Pathogenic fungi, treatment response and prognosis of onychomycosis are different dependent upon the clinical type. Therefore, suitable clinical classification is necessary before treatment. Objective: The primary aim of this study was to investigate the incidence and distribution of 4 clinical types of onychomycosis: distal and lateral subungual onychomycosis (DLSO), superficial white onychomycosis (SWO), proximal subungual onychomycosis (PSO), total dystrophic onychomycosis (TDO). The secondary aim of this study was to clarify the DLSO, the most common type of the onychomycosis, by dividing it into 4 subtypes. Methods: The diagnosis of the patients were confirmed by more than 1 positive test among KOH smear, fungus culture, histopathologic test and KONCPA test. Total 817 onychomycotic nails of 589 patients (331 males, 258 females) who visited Department of dermatology, Catholic University, St. Mary hospital between January, 1999 and December, 2006 were included. Clinical photographs and charts of the patients were thoroughly reviewed and divided into 4 clinical types and 4 clinical subtypes in case of DLSO. Results: 1. Out of 817 nails, 623 (76.3%) fell into DLSO and 82 (10.0%) TDO, 65 (7.9%) SWO, 47 (5.8%) PSO, respectively nails diagnosed to be DLSO were further divided into 4 clinical subtypes: 243 (39.0%) edge types, 211 (33.9%) distal types, 104 (16.7%) linear types and 65 (10.4%) lateral types males with DLSO were further divided into 4 clinical subtypes: 130 (41.3%) edge types, 96 (30.5%) distal types, 54 (17.1%) linear types and 35 (10.6%) lateral types. Females in the same group also exhibited same ranks in terms of prevalence. 4. Out of 457 big toenails, there were 193 edge types (42.2%), 141 distal types (30.9%), 77 linear types (16.8%) and 46 lateral types (10.1%). Out of 105 other toenails, there were 42 distal types (40.0%), 28 edge types (26.7%), 19 linear types (18.1%) and 16 lateral types (15.2%). Conclusion: DLSO composed of 76.3% of onychomycosis were further classified into 4 clinical subtypes. [Kor J Med Mycol 2008; 13(3): ] Key Words: Onychomycosis, Classification, Subtype 서 론 손발톱진균증 (onychomycosis) 은피부사상균 (dermatophytes), 효모균및비피부사상균성사상균 (non-dermatophytic filamentous fungi) 등의진균이손발톱을침범하여손발톱바닥의과다각화증이발생하고, 손발톱판의변색과변형을초래하는질환으로전체손발톱질환의약 20% 를차지하는흔한감염증이다 1. 피부사상균에의한손발톱감염을 Zaias 2 는손발톱판의침범형태, 부위및원인균에따라원위손발톱밑손발톱진균증 (distal subungual onychomycosis; DSO), 표재성백색손발톱진균증 (superficial white onychomycosis; SWO), 근위손발톱밑손발톱진균증 (proximal subungual onychomycosis; PSO) 및칸디다성손발톱진균증 (candidal onychomycosis: CO) 의 4형으로분류하였으나, 최근 Baran 등 3 은원인균보다는손발톱판의침범형태및부위에초점을맞추어원위외측손발톱밑손발톱진균증 (distal lateral subungual onychomycosis; DLSO), 표재성손발톱진균증 (superficial onychomycosis: SO), 근위손발톱밑손발톱진균증 (proximal subungual onychomycosis; PSO), 손발톱내손발톱진균증 (endonyx onychomycosis: EO), 전이상성손발톱진균증 (total dystrophic onychomycosis: TDO) 의다섯가지새로운임상형을제안하였다. 그외손발톱진균증의분류를살펴보면, 1996 년 Drake 등 4 의논문과 1998년 Elewski 등 5 의논문에서는이전 Zaias 2 의분류를따르고있고, 2003년 Roberts 등 6 의논문에서는 DLSO, SWO, PSO, CO, TDO의 5가지분류를사용하였다 (Table 1). 국내에서는 2005년오등 7 과김등 8 은 DLSO, SWO, PSO, TDO의네가지임상형을사용하였고, 특히오등 7 의논문에서는 DLSO의 세부임상형 (clinical subtype) 으로선상형 (linear type) 을제시하였고, 이를다시원위형 (distal type), 측면형 (lateral type), 다발형 (multiple type) 으로세분하여비교분석한바있다. 손발톱진균증은임상형에따라원인균종의분포, 치료에대한반응, 예후등에있어큰영향을줄수있다 3. 본연구에서는한국인의손발톱진균증을임상적으로네가지형, 즉 DLSO, SWO, PSO, TDO으로분류하여그빈도를조사하고이중가장많은분포를차지하는 DLSO를세분하여임상진료에도움이되는분류체계를마련하고자본연구를시행하게되었다. 1. 대상 대상및방법 본연구는 1999년 1월부터 2006년 12월까지 8 년간가톨릭대학교성모병원피부과에내원하여임상소견과손발톱의 KOH 도말검사, 진균배양검사, 병리조직검사, 손발톱조직진균검사 ( 이하 KONCPA 검사 ) 중한가지이상의검사에서양성소견을보여손발톱진균증으로진단을받은환자들중에서임상사진으로정확한분류가가능한 589명 ( 남자 331명, 여자 258명 ) 의 817개의손발톱을대상으로하였다. 2. 연구방법 대상환자 589명의의무기록을후향적으로조사하여, KOH 도말검사, 진균배양검사, 병리조직검사, KONCPA 검사결과를확인하였고환자의임상사진을반복검토하여먼저기본 4개임상형으로분류하고 DLSO는다시 4개의세부임상형으로분류하였다. 1) 기본 4개임상형의임상적특징 (1) 원위외측손발톱밑손발톱진균증 (DLSO) 손발톱판의원위부와모서리또는측면부위에

(2) 표재성백색손발톱진균증 (SWO) 손발톱판의표면에백색반점과반이관찰되며, KOH 도말검사에서양성소견을보이는임상형으로손발톱판의근위부에병변이있을경우에는병소의처음발생부위와진행방향에대한기록을참고하여 PSO와감별하였다 (Fig. 2). (3) 근위손발톱밑손발톱진균증 (PSO) 손발톱판의근위부에서병변이발생하여원위부쪽으로퍼져나가는임상형 (Fig. 3). Clinical findings of TDO 위의세가지형태의손발톱진균증이진행되어진균이전손발톱판의 90% 이상을침범한임상형 (Fig. 4). 2) DLSO 의 4개세부임상형의임상적특징 (1) 원위형 (Distal type) 손발톱판의원위부를부분혹은전반적으로침범한세부임상형 (Fig. 5, 6). (2) 모서리형 (Edge type) 손발톱판의원위부와외측부가맞나는모서리를중심으로하여부채꼴모양으로침범한세부임상형 (Fig. 7, 8). (3) 선상형 (Linear type) 병변부의모양이가로보다세로가길고폭이비교적일정한선모양을보이는세부임상형 (Fig. 9, 10)

8 손발톱진균증은손발톱질환의약 20% 를차지하는매우흔한질환으로서전체인구의약 2~18% 가이환되어있고, 70세까지조사하면 48% 이상이한번은이환된적이있다고보고되고있다 10. 손발톱진균증이잘발생하는조건은남자, 고령, 흡연, 말초혈관질환등이있다 10,17. 본연구에서는총 589명중에서남자가 331명, 여자가 258명으로 1.28:1의비율로남자에서호발하였다. 평균연령은 52.4±15.2세이며 41세에서 60세사이의환자가 257명으로가장많았고, 61세이상의환자가 245명으로두번째로많았다. 41세에서 60세사이의환자가 61세이상의환자보다많았다는점은건강에대한관심도병원접근성등이유병율이외의요인이작용했을것으로사료된다. 손발톱진균증의분류는진균의침범형태와치료결과및예후를예측하는데에있어통찰력 (insight) 을갖게한다 3. 기존의손발톱진균증의분류들을살펴본결과 1972년 Zaias 등 2 의분류와 1998년 Baran 등 3 의분류가대표적이었다. 그밖의다른논문들에서는대체로두저자의분류를따르거나두저자의분류를혼합하여사용하고있었다. 또한 Baran 등 3 의분류가발표된이후에는원인균보다는침습부위에초점을맞춘분류가많이사용되고있다. 본연구에서도손발톱진균증을 Baran 등 3 과김등 8 의분류에기초하여 4가지임상형으로분류해본결과 DLSO가 76.3%, TDO가 10.0%, SWO가 7.9%, PSO가 5.8% 의빈도를보였다. 기본 4개임상형의분류과정에서한개의손발톱에둘이상의임상형이동시에존재하는복합형 (combined type) 이드물게확인되었으며, 이경우에는진료기록상먼저발생하였거나, 범위가넓은임상형으로분류하였다. DLSO는손발톱진균증의가장흔한임상형으로서저자에따라약 70~95% 의빈도를차지하는것으로보고하고있다 8,10,18. 따라서저자들은임상에서 DLSO의다양한형태를경험하면서세 부임상형의필요성을느끼게되었다. DLSO의세부임상형에대해서구체적으로기술한논문은없었으나, 2005년오등 7 은선상손발톱진균증을보고하였고이를다시원위형, 다발형, 측면형으로세분하여그빈도와치료결과를비교하였다. 본연구에서는 1999년 1월부터 2006년 12월까지 8년간본기관에내원하여손발톱진균증으로진단받은환자들의차트와임상사진을비교검토하여 DLSO를원위형, 모서리형, 선상형, 측면형의 4가지세부임상형으로분류하였다. 첫번째원위형은손발톱판의원위부를부분혹은전반적으로침범한형으로측면감염 (lateral involvement) 이있을경우에는제외하였다. 손발톱판의두께와감염범위에따라예후에차이가있으나측면감염이없어비교적치료반응에있어예후가좋을것으로추정되는임상형이다 11. 두번째모서리형은원위부와측부가맞나는모서리를중심으로하여부채꼴의모양으로침범한형으로진균이손발톱판의모서리에서부터원위부와측부를향하여퍼져나가는임상형이다. 세번째선상형은진균이손발톱바닥의종축손발톱주름을따라침범하여병변부의모양이가로보다세로가길고폭이비교적일정한선모양을보이는세부임상형이다. 이아형은 2005년오등 7 이제시한선상손발톱진균증의원위형과다발형을포괄한다. 네번째측면형은진균이측부손발톱주름을통하여손발톱판의측부를부분혹은전반적으로침범한형으로 2005년오등 7 이제시한선상손발톱진균증의측면형에해당한다. 손발톱진균증의치료실패의이유는병변으로의부적절한약물흡수, 진균의내성, 손발톱성장의결함, 면역저하등이있는데, 대부분은진균덩어리 (fungal mass) 로약물이충분한농도로작용할수없는경우이다. 이러한경우로는심한손발톱박리증 (onycholysis), 피부진균종, 선상침범, 심한과다각화증등이있다 15,20. 선상형및측면형과같이선상침범 (linear involvement) 이있는임상형은병변내에케라틴과진균들이서로

9 안효상등 : 한국인손발톱진균증의임상적분류 엉켜손발톱판과손발톱바닦사이에부종을형성하여병변부로약제가충분히침투하지못한다 8,15. 그러므로선상형및측면형은원위형및모서리형에비하여치료반응에있어예후가좋지않을것으로예측할수있다. 측면형은선상형보다대체로손발톱감염의깊이 (depth of invasion) 가깊어 Sergeev 19 가고안한 SCIO 점수가높은경향을보이고, 측부의손발톱판과손발톱바닦은항진균제의약제농도가중앙부보다낮으며, 오등 7 의연구에서도측면형의중증도 (severity) 가원위및다발형보다높았던점을근거로측면형이선상형보다치료반응에있어예후가좋지않을것으로예측할수있다. 실제분류과정에서한개의손발톱에둘이상의세부임상형이동시에존재하는복합형이드물게관찰되었는데, 이경우감염의깊이 (depth of invasion) 를병변의면적보다우선시하여손발톱판의원위부에서부터감염의깊이가더깊은세부임상형으로분류하였다. 본연구에서제시한세부임상형이위와같은치료반응과예후를보이는지에대해서는향후 SCIO 등 19 과같은객관적인지표를통한치료반응평가가필요할것으로사료된다. 결론 1999년 1월에서 2006년 12월까지 8개년간본원에서손발톱진균증으로진단받은환자들의차트와임상사진을바탕으로손발톱진균증을 DLSO, SWO, PSO, TDO의 4가지임상형으로분류하였다. 이중가장높은빈도를보이는 DLSO 에대하여 4가지의세부임상형을제시하였고이를바탕으로 8년간 DLSO로진단된 623개의손발톱을적절하게분류할수있었다. 이분류체계의유용성을평가하기위해서향후 SCIO 등의객관적인지표를통한치료반응평가등의지속적인임상연구가필요할것으로보인다. 참고문헌 1.
