Recently, the reimbursement guidelines for osteoarthritis treatment have changed. Methods: In this study, we sought to describe prescription patterns of nonsteroidal anti-inflammatory drugs (NSAIDs) and gastro-protective agent (GPA) and analyze the clinical and economic impacts of the new policy using the national health insurance claims data. The incidence of upper gastrointestinal adverse event by policy change was identified through the odds ratio, and changes in medicine and medical costs related to osteoarthritis through mean and median. Results: There were 204,552 patients before the reimbursement guidelines relaxation and 239,710 after it, a 17.2% rise. The prescription ratio was 3.3% for the patients prescribed with COX-2 selective NSAIDs alone and 1.3% for those with both COX-2 selective NSAIDs and GPA combination before the reimbursement guidelines relaxation. The reimbursement guidelines relaxation significantly increased their ratios to 6.9% and 2.8%, respectively. Gastrointestinal adverse events significantly reduced by 1.21%p after reimbursement guidelines relaxation. The average medicine cost per person increased significantly to KRW 140,291 from KRW 137,323 after the reimbursement guidelines relaxation, while the average medical cost per person slightly decreased from KRW 311,605 to KRW 310,755 after the relaxation, showing no meaningful difference. Conclusion: The reimbursement guidelines relaxation may influence on decreasing the upper gastrointestinal adverse event, increasing the medicine costs and maintaining the medical costs for osteoarthritis. 2 요양급여심사기준완화에따른비스테로이드성항염제및위장관보호제처방변화및영향분석 / 251 (The Third Canadian Consensus Conference Group) 에서는아세트아미노펜이효과를나타내지못하는골관절염환자에게비스테로이드성항염제 (nonsteroidal anti-inflammatory drugs or NSAIDs) 사용을권고한다. 4,5,6) 아세트아미노펜에비해비선택적비스테로이드성항염제 (non-selective nonsteroidal antiinflammatory drugs or ns-nsaids) 와시클로옥시게나아제-2 선택적비스테로이드성항염제 (Cyclooxygenase-2 selective nonsteroidal anti-inflammatory drugs or COX-2 selective NSAIDs) 는관절염증상및기능개선효과가뛰어나다. 7) ns-nsaids 와 COX-2 selective NSAIDs 의진통, 항염효과는비슷하며골관절염개선에있어서비슷한효과를나타낸다. 7) 시클로옥시게나아제는두개의아형, 시클로옥시게나아제-1(COX-1) 과시클로옥시게나아제- 2(COX-2) 로존재한다. 8,9) 시클로옥시게나아제-1은위혈류와중탄산염발생을촉진시키는위장보호프로스타글란딘의발생과신혈류역학및혈소판응집과정같은일반적인신체기능에관여한다. 반면에시클로옥시게나아제-2는주로염증과관계되는세포 ( 대식세포, 단핵세포, 활액막세포 ) 에서높게발현된다. 10,11) 이론적으로시클로옥시게나아제-2만을선택적으로억제하면위장관보호및혈소판기능방해로인한출혈에영향을주지않고통증및항염작용을나타낼수있다. 11) COX-2 selective NSAIDs 와 ns-nsaids 제제의위장관안전성을비교한대부분의연구는특정환자군에서 ibuprofen, diclofenac, naproxen 등의제한된 NSAIDs와위장관보호제조합으로진행되었다. 12,13,14) 2개이상의약제조합의연구는찾아볼수없었으며, 더욱이실제임상에서는위장관을보호한다는근거가충분하지않은제산제가위장관보호제대신많이쓰이고있는실정이지만국내에서는처방실태연구조차없는상황이다. 2013년 7월 1일, COX-2 selective NSAIDs 제제의국내요양급여심사기준이고시 ( 보건복지부고시제 호 ) 되었다. 기존 1. 상부위장관의궤양, 출혈, 천공의치료기왕력이확인되는경우, 2. Steroid제제를투여중인경우, 3. 항응고제투여가필요한경우, 4. 기존의 NSAIDs에불응성인경우, 5. 대량의 NSAIDs를필요로하는경우요양급여를인정하며, 허가사항범위이지만동인정기준이외에투여한경우는 NSAIDs 약값전액을환자가부담토록한것 ( 보건복지부고시제 호, 2007년 1월 1일 ) 에서연령에대한부분이완화된것이다. 즉, 교과서및국내 외가이드라인등을참조하여기존 65세이상의고령자에서 60세이상고령자로보험급여적용이변경되었다. 이에본연구에서는 COX-2 selective NSAIDs의요양급여심사기준중나이완화로인한월 정책구간별 COX-2 selective NSAIDs 및 ns-nsaids와위장관보호제의처방변화를분석하고정책구간별위장관부작용, 의료비를비교평가하여정책결정이미치는영향을알아보고자한다. 연구방법 연구자료본연구에서는 2011년 7월 1일부터 2015년 6월 30일까지 4년간의전국민 건강보험청구데이터 를이용했다. 건강보험청구데이터 란요양기관이의료서비스를제공한후환자의진료비용중 국민건강보험 이부담하는부분에대해지급의뢰를하기위하여건강보험심사평가원에보험급여청구를하면서발생한자료이다. 청구데이터는명세서일반내역 (Table 20), 진료내역 (Table 30), 수진자상병내역 (Table 40), 원외처방상세내역 (Table 53) 으로구성되어있으며진료내역 ( 처치, 시술, 검사, 처방전약등 ), 진단명, 보험자지급비용, 환자부담금, 환자인구특성, 요양기관정보등과같은다양하고방대한정보를포함하고있다. 15) 진단명은주진단명과부진단명으로나누어져있고국제표준질병분류체계 (International Classification of Disease and Related Health Problems, 10th Revision, ICD- 10) 를기본으로한한국표준질병 사인분류 (Korean Standard Classification of Diseases 6, KCD6) 로입력되어있다. 2003년도에실시된 건강보험질병코드의타당도제고방안및자료의활용방안수립연구 에따르면, 의료청구데이터상의진단명정확성은주상병의경우, 입원은 70%, 외래는 56% 일치도를보였으며, 부상병의경우, 49%, 38% 의일치도를보였다. 또한중증질환이경미한질환보다, 병원이의원보다더높은정확성을보였다. 16) 본연구는건강보험심사평가원의보건의료빅데이터센터에서건강보험청구자료를기반으로한진료정보와의료자원등의데이터를분석하는것으로써, 모든개인정보가암호화된환자정보를제공받았다. 본연구는성균관대학교기관생명윤리위원회로부터심의를거쳐승인받았다. 연구대상및자료구축본연구대상환자는 NSAIDs를처방받은 60-64세골관절염환자 (KCD6: M15-M19) 로동일명세서에골관절염을진단 ( 주부상병 5번째까지 ) 받고 NSAIDs를 30일이상연속처방받은경우이다. 연구대상자는 COX-2 selective NSAIDs의요양급여심사기준중기존 65세이상의고령자에서 60세이상고령자로보험급여적용이변경된것을고려하여, 새로요양급여심사기준에포함된환자로결정되었다. 만성질환인골관절염과상관없이단기적통증감소를위해 NSAIDs를처방받는경우를배제하기위해 30일이상 NSAIDs를연속처방한경우만포함했다. 연구대상환자로추출된환자의약물사용양상, 상부위장관부작용, 의료비및환자특성등분석에필요한정보를환자단위자료로구축했다. 환자가요양급여심사기준확대전에만의료기록이있다면정책구간 ( 전 ) 군으로, 확대후에만의료기록이있다면정책구간 ( 후 ) 군으로분류했다. 요양급여

3 252 / Korean J Clin Pharm, Vol. 27, No. 4, 2017 심사기준확대전 후의료기록이모두있다면확대시점을기준으로나누어정책구간 ( 전 ) 과정책구간 ( 후 ) 로나누어분석했다. 정책구간은요양급여심사기준확대전 후각 2년이다. 연구대상약물연구대상약물은 COX-2 selective NSAIDs를포함한모든 NSAIDs와진료지침이나실제의료현장에서빈번하게사용되는경구용위장관보호제를포함했다. 11,17,18) 연구기간동안우리나라에서처방가능한 COX-2 selective NSAIDs는 celecoxib 가유일했고, ns-nsaids는아세클로페낙, 멜록시캄, 나프록센등을포함한다. 안약과외용제의경우위장관계부작용과관련성이있다고보기어렵기때문에본연구에서는경구제만포함했다. NSAIDs 약물은 ATC code M01A (antiinflammatory and antirheumatic product, non-steroids) 에포함된약제중에우리나라에서처방가능한약제를모두포함했다. 19) 위장관보호제는미소프로스톨 (misoprostol), PPI(proton pump inhibitor, 양성자펌프억제제 ), H2RA (H2-receptor antagonist, 히스타민수용체길항제 ), 제산제 (antacid) 를포함했다. 통계분석연구대상약제중 NSAIDs는계열단위및성분단위로, 위장관보호제는계열단위로분석했다. NSAIDs 계열은 COX-2 selective NSAIDs와 ns-nsaids로, 위장관보호제는미소프로스톨, PPI, H2RA, 제산제로구분했다. ns-nsaids 사용은 1개의 ns-nsaids 사용과 2개이상의 ns-nsaids 사용그룹으로구분했다. 또한, 두계열이상의위장관보호제를사용한경우는 2계열이상위장관보호제로분류하여분석했다. 월별처방패턴은빈도분석을통해처방받은환자수와백분율을분석했다. 정책구간별처방패턴은기술통계분석을통해월별처방받은환자수평균과표준편차을분석하였고정책구간전 후로 regimen 처방에차이가있는지알아보았다. 연구대상환자로결정된이후해당정책구간에발생하는위장관부작용을 NSAIDs로인한부작용으로조작적정의하였으며, 상부위장관부작용결과변수는선행논문고찰을통하여 ICD-10 코드로정의했다 : 위궤양 (Gastric ulcer; K25, K5660), 십이지장궤양 (Duodenal ulcer; K26, K5660), 소화성궤양 (Peptic ulcer; K27), 위공장궤양 (Gastrojejunal ulcer; K28), 위염, 십이지장염 (Gastritis and duodenitis; K2900, K2901, K2940, K2950, K2941, K2951, K2960, K2961, K2920, K2921, K2970, K2990, K2971, K2991, K2980, K2981, K5281), 토혈, 흑색변 (Hematemesis, melena, hemorrhage of the intestinal tract unspecified; K920, K921). 20) 상부위장관부작용은빈도분석을통해정책구간별부작용발생환자수및누적발생률그리고정책구간 ( 전 ) 대비정책구간 ( 후 ) 의부작용발생교차비와 95% 신뢰구간을분석했다. 추가적으로 1개 NSAIDs와 2개이 상의 NSAIDs를처방했을때위장관부작용의발생정도를비교했다. 환자의의료비가정책구간에따라차이가있는지요양급여심사기준완화전 후각 2년을살펴봤다. 골관절염관련된약품비는 NSAIDs와위장관보호제비용을분석하였고골관절염관련된의료비는주상병과제 1-4 부상병에관절염진단이있는명세서의모든비용을보았다. 의료비는기술통계분석을통해평균, 표준편차, 중앙값, IQR을분석했다. 범주형변수는교차분석을연속형변수는독립표본 t-검정을사용했다. 모든통계분석에서유의수준은 0.5% 로하였고, SAS Enterprise Guide 6.1(SAS Institute, Inc., Cary, NC) 을이용하여수행되었다. 연구결과 연구대상자특성골관절염을진단받고관절염치료제를 30 일이상복용한 60-64세노인은정책구간전 후각각 204,552명, 239,710명이다. 남성의비율은정책구간 ( 전 )(27.4%) 과비교해정책구간 ( 후 ) (27.8%) 에 0.4%p 증가했다 (p=0.0014). 의료보장형태는정책 Table 1. Abbreviations: COX-2 (COX-2 selective nonsteroidal anti-inflammatory drugs); GPA (gastro protective agent); ns-nsaids (nonselective nonsteroidal anti-inflammatory drugs). * Chai-square test 구간 ( 전 ) 에는건강보험이 92.1%, 의료급여 7.9% 에서정책구간 ( 후 ) 에건강보험 93.7%, 의료급여 6.3% 로정책구간 ( 후 ) 에건강보험환자가 1.6%p 증가했다 (p<.0001). 정책구간 ( 전 ) 과정책구간 ( 후 ) 외래환자는각각 96.8%, 96.9% 로대다수의환자가외래환자였으며, 정책구간 ( 후 ) 상급종합병원과병원의비율은정책구간 ( 전 ) 보다각각 1.6%p, 0.9%p 높아졌다 (p<.0001). 도시에위치한병원이용률은정책구간 ( 전 )(60.9%) 보다정책구간 ( 후 )(60.0%) 에 0.9%p 낮아졌다 (p<.0001). 월 정책구간별처방패턴변화요양급여심사기준변경시점 (2013 년 7 월 1 일 ) 을기준으로 COX-2 selective NSAIDs 단독, COX-2 selective NSAIDs와위장관보호제병용처방이유의하게증가했다. 급여기준변경전월 (2013년 6월 ) 의 COX-2 selective NSAIDs 단독, COX-2 selective NSAIDs와위장관보호제병용처방을받은환자수는각각 1,011명, 450명이었으나급여기준변경해당월 (2013년 7월 ) 에는각각 1,756명, 651명으로 1달사이에환자수가 73.7%, 44.7% 증가했다. 반면 ns-nsaids 단독및 ns-nsaids와위장관보호제병용처방을받은환자의경우는각각 18,785명에서 21,549명으로 14.7%, 11,162명에서 12,417명으로 11.2% 증가하는데그쳤다 (Figure 1). Table 2는정책구간전 후월별환자수의평균을 regimen 별로비교한것이다. 연구대상환자중, 정책구간 ( 전 ) 의 COX-2 selective NSAIDs 단독처방환자 (3.3%), COX-2 selective NSAIDs와위장관보호제병용처방환자 (1.3%) 의비율은정책구간 ( 후 ) 에각각 6.9%, 2.8% 으로 2배이상증가했다. 반면, ns-nsaids 단독처방환자비율은 62.0% 에서 54.3% 로감소, ns-nsaids와위장관보호제병용처방환자비율은 33.5% 에서 36.0% 로소폭상승했다. COX-2 selective NSAIDs와위장관보호제병용 regimen 중구성비증가율을살펴보면 PPI와 병용이유의하게 49.0% 증가했다. ns-nsaids 단독처방받은월평균환자수는정책구간 ( 전 ) 19,287명에서정책구간 ( 후 ) 19,428명으로소폭증가했다. 하지만정책구간전 후환자수가각각 204,552명, 239,710명으로 17.2% 증가한것을고려하면, 전체중구성비는 12.4% 감소했다. 1개 ns-nsaids regimen에서월평균환자비율이높은약제는 avocado and soyabean oil, unsaponifiables( 정책구간 ( 후 ) 구성비 : 36.8%) > other antiinflammatory and antirheumatic agent, non-steroids (18.4%) > meloxicam(12.0%) > aceclofenac(11.9%) > diacerein (8.9%) 순이다. ns-nsaids 단독군중 avocado and soyabean oil, unsaponifiables과 other antiinflammatory and antirheumatic agent, non-steroids 성분만이정책구간전 후에구성비가증가했다. 2개이상 ns-nsaids regimen에서월평균환자비율이높은약제는 meloxicam+diacerein (11.6%) > Aceclofenac+other antiinflammatory and antirheumatic agent, non-steroids (9.4%) > Meloxicam+other antiinflammatory and antirheumatic agent, non-steroids(8.1%) > Aceclofenac+diacerein (7.3%) > Meloxicam+avocado and soyabean oil, unsaponifiables (7.2%) 순이였다. 1개 ns-nsaids 처방받는환자중에가장빈번하게처방된 avocado and soyabean oil, unsaponifiables(36.8%) 와 other antiinflammatory and antirheumatic agent, non-steroids(18.4%) 는 2개이상 ns-nsaids regimen에서도상위 10 순위중 8개에서적어도한성분을포함하고있었다. Avocado and soyabean oil, unsaponifiables(112201atb, 아보카도소야불검화정량추출물 ) 과 other antiinflammatory and antirheumatic agent, nonsteroids(416601atb, 위령선 괄루근 하고초 에탄올엑스 ; ATB, 당귀 목과 방풍 속단 오가피 우슬 위령선 육계 진교 천궁 천마 홍화 25% 에탄올연조엑스 ) 은생약조성물약제이다. 우리나라에서는골관절염치료를위해 NSAIDs

H2RA 표준용량은소화성궤양의일차혹은이차예방에권고하지않으며위궤양예방에효과가적다. 21,22) PPI의경우상부위장관합병증예방에는도움이되지만하부위장관합병증예방에는도움이되지않는다. 22) 하지만 H2RA보다효과가우수한것으로인정되고있다. 23) 큰구성비감소율을보인제산제는상부위장관증상을차폐함으로써 NSAIDs를단독투여하는경우에비하여궤양합병증의빈도를 2배증가시킨다. 7) 정책구간전 후 찰

COX-2 selective NSAIDs는 ns-nsaids와비교해서약제의가격이높은수준이다. Celecoxib 약가는대부분의연구기간동안 973원이였고 2015년 6월 21일제너릭출시로인해 681원으로하향조정되었다. ns-nsaids의가중평균가 (2011 년하반기 23), 2015 년 24) ) 는단일제로가장많이사용되는 avocado and soyabean oil, unsaponifiables 300 mg(479 원, 412 원 ) 과 other antiinflammatory and antirheumatic agent, nonsteroids(245원 -405원, 174원 -458원 ) 의가격은 celecoxib 보다낮 은수준이다. Meloxicam 15 mg(625원, 411원 ), aceclofenac 200 mg(558원, 414원 ) 가격역시낮다. COX-2 selective NSAIDs를처방받는환자가늘어나면서약품비가함께증가한것으로보인다. 미국에서진행된청구데이터를이용한연구에서는골관절염과류마티스관절염환자를대상으로 celecoxib가급여제한이있는군과제한이없는군에서심각한위장관부작용발생과관련된의료비비교했다. 제한이없는그룹에서 celecoxib의사용이 2배높게나타났다. 심각한위장관부작용은급여제한이있는그룹에서높게나타났으며 (5.4% vs 4.6%, p<.001) 위장관부작용과관련된보정된평균비용은급여제한이있는군에서 15배이상높게나타났다 ($1559 vs $101). 25) 선행연구를통해본연구에서도골관절염관련된약품비가늘어났는데도불구하고골관절염관련된의료비가증가하지않은것은위장관부작용이줄어든것이하나의요소로고려될수있다고생각된다. 비스테로이드성항염제는골관절염뿐아니라류마티스관절염, 강직척추염, 치통, 외상후생기는염증등광범위하게사용된다. 따라서, 골관절염환자의비스테로이드성항염제를복용이골관절염치료목적이라고단정할수없다. 이에본연구에서는만성질환인골관절염과상관없이단기적통증감소를위해 NSAIDs를처방받는경우를배제하기위해 30일이상 NSAIDs 를연속처방한경우만포함하여보완할수있도록했다. 또, 건강보험청구데이터는진료비청구를목적으로하기때문에비급여항목이불포함되었고진단명이정확하지않을수있다. 다른나라에서진행된진단명일치도연구를살펴보면상부위장관출혈과이차소화궤양의양성예측치및민감도는각각 85.2%(95%CI: 80.2%-90.2%) 와 77.1%(95%CI: 69.1%-85.2%) 이다. 26) 위장관부작용은증상만으로진단하기어렵다는것을고려했을때실제발생한것보다낮게진단되었을수있다. COX-2 selective NSAIDs 제제의요양급여심사기준은연령을포함한 5개의임상적기준으로이루어져있다. 본연구에서는환자의연령이완화된부분을고려하여분석이진행되었고, 연령외의 5개의임상적기준에부합하느냐에대한부분은제외되었다. 이 5가지임상적기준에대한영향을

8 요양급여심사기준완화에따른비스테로이드성항염제및위장관보호제처방변화및영향분석 / 257 확인하기위해서는추가적인연구가이루어져야한다. 본연구는요양급여심사기준이변경된환자만을연구대상으로하여비교군이없기때문에처방형태의변화가정책변화의영향만이라고설명하기어려울수있다. 마지막으로해당연구는요양급여심사기준전 후를중심으로비교되었으나 COX-2 selective NSAIDs와 ns-nsaids의약가차이그리고 GPA의위장관보호역할의특성을고려하여 COX-2 selective NSAIDs군과 ns-nsaids군의의료비차이와 GPA 를처방받은군과그렇지않은군의상부위장관부작용발생률을추가연구하는것이필요하다. 결 건강보험청구데이터를이용하여요양급여심사기준변경이라는정책변화로인한 NSAIDs 및위장관보호제처방변화, 부작용누적발생률, 의료비를분석했다. 본연구를통해서 COX-2 selective NSAIDs의급여완화이후 COX-2 selective NSAIDs 처방및 PPI 병용처방의증가와제산제병용처방의감소를확인하였고, 우리나라비스테로이드성항염제와위장관보호제와병용처방실태를파악했다. 또, COX-2 selective NSAIDs 급여완화이후상부위장관부작용이 1.2%p 감소하였고, 의료비는정책시행전 후큰변화가없는것을확인했다. 론 참고문헌 1.
