Clinical manifestation of CDI caused by B1/NAP1 strain is more severe resulting in high mortality and complications, and relapse rate is higher. In Korea, the epidemic feature of CDI is not clearly demonstrated yet. Infection by B1/NAP1 strain was reported recently, but the prevalence of the infections by those strains seems to be not high yet. For the management of CDI, conventional treatment of vancomycin or metronidazole is still the choice of treatment, however, several clinical trials of CDI treatment are progressing; fidaxomicin, monoclonal antibody, toxoid vaccine, and luminal nontoxigenic C. difficile infusion. Thus, it is expected to have more effective treatment or prevention modality of CDI in near future. CDI 는병원내항생제를사용하는중혹은사용후발생하는전형적인병원감염으로서그간안정적인발생률을보였고합병증이나사망률도비교적낮았다. 그러나캐나다퀘벡에서합병증과사망률이높은중증 CDI 가발생하였고 2000 년이후에는북미지역을중심으로빠른속도로전파되고있다 [1]. 이번유행은기존의 C. difficile 에비하여병독성이강한 B1/NAP1 균주가원인이다. B1/NAP1 균주는대륙간전파되어 2008 년에서 2010 년에는아시아태평양지역과중미지역에서도감염이보고되었다 [2]. B1/NAP1 균주의유행으로인하여북미및유럽에서 CDI 역학이변화되었고 CDI 치료에대한관심이크게증가하였다. 이번종설에서는 CDI 의임상양상, 최근 CDI 역학의특징, 기존치료및새로이도입되고있는치료법에관하여간단히살펴보겠다. 2 DOI: /ic Infect Chemother 2010;42(6): 환자에서는여러가지원인으로설사가발생하는데이러한항생제 -연관설사의 15-25% 가 CDI 이다. 그러나항생제사용후발생한위막성대장염의경우는 % 가 CDI 인것으로알려졌다 [3]. 일일 10 회내외의수양성설사가가장흔한증상이고약 28% 의환자에서발열이있으며 50% 의환자에서백혈구증가를보인다. 장내로알부민이배출되어서혈중저알부민증이나타날수있다. 일부중증감염에서무력장폐색증이생겨서설사가없으며독성큰결장증이나타날수있다 [3]. 대장내시경에서위막성대장염이있으면 CDI 로진단되지만약 50% 의환자에서만전형적인소견을보이므로대장내시경의진단적민감도는낮다. 또한환자의 10-33% 에서는병변이상행대장에국한되어서구불결장경검사로는진단을놓치기쉽다 [3, 4]. 복부단층촬영검사도진단에도움이된다. 대장벽이두꺼워져있고장주위가닥 (pericolic stranding), 아코디언사인혹은타겟사인등장점막부종을시사하는소견이나타나며 52% 에서복수가관찰된다 [4]. 그러나복부단순촬영역시대장내시경과같이진단의민감성은 52% 로낮고특이성은 93% 로높았다 [4]. CDI 의주요위험인자는연령, 입원및항생제사용이다 [5]. 연령이높을수록 CDI 발생률이증가한다. 미국 National Hospital Discharge Survey (NHDS) 의결과에의하면 CDI 발생률이 65세이상연령에서 100,000 환자일당 228, 세연령에서 40이었고 세연령에 Cefoxitin 서는 11 이었다. 즉 65세이상연령의 CDI 발생률은 세연령의 5배이었고 세연령보다는 20배높았다 [6]. 항생제사용이 CDI 의위험인자인것은잘알려져있다. 여러항생제를동시에투여하면단일항생제를사용할때보다의미있게 CDI 가증가하였다 (4.2 vs 1.4, P<0.001)[7]. CDI 와유의한상관관계가있는항생제로는 cefotaxime, ceftazidime, amoxicillin, amoxicillinclavulanic acid, antipseudomonal penicillin, quinolone 및 aminoglycoside 등대부분의항생제가포함된다 (Fig. 1)[5]. 항생제이외에도최근영국에서보고된바에의하면위산억제제, 특히 PPI 가지역사회에서발생한 CDI 와유의한상관관계를보였고의미있는위험인자였다 (RR 3.5, 95% CI )[8]. 한편일부의환자군에서는 C. difficile 이감염되더라도증상이없는무증상보균자로남는다. 설사가발생하는환자과무증상보균자를결정하는요인은 C. difficile 의 A독소에대한항체임이밝혀졌다. Kyne 등은입원환자들을대상으로연속적으로혈중 C. difficile A 독소에대한항체를측정하였다. 결과입원당시혹은입원중 C. difficile 이장집락된환자중무증상상태로유지하는환자는설사가발생한환자에비하여 A 독소에대한항체값이유의하게높았고 (P<0.001) ELISA 로측정한혈중항체값이 3.0 ELISA unit 이하인환자들에서주로설사가발생하였다 (adjusted OR 48.0, 95% CI, ). 그러나입원중 C. difficile 장집락이일어난환자와장집락이일어나지않은환자사이에는항체값에차이가없었다. 즉혈중 A 독소에대한항체가장의집락화를막지는못하지만집락화후증상의발현에는영향을주었다 [9]. CDI 의역학 2000 년이후북미에서는 CDI 의발생이크게증가하였다 (Fig. 2) 년미국의 CDI 발생률은 31/100,000 인구였으나 2003 년발생률은 61/100,000 인구로서약 2배증가하였다 [6]. 특히 65세이상의노년인구에서증가폭이가장컸다. 이번유행전미국에서의 CDI 발생률은발생이적었던해는 100,000 환자일당 32-55, 발생이많았던해는 정도이었다 [10]. 또한이번유행으로 CDI 발생이증가한외에도중증감염의빈도가과거에비하여증가되었다. 미국의한병원 Figure 2. 3 364 H Pai Epidemiology and treatment of Clostridium difficile infection 에서는 1999 년 46/100,000 환자일이었던발생률이 에는 121/100,000 환자일로약 3배증가하였고 5.6% 였던중증환자 ( 사망혹은대장절제술 ) 의비율도 8.8% 로유의하게높아졌다 (P=0.004)[11]. 캐나다의한병원에서는 CDI 로인한 30일사망률이 6.9%, 1년사망률이 16.7% 로 CDI 의중증도가매우높음을제시하였다 [12]. 한편미국의펜실베니아, 뉴저지, 오하이오및뉴햄프셔등에서 CDI 의위험요인이없는젊은연령층과출산후산모에서 CDI 가발생한사례들이보고되었다 [13]. 이처럼고위험성균주의유행은북미의 CDI 역학에커다란변화를일으키고있다. 국내의 CDI 역학은많이알려지지않다. 국내한병원의자료를보면 2009 년평균발생률은 67.5/100,000 환자일 ( ) 로 NAP1/027 균주유행전미국병원의발생률과유사하거나조금높은정도로판단된다 [14]. 사망률도낮은편이고수술이필요한중증감염의빈도도 NAP1/027 균주유행이전의미국과유사하다 [15]. 물론일개대학병원자료이고기관마다발생률이다를수있으나아직국내에서는대유행은발생하지않은것으로판단된다. 국내분리균주로시행한검사에서 NAP1/027 균주가몇주분리되었으나임상적의의는아직확실하지않다 [14, 16]. A&B 독소 EIA 검사는특이도는 98% 로좋으나민감도가보고에따라 79-80% 로낮아서문제이다 [19-21]. 배양은독소검사에비하여민감도가높지만국내의경우신선배지를이용한적합한배양이이루어지지않아서배양률이낮은편이다. 한편대변에서균이배양되어도독소를생산하지않는균주들이포함되어있으므로배양된균의독소생산여부를확인하여야한다. 그외표준방법으로세포배양을통한세포독소검사는보고에따라 % 의민감도와 % 의특이도를보이지만검사방법이번거로와서많이이용되지않는다. 최근대변을직접 real-time PCR 하여독소유전자를검사하는방법이나왔는데이는기존의독소 EIA법에비하여민감도가 93% 로우수하다고보고되었으나아직많은경험이없어서추후검증이필요하겠다 [22] 년 Infectious Diseases Society of America (IDSA) 와 Society of Healthcare Epidemiology of America (SHEA) 가이드라인에서는진단에관하여다음과같이권장하였다 [23]. 설사가있는환자에서검사하고치료후재검사는불필요하다. 대변배양이가장민감한검사이다. 독소 EIA 검사는빠르고편리하지만민감도가낮다. GDH (glutamate dehydrogenase) 검사후배양이나세포독성검사를진행하는 2단계방법도쓸수있다. Real-time PCR 은민감하고특이도도좋으나아직자료가많지않으므로추후검증이필요하다. C. difficile NAP1/027 균주의특징 최근의 CDI 역학및임상양상의변화는기존의 C. difficile 에비하여병독성이강한 NAP1/027 균주의유행에서기인한다 [17]. NAP1/027 균주는기존의 A 독소와 B 독소이외에 binary toxin 을추가로생산하지만 binary toxin 의임상적영향은아직명확하지않다. NAP1/027 균주는다른표준균주에비하여 A 독소와 B 독소의생산이각각 16 배와 23배더많은것으로알려졌고독소의과잉생산이중증 CDI 를일으키는원인중하나로생각된다 [14]. 한편 NAP1/027 균주는최근혹은과거에동정된다른균주들에비하여 moxifloxaicn 과 gatifloxacin 내성이높아서최근호흡기퀴놀론을많이사용하면서 NAP1/027 균주가자연선택되어유행이발생한것으로추정된다 [11, 18]. CDI 의진단 CDI 는 A&B 독소 EIA 검사와대변배양으로진단한다 (Table 1). CDI 의치료 CDI의치료제로가장많이사용되는약제는 metronidazole 과 vancomycin 이다. 이외에 rifaximin, nitazoxanide, teicoplanin, fusidic acid, bacitracin 및 tigecycline 을사용할수있으나 metronidazole 과 vancomycin 을제외한다른약제들은임상자료가충분하지않다. Vancomycin 과 metronidazole 의 CDI 치료효과를비교한논문으로서 1983 년 Teasley 등은 vancomycin 과 metronidazole 의치료실패율이각각 0% 와 4.7% 였고재발율은각각 12% 와 4.7% 로치료효과가비슷하다고하였다 [24]. 반면에 2005 년 Musher 의보고에서는 년간일개병원에서발생한 CDI 에서 metronidazole 이 vancomycin 에비하여치료결과가좋지않았다 [25]. 이논문의환자군은사망률이 27% 로매우높았고시기적으로도 NAP1/027 유행이시작된이후라서아마도중증환자들이많이포함되었을것으로생각된다 년 Zar 등은환자의경중도에따라 metronidazole 의반응이달라서경증의환자들은 metronidazole 이 vancomycin 과비 Table 1. 4 DOI: /ic Infect Chemother 2010;42(6): 슷한효과를보이지만 ( 완치율 metronidazole vs vancomycin, 90% vs 98%, P=0.36) 중증환자에서는 vancomycin 의치료효과가우수하다고하였다 ( 완치율 metronidazole vs vancomycin, 76% vs 97%, P=0.02)[26]. 이후 Loui 등도경중도의 CDI 에서는두약의치료효과가같으나중증 CDI 에서 vancomycin 이 metronidazole 보다치료효과도우수하고 (84.8% vs 64.9%, P=0.042) 재발이낮았다고보고하였다 (15.4% vs 25.3%, P=0.005)[27, 28]. 위의자료들을기본으로 IDSA & SHEA 가이드라인에서중증 CDI 의치료에는 vancomycin 을사용할것을권장하고있다 [23]. 중증 CDI 의기준은논문마다약간의차이를보이는데 Zar 등의논문과 IDSA & SHEA 가이드라인에서중증 CDI 의기준은 Table 2와같다. Fusidic acid, teicoplanin, vancomycin 및 metronidazole 의치료효과를비교한논문에서는 teicoplanin 은치료효과가 vancomycin 이나 metronidazole 과유사하였으나 fusidic acid 는 teicoplanin 에비하여낮은치료효과를보였다고하였다. 그러나대상환자수가적어서더검증이필요하겠다 [29]. 그외 CDI 의치료에 nitazoxanide 의치료효과가 vancomycin 과비슷하다는임상연구가있으나환자수가적고추후연구가없으며 [30] metronidazole 에 rifampin 을추가하여치료한임상결과도 metronidazole 단독치료에비하여우수함을입증하지못하였다 [31]. 한편 vancomycin 치료후 2주간 rifaximin 을투여하여서재발율을낮추었다는일부보고가있으나역시자료가충분하지않다 [32]. 최근개발중인약제로서 fidaxomicin (OPT-80) 이있는데 3상임상시험에서 vancomycin 과비교연구를하였다. 치료성공률은 fidaxomicin 과 vancomycin 군이각각 88.2% 와 85.8% 로비슷하였으나재발율은 fidaxomicin 군이 15.4%, vancomycin 군이 25.3% 로 fidaxomicin 군에서재발율이낮았다 [33]. 같은연구에서두약제의투여가장의정상균총에미치는영향을관찰하기위하여 fidaxomicin 군과 vancomycin 군의약물투여전과투여후 14 일에대변의 Bacteroides spp. 의균집락수를비교하였다. 결과약물투여전에는두군에서 Bacteroides 의균집락수가같았으나투여후 14 일에는 fidaxomicin 군에서는 Bacteroides 의정상적인균집락이유지되었으나 vancomycin 군에서는현저하게균집락이감소하였다 [34]. 항생제이외의치료제로서 C. difficile 의독소와결합하는고분자량폴리머경구요법이시도되었다 [35, 36]. Tolevamer 로명명된고분자량폴리머는경구로투여시흡수되지않고 98.5% 이상대변으로분비되고 C. difficile 의독소와결합하여중화시키는작용이있다 [35]. Vancomycin 500 mg/ 일과 tolevamer 6 g/ 일경구요법을비교한임상 2상실험에서 tolevamer 는 vancomycin 경구요법과비교하여열등하지않은치료효과를보였고재발율도열등하지않았다 [36]. 그러나이후진행한임상 3상실험에서 tolevamer 는 vancomycin 에비하여낮은치료효과를보였고햄스터모델과는달리사람장모델을이용한 CDI 시험관적검사에서세포독성을중화시키지않는것으로밝혀졌다 [37, 38]. CDI 의발생을막기위하여여러 probiotics 가항생제와함께투여된다. 그러나 probiotics 의항생제 -관련설사의발생억제효과에관한증 거는불확실하다 년 McFarland 는그간출판된 25개의논문을포함시켜메타분석하였고 probiotics 가유의하게항생제 -관련설사를줄인다고하였다 [39]. 최근 Cochrane systemic review 에서는엄격한기준을적용하여 4개의논문만을메타분석에포함시켰고결과 probiotics 의효과를입증하지못하였다 [40]. C. difficile 장집락후에도무증상보균자로남는환자들에서 A와 B 독소에대한항체값이높다는기존연구에근거하여 [9] A와 B 독소에대한사람단클론항체를투여한임상시험결과가 2010 년보고되었다 [41]. 단클론항체를 10 mg/kg 로일회투여한군과투여하지않은군간의무작위대조시험에서단클론항체투여가 CDI 의중증도에는영향을주지않았으나 84일간의추적관찰에서단클론항체투여군에서유의하게재발률이낮았고 (7% vs 25%, P<0.001), 결과적으로재입원이유의하게감소하였다고하였다 (9% vs 20%, P=0.03). 항체의혈청내반감기는 A 독소항체는 26±8.4 일었고 B 독소항체는 22±13 일이었다. 위의연구가수동적으로면역을증강시키는방법이라면백신은능동면역을증강시키는방법이다. A&B 독소를포함하는백신이개발되었고최근임상연구에서건강인에게투여하였을때부작용이없고항체가잘형성되었다고하였다. 현재임상 2상연구가진행중이고결과가나오면이백신이 CDI 발생을감소시키는지알수있을것이다 [42]. CDI 환자들은항생제사용으로장내정상균총이변화되어있으므로정상인의대변을이식하여장내정상균총을재건하는치료법이그간간헐적으로시도되었다 [43]. 배우자의정상변을필터로걸러내어대장내시경혹은 L-튜브로이식하였고부작용없이안전하게시술되었다. 대부분의논문은소수의증례보고였으나치료성적은비교적우수하였다. 최근메타게놈연구의발전으로장의정상균총에대한연구가활발해지면서대변이식이다시인식되고있다 년한연구에서는계속재발되는 CDI 환자의장내정상균총을분석하고이식후 14 일과 33일의환자대변과제공자대변의정상균총을비교하였다 [44]. 결과, CDI 환자의대변은 Bacteroides 와 Frimicutes 균종이현저히감소되어있었고이식후 14 일에는두균종이증가하여제공자의대변과비슷한분포를보였으며이식후 33일에도장내균의분포가상당히유지되었다고하였다. 추후좀더많은자료가쌓이면대변이식에대한적용여부가결정될것으로생각된다. 비병원성 C. difficile 은자연에흔히분포한다. 이를이용한생물학적치료로서독소를생산하지않는 C. difficile 주입요법이연구중이다. 동물실험에서독소를생산하지않는균을장에정착시킨후독소생산균을투여하면정착된비병원성균이병원성균의감염을억제한다고하였다 [42] 년초건강인에게비병원성균을주입하여안전성을검증하는연구가종료되었으며 2010 년말환자를대상으로하는임상연구가시작된다고한다 년 IDSA 와 SHEA 가이드라인에서는그간임상경험이풍부한 vancomycin 과 metronidazole 을치료의주축으로하였다 [23]. 간단히정리하면, 가능한항생제를끊고병세가중하면검사결과확인이전에항생제를사용할것을권장한다. CDI 가의심되는환자에서검사결과가음성일경우는담당의사의결정에따라항생제를투여한다 ; 경도와중등도 CDI 에서는 metronidazole 을, 중한환자에는초기

5 366 H Pai Epidemiology and treatment of Clostridium difficile infection 에 vancomycin 을권장한다. Metronidazole 은 500 mg을경구로 3회 일투여하고 vancomycin 은 125 mg을일일 4회 일간투여한다. CDI 의합병증으로무력장폐색증이생긴경우는 vancomycin 250 mg을경구혹은관장으로 4회주면서 metronidazole 500 mg을정맥주사로 8시간간격으로투여한다. 임상경과관찰을위해서는백혈구수와혈중젖산수치를측정한다. 임상경과가매우중할때는대장절제를고려하는데가능한직장을희생시키지않는다. 재발된환자의치료법은다음과같다. 처음재발한환자는초기치료와동일한약제로치료한다. Metronidazole 은약독성때문에장기간투여하면안된다. 여러번재발하는환자에서는 vancomycin tapering 요법을시도할수있다 ; 처음 일간 125 mg을 4회투여하고이후일주일간 125 mg을일일 2회, 이후 1주일동안 125 mg을일일 1회투여하며이후2-8 주간 2-3 일에일회투여하는방법이다. 결론 CDI 는북미와유럽에서고병원성균이유행하면서과거와는다른역학을보이고있으며임상양상도과거에비하여훨씬중한양상을보인다. 국내에서는아직고병원성균의빈도가낮은상태이지만추후수년간지속적인관찰이필요한시점이다. CDI 의치료에서는현재사용할수있는치료제가부족하지만항생제로서 vancomycin 과비슷한효과를보이는 fidaxomicin 이개발되었고생물학적치료법으로서비병원성 C. difficile 장내주입치료및독소백신등의연구가진행중이다. 이런연구에서긍정적인결과가나온다면 CDI 치료에중요한전기를맞을수있을것으로사료된다. 한편 CDI 의발생을줄이기위하여서는불필요한항생제의사용을줄이고환자의격리를포함하는 CDI 감염관리에대한노력이필요하겠다.
