용하는것이좋겠다. 중심단어 : 알코올음주, 아스파라진산아미노전이효소, 알라닌아미노전이효소, 미국국립알코올연구소
