We performed quality assessment for white blood cell count, hemoglobin, hematocrit, red blood cell count, platelet count, blood cell morphology, prothrombin time and activated partial thromboplastin time. The response rate was more than 95.2%. The coefficients of variation in hemoglobin, hematocrit and RBC were stable but variable in platelet count and WBC count according to measuring cell counters. Test results of blood cell morphology showed variation among various cell morphologies. Key Words:External quality assessment, Hematology 서 론 재료및방법 2007 년도진단혈액분과위원회의외부정도관리사업은평균 722기관을대상으로백혈구수, 혈색소치, 적혈구용적백분율, 적혈구수, 혈소판수, 세포형태판독등 6종목에대하여 4회실시하였으며, 프로트롬빈시간, 활성화부분트롬보플라스틴시간은 2회실시하였다. 워크샵은 혈액응고의기본개념, 혈액응고검사의원리및방법, 혈액응고 INR 보고및인증시참고사례 에대하여충남대학교의과대학에서 1회시행하였다. 2007 년도제4차부터홈페이지에서결과입력, 결과조회및출력을하게하였다. 교신저자 : 권계철우 ) 301-040 대전광역시중구대사동 640 충남대학교병원진단검사의학과전화 :042)220-7799, FAX:042)257-5365 E-mail:kckwon@cnu.ac.kr 일반혈액검사정도관리사업은백혈구수, 혈색소치, 적혈구용적백분율, 적혈구수, 혈소판수를대상으로 3월, 5월, 9 월, 11월에 4회실시하였으며, 정도관리물질은 1차, 3차, 4차에는동일혈액형을가지고있는정상인으로부터채혈한혈액을영국의 Reardon DM[1] 이개발한방법을이용하여제조하였고, 2차에는 BIO-RAD 사의 Lyphochek TM Hematology-16 Control Normal 를이용하였으며, 각차수마다 vial 간의균질성을보기위하여 20 vial 마다 1 vial 씩약 35 vial 을선택하여표준편차와변이계수를구하였다. 혈액응고시간검사정도관리사업은프로트롬빈시간, 활성화부분트롬보플라스틴시간검사를대상으로 3월, 11월에 2 회실시하였으며, 정도관리물질은 BIO-RAD 사의 Lyphochek R Coagulation control Levels 2를이용하였고, vial 간의균질성을보기위하여 20 vial 마다 1 vial 씩약 15개 vial 을선택하여표준편차와변이계수를구하였다. 세포형태판독을위한정도관리사업은 3월, 5월, 9월, 11월에매회 5개의이미지를홈페이지에게시하고참가기
30 권계철 김선희 김용구 박연보 박종우 박찬정외 관에서조회하여판독할수있게하였다. 정도관리검체의정확하고신속한운송을위하여계약택배를이용하였으며, 2차일반혈액정도관리물질은 1차때함께발송하였다. 정도관리검사결과는대한임상검사정도관리협회홈페이지에서입력하게하였고, 입력기간종료후결과를???.xls' 형식의파일로내려받아오류데이터검증후분석하였다. 검증이끝난결과파일을 진단혈액외부신빙도조사통계분석프로그램 ( 윤종현, 박연보, 권계철, 김진규 ; 2006 년 ; Visual FoxPro 5; Microsoft, USA) 으로분석하였다. 백혈구수, 혈색소치, 적혈구용적백분율, 적혈구수, 혈소판수검사결과의분석은왜곡결과를제외하기위하여 ±3SD 이내기관들로측정기기그룹별과측정방법그룹별로평균, 표준편차, 변이계수를구하였으며, 결과그룹별로도수분포를구하였다. 프로트롬빈시간, 활성화부분트롬보플라스틴시간검사결과의분석은왜곡결과를제외하기위하여 ±3SD 이내기관들로측정기기그룹별과측정시약그룹별, 측정방법그룹별로평균, 표준편차, 변이계수를구하였으며, 결과그룹별로도수분포를구하였다. 결과분석시프로트롬빈시간은 INR 값을적용하였고, 활성화부분트롬보플라스틴시간은각기관의측정결과를각기관의참고치중간값으로나누어산출된값을적용하였다. 세포형태판독에대한분석은각세포별로결과도수별기관수와백분율을보고하였다. 분석한결과를???.csv 형식의파일로내려받아대한임상검사정도관리협회홈페이지에올린후참여기관에서직접조회및출력할수있게하였다. 결과 1. 신빙도조사참여기관일반혈액검사신빙도조사참여기관수는 1~4차까지각각 698기관, 698기관, 743기관, 748기관에검체를발송하여 668 기관 (95.7%), 641 기관 (91.8%), 714기관 (96.1%), 724기관 (96.8%) 의회신율을보여평균 722 기관에검체를발송하여 687 기관 (95.2%) 의회신율을보였으며, 세포형태신빙도조사참여기관수는 1~4차까지각각 5개씩혈액세포형태이미지를홈페이지에게시하여일반혈액검사발송기관기준으로 404 기관 (57.9%), 390 기관 (55.9%), 414기관 (55.7%), 415기관 (55.5%) 의회신율을보여평균 406기관 (56.2%) 의회신율을보였으며, 혈액응고시간검사신빙도조사참여기관수는 1차와 4차에각각 328 기관 348 기관에검체를발송하여 291 기관 (88.7%), 321기관 (92.2%) 의회신율을보여평균 306(90.5%) 의회신율을보였다 (Table 1). 2. 정도관리물질의균질성일반혈액검사정도관리물질의변이계수는 1차 ~4차까지각각백혈구수 1.68%, 1.57%, 2.45%, 3.33% 혈색소치 0.44%, 0.38%, 0.54%, 0.77% 적혈구용적백분율 0.50%, 0.45%, 0.52%, 0.92% 적혈구수 0.47%, 0.30%, 0.48%, 0.83% 혈소판수 1.87%, 2.55%, 1.70%, 2.73% 을보였으며, 1차와 4차에실시한혈액응고검사정도관리물질의변이계수는프로트롬빈시간 1.11%, 2.53%, 활성화부분트롬보플라스틴시간 1.07%, 0.82% 를보였다 (Table 2). 3. 기기및시약이용현황일반혈액검사자동분석기기는제조사별로 Sysmex Co. 269 기관 (37.2%), Beckman Coulter Inc. 117기관 (16.2%), Abbott Lab. 96 기관 (13.3), Bayer diagnostics 95기관 (13.1%), Nihon Kohden Co. 44 기관 (6.1%), ABX diagnostics 39 기관 (5.4%) 등의순으로사용하고있었고, 기종별로는 XE-2100 87 기관 (12.0%), ADVIA-120 50기관 (6.9%), K-4500 49기관 (6.8%), KX 21 39기관 (5.4%), STKS 36기관 (5.0%), ADVIA-2120 33기관 (4.6%), LH-750 27기관 (3.7%), XT-1800 26기관 (3.6%), Cell Dyn 1700 22기관 (3.0%), SE-9000 21기관 (2.9%), Cell Dyn 3500 20기관 (2.8%) MEK 6318K 20기관 (2.8%) 등의순으로사용하고있었다 (Table 3). 혈액응고시간자동분석기기는제조사별로 Sysmex Co. 112기관 (34.9%), Instrumentation Lab. 89기관 (27.7%), Diagnostica Stago 54기관 (16.8%), Behnk Elektronik 30기관 (9.3%) 등의순으로사용하고있었고, 기종별로는 CA 500 series 49기관 (15.3%), CA 1500 32 기관 (10.0%), ACL 9000 30 기관 (9.3%), ACL 100 28 기관 (8.7%), STA Compact 24 기관 (7.5%), 등의순으로사용하고있었다 (Table 4). 프로트롬빈시간검사시약은제조사별로 Dade Behring 128 기관 (39.9%), Instrumentation Lab. 89 기관 (27.7%), Diagnostica Stago 54 기관 (16.8%), Pacific Homeostasis 39 기관 (12.1%) 등의순으로사용하고있었으며, 시약종별로는 Thromborel-S 105기관 (32.7%), PT-fibrinogen HS 74기관 (23.1%), STA Neoplastin-C Plus 54기관 (16.8%), Thromboplastin-D 39 기관 (12.1%), Thromboplastin-C Plus 23기관 (7.2%) 등의순으로사용하고있었다 (Table 5). 활성화부분트롬보플라스틴시간검사시약은제조사별로 Dade Behring 128기관 (40.0%), Instrumentation Lab. 87기관 (27.2%), Diagnostica Stago 57기관 (17.8%), Pacific Homeostasis 41 기관 (12.8%) 등의순으로사용하고있었으며, 시약종별로는 Actin 103기관 (32.2%), PTT A 5 55기관 (17.2%), Kontact APTT reagent 41 기관 (12.8%), Synthasil 41기관 (12.8%), APTT lyophilized silica 39 기관 (12.2%) 등의
진단혈액학검사신빙도조사결과보고 (2007) 31 순으로사용하고있었다 (Table 5). 4. 백혈구수검사결과백혈구수의전체기관변이계수는 1차 6.76% 2차 9.64% 3 차 6.20% 4차 4.29% 를보였고, 기기그룹별변이계수는대부분비슷하였으나몇그룹에서변이계수의차이가있었다 (Table 6). 5. 혈색소치검사결과혈색소치의전체기관변이계수는 1차 1.94% 2차 2.58% 3 차 1.62% 4차 1.82% 를보였고, 기기그룹별변이계수는모든그룹에서안정된결과를보였다 (Table 7). 6. 적혈구용적백분율검사결과 neutrophil 98.6%, 20070353 Reticulocyte 98.1%, 20070354 Othrochromatic erythroblast 92.0%, 20070355 Auer body 95.4% 를보였으며 (Table 12), 20070451 Myeloblast 84.5%, 20070452 Lymphoblast 83.5%, 20070453 Granular lymphocyte 50.6%, Lymphocyte 13.8, 20070454 Fragmented cell (schistocyte) 83.6%, 20070455 Malaria infect red cell 97.3% 를보였다 (Table 12). 10. 혈액응고시간검사결과혈액응고시간검사는 1차, 4차에 2회실시하였으며, 기종별전체기관변이계수는프로트롬빈 INR 1차 18.73%, 2 차 7.42%, 활성화부분트롬보플라스틴 ratio 1차 19.71%, 2차 11.20% 를보여기종별로변이계수의많은차이를보였으며 (Table 14), 측정시약별변이계수도많은차이를보였다 (Table 15, Table 16). 적혈구용적백분율의전체기관변이계수는 1차 3.57% 2 차 6.83% 3차 3.43% 4차 3.77% 를보였고, 기기그룹별변이계수는몇그룹을제외한대부분의그룹에서안정된결과를보였다 (Table 8). 7. 적혈구수검사결과적혈구수의전체기관변이계수는 1차 2.35% 2차 2.30% 3 차 1.98% 4차 2.05% 를보였고, 기기그룹별변이계수는모든그룹에서안정된결과를보였다 (Table 9). 8. 혈소판수검사결과혈소판수의전체기관변이계수는 1차 7.08% 2차 9.80% 3 차 7.29% 4차 6.98% 를보였고, 기기그룹별변이계수는대부분의그룹에서차이가있었다 (Table 10). 9. 혈액세포형태판독결과혈액세포형태판독결과중가장많은비율과의미가있는세포별분포비율은 1차 20070151 Plasma cell 97.3%, 20070152 Myeloblast 96.0%, 20070153 Mature basophil 79.7%, Imnature basophil 11.4%, 20070154 Lymphocyte 43.1%, Granular lymphocyte 36.1% 20070155 Monocyte 98.3% 를보였으며 (Table 11), 2차 20070251 Neutrophilic band granulocyte 77.2%, Neutrophilic segmented granulocyte 19.5%, 20070252 Mature eosinophil 95.6%, 20070253 Monocyte 91.3%, 20070254 Mature basophil 97.7%, 20070255 Platelet 92.2% 를보였고 (Table 11), 3차 20070351 Promyelocyte 85.5%, 20070352 Hypersegmented 고찰 2007 년도에혈액학신빙도조사는총 4회실시하였으며, 그중일반혈액검사와혈액세포형태판독은 4회, 혈액응고시간검사는 2회실시하였다. 일반혈액검사는백혈구수, 혈색소치, 적혈구용적백분율, 적혈구수, 혈소판수를, 혈액응고시간검사는프로트롬빈시간, 활성화부분트롬보플라스틴시간을각각검사하였다. 혈액학신빙도조사참여기관수는년평균 2006 년 623기관, 2007 년 722 기관을보여 99기관이증가하였다. 일반혈액검사는평균 722 기관에검체를발송하여 687 기관 (95.2%) 의회신율을보였으며, 혈액세포형태판독은평균 406 기관에서결과를입력하여 56.2% 의회신율을보였고, 혈액응고시간검사는 338 기관에검체를발송하여 306 기관 (90.5%) 의회신율을보였다 (Table 1). 결과회신율은 2007 년 1차 95.7%, 2차 91.8%, 3차 96.1%, 4차 96.8% 를보여매차수마다 90% 이상을유지하므로서만족할만한결과회신율을유지하고있었다. 일반혈액검사용정도관리물질은 1차, 3차, 4차에는동일혈액형을가지고있는정상인으로부터채혈한혈액을영국의 Reardon DM[1] 이개발한방법을이용하여자가제조하였고, 2차에는 BIO-RAD 사의 Lyphochek TM Hematology-16 Control Normal 을구매하여이용하였다. 정도관리물질을자가제조할때높은변이계수의원인중하나인관리물질의분주에의한오차를배제하기위하여제조시 20 vial 마다하나씩추출하여균질성을조사하였다. 일반혈액검사정도관리물질의변이계수는 1차 ~4차까지각각백혈구수 1.68%, 1.57%, 2.45%, 3.33% 혈색소치 0.44%, 0.38%, 0.54%, 0.77% 적혈구용적백분율 0.50%, 0.45%, 0.52%, 0.92% 적혈구수 0.47%, 0.30%, 0.48%, 0.83% 혈소판수 1.87%, 2.55%, 1.70%,
32 권계철 김선희 김용구 박연보 박종우 박찬정외 2.73% 을보여비교적각 vial 간의균질성이잘유지됨을알수있었다 (Table 2). 일반혈액검사자동분석기기는제조사별로 Sysmex Co. 269기관 (37.2%), Beckman Coulter Inc. 117기관 (16.2%), Abbott Lab. 96기관 (13.3), Bayer diagnostics 95기관 (13.1%), Nihon Kohden Co. 44기관 (6.1%), ABX diagnostics 39기관 (5.4%) 등의순으로사용하고있어 2006년도에비하여사용순위가그대로유지되었으며 (Table 3), 기종별로는 XE-2100 87기관 (12.0%), ADVIA-120 50기관 (6.9%), K-4500 49기관 (6.8%), KX 21 39기관 (5.4%), STKS 36기관 (5.0%), ADVIA-2120 33기관 (4.6%), LH-750 27 기관 (3.7%), XT-1800 26기관 (3.6%), Cell Dyn 1700 22기관 (3.0%), SE-9000 21기관 (2.9%), Cell Dyn 3500 20기관 (2.8%) MEK 6318K 20기관 (2.8%) 등의순으로사용하고있어 (Table 3) 2006년의 XE-2100 67기관 (10.8%), ADVIA-120 57기관 (9.1%), K-4500 46기관 (7.4%), STKS 31 기관 (5.0%), SE-9000 30 기관 (4.8%), KX-21 29기관 (4.7%), Cell Dyn 1700 26기관 (4.2%), Cell Dyn 3500 21 기관 (3.4%) 등의사용순서와비교하여다소변동이있었다. 백혈구수의전체기관변이계수는 1차 6.76% 2차 9.64% 3 차 6.20% 4차 4.29% 를보여 2006년의 1~4 차각각 10.45%, 12.36%, 4.42%, 4.60% 와비슷하였고, 기기그룹별로변이계수의차이가있었으며 2006 년도결과와비교한차이도비슷하였다 (Table 6). 혈색소치의전체기관변이계수는 1차 1.94% 2차 2.58% 3 차 1.62% 4차 1.82% 를보여 2006년의 1~4 차각각 1.81%, 1.95%, 1.73%, 1.92% 과비슷하였고기기그룹별변이계수는 2006 년과같이모든그룹에서안정된결과를보였다 (Table 7). 적혈구용적백분율의전체기관변이계수는 1차 3.57% 2차 6.83% 3차 3.43% 4차 3.77% 를보여 2006년도의 1~4 차각각 4.89%, 6.01%, 3.40%, 3.74% 와비슷하게안정된결과였고, 기기그룹별변이계수는모든그룹에서안정된결과를보였다 (Table 8). 적혈구수의전체기관변이계수는 1차 2.35% 2차 2.30% 3차 1.98% 4차 2.05% 를보여 2006 년도의 1~4 차각각 2.34%, 2.05%, 1.96%, 2.19% 와비슷한결과였으며, 기기그룹별변이계수는모든그룹에서안정된결과를보였다 (Table 9). 혈소판수의전체기관변이계수는 1차 7.08% 2차 9.80% 3차 7.29% 4차 6.98% 를보여 2006 년도의 1~4 차각각 17.32%, 7.58%, 6.66%, 6.59% 과비슷한결과를보였고기기그룹별변이계수도 2005 년과같이몇그룹에서높은변이계수를보였다 (Table 10). 혈액응고시간검사용정도관리물질은 BIO-RAD 사의 Lyphochek R Coagulation control Levels 2를구매하여이용하였으며, vial 간의균질성을보기위하여 20 vial 마다 1 vial 씩총 15개 vial 을선택하여표준편차와변이계수를구하였다. 1차, 4차에실시한혈액응고검사정도관리물질의변이계수는프로트롬빈시간 1.11%, 2.53%, 활성화부분트롬보플라스틴시간 1.07%, 0.82% 를보여각 vial 간의균질성이잘유지됨을알수있었다 (Table 2). 혈액응고시간자동분석기기는제조사별로 Sysmex Co. 112기관 (34.9%), Instrumentation Lab. 89기관 (27.7%), Diagnostica Stago 54 기관 (16.8%), Behnk Elektronik 30 기관 (9.3%) 등의순으로이용하고있어 2006 년의 Sysmex Co. 78 기관 (31.6%), Instrumentation Lab. 62 기관 (25.1%), Diagnostica Stago 42 기관 (17.0%), Behnk Elektronik 22 기관 (8.9%), SEAC 11 기관 (4.5%) 과비교하여이용순위가비슷하였으며, 기종별로는 CA 500 series 49 기관 (15.3%), CA 1500 32기관 (10.0%), ACL 9000 30기관 (9.3%), ACL 100 28기관 (8.7%), STA Compact 24기관 (7.5%) 등의순으로이용하고있어 2006 년의 CA 1500 28기관 (11.3%), CA 500 series 28기관 (11.3%), ACL 9000 23기관 (9.3%), STA Compact 22기관 (8.9%), ACL 100 17기관 (6.9%) 과비교하여약간의이용순위에변동이있었다 (Table 4). 프로트롬빈시간검사시약은제조사별로 Dade Behring 128기관 (39.9%), Instrumentation Lab. 89기관 (27.7%), Diagnostica Stago 54기관 (16.8%), Pacific Homeostasis 39기관 (12.1%) 등의순으로이용하고있어 2006년의 Dade Behring 99 기관 (40.1%), Instrumentation Lab. 61 기관 (24.7%), Diagnostica Stago 43기관 (17.4%), Pacific Homeostasis 38기관 (15.4%) 의이용순위와비교하여변동이없었으며, 시약종별로는 Thromborel-S 105 기관 (32.7%), PT-fibrinogen HS 74 기관 (23.1%), STA Neoplastin-C Plus 54기관 (16.8%), Thromboplastin-D 39 기관 (12.1%), Thromboplastin-C Plus 23기관 (7.2%) 등의순으로이용하고있어 2006년도의 Thromborel-S 75기관 (30.4%), PT-fibrinogen HS 57기관 (23.1%), STA Neoplastin-C Plus 43기관 (17.4%), Thromboplastin-D 38기관 (15.4%), Thromboplastin-C Plus 24기관 (9.7%) 의이용순위와비교하여약간의변동이있었다 (Table 5). 활성화부분트롬보플라스틴시간검사시약은제조사별로 Dade Behring 128 기관 (40.0%), Instrumentation Lab. 87 기관 (27.2%), Diagnostica Stago 57기관 (17.8%), Pacific Homeostasis 41기관 (12.8%) 등의순으로사용하고있어 2006 년도의 Dade Behring 97 기관 (39.4%), Instrumentation Lab. 59 기관 (24.0%), Diagnostica Stago 43기관 (17.5%), Pacific Homostasis 35 기관 (14.2%) 의이용순위와같았으며, 시약종별로는 Actin 103기관 (32.2%), PTT A 5 55기관 (17.2%), Kontact APTT reagent 41 기관 (12.8%), Synthasil 41 기관 (12.8%), APTT lyophilized silica 39 기관 (12.2%) 등의순으로사용하고있어 2006 년도의 Actin 74기관 (30.1%),
진단혈액학검사신빙도조사결과보고 (2007) 33 PTT A 5 43기관 (17.5%), APTT lyophilized silica 36기관 (14.6%), Kontact APTT reagent 35기관 (14.2%), Synthasil 23 기관 (9.3%) 의이용순위와약간의변동이있었다 (Table 5). 혈액응고시간검사는 1차, 4차에 2회실시하였으며, 기종별전체기관변이계수는프로트롬빈 INR 1차 18.73%, 2차 7.42%, 활성화부분트롬보플라스틴 ratio 1차 19. 71%, 2차 11.20% 를보여기종별로변이계수의많은차이를보였으며 (Table 14), 측정시약별변이계수도많은차이를보였다 (Table 15, Table 16). 2006 년부터혈구감별계산을혈액세포형태판독으로대체시행하였으며홈페이지에 5개혈액세포그림을게시한후조회하여판독하게했는데대부분위원회에서의도한결과와 90% 이상일치하는판독을하였으나 1차 20070153 Mature basophil 79.7%, Imnature basophil 11.4%, 20070154 Lymphocyte 43.1%, Granular lymphocyte 36.1% (Table 11), 2차 20070251 Neutrophilic band granulocyte 77.2%, Neutrophilic segmented granulocyte 19.5% (Table 11), 3차 20070351 Promyelocyte 85.5% (Table 12), 4차 20070451 Myeloblast 84.5%, 20070452 Lymphoblast 83.5%, 20070453 Granular lymphocyte 50.6%, Lymphocyte 13.8, 4차 20070454 Fragmented cell (schistocyte) 83.6% (Table 12) 를보이는등미성숙세포나임상도말표본에서자주발견되지않는세포의판독은낮은일치율을보였다. 4월에충남대학교의과대학에서임상화학분과와공동으로실시한 workshop 은 100 명이넘는인원이참가하여성황을이루었다. 오전에 정도관리기본개념, 정도관리 ABC, 정도관리물질의선택, 정도관리물질소개 등정도관리기초이론에대하여실시하였으며오후에는 정도관리활용, Six sigma 을이용한정도관리, 외부정도관리결과활용, Roche My Lab View TM 소개 등정도관리의발전된이론과이용그리고 혈액응고의기본개념, 혈액응고검사의원리및방법, 혈액응고 INR 보고및인증시참고사례 등혈액응고검사의이론에대하여실시하였다. 2005 년도까지는우편으로검체및결과지발송을하였고이후 2006 년도부터 2007 년 3차까지대한임상검사정도관리협회홈페이지의결과입력화면을통하여참여기관에서직접결과를입력하게하여홈페이지에서다운받은참여기관의결과를검증하여오류결과를수정한후 진단혈액외부신빙도조사통계분석및출력프로그램 ( 윤종현, 박연보, 권계철, 김진규 ; 2006 년 ; Visual FoxPro 5; Microsoft, USA) 을이용하여분석한후결과지를출력하여참여기관으로발송하였으며, 2007 년 4차부터는결과지출력하는대신결과파일을내려받아대한임상검사정도관리협회홈페이지의결과올리기프로그램으로결과를올려저장한후참여기관에서직접조회및출력할수있게하였다. 홈페이지를통하여참여기관에서직접결과를입력하고조회하게함으로서위원회에서분석프로그램에결과를입력하는비용 과입력오류를방지할수있었으며, 결과지발송비용과우편발송시분실사고를없앨수있었으며결과지분실원인에따른분쟁을없애는등정도관리업무의향상을이룩할수있었다. 2007 년은충남대학교병원이진단혈액분과정도관리사업을주관한지일곱번째해로서세번째주관기관을시작하는해였다. 그동안진단혈액분과사업을주관하는데행정적인도움을주신충남대학교병원관계자여러분, 검체제조및발송업무에노고가많았던진단검사의학과직원들께감사드리며, 진단혈액정도관리사업의발전을위하여지원을아끼지않으신본회관계자여러분과진단혈액정도관리결과를홈페이지를통하여조회및출력할수있도록프로그램구현에노고가많았던이시소프트관계자께도감사드린다. 2008 년은인터넷을통하여진단혈액정도관리결과입력, 결과조회및출력업무를서비스하는원년으로서참여기관에보다나은서비스를제공하기위하여노력하겠다. 요약 2007 년도에 722 여기관을대상으로 4회실시한진단혈액학검사신빙도조사는다음과같다. 검사종목은일반혈액학적검사는백혈구수, 혈색소치, 적혈구용적백분율, 적혈구수, 혈소판수등 5종목을실시하였으며회신율은 95.2% 였으며, 혈액세포형태판독은 5개의그림을판독하게하였고회신율이 56.2% 였으며, 혈액응고시간검사는프로트롬빈시간, 활성화부분트롬보플라스틴시간등 2종목을실시하였고회신율은 90.5% 였다. 검사방법은전년도와비슷하였다. 혈색소, 적혈구용적백분율및적혈구수의변이계수는안정적이었으나혈소판수와백혈구수의변이계수는각회사의기기마다차이가있었다. 혈액세포형태판독은대부분 90% 이상일치율을보였으나미성숙세포와임상도말표본에서자주발견되지않는세포의판독은낮은일치율을보였다. 혈액응고시간검사의변이계수는기기마다차이가있었다. 워크샵은충남대학교의과대학에서혈액응고에관한주제로 1회실시하였고 100 여명이참가하여높은관심을보여주었다. 2007 년도제4차부터홈페이지에서결과입력, 결과조회및출력을하게하였다. 참고문헌 1.
