1 ORIGINAL ARTICLE 전산화단층사진을이용한하악골 3차원영상에서비대칭진단계측항목의재현도에관한연구 김고운 a ㆍ김재형 b ㆍ이기헌 c ㆍ황현식 d 본연구는하악골 3 차원입체영상에서비대칭계측항목들의재현도비교를통하여안면비대칭진단시유용한계측점설정에도움이되고자시행되었다. 40 명의두경부전산화단층사진을이용하여생성한 3 차원영상에서하악골형태를결정짓는부위로하악과두, 하악우각부, 하악정중부를선택하고각부위의세부위치에따라, 그리고측정시보는각도에따라구분하여하악과두의경우 Cd sup_sup 등 6 개, 하악우각부의경우 Go inf_lat 등 10 개, 하악정중부의경우 Me _ant, Me _inf 등총 18 개의계측점을설정한후이를이용하여하악골비대칭시좌우차이를보일수있는 25 개의계측항목을설정및계측한후조사자간및조사자내의재현도를비교평가하였다. 조사자간재현도의경우 25 개의계측항목중 3 개를제외한모든계측항목에서, 조사자내재현도의경우 2 개항목을제외한모든계측항목에서높은재현도를보였다. 아울러본연구에서설정한 18 개의계측점중 Go mid_lat, Go mid_obl 가포함된계측항목이조사자간및조사자내재현도가떨어지는경향을보였다. 이상의연구결과본연구에서사용된 18 개의계측점중 16 개의계측점 ( 과두를나타내는 Cd sup_sup, Cd lat_ant, Cd lat_lat, Cd post_lat, Cd post_post, S, 하악우각부를나타내는 Go inf_lat, Go inf_inf, Go lat_ant, Go lat_lat, Go post_lat, Go post_post, Ag _lat, Ag _inf, 하악정중부를나타내는 Me _ant, Me _inf ) 은높은재현도를보이므로전산화단층사진을이용한하악골 3 차원영상에서안면비대칭환자의진단에유용하게사용될수있음을시사하였다. DOI: /kjod 래두개안면골격형태의진단, 성장및치료평가뿐만아니라비대칭평가에도사용되어져왔다. 그러나이는 3차원적인입체구조물을 2차원적평면에투영하는것이므로길이, 각도, 형태의왜곡현상이발생하여정량적인평가시부정확한결과를야기할수있어진단과치료계획수립및평가시한계가있어왔다. 5-8 이러한 2차원적진단자료의한계를극복하기위하여 Broadbent 4 는 orientator를, Vogel 5 은 modified compensator를이용하여 2차원적투영시나타나는차등적인확대를보상하고자하였고, Berger 9 는안면비대칭진단시이하두정부사진의사용을주장하는등부가적인상의분석을통해보다정확한정보를얻고자하였다. 또한 Grayson 등, 10 Baumrind 등, 11,12 Bookstein 등 13 은정모와측모두부방사선사진을각각촬영한후이를 3차원으로재구성하여두개악안면골격의 3차원형태를재현하였으며, Kusnoto 등 14 은물방울수평계를부착한특별한 facebow를제작하고이를이용해촬영한정모, 측모, 이 314

2 Vol. 38, No. 5, Korean J Orthod 하악골 3 차원영상에서비대칭진단계측항목의재현도 하두정부사진을 3차원으로재구성하는등두개안면골격을 3차원적으로분석, 진단하려는노력이계속되어져왔다. 그러나이와같이 2차원방사선사진을이용하여구성된 3차원이미지는각각의방사선사진촬영시동일한두부위치의재현이어렵고, 4,10,15 sella 등과같이측모방사선사진에서는인지가되나정모방사선사진에서는인지하기힘든계측점들이있는한편, 볼록한곡면위의한점인 gonion 이나 menton 등은측모방사선사진과정모방사선사진에서서로다르게인지될수있어동일한계측점이각각의사진에서다른위치에설정될수있는문제를내포할수있다. 10,13 아울러이와같이 2차원규격사진을 3차원으로합성한이미지는그물구조 10 라는문제때문에두개안면형태의개괄적관찰이불가능하고실제두개골을보는듯한상을구성할수없는한계를지니고있다. 최근컴퓨터기술의발달로 3차원영상프로그램이개발되어전산화단층촬영 (CT) 을이용하여컴퓨터상에서 3차원입체영상을재구성하는방법이시도되고있다 차원 CT 영상에서는두개안면부의입체적인영상을구성할수있어개괄적인관찰이가능할뿐만아니라, 29,30 컴퓨터상에서직접축을조정하여원하는각도로회전시켜관찰할수있으며 16,24,25 상악이나하악등원하는구조물을따로분리하여관찰할수도있고, 24,31 이러한입체영상에서계측점을 3차원좌표계에나타내어평면또는계측점간의거리및각도를측정할수있는등 15,26,32 많은장점이있다. 현재 3차원영상을이용하여환자의두개골모형을제작하고이를악교정수술의치료계획수립과모의수술에사용함으로써치료결과예측에큰도움을얻고있으며, 19,20,25,26,33,34 simulation을통해치료전후를분석하는등 33,34 교정학의여러분야에서 3차원 CT 영상의활용이시도되고있다. 특히안면비대칭의경우 2차원적인진단자료로그양상을정확히파악할수없으므로 3차원입체영상을이용한진단이반드시필요한바 20 안면비대칭환자의 3차원 CT 영상을채득한후이를임상에적용시키는시도가보고되고있으나, 22,23 이들연구는안면비대칭의양상을개괄적으로파악하는정도에그치고있다. 또한안면비대칭의객관적이고정량적인분석및평가를위해서는비대칭을나타낼수있는주요부위의계측점설정과이를이용한계측항목의설정, 나아가계측기준치가필요하나이에대한체계적인연구는전무한실정이다. 이에본연구는 3차원 CT 영상을이용하여하악 골의형태를결정짓는부위로하악과두, 하악우각부, 하악정중부를선택하고각부위의세부위치에따라, 그리고측정시보는각도에따라여러가지계측점을설정하고이를이용한계측항목의재현도를비교평가하여안면비대칭환자진단시유용한계측점및계측항목설정에도움이되고자시행되었다. 연구방법 연구대상 뚜렷한안면비대칭을보이지않으며상하악중심선의차이가 2 mm 이하, Angle 씨 I 급견치및구치관계를갖고치아의배열이가지런한성인정상교합자 40 명 ( 남자 20 명, 여자 20 명 ) 을본연구의대상으로하였다. 본연구에사용된대상자의평균연령은 25 세 3 개월이었다. 연구방법 전산화단층사진촬영연구대상자를 Frankfort 수평면이바닥에수직이되도록전산화단층사진촬영장치 (Light Speed QX/i, GE Medical Systems, Milwaukee, USA) 에위치시키고치열의정중선이촬영장치의장축과일치되게한후, gantry 각도는 0 o, scanning matrix size 는 pixels, 120 kv, 200 ma 조건하에 2.5 mm thickness, slice pitch 3, scanning time 0.8 초로두개악안면부촬영을시행하였고, 1.0 mm 의 slice thickness 로 Digital Imaging & Communication in Medicine (DICOM) 영상정보를구성하였다. 3 차원 CT 영상제작전산화단층사진촬영으로얻은 DICOM 영상정보를개인용컴퓨터로옮긴후 3 차원영상프로그램 (V-works TM, Cybermed Inc., Seoul, Korea) 을사용하여경조직의 3 차원입체영상을제작하였다 (Fig 1). 차후일부계측항목의계측시기준평면으로사용하기위하여좌우측 porion 과우측 orbitale 의세점을이용하여수평기준평면 (Frankfort horizontal plane, FH plane) 을, 그리고수평기준평면에수직이면서 crista galli 와 opisthion 을지나는수직기준평면 (midsagittal reference plane, MSR plane) 을설정하여컴퓨터에저장하였다. 또한하악골의계측점을정확하 315

B, The SOD file of the mandible was made in order to observe the mandible only. 게설정할수있도록하악골만따로분리하여 SOD (selection of demand) 화일을제작하였다 (Fig 2). 계측점설정및계측 1 계측점설정하악골의형태를결정짓는부위로하악과두, 하 악우각부, 하악정중부를선택하고각부위의세부위치에따라, 그리고측정시보는각도에따라구분하여총 18 개의계측점을다음과같이설정하였다. 하악과두의경우에는세부위치에따라최상방점, 최후방점, 최측방점, sigmoid notch 로구분한후, 이 316

4 Vol. 38, No. 5, Korean J Orthod 하악골 3 차원영상에서비대칭진단계측항목의재현도 를정면, 측면, 후면, 상면, 하면등계측시바라보는각도에따라상면에서본과두의최상방점 (Cd sup_sup ) 과측면에서본최후방점 (Cd post_lat ), 후면에서본최후방점 (Cd post_post), 정면에서본최측방점 (Cd lat_ant ), 측면에서본최측방점 (Cd lat_lat ), 그리고측 면에서본 sigmoid notch로세분화하여 6개의계측점을설정하였다. 하악우각부의 gonion은측모두부방사선규격사진에서사용시하악평면과하악지평면의교점 35,36 또는이것이이루는각을이등분하여정하고있으나, 37 3차원입체영상위의해부학적인 Table 1. 5 김고운, 김재형, 이기헌, 황현식 대치교정지 38 권 5 호, 2008 년 계측점을설정하기위하여하악우각부의만곡부위를세부위치에따라최하방점, 중간점, 최후방점으로구분하였다. 그리고측면과하면에서보았을때 gonion 부위의최하방점 (Go inf_lat, Go inf_inf), 측면과후측면에서보았을때만곡의중간점 (Go mid_lat, Go mid_obl ), 측면과후면에서보았을때만곡의최후방점 (Go post_lat, Go post_post), 정면과측면에서보았을때최외각점 (Go lat_ant, Go lat_lat ), 측면과하면에서보았을때 antegonion notch 의최상방점 (Ag _ant, Ag _inf ) 등 10 개의계측점으로세분화하였다. 하악정중부의 menton 38 은정면과하면에서보았을때각각하악 symphysis 의최하방점 (Me _ant, Me _inf ) 으로구분하여총 18 개의계측점을설정하였다 (Table 1, Fig 3). 2 계측항목의설정및계측안면비대칭시좌우차이를보일수있는하악골계측항목으로 ramus length, mandibular body length, condylar neck length, frontal ramal inclination, lateral ramal inclination 등의계측항목을설정한후각항목을다음과같이분류하였다. Ramus length 는하악과두의최상방점 (Cd sup ) 에서하악우각부의최하방점 (Go inf), 중간점 (Go mid), 최후방점 (Go post), 또는 antegonial notch 의최상방점 (Ag) 까지의거리로, mandibular body length 는하악정중부의최하방점 (Me) 에서하악우각부의최하방점 (Go inf), 중간점 (Go mid), 또는최후방점 (Go post ) 까지의거리로, condylar neck length 는하악과두의최상방점 (Cd sup ) 에서 sigmoid notch 의최하방점 (S) 까지의거리로각각설정하였다. Frontal ramal inclination 은하악과두의최측방점 (Cd lat ) 과하악우각부의최측방점 (Go lat ) 을연결한하악상행지의외측연과수직기준평면 (MSR plane) 이이루는각도로, lateral ramal inclination 은하악과두의최후방점 (Cd post ) 과하악우각부의최후방점 (Go post ) 을연결한하악상행지의후연과수평기준평면 (FH plane) 이이루는각도로각각설정하였다. 이를컴퓨터모니터상에서계측점들이잘보이도록각도를조절하여 ramus length 는측면, 사면, 후면, 또는하면에서바라본 3 차원입체영상에서 Cd sup_sup - Go inf_lat, Cd sup_sup - Go inf_inf, Cd sup_sup - Go mid_lat, Cd sup_sup - Go mid_obl, Cd sup_sup - Go post_lat, Cd sup_sup - Go post_post, Cd sup_sup - Ag _lat, Cd sup_sup - Ag _inf 의 8 개항목으로다시세분화하여계측하였다. 6 Vol. 38, No. 5, Korean J Orthod 하악골 3 차원영상에서비대칭진단계측항목의재현도 length 는정면, 측면, 사면, 후면, 또는하면등에서바라본 3 차원입체영상에서 Go inf_lat - Me _ant, Go inf_lat - Me _inf, Go inf_inf - Me _ant, Go inf_inf - Me _inf, Go post_lat - Me _ant, Go mid_lat - Me _ant, Go mid_lat - Me _inf, Go mid_obl - Me _ant, Go mid_obl - Me _inf, Go post_lat - Me _inf, Go post_post - Me _ant, Go post_post - Me _inf 의 12 개항목으로세분화하여계측하였다. Condylar neck length 는 Cd sup_sup - S 로설정하여측면에서바라본 3 차원입체영상에서계측하였다. Frontal ramal inclination 은정면과측면으로구분하여 Cd lat_ant - Go lat_ant to MSR plane, Cd lat_lat - Go lat_lat to MSR plane 의 2 개항목으로, lateral ramal inclination 은측면과후면으로구분하여 Cd post_lat - Go post_lat to FH plane, Cd post_post - Go post_post to FH plane 의 2 개항목으로각각세분화하여계측하였다 (Table 2). 조사자간재현도를알아보기위해 4 명의조사자로하여금계측을시행하게하였는데이중 2 명은 3 차원영상프로그램으로 30 증례이상의하악골입체영상을다룬경험자 ( 조사자 A, B) 로하였으며, 나머지 2 명은 3 차원입체영상프로그램을사용한경험이없는무경험자 ( 조사자 C, D) 로하였다. 아울러계측시기에따른재현도를알아보기위해 3 차원입체영상프로그램유경험자중한명의경우 2 주후에재측정을하게하였다. 거리계측항목인 ramus length, mandibular body length, condylar neck length 의경우 0.01 mm 단위로, 각도계측항목인 frontal ramal inclination, lateral ramal inclination 의경우 0.01 o 단위로컴퓨터프로그램에있는 3-D measure 기능을이용하여측정하였다. 통계처리 SPSS 통계프로그램을이용하여본연구에서설정한계측항목의평균및표준편차를조사자별로산출하였으며, 조사자간의재현도를비교하기위하여분산분석과급내상관 (intraclass correlation) 분석을시행하여유의차를검정하였다. 아울러 3 차원입체영상프로그램사용경험이있는조사자와경험이없는조사자간의차이를알아보기위하여 Tukey grouping 으로사후검정을시행하였다. 또한조사자내재현도를알아보기위하여한명의조사자가측정한 1 회와 2 회계측치사이의차이를 paired t-test 로비교하는한편 Pearson 상관분석과 reliability coefficient 분석을통하여상관성을평가하였다. 연구성적 계측항목의조사자간재현도 본연구에서사용된계측항목의계측자간재현도를알아보기위하여 4 명의조사자들의계측치간에분산분석과급내상관분석을시행한결과, ramus Table 2. Post-hoc comparison by Tukey grouping in the measurements showing significant difference between examiners Ramus length Measurements Mean ± SD p value Cd sup_sup - Go mid_obl A B C D ± ± ± ± Mandibular body length Go mid_obl - Me _ant D C A B ± ± ± ± Go mid_obl - Me _inf C D A B ± ± ± ± Groups connected by horizontal bars were not significantly different. 우계측경험이있는자가작게측정한반면, mandibular body length 를나타내는 Go mid_obl - Me _ant 와 Go mid_obl - Me _inf 의경우계측경험이없는자가경험이있는자에비하여작게측정하는경향을보였다. 특히 Go mid_obl - Me _ant 의경우유경험자 (A, B) 와무경험자 (C, D) 간의뚜렷한차이를보였다 (Table 4). 계측항목의계측시기에따른재현도 계측시기에따른계측치의재현도를알아보기위하여한조사자 (A) 에서 1 회와 2 회의측정치간에 t-test 를시행한결과, ramus length 의 Cd sup_sup - Go mid_obl 와, mandibular body length 의 Go mid_lat - Me _inf 등두항목을제외한모든계측항목에서통계적유의차를보이지않았다 (Table 5). 또한피어슨상관분석및 reliability 상관분석모든항목에서 0.9 이상의높은상관관계를나타내어조사자내재현도는매우높은것으로나타났다 (Table 6). 고찰 두개안면정중선을기준으로상악또는하악의중심이좌측또는우측으로편위되었거나안면고경또는폭경의좌우차이가있는경우이를안면비대칭으로규정하고있다. 39 이러한안면비대칭환자의진단을위해서현재까지임상에서는정모두부방사선규격사진등 2 차원적인사진이주로사용되어왔으나, 이는 3 차원적두개안면구조를 2 차원으로투영하는것이므로상의확대및왜곡으로인해형태 나크기의정량적평가시부적절한결과를야기할수있다. 이러한문제점을극복하고보다정확한안면비대칭의평가를위해서최근에는 3 차원 CT 영상을이용하여입체영상을제작하고이를활용하는방법들이시도되고있다 본연구는이러한 3 차원 CT 영상에서안면비대칭시좌우차이를보일수있는계측점을설정하고, 이를이용한계측항목의재현도를비교평가하여안면비대칭환자의진단시유용한계측점및계측항목설정에도움이되고자시행되었다. 본연구에서는하악골의형태를결정짓는부위로하악과두, 우각부, 정중부를선택하고각부위의세부위치에따라, 그리고측정시보는각도에따라구분하여총 18 개의계측점을설정하였다. 동일한계측점이라도예를들어같은 gonion 이라할지라도, gonion 부위의최하방점, 중간점, 최후방점등위치에따라계측점을설정할때재현도가달라질수있는데, 실제임상에서계측항목설정시 gonion 부위의특정위치가중요할수있지만임상적으로큰차이가없는경우라면재현도가높은점을사용하는것이타당하리라생각되어본연구에서는같은해부학적계측점이라할지라도필요에따라세부위치를구분하여계측점을설정하고계측항목비교를시도하였다. 또한 2 차원적인측모및정모방사선사진에서는보는각도가정해져있는반면, 3 차원입체영상에서는측정시보는각도에따라재현도가달라질수있을것으로판단되었다. 예를들어 gonion 의최후방점을측면에서설정하였을때의재현도와입체영 321

11 김고운, 김재형, 이기헌, 황현식 대치교정지 38 권 5 호, 2008 년 Go mid_obl 가포함된계측항목임을고려시하악우각부를나타내는여러가지계측점중 Go mid_obl 의사용은재현도측면에서볼때바람직하지않음을알수있었다. 3 차원영상계측경험유무가재현도에미치는영향을알아보기위하여조사자간유의한차이를보이는 3 개의항목에대하여사후분석을시행한결과, 계측경험이없는조사자의경우 ramus length 를나타내는 Cd sup_sup - Go mid_obl 는크게측정한반면, mandibular body length 를나타내는 Go mid_obl - Me _ant 와 Go mid_obl - Me _inf 의경우는작게측정하는경향을보여계측프로그램을사용한경험유무에따른계측치의차이가있음을알수있었다. 이는 Go mid_obl 를포함하는계측항목설정시계측경험이없는조사자의경우계측경험이있는조사자보다 Go mid_obl 를약간하방으로설정하여 ramus length 의 Cd sup_sup - Go mid_obl 는큰값이, mandibular body length 의 Go mid_obl - Me _ant, Go mid_obl - Me _inf 는작은값이측정된것으로여겨진다. 이는 Go mid_obl 의정의가후측방에서보았을때의 Go mid 인데, 경험이없는조사자의경우 3 차원영상을회전시키는과정에서후측방보다는후측하방에서의 Go mid 를설정한것으로나타났다. 본연구결과조사자간재현도차이가나는 3 개항목모두 3 차원영상프로그램사용경험유무에영향을받는경향을보였는데이는 3 차원프로그램사용이숙달된다면조사자간의차이는매우줄어들수있음을시사하여조사자간재현도가임상적으로큰문제가되지않음을, 즉 3 차원입체영상이임상에서큰무리없이사용될수있음을보여주었다. 계측시기에따른재현도를알아보기위하여 2 주간격으로 2 회측정한계측값을 t-test 로비교한결과, ramus length 의 Cd sup_sup - Go mid_obl, mandibular body length 의 Go mid_lat - Me _inf, 2 개의계측항목에서만통계적으로유의한차이가나타났으며, 피어슨상관분석에서도모든계측항목에서 0.9 이상의상관관계를나타내어높은재현도를보였다. Kragskov 등 32 은 3 차원 CT 와 metallic marker 를사용한일반적인측모, 정모두부방사선규격사진과의비교연구에서 3 차원 CT 계측점의재현도가낮아정상인에서보다는악안면기형이있는환자에서사용될수있다고보고한바있다. 이와같이 Kragskov 등의연구에서 3 차원 CT 계측점의재현도가낮게나타난것은 3 차원 CT 영상에서계측점설정시측모및정모두부방사선사진계측치와의비교를위하여 2 차원사진 에서의계측점의정의를그대로 3 차원영상에적용시켰기때문인것으로생각된다. 반면본연구에서는모든계측점을 3 차원영상에서설정할수있도록별도로정의한바그재현도가높아진것으로판단된다. 따라서임상적으로 3 차원입체영상의분석을위해서는 3 차원영상에서의별도의계측점설정이중요함을알수있었다. 본연구에서사용된 18 개계측점중 gonion midpoint (Go mid_lat, Go mid_obl ) 를포함한계측항목들이전반적으로재현도가낮게나타났는데이는 gonion midpoint 의세부위치에대한정의가모호하였기때문으로여겨진다. 따라서계측점정의시에는구체적이고명확한기술이필요할것으로여겨지며이것이불가능할경우에는이의사용을피하고다른위치의계측점을새로이설정하는것이바람직함을알수있었다. 또한 3 차원영상에서같은계측점을측정시보는각도에따라구분하였을경우, 예를들어 mandibular body length 계측시 Go post_lat - Me _ant, Go post_post - Me _ant 를각각비교하였을때, gonion 최후방점을측면에서설정한계측치와후면에서설정하여계측한계측치간에차이가있을것으로예상되었다. 즉같은 Go post 점이라할지라도측면보다는후면에서설정시더욱후방에즉, mandibular body length 가길게측정될것으로생각되었다. 그러나본연구결과이경우각각의계측치는 과 mm 로뚜렷한차이를보이지않아측정시보는각도에따른차이는없는것으로나타났다. 따라서본연구에서는보는각도에따라다양하게계측점을구분하여재현도를비교해보았으나실제임상에서는계측항목의설명이용이하고측정이간편한어느한각도로통일하여계측항목을간단화하는것이가능함을알수있었다. 또한한편으로는계측점정의시세부위치만기술하고측정시보는각도는별도로기술하지않더라도임상적으로큰문제가되지않음을알수있었다. 본연구에서설정된대부분의계측점및계측항목은그재현도가높은것으로나타나비대칭시좌우차이를보일수있는계측점을 3 차원입체영상에서정의하고이를이용하여거리및각도계측을한다면 3 차원입체영상에서도정량적계측이가능하게되어안면비대칭분석에큰도움을줄수있으리라생각된다. 한편본연구에서는 frontal 및 lateral ramal inclination 계측시수평및수직기준평면을사용하였 324

12 Vol. 38, No. 5, Korean J Orthod 하악골 3 차원영상에서비대칭진단계측항목의재현도 는데, 하악골의비대칭을나타내는계측항목의재현도를비교함으로써임상적으로유용한계측점들을알아보는데그목적이있어수평및수직기준평면을한명의조사자가설정한후저장시킴으로써모든계측자는동일한기준평면을사용하게하였다. 그러나실제임상에서는각계측자가별도로기준평면을설정하여야하므로기준평면설정의차이에따라계측치의차이도증가할것으로예상된다. 차후 3 차원입체영상이임상에서보다유용하게사용되기위해서는정확하고도재현가능한기준평면설정에대한체계적인연구가필요할것으로생각된다. 결론 본연구는하악골 3 차원입체영상에서비대칭계측항목들의재현도비교를통하여안면비대칭진단시유용한계측점및계측항목설정에도움이되고자시행되었다. 뚜렷한안면비대칭을보이지않는성인정상교합자 40 명을대상으로두경부전산화단층사진을촬영한후각각의단면촬영영상정보를컴퓨터프로그램을이용하여 3 차원입체영상으로재구성하였다. 하악골형태를결정짓는부위로하악과두, 하악우각부, 하악정중부를선택하고각부위의세부위치에따라, 그리고측정시보는각도에따라구분하여하악과두의경우 Cd sup_sup 등 6 개, 하악우각부의경우 Go inf_lat 등 10 개, 하악정중부의경우 Me _ant, Me _inf 등총 18 개의계측점을설정하였다. 이를이용하여하악골비대칭시좌우차이를보일수있는계측항목으로 ramus length 를나타내는 Cd sup_sup - Go inf_lat 등 8 개의항목, mandibular body length 를나타내는 Go inf_lat - Me _ant 등 12 개의항목, condylar neck length 를나타내는 Cd sup_sup - S, frontal ramal inclination 를나타내는 Cd lat_ant - Go lat_ant 등 2 개의항목, lateral ramal inclination 을나타내는 Cd post_lat - Go post_lat 등 2 개의항목, 총 25 개의계측항목을설정한후조사자간의차이를알아보기위하여 3 차원영상계측경험이있는 2 명의조사자와계측경험이없는 2 명의조사자가각각계측을시행하였으며, 계측시기에따른차이를알아보기위하여한명의조사자의경우 2 주후재계측을시행하고각계측항목의재현도를비교평가하여다음과같은결과를얻었다. 1. 조사자간의계측치비교를통하여조사자간재현 도를알아본결과, 본연구에서사용된 25 개의계측항목중 3 개항목 (ramus length 를나타내는 Cd sup_sup - Go mid_obl 와 mandibular body length 를나타내는 Go mid_obl - Me _ant, Go mid_obl - Me _inf) 을제외한모든계측항목에서재현도가높은것으로나타났다. 2. 조사자간재현도가낮은항목에서계측치간차이를살펴본결과, 3 차원영상계측경험이있는조사자와경험이없는조사자간에차이가나타나는경향을보였다. 3. 계측시기에따른비교를통하여조사자내재현도를알아본결과, 본연구에서사용된 25 개의계측항목중 2 개항목 (ramus length 를나타내는 Cd sup_sup - Go mid_obl 와 mandibular body length 를나타내는 Go mid_lat - Me _inf ) 을제외한모든계측항목에서재현도가높은것으로나타났다. 4. 본연구에서설정한 18 개의계측점중 Go mid_lat, Go mid_obl 가포함된일부계측항목은조사자간및조사자내재현도가떨어지는경향을보였다. 이상의연구결과, 과두를나타내는 Cd sup_sup, Cd lat_ant, Cd lat_lat, Cd post_lat, Cd post_post, S, 하악우각부를나타내는 Go inf_lat, Go inf_inf, Go lat_ant, Go lat_lat, Go post_lat, Go post_post, Ag _lat, Ag _inf, 하악정중부를나타내는 Me _ant, Me _inf 등의계측점은높은재현도를보이므로전산화단층사진을이용한하악골 3 차원영상에서안면비대칭환자의진단에유용하게사용될수있음을시사하였다. 참고문헌 1. Methods: Facial CT images were obtained from forty normal occlusion individuals. Eighteen landmarks were established from the condyle, gonion, and menton areas, and 25 measurements were constructed to represent asymmetry of the mandible; 8 for ramus length, 12 for mandibular body length, 1 for condylar neck length, 2 for frontal ramal inclination, and 2 for lateral ramal inclination. Inter- and intra-examiner reproducibility of the measurements was evaluated. Results: Inter-examiner reproducibility of the measurements proved to be high except for 3 measurements. Intra-examiner reproducibility also proved to be high except for 2 measurements. Interand intra-examiner reproducibility of the measurements including Gonion proved to be low. Conclusions: The results of the present study indicate that the landmarks and measurements constructed in 3D CT images can be used for the diagnosis of facial asymmetry.
