The purpose of our study was to investigate whether tonic vestibular imbalance appeared as spontaneous nystagmus during the wakeful state could present during REM sleep. Materials and Method: Overnight polysomnography (PSG) was performed in 7 patients with spontaneous nystagmus due to vestibular neuritis (VN) and 7 control patients without dizziness or any nystagmus. The numbers of horizontal saccades were counted, during 3 minutes samples of the alert state before and after the PSG and the first and last REM sleep. Results: All patients with VN showed significantly more saccades (fast phases of spontaneous nystagmus) towards the side contralateral to their vestibular lesion in the awake state before and after the PSG compared with control group. By contrast, during REM sleep the patients with VN showed no preponderance in saccade direction (p<0.05). Some brief nystagmoid jerks showed during REM sleep in both patients and controls equally and also had no preponderance in direction. Conclusion: The tonic vestibular imbalance at peripheral level observed during alert state does not appear at the brainstem level during REM sleep. It is suggested that a de-afferentation of the peripheral vestibular input to the REM sleep generating areas may explain an absence of nystagmus during REM sleep in patients with VN. 2 김현아 외 의 어지럼증, 일측 방향으로의 자발 안진이 있으면서 다른 신경학적 증상이나 징후는 없는 것을 특징으로 한다. 전정신경염 환자에서 각성 상태 동안에는 빠른 성분이 건측을 향하는 수평 회전성 안진이 관찰되나 비렘수면(non-REM sleep) 동안에는 빠른 안구 운동이 나타나지 않는다. 렘수면 동안의 빠른 안구 운동은 단 속운동으로 생각되므로 안진은 렘수면 동안에도 관찰 될 것으로 기대된다. 따라서 우리는 전정신경염 환자 에서 각성시 보이는 자발 안진이 렘수면 동안에도 관 찰될는지에 대해 조사하였다. 재료 및 방법 자발안진이 관찰되는 7명의 전정신경염 환자와 안 진이 없는 7명의 대조군을 대상으로 야간 수면다원검 사를 시행하였다. 전정신경염의 진단은 1999년 Strupp 과 Brandt에 의해 제시된 기준을 기초로 하였다. 7) 대 조군은 수면 장애를 가진 환자를 대상으로 하였는데, 이들은 모두 현훈의 병력이 없고 각성 시 프렌젤 안 경(Frenzel glass)을 착용하여도 자발 안진이 관찰되지 않았다. 수면다원검사는 전정신경염의 증상 시작 당일 에서 이후 5일까지의 기간 동안 시행되었다(평균 49시 간, 11~122시간). 전기안구운동기(electro-oculography: EOG)는 전극을 양쪽 눈 옆과 안구 아래위에 붙여 실 시하였다. 전기안구운동기의 보정(calibration)은 수면 다원검사 시작 시 시행되었다. 우측과 좌측(전정신경 염에서는 동측과 반대측) 수평 단속운동은 수면 시작 전 각성상태와 야간 수면 후 각성상태, 그리고 야간 수면 중 첫번째와 마지막 렘수면 구간에서 각각 3분 동안의 기록으로 계산되었다. 수평 단속 운동은 3도 이상의 측방 운동이나 초당 50도 이상의 속도를 보이 는 안구 운동만을 포함하였다. 8) 단속운동이 방향의 우위를 보이면 안진으로 간주하였다. 각성 상태와 렘 수면에서 수평 단속운동과 안진의 방향적 우위를 찾 아내기 위해 우측/동측과 좌측/반대측 수평 단속운동 의 수를 비교하였다. 통계학적 분석은 Mann-Whitney U test를 이용하여 시행하였으며 P값이 0.05 이하일 때 유의한 것으로 판정하였다. 결 과 전정신경염 환자는 대조군보다 나이가 많았으며 (전정신경염 62.29±5.68세, 대조군 44.29±19.31세), 두 그룹 간의 성별 분포는 비슷하였다. 우측 전정신경염 은 5명, 좌측 전정신경염은 2명이었다. 대조군은 폐쇄 성 무호흡을 진단 받은 환자들이었다. 모든 전정신경 염 환자들은 자발 안진을 보였고 이는 프렌젤 안경을 착용하였을 때 가장 잘 관찰되었다. 비디오 테이프로 환자와 대조군이 분석 중인 각성 상태와 렘수면 동안 좌우로 머리를 틀지 않고 바로 누워 있었다는 것을 Table 1. 전정신경염의 진단은 1999년 Strupp 과 Brandt에 의해 제시된 기준을 기초로 하였다. 7) 대 조군은 수면 장애를 가진 환자를 대상으로 하였는데, 이들은 모두 현훈의 병력이 없고 각성 시 프렌젤 안 경(Frenzel glass)을 착용하여도 자발 안진이 관찰되지 않았다.

Some brief duration (2~3 seconds long) of nystagmoid jerks equally found with in both patient and control during REM sleep (arrows). 확인하였다. 전정신경염 환자들은 각성 상태에 병변 방향의 반대쪽으로 통계적으로 유의하게 더 많은 단 속운동을 보였고 이는 자발 안진을 의미한다(P<0.05) (Table 1, Fig. 1-A). 그러나 렘수면 동안에는 어떤 환 자도 자발 안진이나 유의한 정도의 단속운동의 방향 적 우위를 보이지 않았다. 대조군에서는 각성 상태나 렘수면 동안 단속운동의 방향 차이를 보이지 않았다. 전정신경염과 대조군 모두에서 짧은 기간(2~3초 가 량)의 안진양의 격동(nystagmoid jerk)(fig. 1-B)이나 초 당 10도에서 20도 정도의 속도를 가진 느린 수평 안 구 운동이 렘수면 동안 나타났다. 비렘수면 동안에는 두 그룹 모두에서 단속운동이 관찰되지 않았다. 고 찰 내측 전정핵(medial vestibular nucleus)은 렘수면을 만들어 내는데 중요한 역할을 한다고 알려져 있다. 9,10) 우리 환자들에서 말초 수준에서의 긴장성 전정 불균 형이 내측 전정핵까지 성공적으로 전달되면, 안진이 나 유의한 정도의 단속운동의 방향적 우위가 렘수면 동안 관찰되어야 할 것이다. 그러나 전정신경염 환자 중 어떤 환자도 렘수면 동안 안진이나 단속운동의 방 향성의 우위를 보이지 않았는데, 이는 각성 상태 동안 말초 수준에서 일어나는 긴장성 전정 불균형이 렘수 면 동안 뇌간 수준에서는 활성화되지 못한다는 것을 의미한다. 이는 또한 비동기적 수면(desynchronized sleep) 동안의 빠른 안구 운동과 관련된 신경원성 방 전(neuronal discharge)은 전정신경염에서 안진을 만들 어내는 기전에 의해 영향을 받지 않는다는 것을 말해

4 김현아 외 준다. 정적 수면(quiet sleep), 동기화된 수면(synchronized sleep)이라고도 불리는 비렘수면에서는 빠른 안구 운 동이 관찰되지 않는다. 말초 혹은 중추의 병변에 의해 각성 상태 동안 보이던 자발 안진이 비렘수면 동안에 는 사라진다는 여러 보고들이 있다 ) 우리 연구에 서도 전정신경염 환자에서 각성 시 보이던 안진이 비 렘수면 동안에는 관찰되지 않았다. 고양이를 이용한 연구에서 일측 미로절제술(labyrinthectomy) 후 각성 상태 동안 관찰되던 자발 안진이 렘수면 동안에는 대부분 억제되거나 완전히 사라진다 는 보고들이 있으며, 11) 이는 우리 연구의 결과와 일치 한다. 그러나 중추 병변에 의해 안진이 관찰되는 환자 에서 렘수면 동안의 안구 운동에 대해서는 여러 연구 들이 상반된 결과를 보여준다. 이전의 보고들은 중추 병변으로 인해 각성 상태 동안 보이던 안진이 렘수면 동안에는 사라진다고 하였다. 12,13) 반대로 다른 연구들 은 중추 병변으로 인해 각성 상태 동안 안진을 보이 는 환자들에서 렘수면 동안 짧은 기간의 안진양 격동 이 관찰된다고 하였다. 14,15) 그러나 이런 안진양 격동 은 건강한 환자에서도 기록되었으며 13) 우리는 또한 전정신경염 환자와 대조군 모두에서 안진양 격동이 방향적 우위성을 보이지 않는다는 것을 관찰하였다. 따라서 이런 안진양 격동은 렘수면 동안 관찰되는 정 상 소견으로 생각된다. 이런 상반되는 결과들은 아마 1) 빠른 안구 운동의 억제에 대한 정의의 차이(즉, 안 진과 안진양 격동 모두의 소실 혹은 안진의 소실), 2) 안진의 방향과 양상에 대한 기술의 부족, 3) 안진의 원인이 되는 병변에 대한 기술의 부족 등에 의해 설 명될 수 있을 것이다. 렘수면 동안 안진이 소실되는 현상에 대한 신경 생리학적 근거는 명확치 않다. 렘수면 동안에는 운 동 무긴장(motor atonia)이 발생하는데 이는 운동 출 력(motor output)의 원심성(efferentation) 신호 억제 때 문인 것으로 생각되고 있다. 비슷하게 렘수면 동안의 안진 소실은 단속운동 방출 신경원을 포함하는 렘수 면 발생 지역으로 향하는 말초성 전정 신호의 구심성 차단(de-afferentation)이 내측 전정핵으로 들어가는 말 초의 신호를 억제하는 것으로 설명할 수 있을 것이다. 내측 전정핵의 신경원은 렘수면 동안의 빠른 안구 운 동과 동기화되는 전위를 가진다고 알려져 있다. 9,10) 수 면 동안의 안구 운동에 전정 자극이 미치는 영향에 대한 과거의 연구들에서 전정 안구 반사나 시운동성 안진이 렘수면 동안 유발되지 않는다는 비슷한 결과 를 보여주었다. 17,18) 이 또한 말초 전정 신호의 구심성 차단으로 설명할 수 있을 것이다. 우리 연구의 제한점은 두 그룹의 수가 적고 두 그 룹 사이의 연령이 비슷하지 않으며 대조군이 렘수면 에 영향을 미칠 수 있는 수면 무호흡을 가진다는 것 이다. 차후 말초 또는 중추성 안진을 보이는 더 많은 수의 환자에 대한 연구가 이러한 결과를 확인하는데 필요할 것으로 생각된다.
