The aim of this study was to evaluate the performance of Tox A/B Quik Chek test (TECHLAB, Blacksburg, VA, USA) compared to toxigenic culture. Methods: From September 2006 to August 2007, 959 stools were examined by Tox A/B Quik Chek test and toxigenic culture (C. difficile culture plus tcdb PCR using colonies obtained from culture). Results: Compared to the results of toxigenic culture, the sensitivity and specificity of Tox A/B Quik Chek test were 47.5% and 97.5%, respectively. Conclusion: The sensitivity of Tox A/B Quik Chek test was not high, but the specificity was high. Although Tox A/B Quik Chek test alone is not sufficient to diagnose Clostridium difficile-associated diarrhea, it may aid rapid diagnosis, early treatment and prevention of nosocomial spread of the infection, if supplemented by C. difficile culture or tissue culture cytotoxin assay. (Korean J Clin Microbiol 2008;11: ) Key Words: Clostridium difficile, Enzyme immunoassay, Clostridium difficile Toxin A, Clostridium difficile Toxin B 서 Clostridium difficile은대표적인원내감염균중한가지이며, 항균제연관설사의 25% 는이세균이원인이다 [1]. C. difficile 연관설사는 C. difficile이생성하는독소 A와독소 B가원인인것으로알려져있으며, 경한설사에서부터위막성대장염, 독성거대결장까지생길수있고, 심한경우장천공이나사망에이를수도있다 [2,3]. C. difficile 연관설사를조기에진단하고치료하는것은감염증의진행을막고, 다른환자에게이세균이전파되는것을막기위해중요하다 [4]. C. difficile 연관설사를진단하기위해서는분리된 C. difficile이독소생성주인지확인하거나, 변검체내독소를확인하여야한다. 세포배양을이용한독성시험이표준참고검사로알려져있으나검사소요시간이길고, 세포배양이필요하여번거로운것이단점이다 [5]. (Tel) , (Fax) , ( ) leekcp@yuhs.ac 론 (PCR) 으로독소유전자를검출하는방법도소개된바있으나변검체내의 PCR 저해인자를제거하는과정이필요하고, 상품화된시약이드물다는단점이있어널리이용되지못하고있다 [7]. 독소면역시험은세포독성시험이나배양보다민감도가낮으나검사소요시간이짧고시험이간편한장점이있어서, 많은검사실에서사용하고있다 [6-8]. 본연구에서는독소생성 C. difficile 배양결과와비교하여임상검사실에서현재흔히사용되고있는독소막효소면역시험인 Tox A/B Quik Chek (TECHLAB, Blacksburg, VA, USA) 시험의유용성을평가하고자하였다. 재료및방법 2006년 9월부터 2007년 8월까지세브란스병원미생물검사실에의뢰된 Tox A/B Quik Chek 시험과독소생성 C. difficile 배양결과를분석하였다. 보균자를배제하기위해설사변또는무른변만을검사하였으며, 면봉으로채취한검체는양이충분하지않으므로검사를시행하지않았다. Tox A/B Quik Chek 시험은변과희석액, 접합액 (conjugate) 을잘섞어 membrane device에분주하고 15분간배양한후세척하고, 기질액을첨가하여다시 10분간배양하고결과를판정하였다. Membrane device에는두개의줄이있는데하나에는 C. difficile 독소 A와 112

2 Sue Jung Kim, et al. : Rapid Assay of Clostridium difficile Toxins 113 B에대한항체가, 다른하나에는항 IgG 항체가부착되어있으며, 접합액에는 horseradish peroxidase와결합된 C. difficile 독소 A와 B에대한항체가들어있어서, 두줄모두푸른선으로나타나면양성으로판정하였다. Tox A/B Quik Chek 시험을바로시행할수없는경우, 4 o C에냉장보관하였다가 72시간이내에검사하였다. 독소생성 C. difficile 배양 (C. difficile 배양하여생긴집락으로 tcdb를 PCR로검출하는것 ) 에는 Clostridium difficile selective agar (CDSA, Becton, Dickinson and Company, Sparks, MD, USA) 를사용하였다. 혐기성상자 (Forma Scientific, Thermo Electron Corp., Waltham, MA, USA) 안에서 0.01 ml 일회용 loop을이용하여충분한양의설사변을선택배지에접종하고혐기성상자내항온기에서배양하였다. 배양 48시간후에집락이생기는지관찰하고도말염색에서의형태를보고 API rapid ID 32A system (biomérieux, Marcy-l Etoile, France) 을이용하여동정하였다. C. difficile이분리된경우, 배양결과를반정량적으로기록하였는데, 평판의셋째구역이상까지집락이증식된경우를 다수, 둘째구역까지증식되면 중등도, 첫째구역에만증식되면 소수 로하였다. CDSA배지에서증식한집락을이용하여기존의방법을약간변형하여 tcdb PCR을시행하였다 [8]. 즉, 집락부유액을만들고, 7분간끓여서 DNA를추출하여, 상층액 1μl와시발체각각 1μl씩, 그리고증류수 17 μl를 AccuPower PCR premix (Bioneer사, 대전, 한국 ) 에넣었다. 시발체는 NK104 (5 -GTGTAGCAATGAAAGTC CAAG- TTTACGC-3 ) 와 NK105 (5 -CACTTAGCTCTTTGAT TGCT- GCACCT-3 ) 를사용하였고, PCR 조건은 95 o C에서 10분간둔후 94 o C에서 30초, 55 o C에서 30초, 72 o C에서 1분을 35회반복하고, 72 o C에서 7분간두었다. 독소생성 C. difficile 배양양성은배양된 C. difficile이 tcdb PCR 양성인경우이고, 독소생성 C. difficile 배양음성은 C. difficile 배양음성이거나배양된 C. difficile이 tcdb PCR 음성인경우로정의하였다. 결과총 1,198명의환자에서채취한 1,590개의검체에대하여독소생성 C. difficile 배양과 Tox A/B Quik Chek 시험을시행하였다. 이중중복검사및독소생성 C. difficile 배양과 Tox A/B Quik Chek 시험두가지모두를시행하지않은경우를제외하고총 959건의결과를분석하였다. 총 959건중 C. difficile 배양양성검체는 105개 (10.9%) 이었다. 독소생성 C. difficile 배양양성중에 Tox A/B Quik Chek 시험양성은 38예, 음성은 42예이었다 (Table 1). 독소생성 C. difficile 배양음성중에 Tox A/B Quik Chek 시험양성은 22예, 음성은 857예이었다. 독소생성 C. difficile 배양결과를근거로하였을때, Tox A/B Quik Chek Table 1. Abbreviation: CI, confidence interval. 시험과의일치율은 93.3% 이었고, Tox A/B Quik Chek 시험의민감도와특이도는각각 47.5% (38/80) 와 97.5% (857/879) 이었다. 독소생성주 80주의반정량적배양결과와 Tox A/B Quik Chek 시험결과를비교하였는데 (Table 2), 배양결과 소수 인 24예중 7예 (29.2%) 와, 중등도 인 10예중 3예 (30.0%) 가독소시험양성인데비해, 다수 인경우 46예중에서는 28예 (60.9%) 가독소시험양성이었다. 배양결과 중등도 인경우를 소수 와비교하였을때교차비가 1.04이었고, 통계적으로유의하지않았으나 (P value=0.961), 다수 인경우에는 소수 와비교했을때교차비가 3.78이었고통계적으로유의한것으로나타났다 (P value=0.014). 고 C. difficile은병원내환경에흔히있어서원내감염을일으킨다. 따라서, C. difficile 감염증을빠르게진단하여적절히치료하고, 그전파를예방하는것이중요하다 [9]. C. difficile 연관설사의진단에어떠한검사가가장이상적인지에대해서는아직도논란이있다. 세포배양을이용한독성시험이나배양은민 찰

3 114 Korean J Clin Microbiol 2008;11(2): 감도는높으나검사에 48시간이상소요되는단점이있어서근래빠르고간편한독소효소면역시험이널리이용되고있다 [10]. 독소효소면역시험은세포독성시험과비교할때일반적으로민감도가낮다고알려져있으나, 시약, 결과해석방법및연구방법에따라서민감도와특이도가다르게보고되고있다 [11-13]. 상품화된여러가지독소효소면역시험키트가있는데, 독소 A만을검출하는키트와독소 A, B 모두를검출하는키트가있으며, 방법은대부분 microwell 을이용하지만막을이용한효소면역시험도있다. 독소 A 음성 B 양성균주의출현으로독소 A와 B 모두를검출하는키트의사용이더권장되고있다 [14-16]. 독소 A와 B 모두를검출하는막효소면역시험키트로는 Tox A/B Quik Chek, ImmunoCard Toxins A and B (Meridian Bioscience, Inc., Cincinnati, OH, USA), Xpect C. difficile Toxin A/B (Remel, Inc., Lenexa, KS, USA) 등이있다. 막효소면역시험은 microwell을이용한효소면역시험에비해검사소요시간이짧은것이장점이다 [14]. 본연구에서는독소생성 C. difficile 배양결과와비교하여, Tox A/B Quik Chek 시험의유용성을평가하였다. 독소생성 C. difficile 배양결과를근거로하면 Tox A/B Quik Chek 시험의민감도는 47.5%, 특이도는 97.5% 였다. 이는본연구와연구설계에서다소차이가있으나 Tox A/B Quik Chek 시험을평가한다른연구결과와유사하였다. Gilligan[17] 은 368개의검체를대상으로, glutamate dehyrogenase (GDH) 를검출하는막효소면역검사인 C. DIFF CHEK-60 (TECHLAB), 시험후세포독성검사로독소생성여부를확인하는방법을근거로하였을때 Tox A/B Quik Chek 시험의민감도는 43.2%, 특이도는 98.5% 였다고보고하였다. Fenner 등 [18] 은 1,468개의검체를대상으로 C. DIFF CHEK-60 시험양성인경우, Tox A/B Quik Chek 시험으로독소생성여부를확인하였다. 이결과를배양및독소유전자 PCR 결과와비교하였을때, 민감도는 52.9%, 특이도는 97.1% 였다고하였다. 또한, Sloan 등 [19] 은변검체 200개를대상으로 ImmunoCard toxin A and B, Xpect C. difficile Toxin A/B 시험과배양및독소유전자 PCR 결과를비교하였는데, 두독소막효소면역시험의민감도는각각 48% 와 48%, 특이도는각각 99% 와 84% 라고하였다. 국내연구를보면, enzyme linked fluorescent immunoassay 방법인 VIDAS C. difficile Toxin A II (biomérieux) 시험또는 enzyme linked immunosolvent assay 방법인 C. difficile TOX A II (TECHLAB) 시험을배양, 독소유전자 PCR 또는세포독성검사와비교하여평가한보고들이있는데, 연구방법에따라민감도는 %, 특이도는 % 로다양하였다 [15,20,21]. Tox A/B Quik Chek 시험양성이면서배양음성인환자의경우, Tox A/B Quik Chek 시험이위양성일가능성과배양이위음성일가능성이있다. 본연구에서는 Tox A/B Quik Chek 시험의위양성여부를확인할수없었으나배양위음성의원인으 로검체내다른상재균의과증식으로인해 C. difficile이검출되지않았을가능성이있다. 기존연구에의하면, 변을알코올처리하면다른상재균의증식을억제하여분리율을높이고, C. difficile을쉽게식별할수있다고하였다 [22,23]. 본검사실에서 2008년 6월이후, Tox A/B Quik Chek 시험양성인변을냉동보관하였다가, C. difficile 배양이음성인경우, 냉동하였던변을알코올처리하여다시배양하였다. Tox A/B Quik Chek 시험양성이면서배양음성이었던 3건모두에서알코올처리후배양시 C. difficile이분리되었고, 배양세균은 tcdb PCR 양성이었다. 이는본연구에서독소시험과배양결과의불일치를보였던경우, 상당수가배양위음성이었을가능성을시사하는결과라고할수있다. 독소효소면역시험의특이도가일반적으로높다는점을감안하여, 독소시험양성이면서배양음성이었던경우가모두독소시험의진양성이었을것으로추정하면, 독소시험의민감도는 58.8%, 특이도는 100% 로증가될수있다. 이에대해서는앞으로더많은검체를대상으로알코올처리를병행하여배양을하면서확인할필요가있을것이다. 독소생성 C. difficile 배양이양성이지만 Tox A/B Quik Chek 시험이음성인경우, 위음성으로해석할수있는데, 그원인으로검체채취및보관이잘못되었거나변검체내적은세균수로독소양이적어검출되지못했을가능성을생각해볼수있다. C. difficile 독소는불안정하여실온에두면분해되므로독소시험은변을채취한지 2시간내에검사하여야가장좋은결과를얻을수있다 [14]. 채취후즉시독소시험이어려울경우에는 3일간 4 o C에보관할수있으나, 냉장보관중에도독소활성은감소하게된다.[24] 검체를 2일간실온보관시세포독소가거의 2.0 log 감소하고 5 o C 보관시에도실온에비해덜하지만감소하였다는보고가있다 [24]. 독소시험이지연될경우, 냉동보관후검사할수있으나냉동및해동과정에서도독소가상당량손실된다고하였다 [25]. 따라서, 검체채취후검사까지의시간이지연되면 Tox A/B Quik Chek 시험위음성의원인이될수있다. 또한, 변검체의세균의수가적어서 Tox A/B Quik Chek 시험으로검출하지못했을가능성이있다. 본연구에서독소생성주의반정량적배양과 Tox A/B Quik Chek 시험결과를비교하였을때, 배양결과 다수 였던검체가 소수 였던검체에비해 Tox A/B Quik Chek 시험양성일가능성이 3.7배높았다. 이는검체내세균의수가검사결과에영향을준다고추정할수있다. Delmée 등 [26] 은반정량적인배양결과와세포배양을이용한독성시험결과를비교하여, 배양된집락의수가많을수록세포독성시험의양성율이높았다고보고하였고, Yong 등 [27] 은검체를희석하여정량배양을하였을때, 배양된집락의수가많을수록세포독성시험양성율이높아지는경향을보였다고하였다. 따라서, 검체내세균의수가적은경우도 Tox A/B Quik Chek 시험위음성의원인이될수있다고하겠다.
