Methods:DRD2 Taq I A RFLP genotyping was carried out with DNA extracted from blood samples of 75 patients with tic disorders(47 with TS and 28 with CMT) and 90 healthy subjects. Genotype and allelic frequencies for the DRD2 gene polymorphisms of the tic disorder group as a whole were compared to those of the control group. Separating the TS group, thereafter, the frequency of genotypes and alleles were compared to those of the controls. Results:The results demonstrated that genotype and allele distributions for the DRD2 gene polymorphism in the tic disorder as a whole, TS, and control groups were not significantly different. Conclusion:No association was found for DRD2 gene, TS and CMT. The data suggest that DRD2 gene may not be a useful marker for the prediction of the susceptibility of tic disorder. 2 서론 뚜렛증후군 (Tourette syndrome, 이하 TS) 는소아기또는청소년기때운동틱과음성틱이시작되어악화와완화를반복하며지속되는신경행동학적질병으로 Diagnostic and Statistical Manual of Mental Disorders, edition 4( 이하 DSM-IV) 1) 진단기준에의하면 18세이전에증상이시작되어증상이없는기간이 3개월미만이어야한다. 만성운동틱장애 (chronic motor tic disorder, 이하 CMT) 혹은만성음성틱장애 (chronic vocal tic disorder, 이하 CVT) 는만성틱장애 (chronic tic disorder, 이하 CT) 로써운동틱과음성틱이각각한가지만 1년이상나타나는경우로 TS의진단기준에는부합되지않는경우이다. 이와같이진단체계에서는 TS와 CMT 또는 CVT 를구분하고있으나, 이들질환이원인적으로서로관련이있다는제안이꾸준히있어왔다. 아직까지논란이있으나, 2) TS는 CMT 또는 CVT 의보다심한아형으로이해되고있다. 가족및유전연구에서이들질환은유전적으로서로밀접한관계가있는것으로나타나고있다. CT와 TS이같은가계내에서전달되는것으로보고되었고, 3)4) TS 환자의가족력을조사한연구 5) 에서가족내 TS 발병만을조사한경우보다 CT까지포함해서조사했을때질병의침투도 (penetrance) 가더증가했다고하였다. 따라서일종의소위 TS 유전자가일과성틱장애 (transient tic disorder), CT, 그리고 TS와같이다양하게표현되는것으로이해되고있다. CT와 TS을구분해서다양한신경심리학적및정신사회적장해, 동시이환질환과같은변인을비교한연구 6) 에서도두질환모두정상군과는구분되면서서로간에는별차이가없이유사하게나타났다. 이러한연구결과들을종합했을때 CT와 TS이공통된원인인자를갖는서로다른표현형이며, TS이 CT 의보다심한형태의아형으로이해된다. 틱장애는도파민체계의결함으로인해발생하는것으로알려져있는데, 7) 이에대한근거로틱증상이 haloperidol이나 pimozide와같은신경이완제와같은도파민수용체차단제에의해효과적으로조절되고, 그밖에도파민합성을차단하는 alpha-methylpara-tyrosine 8) 이나도파민신경의전접합부 (presynaptic) 에서도파민의저장을차단하는 tetrabenazine 9) 에의해틱 이억제되는현상을들수있다. 도파민수준을증가시키는 L-Dopa 10) 와도파민체계를자극시키는 amphetamine이나 methylphenidate에의해틱이악화되는현상 11) 이나투여하던신경이완제를줄였을때틱이악화되는현상 12) 또한이를뒷받침하고있다. 이로써틱장애는후접합부 (postsynaptic) 도파민수용체의숫자나친화도가높아져전체적으로도파민에대한민감도 (sensitivity) 가증가되어나타나는현상이라는가설이성립가능하다. 도파민 D2 수용체는도파민신경의후접합부 (postsynaptic) 에위치한수용체로써주로중뇌피변연계 (mesocorticolimbic) 경로와관련하여분포되어있다. 13) D2 수용체가매개하는신경전달은사람의운동을조절하는데핵심적인역할을하고또한보상이나강화기전과관련이된다. 이러한도파민 D2 수용체유전자가 TS와관련된다는연구결과가 Comings 등에의해 1991 년에처음으로보고 14) 되었다. 이들연구자들은도파민 D2 수용체유전자의 TaqI A 다형성에대한분석결과, A1 대립유전자 (allele) 가 TS와관련이있으며, 이는또한질병의심한정도와도관련된다고하였다. 14)15) 그러나연관분석 (linkage study) 에서도파민 D2 수용체유전자와의연관이없다는연구결과들이있었고, 16-18) 뒤이은 Gelernter 등의연구 19) 에서이유전자가 TS의심한정도에영향을미치지않는다고하였다. 환자가계를이용한연합연구 (family-based association study) 에서도 Comings 등의결과 14)15) 가재확인되지않았고, 20) 같은방식으로프랑스계캐나다인들을대상으로실시한분석에서도 A1 대립유전자가더많은경향이있는정도로나타났다. 21) 그러나최근에대만에서 151 명의 TS 환자들을대상으로이루어진 Lee 등 22) 의연합연구에서는 TS군에서 A1 동형접합체의빈도가정상군에비해의미있게더많았고, 또한 A1 대립유전자의비율이정상군의것보다의미있게높았다. 이에저자들은도파민 D2 수용체유전자나인접한연결된유전자가 TS에대한취약성을설명한다고주장하였다. 이와같이도파민 D2 수용체유전자의 TaqI A 제한효소분절길이다형성 (restriction fragment length polymorphism, 이하 RFLP) 의 TS와의관련성에대해서는아직까지찬반으로논란이되고있다. 이에본저자들은한국인 TS 그리고 CT 환자들과정상인들을대상으로도파민 D2 수용체유전자의 TaqI A 다형성의빈

3 도를조사하여도파민 D2 수용체유전자와틱장애의유전적관련을파악하고자하였다. 방법 1. 연구대상환자군은 2002년 3월부터 2004년 2월까지순천향대학교부속부천병원과가천의과대학부속인천길병원정신과외래에서 DSM-IV 1) 에근거하여 TS와 CMT 로진단된환자중에아동이나청소년의경우는부모가그리고성인인경우는환자자신이본연구에자발적으로참여하는것에동의한경우를대상으로하였다. 대상군에서제외기준은정신지체와전반적발달장애를함께보이는경우로하였고, 과거력이나현병력에서뇌손상이나경련성질환을비롯한기질적원인이있거나약물이나알코올문제가있는경우도제외하였다. 그밖에도파민체계가발병기전과관련이있을수있는정신분열병, 양극성장애와주요우울장애가있는경우도대상군에서제외하였다. 그결과총 75명의틱장애환자 (TS=47 명, CMT=28 명 ) 가연구에포함되었으며, 이들의평균연령은 11.9±5.3 세 ( 범위 =5~41세 ) 였고, 남녀구성비는 68:7이었다 ( 표 1). 모든대상은가계력상서로관련이없었다. 정상대조군은같은기간에고려대학교건강의학센터를방문한수진자중에기초설문조사에서정신과적치료병력이없는경우를대상으로하였다. 이들중본조사의대조군에포함될수있는지를조사하기위해추가면담과채혈에자발적으로응하는사람총 145 명을대상으로하였다. 선별은일대일면접방식으로진행하였는데우선환자군과동일한배제질환에해당되는지를조사하였고, 두번째로틱장애와유전학적으로밀접한연관이있는강박장애 23) 가과거에있었거나현재있는지를조사하였다. 그리고이와같은배제사유가본인과부계측과모계측 4촌이내의친인척과형제, 자매또는자녀중에있는경우는연구에서배제하였다. 다음으로 틱과관련하여서는눈깜빡거림, 찡그리거나입을벌림, 고개끄덕임, 어깨나배또는상체를으쓱거리거나꿈틀거림, 팔다리의반복되는흔들거림이나꿈틀거림과같은운동틱과킁킁거리거나헛기침, 혀를차거나기타다른소리를반복해서내는음성틱을대상자에게직접시연하여존재여부를조사하였다. 그리고이와같은운동틱과음성틱이본인과부계측과모계측 4촌이내의친인척과형제, 자매또는자녀중에있는경우를제외하였다. 그결과 145명중 90명만이연구에포함되었으며, 이들의평균연령은 48.2±12.7 세였고, 남녀구성비는 75:15이었다. 환자군과대조군의성별분포에는차이가없었으나, 평균연령은환자군에비해대조군에서유의하게높았다 (t=23.0, df=163, p<0.001). 2. 유전자분석환자군과대조군으로부터정맥혈 10cc를 EDTA 시험관에채혈하여냉장보관하였다가 24시간이내에 Wizard DNA 추출키트 (Promega, 미국 ) 를이용하여 DNA 를추출하였다. 시발체 (5'-CCGTCGACGGCTG GCGAAGTTGTCTA-3'/5'-CCGTCGACCCTTCCT GAGTGTCATCA-3') 를사용하여 Genomic DNA 를중합효소연쇄반응 (polymerase chain reaction, 이하 PCR) 시켰다. 사용된표준 PCR은 100ng의게놈 DNA 가포함된 25μl 의시료에 10pmol 의시발체, 50mM KCl, 10nM Tris-HCl(pH 8.3), 1.5mM MgCl 2, dntp 각 200μmol과 0.3U Taq DNA Polymerase(Takara, 일본 ) 을같이넣어서시행하였다. 자동온도조절기 (9000 M Perkin-Elmer, 미국 ) 를이용하여증폭하였으며, 95 에서첫 5분간변성을시킨후에 95 에서 30초, 50 에서 30초, 72 에서 30초의 3단계를 35주기반복한후, 72 에서 10분간유지하였다. PCR 산물을 TaqI A 제한효소 (New England Biolabs, 미국 ) 로 16시간반응시킨후에, 2% agarose gel 에서 100bp DNA 분자량표지자 (molecular weight marker, Takara, 일본 ) 과함께전기영동하여절단양상을확인하였다. 분석은환자군과 Table 1. 4 대조군여부를모르는연구자에의해이루어졌는데, 절단된산물중에 310bp 한개의 band 로나타나는것을 A1 대립유전자로, 그리고 130bp 와 180bp 두개의 band 로나타나는것을 A2 대립유전자로분류하였다. 유전자형 (genotype) 은한개의 310bp band로나타나는경우는 A1A1 동형접합체, 310bp, 180bp, 130bp 세개의 band 로나타나는경우는 A1A2 이형접합체, 그리고 180bp, 130bp 두개의 band로나타나는경우를 A2A2 동형접합체로판정하였다. 3. 통계방법환자군과대조군사이의유전자형과대립유전자의발현빈도는 χ 2 검증을통해분석하였다. 우선 TS 와 CMT 전체틱장애군과대조군을비교하였고, 다음으로환자군중에서 TS군만을별도로대조군과비교분석하였다. 유전자형의분포를 Hardy-Weinberg 평형의예측치와비교하였다. 통계분석은 SPSS v10.0(spss Inc., Chicago, IL, 미국 ) 를이용하였고, 통계적유의성수준은 p<0.05으로하였다. 결과 환자및대조군에서개인별로 DRD2 유전자의 TaqI A 다형성이분석되었다. 그결과나타난유전자형의분포는전체틱장애군 (p=0.55), TS군 (p=0.54), 그리고대조군 (p=0.61) 에서모두 Hardy-Weinberg 평형을위배하지않았다. TS와 CMT 로구성된전체틱장애군에서 DRD2 유전자형의빈도는 A1A1, A1A2, A2A2 가각각 17.3%, 56.0%, 26.7% 으로, 대조군의 12.2%, 53.3%, 34.4% 와차이가없었다 ( 표 2). 전체틱장애군의대립유전자의빈도는 A1과 A2가각각 45.3% 와 54.7% 로, 대조군의 38.9% 와 61.1% 유의한차이가없었다. TS군만을별도로분석하였을때 A1A1, A1A2, A2A2 유전자형의빈도가 19.1%, 53.2%, 27.7% 로대조군의빈도와차이가없었고, A1과 A2 대립유전자의빈도가 45.7% 와 54.3% 로역시대조군의빈도와유의한차이가없었다. 고찰 뚜렛증후군을포함한틱장애는도파민계신경회로가병태생리학적으로관여하는유전성이강한질환이다. 24) 틱장애와대조군을대상으로한연합연구에서조사된후보유전자에는도파민수용체 (DRD1, DRD2, DRD4, DRD5), 도파민운반체, 노르아드레날린성 (ADRA2a, ADRA2c, Dopamine beta hydroxylase) 그리고세로토닌운반체유전자등이있다. 25) 실제로도파민 D2 수용체는틱장애연구에있어매우좋은후보유전자이다. 그기능이나발현되는부위때문만이아니라신경이완제의일차적인작용부위에해당되기때문이다. DRD2 유전자다형성부위중 TaqI으로소화시켰을때분절되는 RFLP 중 A1 대립유전자가알코올중독에서유의하게높게나타났다는보고 26) 가있은이래여러정신과적질환에서이유전자부위에대한연구가진행되어왔다. 아직까지논란의여지는있으나실제로 A1 대립유전자가뇌의낮은 D2 수용체밀도와관련이있다는연구결과들도뒷받침되고있다. 27)28) 본연구에서는 TS와 CMT 환자가이 DRD2 유전자 TaqI A 다형성과관련이없는것으로나타났다. 환자군 Table 2. 5 중에보다심한형태인 TS만을별도로분석하였을때에도관련이없었다. 이러한결과는초기의 Comings 등의연구들 14)15)29) 과최근대만인을대상으로실시한 Lee 등 22) 의연구결과들을반박하는것이다. 특히 Lee 등 22) 은 TaqI DRD2 A1 동형접합체의경우 A2 동형접합체에비해 TS가발병할 odds ratio 가 2.253라고제시하기도하였다. 그러나 Nothen 등 20) 과 Diaz-Anzaldua 등 21) 의연구에서는본연구와마찬가지로이전에보고된관련성을재확인할수없었다. 이와같이연구결과들이일관되지않게나타나는이유는여러측면에서생각해볼수있다. 우선기본적으로해당유전자가작은효과를갖고있기때문일수있다. 최근견해로주된역할을하는단일유전자대신작은효과를갖는여러유전자들이복합적으로작용하여 TS의발병에관여하는것으로이해되고있다. 25) 다른이유로인종학적다양성 (heterogeneity) 때문일수있다. 서구에비해한국인들은비교적유전적으로동질성을지닌것으로간주된다. 인구층화 (population stratification) 가단점으로작용하는환자-대조군연합연구의방법론적인측면을고려했을때한국인들을대상으로실시한본연구가나름의의의가있다고생각된다. 세번째로틱장애는감염과같은환경적인영향, 자가면역학적영향, 후성학적인요소 (epigenetic factor) 등유전자와복잡한상호작용을통해발병할수있다. 따라서보다포괄적인연구방법론이적용될필요가있다. 본연구의제한점은다음과같다. 첫째, 연구의표본크기가작고, 둘째, 환자군과대조군간에평균연령의차이가있다는점이다. 우리나라의임상실제에서틱장애로병원에내원하는경우는대개소아혹은청소년기때가많다. 그런데 DSM-IV 의진단기준에는 TS와 CMT 가 18세이전에발병해야한다고명시하고있기때문에이를감안했을때아직까지병발할가능성이있는 18세미만의아동이나청소년은대조군에포함시킬수가없다. 따라서자연히환자군과대조군사이에는평균연령에차이가있을수밖에없다. 이에본연구에서는대조군의높은연령때문에과거에틱장애가있었으나이를망각하거나부정할가능성을최소화하기위해선별과정에서소아청소년정신과의사가각틱증상을직접시연하여해당되는경우배제하는방식을택했다. 셋째, 본연구는틱장애군에서 DRD2 유전자다형성과연관되어있는것으로보고된바있는강박장애 30) 와주의력결핍 과잉행동장애 31) 등과같은다른정신과질환의공존을배제하지않았다는점이제한점으로생각된다. 마지막으로설문조사를통한가족력에대한조사의한계점을들수있다. 본연구의결과는 DRD2 TaqI A 다형성이한국인의틱장애와는관련되어있지않다는것을의미한다. 본연구를포함하여 DRD2 유전자와틱장애의유전적관련성을규명하기위한현재까지의연구결과들을종합해보았을때연구에따라상반되는결과가나타난다는것을알수있었다. 향후 TS와 CMT 를구분하여보다많은환자와대조군을포함한대단위연구를통해 DRD2 유전자와의관련성을확인해보는과정이필요하다. 중심단어 : 뚜렛증후군 만성운동틱장애 유전자다형성 도파민 D2 수용체 TaqI RFLP. 참고문헌 1.
