1 대한안과학회지 2009 년제 50 권제 2 호 J Korean Ophthalmol Soc 2009;50(2): DOI : /jkos 우세안과비우세안의굴절이상 조경진 1 김소열 2 양석우 1 가톨릭대학교의과대학안과및시과학교실 1, 평화의빛성모안과 2 목적 : 우세안의결정에관여하는요소중굴절이상에대하여분석하여우세안과비우세안과의차이를알아보고자하였다. 대상과방법 : 근시성난시환자 62 명을대상으로양안시력교정후, hole-in-the-card test 의방법을사용하여우세안을결정한후, 나안시력, 굴절검사, 안압등을측정하였다. 결과 : 우세안의나안시력은평균 0.41, 비우세안은 0.39 이었으며, 우세안이더좋은경우가 18 명, 나쁜경우가 18 명이었다. 근시의양은우세안이평균 3.2, 비우세안이 디옵터로유의한차이가없었고, 우세안이비우세안보다근시의양이더많은경우는 24 명, 적은경우는 32 명이었다. 우세안의난시정도가비우세안보다적은경우가 35 명 (56%) 으로난시가적은쪽이우세안이되는경우가유의하게많았다 (P=0.0014). 안압은우세안이 15.4 mmhg, 비우세안이 15.7 mmhg 으로유의한차이는없었다. 결론 : 근시성난시환자의경우우세안에서난시가적었다. 우세안이비우세안에비해근시의진행정도가크다는과거의주장은더욱많은연구가필요할것으로생각된다. < 대한안과학회지 2009;50(2): > 양측으로존재하는신체기관에서는대체적으로우세함을보이는어느한쪽이있다. 이현상은손과발에서뚜렷이나타나며눈에서도우세함을보이는쪽이있다. 정상인에서우안이우세안인경우의빈도를높게보고하고있으며사시환자에서는비편위안이우세안과관련이높고시력과도상관관계가있다고하였다. 1 또한우세안의지속적인조절로인하여우세안의근시가더진행하며, 조절시필요한부교감신경의흥분으로우세안의안압이비우세안보다높다는보고도있다. 1-3 이처럼우세안의결정에는운동, 감각, 안질환, 시력등매우다양한요소와관련이있다고알려져있다. 본연구에서는이러한다양한요소중굴절이상에대하여분석하여우세안과비우세안과의차이를알아보고자한다. 접수일 : 2007 년 8 월 20 일 대상과방법 2005년 3월부터 2005년 10월까지성바오로병원안과에시력교정을목적으로내원한 15세이상 45세이하의모든환자중안질환이없는근시성난시환자 62명을대상으로 심사통과일 : 2008 년 9 월 2 일 통신저자 : 양석우서울시서초구반포동 505 가톨릭대학교강남성모병원안과 Tel: , Fax: yswoph@catholic.ac.kr * 본논문의요지는 2006 년대한안과학회제 96 회추계학술대회에서포스터로발표되였음. 하였고, 최대교정시력이 1.0이되지않는환자는제외되었다. 모든환자를 1.0으로교정한후양안이동일한시력하에서우세안검사를하였으며, 나안시력, 굴절검사, 안압, 그리고오른손잡이및왼손잡이여부를조사하였다. 우세안검사는 hole in the card test를사용하여가운데작은구멍이뚫린카드를환자가양손으로잡게하고양안을뜨고정면주시상태에서카드의구멍을통하여먼곳의목표물을주시하게하였다. 그상태에서한쪽눈씩교대로감게하여어느쪽이우세안인지를결정하였으며, 근거리와원거리모두에서우세안검사를시행하였다. 굴절검사는자동굴절검사기 (Auto Ref Keratometer RK 3. CANON) 를이용하여측정후최적구면도수 (manifest spherical equivalent) 로환산하였으며, 안압측정은자동안압기 (Tonometer TX-10, CANON) 를사용하였다. 결과 대상자인 62명중여자가 38명, 남자가 24명이었으며, 연령은여자가 15~45 세, 평균 31.4 세 (±7.3), 남자가 15~43 세, 평균 33.7세 (±8.2) 로남자와여자간에통계적으로유의한차이를보이지않았다. 우세안이우안인경우가 41명 (66%) 이었으며, 좌안인경우 21명 (34%) 보다많았다. 또한오른손잡이가 58명 (94%), 왼손잡이는 4명 (6%) 이었다. 모두근거리와원거리에서우세안이다른경우는없었다. 또한성별로나누었을때, 여자의경우우세안이우안인경우 24명 (65%), 좌안인경우 13명 (35%) 이었고남자의경 275

2 - 대한안과학회지 2009 년제 50 권제 2 호 - 우우세안이우안인경우 17명 (68%), 좌안인경우 8명 (32%) 으로남자와여자간우세안의유의한차이를보이지않았다 (p=0.800). 오른손잡이중우세안이오른눈인경우는 38명 (66%), 왼눈인경우는 20명 (34%) 이고왼손잡이중우세안이오른눈인경우는 3명 (75%), 왼눈인경우는 1명 (25%) 으로, 오른손잡이와왼손잡이간에우세안의유의한차이를보이지않았다 (p=0.700). 나안시력은우세안이평균 0.41, 비우세안이 0.39로우세안과비우세안의나안시력평균은통계적으로유의한차이가없었다 (P=0.431)(Table 1). 우세안의나안시력이비우세안보다더좋은경우는 18명 (29%), 더나쁜경우또한 Table 1. Characteristics of dominant eye compared with non-dominant eye No. of patients (%) Better or more Worse or less Same value UCVA * 1 (29%) 18 (29%) 26 (42%) Degree of myopia 24 (39%) 32 (52%) 6 (9%) Degree of astigmatism 14 (23%) 35 (56%) 13 (21%) Intraocular pressure 14 29%) 20 (41%) 15 (30%) * UCVA=uncorrected visual acuity. 18명 (29%) 으로같았다 (Table 2). 굴절이상은우세안이평균 3.2디옵터, 비우세안이 3.43디옵터로우세안과비우세안의근시의양은통계적으로유의한차이가없었다 (p=0.282)(table 1). 또한우세안이비우세안보다근시가더많은경우는 24명 (39%) 이었고, 우세안이근시가더적은경우는 32명 (52%) 이었다 (Table 2). 1 디옵터이상의양안부등시성근시가있는 16 명중 6명만이우세안의근시정도가더많았다. 평균난시정도는우세안이 0.95, 비우세안이 1.91 로우세안과비우세안간난시정도에유의한차이를보였다 (P<0.001)(Table 1). 또한우세안의난시정도가비우세안보다적은경우가 35명 (56%) 으로, 난시가적은쪽이우세안이되는경우가유의하게많았다 (p=0.0014)(table 2). 본연구에서는자동굴절검사기를사용하여난시측정을한것으로두눈간에차이가 0.5디옵터이상일때난시의차이가있다고했으며 0.5디옵터미만은오차의가능성을고려하여난시의차이가없다고하였다. 우안이우세안인 41명중 20명 (49%) 이우안난시가좌안보다적었고, 12명 (29%) 은같은정도의난시가있었으며 9명 (22%) 은우안난시가좌안보다많았다 (Table 3). 좌안이우세안인 21명중 15명 (71%) 이비우세안인우안에난시가더많았고, 1명 (5%) 은같은정도의난시가있었으며 5명 (24%) 만이우세안인좌안에난시가더많았다 (Table 4). 평균안압은우세안이 15.4 mmhg, 비우세안이 15.7 mmhg이었으며우세안과비우세안간에유의한안압차이는없었다 (P=0.338)(Table 1). 우세안의안압이더높은경우는 14명 (29%) 으로, 우세안과비우세안간의차이라기보다는일반적으로좌안의안압이더높았으나 (47%) 통계적으로유의한수준은아니었다 (P=0.20)(Table 2). 3 - 조경진외 : 우세안과비우세안의굴절이상 - 고찰 우세안은 1593년처음소개되었으며, 주시와주의그리고인지시에비우세안에비해우세안인한쪽눈으로부터시각정보를얻으려는성향으로정의되고있다. 4-6 우세안의존재는다양한매개변수에의해서설명되어질수있는데, 부등시성근시에서근시의정도, 7 조절반응, 8,9 주시와주의, 10 안구운동, 11 그리고노안 12 이나편안의황반열공 13 같은안질환등이해당된다. 우세안의결정은통상근시가진행하기전인 3세전에이루어진다고알려져있으며, 유전적으로결정되는경향이있고 14 우세안은매우안정적이어서, 5 우세안의시력저하가심하지않으면변화하지않는다고알려져있다. 따라서우세안은근시가나타나기전에결정되어지며우세안과굴절력은관련이없다고알려져있다. 4 하지만다른연구에서는우세안은비우세안에비해높은굴절이상을보이고, 안축장이길며특히부등시성근시환자에서우세안의근시정도가더심하다고하였다. 7 이는지속적인조절로인한모양체근의피로가근시진행의원인요소로서지목되고있으며, 우세안의근시진행이비우세안보다더많다는주장이다. 따라서치료적관점에서도안약사용및굴절교정을통한근시진행의억제를생각하기도한다. 부교감신경계의작용과안압상승또한우세안과관련해서생각해볼수있는데, 교감신경계의자극은안압을낮추게되는 반면에부교감신경계의자극은안압을높이게된다고알려져있다. 18,19 성별에따라서는남자의경우우안에서안압이좌안에비해더높고, 여자의경우는차이가없다고한다. 또, 오른손잡이의경우우안의안압이좌안에비해더높다고한다. 이는초점을맞추는눈에부교감신경의자극이강하게작용하고따라서안압을높이게되는데, 남자의경우우안이우세안인경우가많고뇌의구조와기능이여자에비해비대칭이기때문이라고한다. 20,21 마찬가지로오른손잡이의경우도우안이우세안인경우가많고왼손잡이에비해뇌의구조와기능이비대칭이기때문이라고설명한다. 22,23 본연구에서는 1) 근시성난시환자에서우세안의나안시력이비우세안의나안시력과의미있는차이를보이지않았다. 2) 우세안의근시정도는비우세안에비해적은경우가많았으나, 우세안과비우세안의근시의양은통계적으로유의한차이가없었다. 3) 안압은우세안과비우세안에서유의한차이가없었다. 4) 우세안은비우세안에비하여난시의정도가유의하게적었으며, 난시가적은쪽이우세안인경우가유의하 게많았다. 따라서우세안의사용에의한조절과다로근시가비우세안에비해우세안에서더많이진행한다는과거의주장에대해서는더욱많은연구가필요할것으로생각되며, 우세안에부교감신경이강하게작용하여안압이높아진다는주장또한근거가부족한것으로생각된다. 또한, 저자들은양안교정시력이동일한근시성난시환자에서우세안의난시가평균적으로적음을확인하였다. 난시의정도는연령이증가함에따라변화가있는데, 출생시에는대부분난시안이지만생후 1년동안은난시의정도가급격히감소되고 1세이후부터는변화가점진적으로감소되며 8세이후부터는크게변하지않는다고한다.24 따라서근시가진행하기시작하는 3세이전에우세안이결정된다고하면, 3세이전까지의난시정도가우세안의결정에영향을줄것으로생각된다. 하지만난시정도가많은눈이우세안인경우가 23% 이고비슷한경우도 21% 이었다. 이는난시정도가적은눈이우세안이되는경우 (56%) 보다는유의하게적었지만 (p=0.0014) 우세안이결정된 3세이후에도난시가변할수있으며우세안의결정에난시와더불어운동, 감각, 그리고시력등이함께복합적으로작용할것으로생각되는바이에대해서도더많은연구가필요할것이다. 참고문헌 1) Koo BS, Cho YA. Methods: Sixty two subjects without underlying ocular disease were recruited. Ocular dominance was determined using the holein the card test. Uncorrected visual acuity, refractive error, and intraocular pressure (IOP) were checked in both eyes. Results: Mean uncorrected visual acuity of the dominant and non dominant eye were 0.41 and 0.39, respectively. The number of patients whose uncorrected visual acuity of the dominant eye was superior to the non dominant eye was 18(29%), and inferior to the non dominant eye was 18(29%). Mean refractive power in the dominant eye was -3.2 diopter and 3.43 diopter degrees in the non dominant eye, therefore no difference in degrees of myopia between the groups (P=0.282) was observed. The number of patients whose dominant eye had a greater degree of myopia than the non dominant eye was 24(39%) and the patients with a lower degree of myopia in the dominant eye was 32(52%). Astigmatism of the dominant eye was lower than the non dominant eye in 35(56%) of the patients, thus a significant relationship was shown between astigmatism and the dominant eye (P=0.0014). The mean IOP of the dominant eye was 15.4 mmhg and the non dominant eye was 15.7 mmhg, showing no significant difference between eyes. Conclusions: The dominant eye showed a lower degree of astigmatism than the non dominant eye. The previous belief that myopia is more progressed in the dominant eye than the non dominant eye because of excessive accommodative use of the dominant eye requires further study.
