This report describes a 24-year-old woman who experienced syncope due to complete atrioventricular block complicated by KSS. At 12 years old, she was diagnosed KSS. The consecutive change of EKG shows typical progression of cardiac conduction disturbance of KSS. She was successfully treated with implantation of a pacemaker.(korean J Med 70: , 2006) Key Words : Kearns-Sayre syndrome, Complete atrioventricular block, Cardiac pacemaker 서론 Kearns-Sayre 증후군은미토콘드리아 DNA 이상으로인해진행성외안근마비에따른안검하수, 비특이성망막색소증과함께심전도장애를특징적으로보이는매우드문질환이다 1). 특히심전도장애는완전방실차단으로진행되며이에따른사망률이 20% 로높게보고되고있다. 본저자등은완전방실차단이발생하여영구심장박동조율기를삽입한 Kearns-Sayre 증후군 1예를경험하여문헌고찰과함께보고하는바이다. 증례 환자 : 성 라, 여자 24세주소 : 실신 접수 : 2005년 3월 30일 통과 : 2005년 6월 2일 교신저자 : 정보영, 서울시서대문구신촌동 134, 연세대학교의과대학내과학교실 ( ) cby6908@yumc.yonsei.ac.kr 현병력 : 내원 3일전부터간간히현기증이있었으며, 내원당일약 10분간의실신을주소로인근병원내원하여본원으로전원되었다. 과거력 : 환자는 12년전에안검하수증상으로본원내원하여안과에서눈꺼풀성형술 (blepharoplasty) 을시행받았으며이후 KearnsSayre 증후군으로진단받았다. 6년전부터당뇨로경구혈당강하제를복용중이었으며그외간염, 폐결핵의과거력은없었다. 가족력 : 특이사항없었다. 이학적소견 : 내원당시혈압은 90/60 mmhg, 맥박수는분당 33회, 호흡수는분당 20회, 체온은 36.0 이었다. 의식은명료하였으나급성병색을보였으며호흡곤란을호소하였다. 흉부청진상우측폐하엽에서흡기시에염발음이들렸으며, 심잡음은청진되지않았다

7 g/dl, 헤마토크릿 39.8%, 혈소판수 202,000/mm 3 이었다. 일반화학검사상 BUN 13.3 mg/dl, 크레아티닌 0.8 mg/dl, Na+/K+/Cl- 140/4.8/103 meq/l, 혈청총단백질 6.0 g/dl, 알부민 3.4 g/dl, AST/ALT 75/69 IU/L, CK 17 IU/L, CK-MB 16.

The lead of temporary pacemaker was inserted at apex of right ventricle 검사실소견 : 내원당시시행한일반혈액검사상백혈구수 11,680/mm 3, 혈색소 12.7 g/dl, 헤마토크릿 39.8%, 혈소판수 202,000/mm 3 이었다. 일반화학검사상 BUN 13.3 mg/dl, 크레아티닌 0.8 mg/dl, Na+/K+/Cl- 140/4.8/103 meq/l, 혈청총단백질 6.0 g/dl, 알부민 3.4 g/dl, AST/ALT 75/69 IU/L, CK 17 IU/L, CK-MB IU/L, Tn-T ng/ml 이었다. 심전도및방사선학적소견 : 내원당시심전도상심박동수 33회의완전방실차단소견을보였다 ( 그림 1). 단순흉부 X-선상양폐야에폐부종과우폐중엽에간유리혼탁화 (ground glass appearance) 소견이관찰되었다 ( 그림 2). 치료및경과 : 환자는의식소실과호흡곤란을동반한완전방실차단소견을보여일시적심장박동조율기를삽입하였다. 과거심전도상 9세에처음본원에내원하여시행한심전도는전도장애가없는정상이었으나 ( 그림 3), 14세때심전도는좌전각차단 (left anterior hemiblock) 소견을보였고 ( 그림 4), 16세에시행한심전도는우각차단 (right bundle branch block) 과좌전각차단이동시에관찰되었다 ( 그림 5). 심초음파검사상좌심실하벽에중등도의운동저하소견이기저부에서관찰되었지만좌심실구혈률은 65% 로정상이었다. 입원 3일째 DDD 유형의영구심장박동조율기를좌측쇄골하정맥을통하여삽입하였다 ( 그림 6). 당시시행한관동맥조영술 (coronary angiography) 상관동맥질환을시사하는소견은없었다. 고찰 Kearns-Sayre 증후군은 20세이전에병발하고유전되는것이아니라산발적으로발생하는것으로알려져

When she was 14-year old, EKG showed normal sinus rhythm with anterior fascicular block 있다 2). 병태생리학적원인은미토콘드리아 DNA 결손으로인해미토콘드리아내 ATP 생성에이상이생기며 3), 이런변형된미토콘드리아가근육뿐아니라뇌, 심장등에축적됨으로써증상이드러나게된다 4). 진단은이상증상을보이는장기에서조직검사를하여특징적인미토콘드리아의변형을확인함으로확진을하게된다. 본증례의경우도 9세때부터안검하수증세가있었고, 외안근 마비가발생하여본원신경과에서근조직검사를하였으며조직검사상미토콘드리아질환에특징적인불균일적색근육섬유 (ragged-red-fiber) 소견을보였고 ( 그림 7), 이후비특이성망막색소증을확인함으로써 Kearns- Sayre 증후군으로진단된경우였다. Kearns-Sayre 증후군에서는다른미토콘드리아질환과달리심부전보다는심전도장애가더특징적으로나타나게되며심전도의장애는결국완전방실차단으로진행하게된다.

Chest PA after DDD pacemaker implantation 콘드리아의변형이축적되며이런심장근육세포의변형으로인해심전도장애와심근수축력의저하가발생된다 5). Kearns-Sayre 증후군에서는다른미토콘드리아질환과달리심부전보다는심전도장애가더특징적으로나타나게되며심전도의장애는결국완전방실차단으로진행하게된다. 이로인한사망률은 20% 정도로높 아서 2각이상의전도장애를보일경우무증상이라고하더라도예방적으로영구심장박동조율기를삽입하는것이권장되고있다 5, 6). 본증례는 13년전진단당시에는심전도장애를보이지않았으나 7년전에 2각전도장애를보였으며당시무증상이었으나박동기삽입또는주의깊은추적관찰이필요하였을것으로생각된다. Kearns-Sayre 증후군에서초기증상이외안근마비에따른안검하수로나타나므로주로안과나신경과에서처음진단을받게되고추적관찰을하게된다 7). 하지만병이진행됨에따라심부전및심전도장애가발생하므로이에대한주의및적절한치료가필요할것이다
