Especially, women who have anti-ssa/ro and anti-ssb/la auto-antibodies increase the risk of developing fetal atrioventricular (AV) block. It is difficult to indentify fetal AV block in the early stage of the disease, which is consequently found after the establishment of complete AV block as the form of fetal arrhythmia. To have a chance to reverse this serious disorder, we should detect fetal AV block in the early stage before it progresses to complete AV block. We experienced a case of pregnant systemic lupus erythematosus woman who has anti- SSA/Ro and anti-ssb/la auto-antibodies monitored by cardiac doppler sonography; We detected fetal first degree AV block and successfully treated with dexamethasone. We report systemic lupus erythematosus women who increased risks of fetal AV block with a brief review of relevant literatures. Keywords: Systemic lupus erythematosus; Congenital heart block; Atrioventricular block; Anti-SSA/Ro and anti-ssb/la antibodies 전신홍반루푸스 (systemic lupus erythematosus, SLE) 가있는여성이임신을하는경우임신중산모나태아에게여러가지합병증들이나타날수있는데, 특히 SLE에서확인되는자가항체중 anti-ssa/ro 또는 anti-ssb/la 항체가양성인경우, 태아의선천성심장방실차단의발생과연관성이높다고알려져있다 [1]. 태아의선천성심장방실차단은 1도나 2도차단의경우서맥이나타나지않기때문에조기진단이어려운경우가많다. 따라서심장방실차단이발견되는경우대다수에서서맥을동반하는 3도방실차단일때가많다. 태아의심장방실차단중 1, 2도차단은 fluorinated steroids (dexamethasone, betamethasone) 의투여로치료될수있는가역적인상태이고, 3도차단은치료되지않는비가역적인상태이다. 따라서태아의심장방실차단은치료가어려워지는비가역적인상태로진행하기전에조기진단하여치료하는것이무엇보다중요하다. 도플러커서 를위대정맥 (superior vena cava) 과오름대동맥 (ascending aorta) 을교 차하도록위치시키면이부위에서위대정맥과오름대동맥의혈류속도 를측정할수있다. 위대정맥의역행성혈류는심방의수축기를의미하 며, 오름대동맥의전진성혈류는심실의수축기를의미한다 (Fig. 1) [2]. 그시작점의시간적차이를직접측정하였다.

These values were within the normal range (reference range, 111±17 ms). 1. 초음파를이용한 AV time interval 의측정방실차단의유무를진단하기위해서초음파기기 (Prosoundα10, ALOKA, Korea) 의 M-mode, pulsed Doppler 를이용할수있으며, 상 기환자에서는 pulsed Doppler 를이용하여측정하였다. 도플러커서 를위대정맥 (superior vena cava) 과오름대동맥 (ascending aorta) 을교 차하도록위치시키면이부위에서위대정맥과오름대동맥의혈류속도 를측정할수있다. 위대정맥의역행성혈류는심방의수축기를의미하 며, 오름대동맥의전진성혈류는심실의수축기를의미한다 (Fig. 1) [2]. 그시작점의시간적차이를직접측정하였다. 정상 AV time interval 은 111 ± 17 ms 이다 [2]. These values were prolonged compared to normal range (reference range, 111±17 ms). 치료받고있던자로자연임신되어임신 8 주 3 일에본원산부인과를 내원하였다. 환자는산과력 인초산모였고, 임신전월경력상 간격은 30 일로규칙적이었고월경량은보통이었으며월경통은없었다. 최종월경시작일이 2009 년 12 월 28 일로분만예정일은 2010 년 10 월 13 일이었다. 산모는과거병력상 SLE 외에는특별한병력이없었고내 원당시 SLE 치료제로 prednisolone 10 mg 을매일복용중이었다. 산모는임신초기에자가면역항체검사를시행하였고 anti-ssa/ro 와 anti-ssb/la 항체가모두 >200 u/ml 으로양성이었다. 따라서산 모는산전검사로내원시마다초음파로태아의 AV time interval 을연속 적으로확인하였다. 임신 26주 1일에시행한초음파검사상 AV time interval 이 138 ms로정상범위보다길어져 1도방실차단을진단하였다 (Fig. 2). 그리고방실차단진단직후산모에게 dexamethasone 을매일 4 mg씩복용하게하였고복용 1주일만에 AV time interval 은 114 ms로정상화되었다 (Fig. 3). 그이후로도매주초음파검사를시행하였고 AV time interval 은정상범위를유지하였다. 진단이후, 산모는 dexamethasone 을매일 4 mg씩지속적으로복용하였고복용 9주째되던임신 35주에심한자궁내성장지연소견을보여복용용량을반으로감량하였다. 산모는임신 37주에연고지상타병원에서자궁내성장지연으로유도분만시행하여 1.58 kg의여아를분만하였고신생아선천성심장방실차단은발견되지않았다. 고찰 전신홍반루푸스는자가항체와면역복합체에의해조직특히결체조직및세포가손상을받는원인불명의자가면역질환이다. 여성대남성의발병비는 9대 1이며, 주로 10대후반에서 40대초반의가임기여성 503

3 KJOG Vol. 55, No. 7, 2012 에서빈발하므로 SLE 환자의임신과그결과는 SLE 환자관리에있어서매우중요하다 [3]. 일반적으로 SLE는여성의수태능에영향을미치지는않지만, SLE 환자가임신을하는경우산모에게는자간전증의발생이증가하고태아에게는유산, 사산, 조기진통및자궁내성장지연등의발생빈도가증가하며출생후신생아루푸스증후군이나타날수있다 [4]. 신생아루푸스증후군이란 anti-ssa/ro 또는 anti-ssb/la 항체가양성인 SLE나, Sjogren s syndrome 같은류마티스질환이있는산모에서태어난아기가일시적피부염, 혈소판감소증및선천성심장방실차단등의특징을갖는질환이다. 이중선천성심장방실차단은다른증상이출생 6개월이내에소실되는것과달리영구적이고치명적이다 [4]. SLE 산모에서태아의선천성심장방실차단의발생은 SLE 자체가위험인자가아니라 anti-ssa/ro 또는 anti-ssb/la 항체의존재에의한것이다 [4]. 따라서이자가항체가양성인다른자가면역성결체조직질환에서도태아의심장방실차단이발생할수있다. 태아에서선천성심장방실차단이발생하는빈도는 15,000-22,000 명의출생아중한명정도인것에비해산모에서 anti-ssa/ro 또는 anti-ssb/la 항체가양성인경우는그빈도가 1%-2% 로매우증가한다. 이는이러한자가항체들이태반을통과해태아의심장전도계에영향을주어섬유화나석회화를유발하기때문이다 [5]. 신생아에서선천성심장방실차단이진단된경우 90% 이상에서진단당시 3도완전방실차단상태로진단된다 [6]. 완전방실차단은비가역적이며, 한연구에의하면전체사망률이 19% 로보고되었고이중자궁내에서사망하는경우가 27%, 출생후 3개월이내사망하는경우가 45% 에달했다고한다 [7]. 완전방실차단으로출생한신생아의경우태어나자마자인공심박조율기를필요로하는경우가많은데이러한치료를조기에시행함에도불구하고나중에후발성확장성심근병증 (late-onset dilated cardiomyopathy) 이발생하기도한다 [8]. 따라서선천성심장방실차단을치료가가능한 1도또는 2도방실차단상태에서조기진단하여완전방실차단으로진행하는것을막는것이중요하다. 선천성심장방실차단의치료에는 fluorinated steroids (dexamethasone, betamethasone) 가사용된다. Fluorinated steroids 는지난 30년간태아의폐성숙을위해사용되었던약물로약 20년전태아의방실차단치료를위해처음사용되었다 [6]. 이러한 fluorinated steroids 는태반에서부분적으로만대사되어태반을통과하여활성화형태로태아에게작용함으로써불완전방실차단이나태아수종등의치료에유용하게사용된다 [9]. 반면 non-fluorinated steroids (prednisone, prednisolone, methylprednisolone) 는태아가아닌산모의치료적응증에따라제한적으로사용하는약물로태반을통과하지않아태아의방실차단치료에는효과가없다 [10]. Betamethasone 과 dexamethasone 의효과및부작용등의차이에대한대규모무작위연구는아직없으나몇몇연구에서산모에게 betamethasone 을사용하는것이 dexamethasone 에비해태아신경발달에더좋다고보고하였고 [11,12], Brownfoot 등 [13] 은조기진통이있는산모에게태아폐성숙을위해 dexamethasone 을사용 한경우 betamethasone 에비해뇌실내출혈이적었다고보고하였다. 저자들은둘중에서본원에서처방가능한 dexamethasone 을환자에게투여하였다. 저자들은태아에서 1도방실차단이확인된산모에게 dexamethasone 을투여함으로써 AV time interval 이정상화되는것을경험하였다. 이처럼 dexamethasone 을투여하여 1도방실차단을치료하였다는몇몇외국의연구보고가있다. Vesel 등 [5] 에의한연구에서는 anti-ssa/ro 항체가양성인산모에서임신 25주에 1도방실차단을확인하였고진단이후 dexamethasone 4 mg을매일복용시켰다. 그결과 2주만에 AV time interval 이정상화되었고이후 2주간약을더복용하던중양수과소증이발생하여약물복용을중단하였으나이후검사에서도 AV time interval 은정상범위를유지하였다. Rein 등 [6] 에의한연구에서는 anti-ssa/ro 또는 anti-ssb/la 항체가양성인 70명산모의태아를대상으로연구하였다. 이중 6명의태아에서임신 21-34주사이에 1도방실차단이확인되었고진단직후산모는 dexamethasone 4 mg을매일복용하였다. 그결과치료시작후 3-14일내에 AV time interval이정상화되었고약물복용은분만전까지유지하였으며 AV time interval 도정상범위를유지하였다 [6]. 국내에서는태아심장방실차단의치료에대한보고는아직없으며정등에의한증례보고에서임신 29주산모의태아에게완전방실차단소견이보여무증상의산모에게 anti-ssa/ro 와 anti-ssb/la 항체검사를시행하였고, 그결과항체가양성임을확인하였다. 그후경과관찰하던중만삭에출산한태아에게완전방실차단을동반한신생아루푸스증후군이확진되었던증례를보고한바있다 [14]. 임신중 fluorinated steroids 의사용은산모에게감염, 골다공증, 골괴사, 당뇨등의위험을높이고, 태아의중추신경계, 망막, 폐, 부신등의발달에유해한영향을줄수있으며, 특히조산이나자궁내성장지연, 양수과소증을유발한다고알려져있다 [11]. 따라서약물을사용하는경우부작용발생유무를잘확인하며이에주의해서사용하여야한다. 저자의증례에서도 dexamethasone 을복용한산모의태아에서자궁내성장지연이나타났다. 이는 SLE의합병증일수도있고방실차단치료를위해투여한 dexamethasone 의부작용일수도있다. 1도방실차단은상당수에서스테로이드치료없이도정상화된다고하나일부에서는완전방실차단으로진행된다 [15]. 완전방실차단은심장전도계의섬유화및석회화로스테로이드투여로치료되지않고, 출생후심근병증등의합병증이발생할수있어치명적이다. 따라서저자들은산전에 1 도방실차단을진단한경우치료없이 PR interval 이정상화되길기다리는것보다는 fluorinated steroids 를사용하여이것들의합병증보다더치명적일수있는완전방실차단으로의진행을예방하는것이더중요하다고판단하였다. SLE나기타자가면역성결체조직질환이있는여성이임신을한경우산전에 anti-ssa/ro 와 anti-ssb/la 항체유무를확인해야하며이것이양성인경우에는산전검사에서태아심장의 AV time interval 을연속적으로확인하여심장방실차단을조기에발견하는것이중요하다. 이렇게진단된 1도방실차단은 dexamethasone 투여를통해치 504
