However, unlike autosomal genes, the Y chromosome does not undergo recombination, and therefore individuals from different geographical regions may have differing distribution patterns with respect to Y-chromosome mutations. More detailed knowledge and information regarding Y-chromosome mutations might therefore provide insights into phylogenetic history and personal identification. Here, we describe a case study involving genotype-phenotype discrepancy in an Indian male individual. We found that the mistyping in sex determination was caused by a deletion in the amelogenin Y (AMEL Y) gene. Furthermore, on examining the short tandem repeat (Y-STR) loci using the PowerPlex Y23 System, we found four more deleted loci on Yp11.2 (DYS576, DYS481, DYS570, and DYS458) in this sample. We performed deletion mapping for this sample, and we propose that the microdeletion on the Yp11.2 locus occurred approximately in the 6.44 Mb to 9.75 Mb region. Previous studies have reported that the AMEL Y deletion is a common mutation in the Indian population. Taking into account regional differences, we also analyzed several area-specific Y- chromosome mutations. Key Words : AMELY, Y-STR, Deletion, Yp11.2 서론친자확인이나혈연인척관계확인을위한유전자검사를할경우, 또는개인식별의경우성별은매우중요한요소이다. 표현형은남성인데, 유전자형이여성으로나올경우부정확한결과를도출하여혼동을줄수있다. 인도인의시료를분양받아민족간유전자정보를수집하던중, 표현형은남성인사람의시료의상염색체짧은연쇄반복 (short tandem repeat, STR) 결과가 XX임을발견하였다. 동일인의 Y-STR 결과를살펴보니 23개의유전자위중 amelogenin Y ( 이하 AMELY) 와근접한지역의 4개의유전자위에서유전자형이검출되지않은것을발견하였고, 이사람을 AMELY null 남성으로간주하였다. 추가적인연구를통해진짜 AMELY null 남성인지, 그렇다면어느부위까지결실이일어났는지를파악하였다. 결실이일어난것으로추정되는 Yp11.2부위는 Y 염색체의성결정영역 (sex determinating region, SRY) 유전자와는관련 116 ccopyright 2014 by the Korean Society for Legal Medicine This is an Open Access article distributed under the terms of the Creative Commons Attribution Non-Commercial License ( by-nc/3.0) which permits unrestricted non-commercial use, distribution, and reproduction in any medium, provided the original work is properly cited. pissn eissn

Y 염색체의단완 11.2 (Yp 11.2) 부위의유전자성별감식시동체 ( 프라이머, primer) 결합부위에전좌또는돌연변이, 결실로인해 amelogenin Y gene (AMELY) 이증폭되지않아실제법의학적용과산전진단에서성별입력에대한오해의소지가있을수있다는보고가있었다. 1) AMELY 을포함한 Yp11.2 부위의결실은인도인과특히동남아시아에 2, 3) 서많이발견되었다고한다. 이를관련논문들에대한고찰과함께, 이러한사례를증례의형태로보고하여 amelogenin 유전자를이용하여성별검사를하였을때돌연변이에의해초래할혼동을줄이고자한다. 증 아시아지역여러민족들의유전적특징을비교하는목적으로연구를수행하던과정중인도인의시료를분양받아유전자를분석하는과정에서표현형은남성인사람의시료의상염색체 STR 결과가 XX임을발견하였다. 진짜 AMELY null 남성인지재확인이필요하였고, 이를위해추가적인검사를실시하였다. 시료들은 DNA 원액의형태로검사를진행하는연구자에게전달되었고, 연구자들은시료가인도인남성에게서유래하였다는정보를제외하고는모든인적사항에대해서는모르는상태에서검사가진행되었다. 본보고에대한사항은서울대학교의과대학및서울대병원임상시험심사위원회 (institutional review board, IRB) 심의면제과정을거쳤다. 상염색체 STR검사는 PowerPlex 16 kit (Promega Corporation, Madison, WI) 를사용하였다. 이키트는법의유전 례 학분야에서흔히사용되는상업용키트로상염색체상의 16개 STR 유전자위를검사할수있다. 제조사가권하는방법을사용하여 PCR을진행하였고, 확실한결과도출을위하여반복검사를진행하였다. 반복검사모두에서 amelogenin에서여성의소견을보이는것을제외하고는일반적인결과를얻을수있었다 (Fig. 1). Y-STR검사는 PowerPlex Y-23 kit (Promega) 를역시제조사가권하는방법을따라사용하였다. 이키트는 Y 염색체상의 23개의 STR 유전자위들을검사할수있는데, 본대상인에대해서는반복적인실험을진행하였음에도불구하고 Y 염색체단완부위중에서 AMELY가위치한 Yp11.2에존재하는 DYS576, DYS481, DYS570, DYS458의 4개의유전자위에서는결과를얻을수없었다 (Fig. 2). 이남성의 Y염색체단완부위결실의여부와그위치를판별 Fig. 1. STS마커는 sy877, sy1012, sy716, sy1242, sy881, sy939, sy1101, sy57. sy1293, sy59, sy1243, sy1244, sy714 마커를사용하였다. AMELY에결실이없는다른인도인과 9948A (Promega) 와함께전기영동을진행하여밴드가관찰되지않으면결실로간주하였다. 본대상자는 SY1242부터 SY1244까지의 STS 마커 PCR 결과가나오지않았다. 따라서 5.42 Mb부터 Mb 사이에서결실이시작되어 9.46 Mb에서 9.75 Mb 사이에서결실이끝나는것으로판단하였다. Deletion around Yp11.2 occurred approximately in the range of 6.44Mb to 9.75 Mb. 고찰본연구결과를 YHRD ( Amelogenin+Y+deletions) 에공개되어있는다른 AMELY deletion 결과 4-8) 들과비교한내용을 Table 1에요약하였다. 인도인중하플로그룹 (haplogroup) J2e1의결과와 STR과 STSs 결실결과가유사한것을알수있었지만본사례에서는제한된양의 DNA 시료를제공받아실험을수행한상황으로더세밀한결실부위를알수있는세부적인 STS 마커에대한실험과하플로그룹을알수있는추가실험을진행하지는못하였기에타인도인과의직접적인비교는불가능하였다. Y- STR 타이핑결과를기초로하플로그룹을예측해주는프로그램인 Whit Athey s Haplogroup Predictor 27-haplogroup program ( hapest5.htm) 을이용하여, 지역선택에서 South Asia와 Equal Priors 선택에관계없이 100% 의동일한결과인 J2b를얻었다. 이는위에서언급된하플로그룹과다소차이가있는데, 프로그램에서활용하고있는하플로그룹이다양하지않고, 본사례에서결실때문에활용할수있는유전자의수가적어위와같은차이가발생한것으로추측한다. 한편 AMELY결실이일어난사람들이주로인도와인도아대륙에서빈번하다는보고들로볼때민족의차이또는지역의차이에초점을맞추어다양한 Y염색체단완부위의결실위치를분석하고, 분류한다면 Y 염색체의결실부위도민족을구분하는데또하나의파라미터가될수있을것으로예상한다. 최근 AMELY 결실에대한성별오류를보완하는목적으로 DYS391 마커를추가적으로 Combined DNA Index System (CODIS) 와 European Standard Set (ESS) 에포함하여야한 다는의견이있고 9) 이에따라 Promega에서는 DYS391 마커가포함된 PowerPlex Fusion System 키트를선보였고, Life Technologies사에서는 GlobalFiler Kit를선보였다. 하지만 DYS391 마커의큰크기 ( bp) 로인해손상된 DNA 시료에서는분석에오류가있을수있다는보고 10) 가있어손상된시료분석시에는주의가요구된다. 우리나라에서도외국인과의교류가빈번해지고있으며, 이에따라우리나라에서발생하는사건에서혹은거주자에서도이러한유형의사례들이관찰될가능성이있고, 위에서설명한것과같은지역성을반영한결실에대한이해는유전자검사결과해석에적지않게도움될것으로예상한다. 이논문은 2013년도정부 ( 미래창조과학부 ) 의재원으로한국연구재단바이오. 의료기술개발사업의지원을받아수행된연구입니다 (No ). 참고문헌 1.
