Stool culture, however, has a problem because of relatively low sensitivity and long turnaround time. In this study, we evaluated multiplex PCR applied on stool specimens directly to diagnose enteropathogenic bacteria. Methods: From June to September 2009, 173 diarrheal stools submitted for stool cultures were tested by Seeplex R Diarrhea ACE Detection kit (Seegene, Korea) to detect 10 enteropathogenic bacteria. Specimens were cultured for Salmonella, Shigella, Vibrio, and Yersinia. Late 50 specimens were also cultured for Campylobacter. The specimens positive for verotoxin-producing Escherichia coli (VTEC) were further subcultured for detecting enterohaemorrhagic Escherichia coli O157:H7. Electronic medical records were reviewed for clinical and laboratory findings. Results: Of 173 specimens, multiplex PCR and cultures identified enteropathogens in 36 (20.8%) and 8 specimens (4.6%), respectively. While multiplex PCR detected 5 Salmonella, 15 Campylobacter, 1 Vibrio, 4 Clostridium difficiles toxin B, 5 Clostridium perfringens, 1 Yersinia enterocolitica, 5 Aeromonas, and 2 VTEC, cultures detected 5 Salmonella, 1 Vibrio, 1 Y. enterocolitica, 1 Aeromonas, and 2 E. coli O157:H7. Conclusion: Multiplex PCR would be useful to detect Campylobacter, VTEC and C. perfringens, as well as have equivalent sensitivity to conventional culture for ordinary enteropathogens such as Salmonella, Shigella, Vibrio, Y. enterocolitica. Direct application of multiplex PCR combined with conventional cultures on stool warrants remarkable improvement of sensitivity to diagnose enteropathogenic bacteria. (Korean J Clin Microbiol 2010;13: ) Key Words: Multiplex PCR, Infectious diarrheal disease, Enteropathogen 서 설사는아직도전세계적으로 5세이하소아사망원인의두번째흔한원인으로서매년 2백만명의소아환자들이설사로인해목숨을잃고있다 [1,2]. 이중감염성설사가전체설사로인한사망원인중가장흔한원인이다 [3]. 설사원인균의신속한진단은항균제사용여부등의치료방법을결정하는데필수적일뿐아니라감염성설사의전파를방지하는데중요한역할을한다 [4-6]. 세균성설사는주로설사원인균에오염된음식물이나물을통해감염되는데, Campylobacter, Salmonella, Shigella와 Escherichia coli 등이주요원인균이다 [1,7-9]. 설사원인균의빈도는국가의경제수준과밀접한관련이있는것으로알려져있다. 개발도상국가에서는 Shigella가많고, 다음으로 Vibrio cholera, Received 17 May, 2010, Revised 5 August, 2010 Accepted 10 September, 2010 Correspondence: Mi-Na Kim, Department of Laboratory Medicine, Asan Medical Center and University of Ulsan College of Medicine, Pungnap 2-dong, Songpa-gu, Seoul , Korea. (Tel) , (Fax) , ( ) mnkim@amc.seoul.kr 론 Yersinia, Salmonella 균종들이주요장병원성균인데비해경제수준이높은국가에서는 Campylobacter, Salmonella가가장높은빈도를나타내며 Shigella, E. coli O157:H7, Vibrio, Yersinia 등이다음으로높은빈도를보인다고한다 [7,8]. 국내대학병원에서 30년간조사한자료 [10] 에서도국내설사원인균은 60, 70 년대에는 Shigella가가장다빈도원인균이었던것이 80년대후반부터는 Salmonella가가장빈도가높아졌고 Campylobacter 가 2번째로많이분리되는균종이었다. 질병관리본부의 2008 년전국규모실험실감시사업통계 [11] 에서는 Shigella가 0.1% 로매우낮은빈도를보이는것은이전보고와유사하다. 하지만이통계보고에서는새로이분자생물학적기법을사용하여설사원인균중가장흔한것이전체양성검체의 45% 를차지한장병원성 E. coli임을규명하였고, 이어서 Salmonella, Campylobacter를각각 2, 3위로보고하였다. 배양검사만을사용했던이전논문에비해이번통계보고에서는 E. coli와 Campylobacter의빈도가훨씬높아져서, 검사기법에따라장병원성균의빈도분포는큰영향을받는다는것을알수있다. 통상적으로설사원인균동정에세균배양방법을사용한다. 162

2 Min-Chul Cho, et al. : Application of Multiplex PCR on Stool 163 하지만이들방법은균동정까지 3 5일이라는긴시간이필요하며검체의종류, 채취및운송방법에따라민감도가떨어지기도한다 [12]. 또한원인균의동정보다는세균독소를검출하는것이임상적으로더중요한콜레라, verotoxin 생산 E. coli (verotoxin-producing E. coli, VTEC), Clostridium difficile 등의경우통상배양보다는효소면역검사법이나직접독소검사법등이사용된다. 하지만효소면역검사는다른검사법들과비교했을때민감도와특이도가떨어지고, 직접독소검사에는세포독성검사가 gold standard 로서사용되지만, 까다롭고수기시간이긴단점을가진다 [13]. 따라서최근분자생물학적방법을사용하여다양한원인균들을신속하고민감하게검출할수있고, 독소생성유무도함께진단하는시도가이루어지고있다 [12,14-19]. 이번연구에서는세균성원인균의신속검출을위해다중중합효소연쇄반응검사법을설사환자의대변검체에직접적용하여그임상적유용성을점검하고자하였다. 대상및방법 1. 대상과배양검사 2009년 6월에서 9월사이에대변배양이의뢰된검체중설사변검체를골라서통상적인배양을위해혈액한천배지 (BAP), MacConkey 배지, xylose-lysine-deoxycholate 배지에접종하였다. 후반부 50개의검체는 Campylobacter 배양을위해 Campy BAP (Hanil-Komed, Sungnam, Korea) 에접종하여 Anaero- Pack R ㆍMicroAeroa (Mitsubishi Gas Chemical, Japan) 를이용한미호기성조건에서 3일간배양하였다. 검체접종시대조균 (Campylobacter fetus) 을함께배양하여배양과정의문제가없음을확인하였다. 다중중합효소연쇄반응검사에서 VTEC, E. coli O157:H7 이검출되고임상적으로감염이의심되는경우균을검출하기위해대변검체를 sorbitol-macconkey 배지에접종하였다. 2. 다중중합효소연쇄반응접종후남은대변검체에서배양이의뢰된당일핵산추출을하였다. 핵산추출은 intron Viral Gene-SpinTM viral DNA/ RNA Extraction kit (intron biotechnology, Korea) 를이용하여시행하였으며, 핵산추출액은 20 o C 냉동고에보관하였다가검사할때해동하여사용하였다. Seeplex R Diarrhea ACE Detection kit (Seegene, Seoul, Korea) 를이용하여다중중합효소연쇄반응을실시하였다. 키트는각목표균종과독소유전자특이적인시발체조합들로이루어진두개의패널인 diarrhea-b1 (DB-1), diarrhea-b2 (DB-2) 로구성되어있다. DB-1는 Salmonella, Shigella, Vibrio 속특이적인시발체와, C. difficile toxin B (CDB) 시발체를포함하는패널이며, DB-2는 Clostridium perfringens, Yersinia en- Table 1. Target genes and their product size of multiplex PCR Primer set Diarrhea-B1 ACE Diarrhea-B2 ACE Pathogens Campylobacter spp. Campylobacter jejuni Campylobacter coli Shigella spp. Salmonella spp. Clostridium difficile Toxin B Vibrio spp. Vibrio cholerae Vibrio parahaemolyticus Vibrio vulnificus Target gene hip asp vif, ipah sopb tcdb hly tlh vvh Product size (bp) Aeromonas spp. hly, ela 217 VTEC VT1, VT2 291 Escherichia O157 rfb 476 Escherichia H7 flic 370 Yersinia enterocolitica inv 580 Clostridium perfringens cpa 700 terocolitica, E. coli O157, E. coli H7, verotoxin-producing E. coli (VTEC), Aeromonas 속특이적인시발체로이루어진패널로구성되어있다 (Table 1). 각각의시발체에의해증폭되었을때기대하는다중중합효소연쇄반응산물의크기와비교하여상기균종들을동정하였다. 단, E. coli O157, E. coli H7, VTEC 가양성일때장출혈성 E. coli O157:H7 으로해석하였다. 5 μl 의핵산추출액을사용하여증폭한후 2% agarose gel에전기영동하여증폭산물의크기에따라판독하였다. 3. 환자의임상양상조사 다중중합효소연쇄반응양성인환자의의무기록을조사하여환자의성별, 나이, 대변배양이의뢰된당시의발열과복통의유무, 입원일, 설사발병일, 대변검체에서의잠혈과백혈구존재유무, 말초혈액백혈구수, C-반응단백 (C-reactive protein, CRP) 결과를확인하였다. 결 전체 173건의대변검체중다중중합효소연쇄반응검사에서 36검체 (20.8%) 에서 38건의의미있는장병원성세균이검출되었다. Salmonella 5건, Campylobacter 15건, Vibrio 1건, CDB 4건, C. perfringens 5건, Y. enterocolitica 1건, Aeromonas 5건, E. coli O157과 VTEC 둘다양성이 2건등이검출되었고이중 Aeromonas 2건은각각 Campylobacter 양성검체와 Vibrio 양성검체에서동시에검출되었다. E. coli H7만검출된 25건의검체는다중중합효소연쇄반응에서장병원성세균음성으로간주하였다. 다중중합효소연쇄반응에서 Salmonella가검출된 5건과 Vibrio 가검출된 1건은배양에서도양성으로균이동정되어다중중 과

3 164 Korean J Clin Microbiol 2010;13(4): 합효소연쇄반응결과와배양결과가모두일치하였다. 반면후반에 Campylobacter 배양을실시했던 50검체중 4개가 Campylobacter 다중중합효소연쇄반응양성이었지만배양은모두음성이었다. 다중중합효소연쇄반응에서 Aeromonas 양성 5건중 1건만배양에서균이분리되었다. E. coli O157, H7, VTEC이양성인 2건은 sorbitol-macconkey 배지에서선택한무색 E. coli 집락이 O157 항혈청에응집하였고 verotoxin 효소면역검사에서 verotoxin 양성임을확인하였다 (Table 2). 다중중합효소연쇄반응에서 Campylobacter가검출된 15명중복통이 13명 (86.7%) 있었고이중 12명 (80.0%) 은발열도있었다. 2명 (13.3%) 은설사만있었다. 혈액배양검사에서는모두 Table 2. Abbreviations: CDB, Clostridium difficile Toxin B; VTEC, verotoxinproducing E. coli. 균이검출되지않았다. 대변잠혈검사를실시한 13명중 5명이양성이었고, 대변백혈구검사를실시한 6명모두양성이었다. CRP를검사한 13명은모두 CRP가상승하였다. 설사만있던환자 2명은대변잠혈검사에서 1명이양성이었고, 말초혈액백혈구검사는하지않았으며, CRP는상승되어있었다. Campylobacter가검출된환자는모두설사를주소로내원한환자였으며, 1명의환자가간농양으로진단받은외에는모두특별한기저질환없는환자들로감염성설사로진단받았고이중 12명은항생제치료를받았다. 66.7% 환자가성인환자였으며남녀의비율은 2.8:1로남자가더많았다. Salmonella 양성환자 5명은모두발열과복통이있었고대변백혈구는없었으나, 2명은혈액배양에서동일한균이검출되었다. Aeromonas가단독으로검출된 3명의환자는발열이없었고 2명은복통이있으면서 CRP가상승하였다. 이중 1명은골수이식을받은급성골수성백혈병환자로서급성이식편대숙주병으로입원하였으며, 1명은간이식환자로서장염증상으로입원하였다. 다른 1명은조기위암으로내시경수술을받기위해입원하였는데입원당일 2차례의설사가있었으나, 복통, 발열, CRP 증가는없었다. C. perfringens 양성인 5명은모두복통과설사만있었고, 발열, 대변잠혈, 대변백혈구, CRP는음성이었다. 4명은소아였다. CDB가검출된환자는전체 4명중 3명은원내감염에의한것이었고나머지장병원성균들은모두지역사회획득감염이었다 (Table 3). 고찰대변검체에다중중합효소연쇄반응을직접적용했을때 20.8% 에서장병원성세균을검출한것에비해통상적인배양 Table 3. 4 Min-Chul Cho, et al. : Application of Multiplex PCR on Stool 165 검사는 4.6% 에서만검출할수있었다. 통상배양법에의한장병원성세균검출률은국내나국외보고에서 % 인것 [10,11,20] 과비슷한데비해다중중합효소연쇄반응법에의한검출률은매우높은편이다. 이는다중중합효소연쇄반응방법이 10종의다양한장병원성균을목표로하여통상배양에서검출하지못하는균을동정할수있었기때문이다. Salmonella, Shigella, Campylobacter, E. coli O157을대상으로하는다중중합효소연쇄반응과통상배양법을비교한논문 [20] 에서는각각 7.6%, 5.4% 의양성률을보여서다중중합효소연쇄반응에서더높은검출률을보였다. 이번연구에서도배양양성일때다중중합효소연쇄반응에서놓친경우는없었으며, 배양검사를실시한모든균종에대하여배양보다높은검출률을나타내었다. 다중중합효소연쇄반응검사법의더높은양성률은 Campylobacter, VTEC, C. perfringens 의배양검사에서목표로하지않은다양한장병원성균이검출된데기인하였다. 따라서, 직접검체에다중중합효소연쇄반응을적용함으로써장병원성세균에의한설사의원인균검출률을높일수있을것으로판단하였다. 이번연구에서다중중합효소연쇄반응검사와배양검사의양성률이 4.5배정도차이가나는가장중요한원인은 Campylobacter가다중중합효소연쇄반응검사에서전체양성병원균의 39.5% 를차지하는가장빈번한원인균인데비해배양검사는후기 50 검체에서만이루어졌고배양을실시했던 50 검체에서도배양되지않았기때문이다. 운송과정에서 Campylobacter 균이사멸하여배양에서는음성결과를보이나균의 DNA는존재하기때문에다중중합효소연쇄반응에서양성을보였을수도있고, 민감도가낮은배양법을사용하고있을가능성도존재한다. 실제로 Campylobacter 양성환자는대부분발열, 복통, CRP 상승등전형적인세균성장염소견을보였으며, 모두임상적으로지역사회획득세균성장염으로진단받았던환자들로 Campylobacter 장염에합당하였다 [21]. 배양검사를하였던 50검체중 Campylobacter 다중중합효소연쇄반응양성이었던 4주를배양에서모두검출하지못하였는데임상적으로 Campylobacter 감염에합당했던환자들로중합효소연쇄반응검사의위양성이기보다는배양민감도가떨어진데기인한것으로추정하였다. 이전에보고된논문 [20] 에서 Campylobacter 중합효소연쇄반응의위양성문제를해결하기위해다른시발체세트를이용한중합효소연쇄반응으로재검했을때첫번째다중중합효소연쇄반응양성 12검체중 10검체는두번째중합효소연쇄반응에서도양성으로검출되어진양성임을확인하여, 배양법에비해중합효소연쇄반응검사가훨씬민감함을보여주었다. 따라서, 대변검체에직접다중중합효소연쇄반응으로병원균을검출하는방법이배양조건이까다롭거나보관하면생존력이떨어지는균에대하여특히유용할것으로보인다. 하지만, 이번연구에서 Campylobacter에대한통상배양이모든검체에서이루어지지않은것과배양음성인다중중합효 소연쇄반응양성인균주의위양성결과에대한평가가부족하고, 이경우다른검사법으로검증하지못한것은이번연구의제한점이다. 다중중합효소연쇄반응검사에서 Campylobacter가 39.5% 를차지하여가장높은빈도를나타내었고, Salmonella, Aeromonas, C. perfringens, CDB, E. coli H7:O157, Vibrio, Y. enterocolitica 순이었다. 이는선진국에서는 Campylobacter, Salmonella가주요원인균이었다는보고 [7,8] 를고려할때우리나라는장병원성균종들이선진국형분포를보였다. 질병관리본부자료에서지역사회설사환자의원인균으로서 Salmonella가 Campylobacter 의 2.3배의빈도를보였던것 [11] 에비해이번연구에서는 Campylobacter가 Salmonella의 3배를차지하였다. 이는연구에포함된검체들이입원이필요할만큼복통과발열등증상이심했던환자들이었기때문에통상적인지역사회기반검사실의설사원인균분포와는다를수있다. VTEC 는국내설사환자의 0.1% 에불과하다고보고되는데비해 [11] 1.2% 를차지하였고, 이들은감염성장염으로입원한환자에서분리된것으로역시증상이심한환자들에서 VTEC 빈도가더높았을것으로추정된다. 다중중합효소연쇄반응법을 Campylobacter, E. coli O157: H7, C. perfringens 등에적용하면검출룰을획기적으로개선할수있으며, 본연구처럼이들균의빈도가훨씬증가하는쪽으로국내설사원인균분포가바뀔것으로생각된다. Salmonella, Shigella, Yersinia, Vibrio 등통상배양검사가이루어진균종들은다중중합효소연쇄반응과배양법의결과가높은일치율을보인다. 한주도분리되지않은 Shigella를제외하고모두일치하였다. 이전논문들에서도이들균종들에대해서는통상배양법과중합효소연쇄반응의민감도가유사하였다 [14,20]. 따라서, 다중중합효소연쇄반응은이들균종들의검출민감도를높이지는못하지만직접검체에적용함으로써신속한진단을할수있다는장점이있다. 하지만, 균종의동정은배양검사에서균의분리가필수적이기때문에다중중합효소연쇄반응단독으로사용하기보다는배양검사와함께사용하여선별검사로서역할을할수있을것으로보인다. Aeromonas는다중중합효소연쇄반응검사에서유일하게장병원성세균이중복되어나오는균종으로, Campylobacter, Vibrio와함께검출되었던 2명에서는병원균으로의의를판단할수없다. 단독분리되었던 3명중 1명은이식편대숙주병의기저질환으로인한설사를배제할수없었으나, 2명은입원시지역사회획득성설사가있었으며, 다른원인균이밝혀지지않아서 Aeromonas가설사원인균일가능성이있다. Aeromonas 장염은다양한정도의임상증상을보일수있고 [22] 기저질환이있는균혈증환자에서여름철에설사가동반되는것으로알려져있다 [23]. 우리나라에서는생선회나익히지않은해산물의섭취가많아성인에서 Aeromonas의장내집락률이높은것으로추정하고있다 [24]. 이러한이유로설사환자의대변검체

5 166 Korean J Clin Microbiol 2010;13(4): 뿐아니라건강한정상인의대변에서도 % 분리되는것으로알려져있다 [22]. 다중중합효소연쇄반응의결과와배양결과가불일치하는원인으로 Aeromonas의중합효소연쇄반응위양성가능성을배제할수없다. 하지만, 장염을일으키지않고집락화된상태라면균밀도가낮아서중합효소연쇄반응에서양성이지만배양에서는음성일수있을것이다. Aeromonas 가양성인두검체가진정한원인균인 Campylobacter나 Vibrio 와함께검출된것은이를지지한다. Aeromonas는배양에서 1 주만분리되어배양법에비해다중중합효소연쇄반응검사법이더민감했지만, Aeromonas를민감하게검출하는것이임상적으로유용한지는판단하기어렵다. 따라서, 대변검체의다중중합효소연쇄반응검사에서 Aeromonas 양성인경우장염의원인균으로서의가능성에대한추후연구가더필요할것으로생각하였다. VTEC, C. difficile, C. perfringens는독소가장병원성의주요원인이고, 장에정상세균으로존재하며 [25] 일부의세균만이독소를만들기때문에 [16,26] 배양하는경우독소생성균주임을증명해야장염의원인균으로해석할수있으나, 독소생성균주집락을찾아내기는어려워서통상적으로권장되지는않는다. 독소를직접검출하는효소면역법이나유전자검사가표준검사로사용된다 [14,16,17,26-28]. 이번연구에서는 E. coli O157:H7, VTEC 양성인 2 검체는 sorbitol-macconkey 배지를이용한배양에서도균을분리하여다중중합효소연쇄반응결과를확인할수있었다. VTEC 양성인 2명에서모두 E. coli O157: H7이검출된것은그동안국내 VTEC는 O157 이외의다른혈청형이더중요한원인이었을것이라는보고들과상반된다 [27]. 대변검체에다중중합효소연쇄반응검사를직접적용했던이전논문 [20] 에서 E. coli O157을검출한것중일부도 toxin 음성이거나위양성이었음이밝혀졌다. 이와비교하여이번에사용한다중중합효소연쇄반응법은 O157, H7, verotoxin을한번에검출할수있어특이도가높은검사법이다. 따라서, 다중중합효소연쇄반응검사를대변검체에직접적용하는것이 E. coli O157:H7 장염환자의조기감별진단과국내 VTEC 역학을규명하는데유용할것이다. 결론적으로다중중합효소연쇄반응은통상배양에서검출하기어려운 Campylobacter, VTEC, C. perfringens를진단하는데유용하였고, Salmonella, Vibrio, Y. enterocolitica는배양법과유사한민감도를보였다. 따라서통상배양검사와더불어다중중합효소연쇄반응을대변검체에직접적용하는것은세균성장염원인균진단의민감도를크게향상시킬수있을것으로판단하였다. 참고문헌 1. 7 168 Korean J Clin Microbiol 2010;13(4): = 국문초록 = 세균성설사원인균의신속검출을위한다중중합효소연쇄반응검사의대변검체에대한직접적용 울산대학교의과대학서울아산병원 1 진단검사의학교실, 2 소아과학교실조민철 1, 노신애 1, 김미나 1, 김경모 2 배경 : 감염성설사원인균의진단은주로배양에이해이루어졌으나이러한방법은민감도가상대적으로낮고시간이오래걸리는단점을가진다. 본연구에서는다양한설사원인균의신속동정을위해다중중합효소연쇄반응을환자의대변검체에직접적용하여평가하였다. 방법 : 2009년 6월부터 9월까지대변배양이의뢰된 173건의설사검체에 Seeplex R Diarrhea ACE Detection kit (Seegene, Seoul, Korea) 를이용하여다중중합효소연쇄반응을실시하였다. 모든검체에대하여다중중합효소연쇄반응결과와 Salmonella, Shigella, Vibrio, Yersinia를검출하기위한통상적인배양법결과를비교하였다. 후반부의 50 검체에대하여는 Campylobacter 배양이이루어졌다. 다중중합효소연쇄반응에서 verotoxin 생산 Escherichia coli (VTEC) 가양성인검체는장출혈성 E. coli O157:H7 진단을위해계대배양을실시하였다. 또한환자의의무기록을후향적으로조사하여임상증상과검사결과를확인하였다. 결과 : 총 173건의검체중다중중합효소연쇄반응과배양방법에의해각각 36검체 (20.8%) 와 8검체 (4.6%) 에서설사원인균이동정되었다. 다중중합효소연쇄반응에서는 Salmonella 5건, Campylobacter 15건, Vibrio 1건, Clostridium difficile toxin B 4건, Clostridium perfringens 5건, Yersinia enterocolitica 1건, Aeromonas 5건, VTEC 2건으로검출되었고, 배양검사법으로는 Samonella 5건, Vibrio 1건, Y. enterocolitica 1건, Aeromonas 1건, E. coli O157:H7 2건이진단되었다. 결론 : 다중중합효소연쇄반응방법은 Campylobacter, VTEC and C. perfringens 등과같은배양이어려운장병원성균의검출에매우유용하였을뿐만아니라, 일반적인장병원성균인 Salmonella, Shigella, Vibrio, Y. enterocolitica에대해서는통상배양법과유사한민감도를나타냈다. 통상배양검사와더불어다중중합효소연쇄반응을대변검체에직접적용하는것은세균성장염원인균진단의민감도를크게향상시킬수있을것이다.
