Due to the difficulty of making a decision about the pathogenicity of mycobacteria, species-level identification is very important for patients diagnosis and treatment. The purpose of this study was to identify mycobacteria species using a high performance liquid chromatography (HPLC) method and to provide an initial database for the distribution of mycobacteria in Korea. Methods : Acid fast bacteria isolated from 3,107 clinical specimens were identified by mycolic acid analysis using HPLC. The HPLC patterns were compared with those of standard mycobacteria species. Results : The HPLC patterns were divided into single, double, and triple cluster groups, each group comprising 9, 20, and 4 species, respectively. Mycobacteria and non-tuberculous mycobacteria (NTM) were identifies by HPLC at the rates of 99.5% and 95.6%, respectively. NTM was isolated in 12.4% of the mycobacteria positive specimens. This study also found that there were 20 different NTM species with the distribution of each species ranging from 0.3% to 15.9% of the total NTM. While the rate of NTM has been increasing in Korea, M. avium-intracellulare, M. fortuitum, and M. chelonae are relatively decreasing, and M. kansasii and M. gordonae are relatively increasing. Conclusions : HPLC method was highly discriminative for the identification of NTM in clinical specimens. 2 HPLC 법을이용한 Mycobacteria 의동정 25 여부를판단하기힘들며 [9, 10], 일반적으로결핵균에유효하다고알려진항결핵제들이 NTM에는효과가없는경우가많고, NTM 의종류에따라치료가다른경우가많으므로 [11-14], mycobacteria의균종감별은환자의적절한치료에대단히중요하다. 현재까지우리나라의대한결핵협회와 3차의료기관을포함한대부분의의료기관에서는 mycobacteria의균종감별을위해전통적인생화학법이나중합효소연쇄반응법, 유전자소식자법등을많이사용하고있으나 [4, 15-17], 이방법들은동정시간이오래걸리거나, 오염에의한위양성률이높거나, 다양한 NTM을모두검출할수없거나, 각균종별로검사를시행해야하는단점들이있다 [15-21]. 반면, high performance liquid chromatography (HPLC) 법은결핵균및다양한 NTM의균종특이적 mycolic acid의분해산물을분석하여동정하는방법으로상기의단점들이극복된정확한방법으로알려져있다 [22-26]. 또한우리나라에서의 NTM 감염증의빈도는정확히알려져있지않으며, 임상검체에서분리되는 mycobacteria의종별빈도에관한보고도많지않다. 이에본연구에서는 HPLC법을이용하여결핵균및다양한 NTM을동정하여임상검체에서의 mycobacteria의균종별분포를파악하여우리나라에서의결핵균및다양한 NTM의분포에대한기초자료로활용하고자하였다. 1. 대상 대상및방법 2002년 1월부터 2006년 12월까지 5년간울산대학교병원진단검사의학과에항산균배양이의뢰된검체중초진이거나최근 6개월간동일검체의항산균배양검사에서음성으로판정되었던환자에서초기 3회의항산균배양검사로제한하여, 12,821명의 25,044검체들을검체종류에관계없이모두항산균배양의 Clinical and Laboratory Standards Institute (CLSI) 권고안에따라고체배지인 Eiken 3% 오가와배지 (Eiken, Tokyo, Japan) 와액체배지인 Mycobacteria Growth Indicator Tube (Becton Dickinson, Baltimore, Maryland, USA) 에각각접종하여두배지중한개이상의배지에서항산균배양양성으로확인된 1,342명의 3,107검체를대상으로하였다. 2. Mycobacteria 의동정 항산균배양양성으로판정된경우, mycobacteria의균종특이적 mycolic acid를 HPLC법으로분석하여얻은패턴을표준균주 들의패턴과비교후동정하여균종분포를조사하였다 [22-26]. 표준균주들은 American Type Culture Collection (ATCC) 표준균주 28종과 Korean Type Culture Collection (KTCC) 표준균주 5종, 총 33종을사용하였다 (Table 1). HPLC 분석에사용된구성물로용제를분출시키는펌프와기화시키는분사기를포함하는본체는 waters 2690 seperation module (Waters, Milford, MA, USA) 을, 분리를위한컬럼은 reverse phase analytical cartridge column, mm, packed with 3 m silica (Nova-Pak C18) 컬럼을, 검출을위한자외선검출기는 photodiode array detector (Waters, Milford, MA, USA) 를이용하였다. 내부고분자량및저분자량표준물질로는각각 - bromophenyl ester와 6,7-dimethoxy-4-coumarinylmethyl ester를사용하였다. 분석과정은먼저 methanolic potassium hydroxide를가하여 mycobacteria 세포를분해하여비누화시키고, 클로로포름을용매로지방산을추출하였다. 추출한성분에 potassium bicarbonate 를가하여기화시키고, 지방산의파생물을정제한후컬럼을통과시켜자외선검출기로 HPLC 패턴을검출하였다. 검출된 HPLC 패턴은모두저분자량과고분자량표준물질의 peak들사이에나타난특징적인 cluster 패턴을이용하여분류하였다. 각 cluster 그룹에속하는균주는각각의 cluster내의 peak 수, peak 유지시간및 peak 높이비율을기준으로표준균주들의 HPLC 패턴과비교하여최종동정하고균종분포를조사하였다. 항산균의 HPLC 패턴은 single-cluster 그룹에는 M. asiaticum, Table 1. 3 26 정윤성 김성률 장철훈외 1 인 M. bovis, M. gastri, M. gordonae, M. kansasii, M. marinum, M. szulgai, M. triviale 및 M. tuberculosis complex 등의 9종, doublecluster 그룹에는 M. abscessus, M. acapulsensis, M. agri, M. avium, M. celatum, M. chelonae, M. diernhoferi, M. flavescens, M. fortuitum, M. gilvum, M. intracellulare, M. mucogenicum, M. nonchromogenicum, M. peregrinum, M. phlei, M. porcinum, M. scrofulaceum, M. smegmatis, M. terrae 및 M. xenopi 등의 20종, triple-cluster 그룹에는 M. austroafricanum, M. pulveris, M. simiae 및 M. vaccae 등의 4종이각각포함되었다. 네개이상의 peak cluster를가지는 multi-cluster 그룹은현재까지병원성유무가확인되지않은그룹에속하여제외하였다 [22]. HPLC 패턴이표준균주 33종의어느것과도일치하지않는경우 unclassified NTM으로분류하여균종분포에포함시켰다. 결 과 1. HPLC 법에의한 mycobacteria 의동정 Mycobacteria 표준균주 33종과항산균배양양성으로확인된 1,342명의 3,107검체의 HPLC 검사에서 mycolic acid peak들이모두저분자량과고분자량표준물질의 peak들사이에특징적인 single, double 및 triple-cluster로나타났다. 3,107검체중 3,090 검체 (99.5%) 의 HPLC 패턴은표준균주 33종중어느한패턴과일치하여이패턴의표준균주로동정하였다. 나머지 17검체 (0.5%) 의 HPLC 패턴은표준균주들과일치하는패턴이없어 unclassified NTM으로분류하였다. M. tuberculosis를제외한 384검체는 19종의 NTM과 unclassified NTM으로동정되었고, single-cluster 그룹에는 M. asi- A B C D E F G H I Fig. 1. (Fig. 2 continued next) aticum, M. gordonae, M. kansasii, M. marinum, M. szulgai 및 M. triviale 등 6 종 (Fig. 1), double-cluster 그룹에는 M. abscessus, M. acapulsensis, M. avium, M. chelonae, M. fortuitum, M. intracellulare, M. mucogenicum, M. nonchromogenicum, M. peregrinum, M. scrofulaceum, M. terrae 및 M. xenopi 등 12 종 (Fig. 2), triple-cluster 그룹에는 M. simiae 1 종이각각포함되었고 (Fig. 3), multi-cluster 그룹은검출되지않았다. 또한초진이거나최근 6 개월간동일검체의항산균배양검사에서음성으로판정되었던환자에서초기 3회의항산균배양검사로제한한본연구에서는모든환자에서동일한균주가검출되었다.

(S) M. terrae and (T) M. xenopi. 2. 임상검체에서 NTM 의균종분포 1,342 명의 3,107 검체중 M. tuberculosis 는 1,141 명 (85.0%) 2,723 검체 (87.6%) 였고, NTM 은 201 명 (15.0%) 384 검체 (12.4 %) 였다 (Table 2). NTM 으로동정된 384 검체는 M. kansasii 61 건 (15.9%), M. intracellulare 49건 (12.8%), M. gordonae 44건 (11.5%), M. avium 38건 (9.9%), M. fortuitum 34건 (8.9%), M. abscessus 25건 (6.5%), M. peregrinum 22건 (5.7%), M. szlugai 19건 (5.0%), M. marinum 14건 (3.7%), M. mucogenicum 14 건 (3.7%), M. xenopi 10건 (2.6%), M. terrae 8건 (2.1%), M. scrofulaceum 7건 (1.8%), M. chelonae 6건 (1.6%), M. triviale 5건 (1.3 %), M. simiae 4건 (1.0%), M. acapulsensis 3건 (0.8%), M. asiaticum 3건 (0.8%), M. nonchromogenicum 1건 (0.3%) 및 unclassified NTM 17건 (4.4%) 의분포를보였다 (Table 3). 검체별로는객담 244 (63.5%), 기관지흡인액 29 (7.6%), 소변 Table 2. M. avium, M. fortuitum, M. abscessus, M. peregrinum 및 M. szlugai 를 1 년단위로분석했을때각균종별검출률은차이가없었다 (Table 4). 고찰임상검체에서결핵균의동정은결핵균감염증을진단하기위한최선의방법이며, 최근후천성면역결핍증의증가와면역억제요법의일반화에따라의미있게증가하고있는 NTM의동정도매우중

NTM은현재까지 91종이상이알려져있으며 [27], 물, 토양, 공기및환자등에서검출된 NTM 중현재까지알려진균종에포함되지않는경우가 30% 에이른다고알려져있다 [28]. 임상검체에서 NTM이검출될경우기회감염균으로알려져있으나, 최근후천성면역결핍증의증가와면역억제요법의일반화에따라 NTM 감염증도의미있게증가하고있으며 [10, 11, 29, 30], 면역억제환자뿐만아니라정상면역능을가진사람들에서도 NTM 의검출빈도가증가하고있다 [5-8, 31-33]. 동시에 NTM 감염증은결핵과는달리개발도상국과선진국모두에서증가추세이다 [30]. 그러나현재까지 mycobacteria의균종분포를확인하기위한검출방법으로널리이용되고있는전통적인생화학법은민감도가낮거나검출에오랜시간이걸리고, 균종단계까지동정하기어렵다. 또한 mycobacteria의균종감별에빠르고정확하다고알려진중합효소연쇄반응법, 유전자소식자법등을많이사용하고있으나, 이방법들은오염에의한위양성률이높거나, 다양한 NTM을모두검출할수없거나, 각균종별로검사를시행해야하는단점들이있다 [15-21, 34]. 반면, HPLC를이용한 mycobacteria 동정법은균종특이적 mycolic acid를분석하여얻은 HPLC 패턴을표준균주들의 HPLC 패턴과대조하여동정하는방법으로상기의단점들이극복된정확한방법으로알려져있다 [22-26]. 본연구에서는 mycobacteria의균종분포를확인하기위해 33 종의 mycobacteria 표준균주를이용한 HPLC법으로동정하였는데, 3,107검체중 17검체를제외한 3,090검체의 HPLC 패턴이표준균주 33종중어느한패턴과일치하여 99.5%, M. tuberculosis를제외한 NTM의경우만으로제한하면 384검체중 367 검체 (95.6%) 에서동정이가능하여우수한변별력을확인할수있으며, 본연구에서는 unclassified NTM을포함하여총 20종으로 동정되었고, multi-cluster 그룹은검출되지않았다 년대 NTM 의국내빈도 [35, 36] 는배양양성검체중 1% 미만으로, 1970년김등 [37] 은객담에서 NTM이 2.7% 로보고하였는데, 본연구에서는 NTM이 384건 (201명) 으로배양양성검체의 12.4%, 환자의 15.0% 로나타나 70년대이후국내에서도 NTM의검출률이대단히증가한것을확인할수있었다. 또한임상검체에서분리되는 NTM의종별빈도와균종분포에관한보고도많지않은데, 1995년대한결핵및호흡기학회에서조사한 NTM 전국실태조사 [4] 에서는 1981년부터 1994년까지 14년간대한결핵협회결핵연구원에의뢰된검체중총 158 검체에서 159건의 NTM을전통적인생화학적방법으로동정하였다. 이조사에서 NTM의균주분포는 M. avium-intracellulare 104건 (65.2%), M. fortuitum 20건 (12.7%), M. chelonae 15건 (9.5%), M. gordonae 7건 (4.4%), M. terrae 5건 (3.2%), M. scrofulaceum 3건 (1.9%), M. kansasii 2건 (1.3%), M. szulgai 2건 (1.3%), M. avium-intracellulare & terrae 1건 (0.6%) 였다. 이에반해본연구에서는 2002년부터 2006년까지 5년간울산지역에서검출된 NTM이 384건으로 1981년부터 14년간전국적으로검출한 159건보다 2.5배정도로많았으며, 균종별빈도는 1995년대한결핵및호흡기학회에서조사한 NTM 전국실태조사 [4] 의검출된균종은총 9종임에반해본연구의임상검체에서검출된 NTM의종류는총 20종으로증가하였는데, 이는동정에 HPLC 법을이용하여보다다양한 NTM이동정되었고, NTM 감염증이증가추세에있는두가지이유때문으로판단된다. 그러나본연구에서 5% 이상검출된 M. kansasii, M. intracellulare, M. gordonae, M. avium, M. fortuitum, M. abscessus, M. peregrinum 및 M. szlugai를 1년단위로분석했을때각균종별검출률은차이가없었기때문에검출된균종의증가는 NTM 감염증의증가보다는 HPLC법을이용한동정으로보다다양한 NTM이검출된때문으로판단된다. 또한본연구의균종분포에서는 M. avium과 M. intracellulare 를포함하는 M. avium complex의경우검출빈도는 22.7% 로가장높지만 1995년대한결핵및호흡기학회에서조사한 NTM 전국실태조사 [4] 에서의 65.2% 에비해상대적으로감소하였고, M. fortuitum과 M. chelonae 등의경우에도본연구에서의빈도는각각 8.9%, 1.6% 로 1995년대한결핵및호흡기학회에서조사한 NTM 전국실태조사 [4] 의 12.7%, 9.5% 에비해상대적으로감소하였다. 반면 1995년 NTM 전국실태조사 [4] 와비교해 M. kansasii는 1.3% 에서 15.9% 로, M. gordonae는 4.4% 에서 11.5 % 로, M. abscessus는 0% 에서 6.5% 로, M. peregrinum은 0% 에서 5.7% 로, M. szlugai는 1.3% 에서 5.0% 로, M. marinum과 M.

8 HPLC 법을이용한 Mycobacteria 의동정 31 mucogenicum은 0% 에서 3.7% 로, M. xenopi는 0% 에서 2.6% 로증가하였는데, 이증감추세는미국의질병관리센터에서 1993년부터 1996년까지조사한 NTM 증감추세 [38] 와유사하며, 이는보다발전된정확한검출법으로다양한 NTM이동정되고, 전세계적으로 NTM 감염증이증가추세에있기때문인것으로판단된다. 또한 2006년고등 [39] 은 NTM 폐질환자의원인균은 M. avium complex (48%) 와 M. abscesssus (33%) 가가장많다고한결과와비교하면, 본연구에서는 M. kansasii 증가가특징적인데, 이는본연구에서호흡기및비호흡기의모든검체를대상으로검사하였고, NTM의지역적인분포특성등이원인으로판단된다 [40]. 보였고각각 9, 20, 4종이포함되었다. HPLC법으로 mycobacteria 동정성공률은 99.5%, M. tuberculosis를제외한 NTM의경우 95.6% 였다. 항산균배양양성검체중 NTM의비율은 12.4 % 였고, 총 20종의 NTM은 % 까지의분포를보였다. 결론 : HPLC법은결핵균및 NTM 동정에우수한변별력을보였고, 우리나라에서도 NTM의검출률이대단히증가한것을확인하였다. NTM의경우M. avium-intracellulare, M. fortuitum, M. chelonae 등은상대적감소를, M. kansasii, M. gordonae 등은상대적증가추세를보였다. 참고문헌 본연구에서의 NTM 의검체별빈도는객담, 기관지흡인액, 흉 수액을포함하는호흡기계검체에서 296검체 (77.1%) 가검출되어 NTM도결핵과같이호흡기계에서가장많이검출되었고, 비호흡기계에서는소변, 피부조직, 림프절, 농, 뇌척수액, 위장관흡인액, 골수, 관절액등의다양한검체에서검출되었다. 결론적으로 HPLC법에의하여임상검체에서배양된항산균의 99.5% 및 NTM 균주의 95.6% 를동정할수있었으며, 20종의 NTM이배양항상균의 12.4% 를차지하였다. NTM의종별빈도에서는 M. avium-intracellulare, M. fortuitum, M. chelonae 등은상대적감소를, M. kansasii, M. gordonae 등은상대적증가추세를보였다. 그러므로 NTM 감염증의적절한진단과치료를위해서는임상검체에서 NTM을검출하기위해변별력이뛰어난검출법을이용하여야할것으로생각되며, NTM이검출될경우본연구의균종분포를진단과치료에기초자료로활용할수있을것으로판단된다. 1.
