1 대한고관절학회지제 20 권제 1 호 Vol. 20, No. 1, March, 2008 부산대학교의과대학정형외과학교실 목적 : 본연구의목적은환형미세공근위부피복및원위원추형대퇴스템과미세공피복비구컵을이용한고관절전치환술후 5년이상의추시결과의분석이다. 대상및방법 : 1998년 9월부터 2001년 12월까지 Versys Fiber Metal Taper 대퇴스템 (Zimmer, Warsaw, IN) 및무시멘트 Trilogy 비구컵 (Zimmer) 을사용하여고관절전치환술을시행받고, 최소 5년이상추시관찰이가능하였던 52명 57예를대상으로하였다. 평균연령 46.7세, 평균추시기간 5.6년이었고임상적방사선학적평가를시행하였다. 결과 : 평균 Harris 고관절점수는술전 56.6점에서최종추시시 96.0점이었다. 대퇴스템의위치의변화나수직하강은발생하지않았다. 최종추시시대퇴스템의골내막골형성은주로 Gruen 제 2, 6구역에서 53예 (92.9%) 가나타났으며, 대퇴부골용해는없었다. 비구컵의수직, 수평이동이나경사각의의미있는변화및비구컵주위의골용해는관찰되지않았다. 무균성해리로인한재치환술의시행여부를고려한대퇴스템및비구부의생존율은 100% 이었다. 결론 : Fiber Metal Taper 대퇴스템과 Triology 비구컵을이용한고관절전치환술은최소 5년추시결과안정된골성고정을보였으며대퇴부와비구컵주위골용해가없어우수한임상적, 방사선학적결과를보였다. 향후더장기적인추시관찰이요할것으로사료된다. 색인단어 : 무시멘트형고관절전치환술, Fiber Metal Taper 대퇴스템, Trilogy 비구컵 서 시멘트를이용한인공고관절전치환술에서단기간추시관찰시수술직후안정된고정으로조기의체중부하가가능한점등의장점에비하여장기추시결과무균성해리와시멘트주위의과도한골용해등의여러합병증이발생하여 4) 생물학적고정이가능한무시멘트형고관절전치환술이개발되었고많은발전이이루어졌다. 무시멘트형고관절전치환술에서는비구컵과대퇴스템의견고한압박고정을통해조기에안정성을얻고이들의표면에이차적인골내막골형성을통한생물학적고정으로지속적인 통신저자 : 서근택부산광역시서구아미동 1-10 부산대학교병원정형외과 TEL: FAX: kuentak@pusan.ac.kr 본논문의요지는 2007 년도대한정형외과학회추계학술대회에서발표되었음. 론 안정성을얻는것이중요하다 17,20). 초기에는대퇴스템의근위부가비환형으로피복된대퇴스템이주로사용되었으나이러한경우폴리에틸렌마모입자가대퇴원위부로이동하는것을막지못하여골용해가빈번히발생하였고, 원위부가원주형인대퇴스템을사용했을경우대퇴골의근위부의응력방패현상으로근위대퇴골의소실이초래되었다 2,48). 비구컵에서는폴리에틸렌과금속비구컵사이의미세운동에의한 back side 마모및폴리에틸렌과금속비구컵사이의분리가문제가되었다 52). 그래서최근에는무시멘트스템의경우폴리에틸렌마모입자가대퇴골수강원위부로이동하는것을막기위해서환형미세포말형이며, 응력방패현상을최소화하여근위대퇴골의소실을방지하기위하여원위원추형인대퇴스템이사용되고있다 32,33). 무시멘트비구컵의경우마모입자의이동에의한골용해를방지하기위하여나사못고정을위한구멍이적은비구컵을저확공 (underreaming) 하여압박고정하는방법이선호되며, 폴리에틸렌과금속비구컵사이의미세운동과분리를방지하기위하여잠금장치가개선된비구컵이사용되고있다. 59

2 본연구는근위부가환형으로미세피복되고원위원추형스템인 Versys Fiber Metal Taper 대퇴스템 (Zimmer, Warsaw, IN) 과잠금장치가개선되고나사못고정용구멍이적으며미세공피복된 Trilogy 비구컵 (Zimmer) 을이용한고관절전치환술을시행하고최소 5 년이상추시관찰이가능했던환자들에대하여임상적및방사선학적결과를분석하고자한다. 대상및방법 1998 년 9 월부터 2001 년 12 월까지본원정형외과에서근위부 1/3 이환형미세공으로피복된 Versys Fiber Metal Taper 대퇴스템및무시멘트 Trilogy 비구컵을사용하여고관절전치환술을시술받은환자 90 명, 99 예중최소 5 년이상추시가가능하였던 52 명, 57 예 (57%) 을대상으로하였다. 전례에서저자가직접면담과신체검사를통해환자로부터 Harris 고관절점수를측정할수있었고, SF-12(Short Form-12), WOMAC (Western Ontario and McMaster Universities Osteoarthritis Index) 등의환자기입식설문조사를하였다. 추시기간은최소 5 년, 최고 8 년으로평균 5.6 년이었다. 성별은남자가 31 명, 여자는 21 명으로남자가많았고, 고관절전치환술시행당시의연령분포는 22 세에서 66 세까지로평균 46.7 세였다 (Table 1). 원인질환은대퇴골두무혈성괴사가 40 예 (70.1%) 로가장많았고퇴행성관절염 12 예 (21.0%), 대퇴골경부골절 2 예 (3.5%), 강직성척추염 1 예 (1.8%), 결핵성관절염의후유증 1 예 (1.8%), 화농성관절염후유증 1 예 (1.8%) 였다 (Table 2). Versys Fiber Metal Taper 대퇴스템은 straight 형으로, 근위부 1/3 은 titanium fiber metal 로미세공피복이되어있고중간부 1/3 은강옥 (corundumized) 으로표면을거칠게하였고, 원위부 1/3 은매끄럽고원추형으로되어있다. 근위부의사다리꼴 (trapezoidal shape) 모양은근위간단부및간부의 fit and fill 을얻고, 강도 (stiffness) 감소및회전안전성 (rotational stability) 을향상시킨다. 사용된대퇴스템은스템의직경을기준으로 10 mm 가 1 예, 11 mm 가 6 예, 12 mm 가 9 예, 13 mm 가 20 예, 14 mm 가 13 예, 15 mm 가 7 예, 16 mm 가 1 예였다. 대퇴골두는초 Table 1. Data on the patient who received primary total hip arthroplasty Number of cases 57 Age (yr) 46.7 Weight (kg) 62.4 Gender (men/women) (number of cases) 31/21 Duration of follow-up (yr) 05.6 기 25 예에서 zirconia ceramic head 를, 이후 32 예에서 cobalt-chromium 합금을사용하였고, 크기는 22 mm 가 5 예, 28 mm 가 52 예였다. 비구컵은 titanium 합금의반구형컵인 Trilogy 를사용하였는데, titanium fiber-mesh 의외벽과매끈한내벽으로이루어져있다. Trilogy 비구컵은 multi-hole 형이 5 예, cluster-hole 형이 52 예가사용되었고, 컵의직경을기준으로 42 mm 가 1 예, 46 mm 가 1 예, 48 mm 가 3 예, 50 mm 가 11 예, 52 mm 가 16 예, 54 mm 가 10 예, 56 mm 가 7 예, 58 mm 가 6 예, 62 mm 가 1 예, 66 mm 가 1 예였다. 초기 24 예에서초고분자량폴리에틸렌 (ultra-high molecular weight polyethylene) 삽입물과 zirconia ceramic head 를사용하였고, 이후 33 예에서 highly cross-linked 폴리에틸렌삽입물과 cobalt-chromium head 를사용하였으며, 1 예에서 highly cross-linked 폴리에틸렌삽입물과 zirconia ceramic head 를사용하였다. 수술전방사선사진으로수술예정인인공삽입물의크기결정및다리길이의변화에대한충분한검토후후외측도달법과후방연부조직봉합술을이용하여모두동일한술자 (S.K.T.) 에의하여시행되었다. 비구컵의삽입은 1~2 mm 적게확공하여압박고정을한뒤나사못을삽입하였고, 필요한경우비구컵주위에자가골이식을시행하여조기안정성을얻었다. 대퇴스템은압박고정을위하여가능한큰크기의 rasp 을대퇴골에삽입한후회전시켜봄으로서안정성에대한검사를시행하였다. 대퇴스템삽입후골결손등의소견이보이면자가골이식을시행하였다. 수술후봉합시에는관절낭과고관절외회전근및대둔근을견고히재부착하여탈구율을줄일수있도록하였다. 수술직후심부정맥혈전증을방지하기위하여색전방지스타킹을양하지에착용시키고고관절을외전시킨채로침상안정가료시켰다. 수술다음날부터목발이나보행기를이용하여체중부하없이기동을허용하였으며, 6 주부터부분체중부하를시작하여수술후 12 주째에완전체중부하를할때까지점차적으로부하를증가시켰다. 고관절의임상적평가를위하여술전및최종추시시의 Table 2. 3 Harris 고관절점수 25), SF-12, WOMAC 과환자의임상증상및탈구감염등의합병증유무를조사하여비교분석하였다. Harris 고관절점수는 90 점이상을우수, 80 점에서 89 점을양호, 70 점에서 79 점을보통, 70 점미만을불량으로생각하였다 24). 환자의포괄적삶의질평가를위해자기기입식설문조사를시행하였으며이는 SF-12 와 WOMAC 을이용하였다. SF-12 는 SF-36 을간단하게축약한것으로 9 개의하부영역으로나뉘고총 12 개문항으로구성되어신체적건강지수 (physical component score) 와정신적건강지수 (mental component score) 에대한내용을측정할수있다. SF-12 는 version 1 의한글판을사용하였다. SF-12 설문지를이용하여각각의요인을이용한계산방법은 SF- 12 의저작권자인미국 Qulitymetric 사의계산방법을이용하였다. WOMAC 은 48 시간동안의통증과관절강직, 그리고신체적기능을 VAS scale 을이용하여 0 부터 100 까지의숫자로평가하는제도로점수가높을수록증상이심각한것이다. SF-12 는 score 가 0~19 사이는불량, 20~39 사이는보통, 40~59 사이는양호, 60~79 사이는우수, 80~100 사이는매우우수로표시하였다. 수술전전후면및측면방사선사진으로 Dorr 등 12) 의기준에따라대퇴거 - 골수강비율과대퇴피질골두께를각각측정하고대퇴골을골밀도에따른형태로분류하였으며전후면방사선사진상내측과외측의피질골의두께가두껍고측면방사선사진상에서도후방피질골의두께가두꺼운경우를 A 형, 전후면방사선사진상내외측피질골의두께가두껍고측면방사선사진상후방피질골의두께가얇을경우를 B 형, 전후면및측면방사선사진모두에서피질골의두께가얇아져있을경우를 C 형으로구분하였다. 방사선학적평가를위하여수술후매년표준화된전후면및측면고관절단순방사선사진을촬영하여수술직후방사선사진과비교하여비구컵에대하여는이동, 경사각의변화, 비구컵주위의골용해, 방사선음영감소선등을, 대퇴스템에대하여는골내막신생골형성, 대퇴피질골의경화, 대퇴스템원위부의 pedestal 의형성, 대퇴스템의수직하강, 대퇴스템주위의방사선음영감소선, 대퇴스템주위의골용해등을관찰하였다. 수술후방사선사진상대퇴스템의위치에따라중립, 내반, 외반위치로분류하였으며, Brooker 등 3) 의분류에따른이소성골형성도관찰하였다. 비구컵의구역은 DeLee 와 Charnley 10) 의방법을이용하여세구역으로나누었으며비구컵의수직이동은컵의회전중심에서양측의 teardrop 의끝을연결하는선까지의수직거리의변화로, 수평이동은컵의회전중심에서 teardrop 의중심을통과하는수직선까지의수평거리의변화로측정하였다. 수직이동과수평이동은수술직후방사선사진과비교하여 2 mm 이상, 경사각은 5 이상의 차이가있을때의미있는것으로판정하였다. 그리고비구컵의수직및수평이동이나경사각의변화가있을때나비구컵의주위구역에연속적인방사선투과선이 2 mm 이상이면서진행되는경우를해리로하였다 37). 폴리에틸렌의마모는 Devane 등 11) 의방법을사용하여수술후방사선사진과 1 년마다추시촬영한방사선사진을비교하여컴퓨터를이용하여측정하였다. Devane 등 11) 의방법은일반방사선전후면과측면사진을 scanning 한후, 비구컵의외측경계, 전방경계, 후방경계선, 대퇴골두의경계선을 digitalized tablet 을이용하여디지털화한다. 이런접점들은개발된소프트웨어를통해비구컵과대퇴골두의 3 차원적영상을만들어주며, 비구컵의크기와중심점, 전염각, 골두의크기와위치, 중심점등이계산된다. 수술직후의사진과가장최근에추시되어촬영한방사선사진을이용하여 3 차원적으로비구컵내로대퇴골두의전위된수치가최종계산되어폴리에틸렌마모를예측한다. 대퇴스템의구역은 Gruen 구역 19) 에따라전후, 측면각각 7 구역으로구별하였으며대퇴스템의수직하강정도는미세공피복부의내측근위부의절흔으로부터소전자부의최근위부까지의거리를측정하여 5 mm 이상변화가있을때의미있는것으로정의하였고 5) 방사선투과성선이삽입물표면에각각의영역길이의 50% 이상나타날때의미가있는것으로판정하였다 51). 대퇴스템원위부의 pedestal 형성은최종추시관찰방사선소견상대퇴스템말단부의골수강내를일부또는완전히채우는골내막신생골형성으로정의하였다 17). 골용해는국한된확장형피질골의낭포성미란으로직경이 5 mm 이상인경우로하였다 50). 대퇴스템의진행하는수직하강이나이동이발생하거나대퇴스템주위의방사선투과성선이 2 mm 보다큰두께로나타났을때를대퇴스템의방사선학적해리로판정하였다 17). 수술중합병증으로근위대퇴골골절발생유무를확인하였고, 수술후추시관찰시발생한탈구, 심부감염, 대퇴스템주위골절, 이소성골형성발생등을수술후합병증으로고려하였다. 수술전후의 SF-12, WOMAC 결과에대한분석은 paired t-test 를이용하였고, highly cross-linked 폴리에틸렌과 zirconia ceramic head 를사용한초기군과초고분자량폴리에틸렌삽입물과 cobalt-chromium head 를사용한후기군의마모율, 마모량결과에대한분석은 Mann-Whitney U test 를이용하였다. 1. 임상적결과 결 Harris 고관절점수는수술전평균 56.6 점에서최종추시시의평균 96.0(80-100) 점으로향상되었는데 과 61

4 (p<0.001), 우수가 49 예 (86.0%), 양호가 8 예 (14.0%), 보통과불량인예는없었다 (Table 3). 대퇴부동통을호소한환자가 1 예 (1.8%) 있었으나추시관찰상호전되었다. SF-12 평가법에따른결과로는신체건강요인은술전평균 36.5 에서술후평균 48.6 으로증가하였고 (p<0.001), 정신건강요인은술전평균 36.8 에서술후평균 49.7 로증가하였다 (p<0.001). WOMAC 은술전평균 45.1 에서술후평균 22.7 로감소되었다 (p<0.001, Table 4). 신체건강요인은술전보통 50 예 (87.7%), 양호 7 예 (12.3%) 에서술후보통 8 예 (14.0%), 양호 46 예 (80.7%), 우수 3 예 (5.3%) 로나왔으며, 정신건강요인은술전보통 50 예 (87.7%), 양호 7 예 (12.3%) 에서술후보통 7 예 (12.3%), 양호 43 예 (75.4%), 우수 6 예 (10.5%), 매우우수 1 예 (1.8%) 로나왔다 (Table 5). 2. 방사선학적결과 Dorr 등 12) 의분류에따라 57 예의대퇴골의수술전골밀 Table 3. Results of Harris hip score at the last follow up No. % Excellent Good Fair Poor Total 도를측정한결과 A 형이 40 예 (70.2%), B 형이 15 예 (26.3%), C 형이 2 예 (3.5%) 였다. 수술후방사선사진상대퇴스템의위치는 41 예 (71.9%) 가중립이었으며 11 예 (19.2%) 는내반위치였고 5 예 (8.7%) 는외반위치였다. 추시방사선사진상비구컵의수직, 수평이동이나경사각의의미있는변화및비구컵주위의골용해는관찰되지않았고, 12 예 (21.0%) 에서방사선투과선이관찰되었으나 2 mm 미만의비진행성이었다. DeLee 와 Charnley 8) 의 zone 1 에 8 예, zone 2 에 11 예, zone 3 에 8 예가관찰되었다. 추시방사선사진상대퇴스템의위치의변화나수직하강은발생하지않았다. 대퇴스템의골내막신생골형성은최종추시상 53 예 (92.9%) 에서나타났다 (Fig. 1). 전후면및측면방사선사진상 Gruen 제 2, 3, 5, 6 구역에서많이관찰되었으며대퇴스템근위부 1/3 의미세공피복된부위와중간 1/3 의표면이거칠게처리된부분사이에서주로관찰되었다 (Fig. 2). 대퇴피질골비후는 6 예 (10.5%) 에서관찰되었으며 Gruen zone 3 에 3 예, zone 2 에 2 예, zone 1, 5, 6 에서각각 1 예씩이관찰되었다. 방사선투과성선은 23 예 (40%) 에서주로 Gruen 제 1, 4, 5 구역에서발생하였으며전례에서 2 mm 미만으로비진행성이었다. 대퇴스템원위부의 pedestal 의형성은 30 예 (52.6%) 에서관찰되었으나안정성이었으며해리소견이나임상적으로특이소견은없었다 (Fig. 3). 대퇴스템주위의골용해소견은없었다. 대퇴거흡수는 36 예 (63.1%) 에서관찰되었으나전례에서심한대퇴골골흡수나응력차단현상을보인예는없었다. 대퇴스템의고정상태는 54 예 (94.7%) 에서골성고정이되었으며, 섬유성안정고정은 3 예 (5.2%) 에서관찰 Table 4. 5 되었으며불안정성고정이나해리소견은없었다. 초기 44% 의환자에서는 zirconia ceramic head 와초고분자량폴리에틸렌삽입물을함께사용하였고, 후기 56% 의환자에서는 cobalt-chromium head와 cross-linked 폴리에틸렌을함께사용하였다. 전체폴리에틸렌의평균선형마모는 0.53 mm, 평균마모량은 mm 3 이었 B A Fig. 1. 6 고, 연평균선형마모율은 0.09 mm/year, 연평균마모량은 33.4 mm 3 /year 이었다. 초기 24 예의 zirconia ceramic head 를사용한군에서초고분자량폴리에틸렌의연평균선형마모율은 0.11 mm/yr, 연평균마모량은 47 mm 3 /year 로관찰되었고, 후기 33 예의 cobalt-chromium head 를사용한군에서 highly cross-linked 폴리에틸렌의연평균선형마모율은 0.08 mm/year, 연평균마모량은 23 mm 3 /year 으로측정되었다 (Table 6). 연평균선형마모율은두군간에유의한차이가없었으며 (p=0.11), 연평균마모량은두군간에유의한차이가있었다 (p<0.001) 3. 합병증 합병증없이골내성장을얻을수있었다 (Fig. 4). 수술후발생한골절 1 예는침강소견을보이며불완전골절로침상안정을통한골성고정을얻었다. 수술중대퇴스템주위골절이있었던환자중 1 예에서추시관찰시심부감염이발생하여변연제거술및대퇴골두와폴리에틸렌의교환을시행하였다. 탈구는 1 예에서전방탈구가발생하여도수정복하여치료하였으며, 수술후 2 년뒤에뇌경색이발생하여서재발성전방아탈구가발생하여 10 거상된폴리에틸렌의전상방전위및 long neck 을사용하여재치환을시행하였다. 최종추시시이소성골형성은 Brooker 3) 제 1 급이 7 예, 2 급이 8 예, 3 급이 1 예였다. 수술중근위대퇴골의골절이 3 예발생하여골절부주위환형강선고정술로고정하였으며추시기간중별다른 B A Fig. 3. Results of Kaplan-Meier analysis of probability of acetabular cup survival over time 4. 생존율 심부감염 1 예, 재발성전방아탈구 1 예로인한폴리에틸렌및대퇴골두교환술이시행되어 Kaplan-Meier 에의한평균추시기간 5.6 년생존율은 96.5% 이었다. 무균성해리로인한재치환술시행여부를고려한대퇴스템과비구부의 Kaplan-Meier 에의한평균추시기간 5.6 년생존율은 100% 이었다 (Fig. 5). 인공고관절전치환술후대부분의환자에서동통이완화되고술전에비해만족도가높은삶을유지하고있어많이시행되고있으나치료의결과는의료인들이작성한기준에근거하여판정되고있어환자의주관적인평가에대한연구가부족한실정이다. 많은분석법들이있겠으나본연구에서는최근에많이이용되고있는 SF-12 와 WOMAC 을이용하여분석하였다. 본연구의결과 SF-12 의신체건강요인과정신건강요인이모두유의하게증가되었고, WOMAC 수치또한유의하게증가되었다. 근위부환형미세포말처리된 2 세대대퇴스템을이용한연구에서대퇴부동통의발생은 1.4~10% 까지다양하게보고되나원위원추형스템에서대체로적은빈도를보이며 2,40,48), hydroxyapatite (HA) 를도포한대퇴스템을사용한연구에서대퇴부동통은 4.0~5.0% 정도의비교적적은빈도로보고되었다. 대퇴부동통의확실한원인은아직밝혀지지않았으며대퇴스템의미세운동, 대퇴원위부로의국소적체중전달, 대퇴삽입물과골사이의강도차이등이원인이될수있다 1,4,7). VerSys Fiber Metal Taper 대퇴스템및무시멘트 Trilogy 비구컵을이용한무시멘트고관절전치환술후대퇴부동통은 1 예에서관찰되었으나경과관찰중호전되었다. 본연구의경우근위부환형미세공피복및원위부원추형대퇴스템의사용으로대퇴 65

8 원위부로국소적체중전달이감소되었으며미세공피복부위의조기골내성장을통한안정된고정을얻었기때문에대퇴부동통을호소한경우가적었다고사료된다. 미세공피복은골내성장을유도하여무시멘트고관절전치환술에서그성공적결과에중요한비중을차지한다. 미세공피복된스템을사용한여러연구에서중기추시결과 88.6~100% 의골내막신생골형성을보고하였다 40,43,48). 저자의경우골내막신생골형성이최종추시시 53 예 (92.9%) 에서나타났으며이는대퇴삽입물의우수한접촉과미세포말스템으로의우수한골내막신생골형성의결과로생각된다. 이에비해비환형대퇴스템을사용한경우 Petersilge 등 42) 은 89%, Maloney 등 35) 은 83% 의골내막신생골형성을보고하였다. 주로골내막신생골형성을보인부위는대퇴스템근위부 1/3 의미세공피복된부위와중간 1/3 의표면이거칠게처리된부분의사이인 Gruen 제 2 구역과 3 구역의경계및제 5 구역과 6 구역의경계지역에서주로관찰되어다른대퇴삽입물과비슷하였다. 대퇴피질골비후는대퇴스템에서대퇴골로의응력전달및직선형대퇴주대의대퇴골내에서외측굴곡능률 (lateral bending moment) 생성이그원인으로생각되며 38), 다른디자인의무시멘트대퇴스템을이용한여러연구결과에서 14% 에서 30% 정도의원위부피질골비후소견을보고하였다 2,9,20). 본연구에서는 6 예 (10.5%) 가관찰되었으며, 대퇴근위부에서원위부로균등한체중부하전달이이루어짐과동시에안정된고정으로인해적게발생한것으로사료된다. Pedestal 반응은 30 예 (52.6%) 에서발생하였으나의미가있는경우는없었으며, 원위대퇴스템이 pedestal 과맞닿아있고대퇴스템첨단부주위에방사선음영감소선이없어 Engh 등 17) 의분류상모두안정한원위대퇴스템과관련되어있었다. 대퇴거흡수는 36 예 (63.1%) 에서관찰되었으나대부분 rounding off 형으로전례에서심한대퇴부골흡수나응력방패현상을보인예는없었으며대퇴스템의근위부환형미세공피복부위보다근위부에서주로발생하였다. 이는대퇴스템의미세공피복부위로골내성장이만족스럽게이루어져대퇴골근위부의응력방패현상을최소화하여골의소실이방지된것으로사료된다. 고관절전치환술시골용해는첫 2 년동안발생할수는있으나 34) 수술후평균 5 년후더흔하게관찰되며, 무시멘트고관절전치환술시골용해발생빈도는 5 년추시시대략 10~32% 로보고되고있다 17,26,36,42). 대퇴스템주위의골용해를감소시키기위해서는대식세포를활성화시키는마모입자가골과스템사이를통하여골수강내로접근하는것을차단하는것이중요하며, Emerson 등은삽입물의근위부에환형미세포말처리를시행하면근위부에서 골내성장을이루어골용해를일으킬수있는미세입자의이동을방지한다고보고하였다. 대퇴원위부의골용해는비환형미세공피복대퇴스템을사용하였을때 Petersilge 등 42) 은 12%, Martell 등 36) 은 8% 로보고하고있으나대조적으로환형미세공피복대퇴스템을사용하였을때 Emerson 등 15), Hofman 등 27), Sharkey 등 46) 은모두 0% 로보고하고있다. 본연구에서는대퇴스템주위로골용해소견을보인예는없었으며이는 highly crosslinked 폴리에틸렌등의성능이향상된관절면을사용함으로써마모입자의발생을줄인것과대퇴스템근위부의환형미세포말에의한조기골내성장으로대퇴스템의표면과골사이에강한결합이형성되어마모입자가원위부로이동하는것을효과적으로차단한것으로판단된다. 그러나 Emerson 등 15) 은골내성장에도불구하고관절액의접근을허용하는 interconnecting porous channel 이존재할수있다고가정하였고, 미세운동에의한원위부에서의국소적마모파편이형성되거나, 활동적인환자에서스템원위부에서응력이높아져삽입물과골내막사이에서움직임이생겨원위부에골용해를발생시킨다는가정들이제시되고있으므로이에대하여는향후더추시관찰이필요하다고사료된다. 시멘트형삽입물에서평균폴리에틸렌의선형마모율이 0.05~0.21 mm/year 으로보고되었으며 6,31), 무시멘트형삽입물의평균선형마모율은 mm/year 으로보고된바있다 22,30,39,47). 본연구에서폴리에틸렌의마모율은평균 0.09 mm/year 으로시멘트형및다른무시멘트형삽입물에비해좋은결과를보였다. 초고분자량폴리에틸렌의평균선형마모율은 Sychterz 등 49) 이 0.17 mm/year, Dowd 등 14) 이 0.18 mm/year, Pedersen 등 41) 이 0.14 mm/year 으로보고하였으며, highly cross-linked 폴리에틸렌의평균선형마모율은 Dorr 등 13) 이 mm/year, Heisel 등 26) 이 0.02 mm/year 로보고하였다. 이는 polymer 의 cross-linking 으로이차적인산화에대한내구성이더강해진것때문으로사료된다. 본연구의경우초기 24 예의초고분자량폴리에틸렌의평균선형마모율은 0.11 mm/year, 후기 33 예의 highly cross-linked 폴리에틸렌의평균선형마모율은 0.08 mm/year 으로관찰되었다 (p=0.11). 초고분자량폴리에틸렌삽입물과함께 zirconia ceramic head 를사용하여마모에대해보고한논문들에의하면 Haraguchi 등 23) 은 phase transformation 으로표면거칠기가증가한다고하였고 Wroblewski 등 53) 은 0.22 mm/year, Kim 등 29) 은 0.08 mm/year 의결과를보고하였다. Kim 등 29) 은 zirconia ceramic head 와 cobalt-chromium head 를비교한논문에서 zirconia ceramic head (0.08 mm/year) 가 cobaltchromium head (0.17 mm/year) 에비해 wear 에대해우수하다고보고하였다. 본연구의경우초기 25 예의 66

9 zirconia ceramic head 를사용한군의폴리에틸렌평균선형마모율은 0.11 mm/year, 후기 32 예의 cobaltchromium head 를사용한군의폴리에틸렌평균선형마모율은 0.08 mm/year 으로관찰되었으나 (p=0.11), 저자들의경우 zirconia ceramic head 는오직초고분자량폴리에틸렌삽입물에, cobalt-chromium head 는오직 highly cross-linked 폴리에틸렌에사용되었기에단순비교가불가능하여따라서그차이에대한의의는적다고사료된다. Trilogy 비구컵은미세공피복된반구형의조합형삽입물이다. 미세공피복된비구컵은골내성장이조직검사상우수하고, 중단기간추시결과가많은경우에우수한것으로보고되고있다. Alejandro 등 52) 은 Triolgy 비구컵을사용하여일차성인공관절전치환술을시행하고 4~7 년추시관찰한결과 99% 의골내성장, 5% 의비구부골용해를보고하였다. George 등 21) 은반구형비구컵은일차성인공관절전치환술중비구부골절의빈도가타원형비구컵에비해적다고보고하였다. 조합형비구컵에서는관절면과금속 shell 이접촉하는 backside 에서폴리에틸렌마모입자가형성되고 28), 비조합형비구컵은제조사에서폴리에틸렌이미리조립되기때문에폴리에틸렌과 shell 사이에움직임이없다 45). 그래서조합형비구컵에서비조합형비구컵에비해삽입물주위골용해의빈도가더많을것이라고생각할수있으며, Crowther 등 8) 은 1 세대 Harris- Galante 비구컵을사용하여 9~14 년추시관찰한결과 23% 의비구부골용해를보고하였다. Backside 마모를줄이기위해최근의조합형비구컵에서는잠금장치를개선시키고비구컵과폴리에틸렌사이의외형일치를최대화하였다. Alejandro 등 18) 은 Trilogy 비구컵과비조합형 Implex 비구컵을각각사용하여 5~8 년추시관찰결과마모율와삽입물주위골용해는두군에서차이가없었다고보고하였다. 결 VerSys Fiber Metal Taper 대퇴스템과무시멘트 Trilogy 비구컵을이용한무시멘트고관절전치환술을시행한후최소 5 년이상추시관찰한결과임상적및방사선학적으로우수한결과를나타내었다. 비구컵의이동이나비구컵주위의골용해가관찰되지않았으며, 대퇴스템근위부의환형미세공피복으로인한골내성장및원추형원위부에의하여응력방패현상을최소화하여근위대퇴골의소실을방지하였다. 또한 cobalt-chromium head 와 highly-cross linked 폴리에틸렌의사용으로마모를줄일수있었다. 따라서우수한임상적결과와방사선학적안정성을얻을수있었으며, 향후더장기적인추시관찰이요할것으로사료된다. 론 REFERENCES 01) Barrack RL, Jasty M, Bragdon C, Haire T and Harris WH: Thigh pain despite bone ingro wth into uncemented femoral stems. J Bone Joint Surg, 85-B: , ABSTRACT Mid-term Results of Primary Cementless Total Hip Arthroplasty with a Fiber Metal Taper Femoral Component and Trilogy Acetabular Cup Kuen Tak Suh, M.D., Jong Ki Shin, M.D., and Jung Sub Lee, M.D. Department of Orthopedic Surgery, Pusan National University School of Medicine, Busan, Korea Purpose: This study examined the results of cementless total hip arthroplasty using a circumferentially porous coated tapered femoral stem and a porous coated acetabular cup after a follow up of more than 5 years. Materials and Methods: Fifty-seven hips in 52 patients, who underwent primary total hip arthroplasty with a Versys Fiber Metal Taper femoral stem (Zimmer, Warsaw, IN) and Trilogy acetabular cup (Zimmer) between September 1998 and December 2001, were followed up for more than 5 years after surgery. Results: The mean Harris hip score increased from 56.6 to 96.0 points. There was no osteolysis in the femur and acetabulum. Endosteal bone formation was observed in 53 hips (92.9%) mainly in Gruen zones II and VI. There was no stem subsidence, and no significant migration or tilting of the acetabular cup. The survival with an acetabular and femoral revision for aseptic loosening was 100%, respectively. Conclusion: After a follow-up of more than 5 years, cementless total hip arthroplasty using a circumferentially porous coated tapered femoral stem and a porous coated acetabular cup showed excellent results with good bone ingrowth, as well as no stem and cup migration and no osteolysis around the distal femur and acetabulum.
