4 History of NPSG s The Joint Commission first implemented the first set of NPSG s for healthcare organizatio ns in January 1, Hospital Accreditation is dependent on co mpliance with the NPSG s.

5 Identify Patients Correctly NPSG : Identify Patients Correctly Use at least two patient identifiers when providing care, treatment, and services. 8 Use Medicines Safely NPSG : Before a procedure, label medicines that are not labeled. Elements of Performance If not immediately administered Labeling occurs when any medication or solution is transferred from original packaging to another container Labels include name, strength, quantity, dilutent and volume, prep date, exp date when not used within 24 hours, exp time when less than 24 hours Verify both verbally and visually. 2 people required if the person preparing is NOT the person administering. 10 Use Medicines Safely NPSG : Maintain and communicate accurate patient medication information (record and pass along/change of shift report). Elements of Performance Find out what medicines the patient is taking. Compare those medicines to new medicines given to medicines the patient is taking. Make sure the patient knows which medicines to take when they are at home. Explain the importance of managing medication information to the patient. Tell the patient it is important to bring their up-to-date list of medicines every time they visit a doctor.

11 Use Alarms Safely NPSG Make improvements to ensure that alarms on medical equipment are heard and responded to on time. Improve the safety of clinical alarm systems Elements of Performance A 1. 22 고위험의약품분류 Institute for Safe Medication Practices (ISMP): 22개의범주와 12개의특정약물을고위험의약품으로분류 Institute of Healthcare Improvement (IHI): Five Million Live Campaign 을시행하면서항응고제, 마약, 수면 / 진정제, 인슐린을고위험의약품으로분류 Joint Commission International (JCI): 항암제와유사발음 / 유사외관약물을추가하고병원에서목록을만들어관리

23 고위험의약품보관 고위험의약품의보관을각병동에서약국으로일원화하는방법이고위험의약품의사고를줄이는가장효과적인방법들중하나라고보고 부서보관 : 다른일반의약품들과장소를분리보관하고, 보관장소와약물에각각라벨을부착하여늘같은장소에같은약이보관 약국보관 : 고위험의약품은지정된장소에보관하고, 장소에라벨을부착하여임의로변경할수없도록함

24 고위험의약품처방 전체투약오류중처방에의한오류비율 : 3% ~ 79% 오류종류 : 읽기어려운처방, 미완성처방, 금기처방, 부적절한용량등 고위험의약품처방시오류감소방법 : 처방의표준화 (IHI) 사례 처방표준화 : 용량, 용법, 투약경로등으로가능한한처방의형태를단순화함으로써오류의가능성을줄일뿐아니라오류가발생하더라도누구나쉽게알수있도록하자는의도 항암제 : 주기적으로환자에게투여되는모든의약품일정포함 염화칼륨과같은고농도전해질 : 중심정맥용과말초정맥용정도로구별하여각각 1-2가지의희석된형태로만처방가능 헤파린과같은항응고제 : 체중에근거한프로토콜을사용함으로써처방이잘못되는것을미연에방지함과동시에조제및투여시에도쉽게오류발견

25 고위험의약품조제 조제전처방전감사 : 처방오류의약 70% 를걸러낼수있다고보고 조제오류 : 전체투약오류중 4% ~ 42% 모든고위험의약품은조제후약물고유의라벨이외에고위험의약품라벨을붙임으로써누구나인지하게하여오류를최소화

26 고위험의약품투여 1993년부터 1998년까지투약오류로사망하여식품의약국에보고된사고조사한결과 : 과용량에의한사고가가장많았고, 타환자에게투약된경우가그다음으로많이보고 원인을분석한결과 약 65.2%: 단순실수를포함하는인적요인 개인적인문제보다는과로, 주변환경, 교육등과같은시스템적인문제 간호사의근무시간이 12.5 시간을지나면오류가증가 약 15.8%: 의사전달의오류 IHI 는고위험의약품의투약시두사람이독립적으로확인하는절차요청

27 고위험의약품모니터링 약물부작용으로보고된사건들중예방가능했던 317 례분석결과 전체 50%: 헤파린과용량으로인한출혈, 마약의과용량또는다른의약품상호작용에의한호흡곤란, 인슐린과용량으로인한저혈당 IHI는고위험의약품의경우처방의표준화단계에서약물투여중무엇을모니터링할것인가를결정할것을권장

28 고위험의약품교육 약물투여오류원인에대한간호사설문조사결과 : 신규인력 (37.5%), 약물에대한이해부족 (31.9%), 환자상태에대한이해부족 (22.2%), 불충분한훈련 (15.3%) 약물에대한교육은의사에게도중요한사항으로신규약물이입고되면이에대한충분한교육필요 약물에대한정보를늘접근이가능하도록하는약품정보시스템을갖추는것도매우중요한방법
