In this study, we analyzed the laboratory and clinical characteristics of candidemia caused by four different Candida species ( albicans, parapsilosis, tropicalis and glabrata) occurring at Chonnam National University Hospital (CNUH). Methods : The demographic, clinical and microbiological data of 157 patients with candidemia at CNUH from 1996 to 2002 was analyzed, retrospectively. The etiologic agents for 157 cases of candidemia were albicans (n=48), parapsilosis (n=48), tropicalis (n=32) and glabrata (n= 29). The characteristics of candidemia due to each single Candida species were compared with those with all other species combined. Results : Although the majority (77%) of candidemic patients were adults, candidemia due to albicans or parapsilosis occurred significantly more often in premature infants (15%, retrospectively, P=0.002), in comparison with other Candida species (0%). Candidemia due to glabrata was more common in patients with neutropenia (41%, P<0.001), and they also occurred frequently in the absence of central venous catheter related candidemia (86%, P<0.001). Bloodstream infections with parapsilosis were more frequently the cause of catheter related candidemia (56%, P=0.012), and they had a better clinical outcome (90%, P=0.004) than those with other Candida species. Conclusions : This study confirms that some characteristics of candidemia such as age, underlying conditions, relatedness of catheter, and outcome can be different according to the species of Candida. (Korean J Lab Med 2005; 25: ) Key Words : Candidemia, Clinical characteristics, glabrata, parapsilosis 서 칸디다혈증의발생빈도는최근수십년간점차증가하여, 현재입원환자에서유병률과치명률이높은중요한질환이되었다 [1, 2]. 미국의경우칸디다혈증은병원성혈류감염의약 8% 를차지 접수 : 2005년 6월 8일접수번호 : KJLM1861 수정본접수 : 2005년 7월 25일교신저자 : 신종희우 광주광역시동구학동 8 전남대학교병원진단검사의학과전화 : , Fax: shinjh@chonnam.ac.kr 론 하며 [3], 집중치료를받는환자의혈액배양에서 4번째로흔히분리되는균이다 [7]. 칸디다혈증의유병률이증가한원인으로는화학요법, 이식수술및집중치료를받는환자수의증가등여러가지가제시되고있다 [1, 4]. 칸디다혈증의원인균으로는이전에는 Candida albicans가대부분이었으나 [5, 6], 최근에는 glabrata 나 tropicalis 등과같은 albicans 이외의칸디다균종에의한혈류감염의발생이증가하고있다 [8]. 최근구미에서는혈액배양에서분리되는칸디다균종의분포와이들의임상및역학양상에관해다양한연구가진행되고있는데 [5, 6, 9, 10], 이보고들에의하면병원, 지역및국가에따라 317

2 318 오봉준 최현우 이진솔외 6 인 혈액배양에서분리되는칸디다균종의분포와임상적양상이다름을알수있다. 이러한차이는연구의대상으로선택된환자군간에항진균제사용과감염관리정책등이다르게적용되고있기때문인것으로생각되고있다 [10]. 특정지역이나병원의혈액배양에서동정되는세균의균종이환자의치료및예후에미치는영향이큰것처럼, 혈액에서분리되는칸디다균종또한칸디다혈증의치료및환자의예후에관한중요한정보를제공할수있다 [5]. 현재칸디다혈증의주요원인균으로는 albicans, tropicalis, glabrata 및 parapsilosis 등이가장잘알려져있는데, 칸디다혈증의원인균종과이의임상적특성을분석한보고는많지않은실정이다 [11, 12]. 본연구에서는최근전남대학교병원에서발생한네가지주요칸디다균종에의한칸디다혈증의임상및검사실적특성을원인균별로비교분석하여보았다. 대상및방법 1. 대상 3. 임상적특성칸디다혈증환자의나이, 성별, 기저질환, 선행인자, 치료및예후등임상적특성과혈액배양양성횟수, 중심정맥관 tip 배양등검사실적성적을후향적으로조사하고, 이를원인균종에따라비교하여보았다. 환자의연령은 18세를기준으로하여그이상인경우성인으로, 그이하인경우소아로구분하였으며, 수술을받은과거력을제외한나머지의임상정보는환자에게칸디다혈증이나타나기 7일전후의것만을고려하였다. 백혈구감소증은감염이발생할당시의백혈구수가 1,000/mm 3 미만인경우로정의하였으며, 칸디다혈증환자에서분리한중심정맥관의 tip을반정량적으로배양하여동일칸디다균종의집락이 15개이상자라는경우중심정맥관관련칸디다혈증으로진단하였다 [15, 16]. 칸디다혈증환자의경과를 3개월동안관찰하여혈액배양에서균이더이상자라지않고, 발열이나저혈압과같은혈류감염의임상증상이없는경우칸디다혈증이호전된것으로판단하였다 [17]. 4. 통계처리 1996년 1월부터 2002년 12월까지의 7년동안전남대학교병원에서실시한혈액배양에서 1회이상칸디다가분리된환자를대상으로하였다. 각칸디다균종에의한칸디다혈증의발병빈도를연도별로조사하였고, 4가지주요칸디다균종 ( albicans, tropicalis, parapsilosis 및 glabrata) 이분리된환자들중한가지균종만분리된환자들을대상으로임상적및검사실적특성을조사하였다. 병력조사가가능했던환자는 157명이었으며, 칸디다혈증원인균종별로는 albicans 48명, parapsilosis 48명, tropicalis 32명및 glabrata 29명이었다. 2. 혈액배양과칸디다균종의동정법혈액배양은 BACTEC 9240 system (Becton Dickinson, Sparks, USA), Vital system (biomerieux, Hazelwood, USA) 과 BacT/Alert system (biomerieux, Hazelwood, USA) 을이용하였다. 임상적으로균혈증이의심되는환자에서 6-10 ml을채혈하여이중 3-5 ml씩을호기성및혐기성배양병 (BacT/Alert culture medium, BACTEC Plus Aerobic/F 및 BACTEC Plus Anaerobic/F medium 혹은 BAC TECPEDS Plus/F medium) 에넣고 에서배양하여 BACTEC이나 Vital 기계에서 CO 2 생산량을측정하여양성으로판정될때까지혹은 5일간배양하였다. 양성인혈액배양병은배양액일부를취하여그람염색하고계대배양을시행하였다. 칸디다균종의동정에는발아관시험, API 20C (biomerieux, USA) 와 ATB 32C system (biomerieux, USA) 검사성적및 cornmeal agar와 CHROMagar Can- dida (Hardy diagnostics, Sarta Maria, USA) 에서 48시간배양후형태관찰등을이용하였다 [14]. 각칸디다균종에의한칸디다혈증의임상및검사실적특성은다른 3종칸디다에의한칸디다혈증의특성을합한성적과비교하여통계처리하였다. 각군간의통계적유의성은 Pearson's 2 test를이용하여검정하였고, P<0.05인경우유의한것으로간주하였다. 통계처리는 SPSS 12.0 (SPSS Inc., Chicago, USA) 을이용하였다. 결과 1. 칸디다혈증의발생빈도 1996년부터 2002년까지의 7년동안전남대학교병원에의뢰된혈액배양에서칸디다가분리된환자수는총 289명이었으며, 환자 100명당발생률은 0.012였다. 1996년은감염률이 0.006이었으나, 1997년 0.011로증가하였고그후 내외의빈도를보였다. 원인균으로는전체적으로 albicans 79예 (27%) 에비해 albicans가아닌칸디다균종이 73% 로훨씬많았다. 각균종별로는 parapsilosis가 92예 (32%) 로가장많았으며, albicans 79예 (27 %), tropicalis 35예 (12%), glabrata 31예 (11%) 의순이었다. 4가지주요칸디다균종중 albicans는 1998년과 2002년에가장높은빈도를보였고, parapsilosis는 1999년부터 2001년사이에가장많이분리되었다 (Table 1). 2. 칸디다혈증환자의연령, 성별, 기저질환및선행인자조사가가능했던환자 157명중 98명 (62%) 이남자이고, 121명 (77%) 이성인이었다 (Table 2). glabrata 칸디다혈증은여성

4 320 오봉준 최현우 이진솔외 6 인 맥관을유치하고있었는데, 이중 63% (82/130) 에서중심정맥관을제거했으며, 중심정맥관관련칸디다혈증으로확진된예는총 42% (66/157) 이었다. 전체칸디다혈증중중심정맥관관련칸디다혈증으로확진된예는 albicans 42%, parapsilosis 56%, tropicalis 47% 및 glabrata 14% 로서 glabrata가다른균종에비해의의있게빈도가낮았다 (P<0.001) (Table 3). 칸디다혈증이발생하기이전에항진균제치료를받은환자는전체의 3% (4/157) 이었으며, glabrata 칸디다혈증의경우 14% (4/29) 로다른칸디다혈증에비해높았다 (P<0.001). 칸디다혈증이발생한이후에항진균제치료를받은환자는 61% (95/157) 이었는데, 그중 glabrata 칸디다혈증의경우항진균제치료율이 41% (12/29) 로가장낮았고 (P=0.020), 칸디다혈증의회복률도 59% (17/29) 로가장낮았다 (P=0.030). 칸디다혈증이회복된경우는전체칸디다혈증의 75% (117/157) 였는데, parapsilosis 칸디다혈증의경우 90%(43/48) 로회복률이가장높았다 (P=0.004) (Table 3). 고찰칸디다혈증의발생빈도는외국 3차병원의경우입원환자 10,000 명당약 8예로보고된바있으며 [19, 29], 집중치료실환자의경우입원환자 10,000명당 예로보고되고있다 [22]. 전남대학교병원의최근 7년간칸디다혈증의총발생률은환자 100명당 0.012로서외국의보고보다는낮고, 국내다른대학병원과는유사하였다 [25]. 칸디다혈증의원인균은조사한시기, 지역및병원등에따라다르다 [5, 6] 년동안미국, 캐나다및중남미지역의 71 개병원을대상으로조사한결과, 전체적으로는 albicans가 55% 로가장많았고, albicans가아닌칸디다균종중에서는미국에서는 glabrata가가장많은반면, 라틴아메리카, 캐나다및유럽에서는 parapsilosis가가장많았다 [5]. 국내에서강등 [12] 은 albicans (40.8%), parapsilosis (17.3%) 및 tropicalis (16.5%) 의순으로보고하였고, 어등 [13] 은 albicans (43.8%), parapsilosis (20.8%) 및 glabrata (13.8%) 의순으로보고하였다. 본연구에서는 parapsilosis (31.8%), albicans (27.3 %) 및 tropicalis (12.1%) 의순으로서칸디다혈증의원인균종이지역과병원에따라다름을확인할수있었다. 본연구결과, tropicalis와 glabrata 칸디다혈증은각각 97% 및 90% 가성인에서발생한반면, albicans와 parapsilosis는각각 58% 및 75% 로더낮아균종간에유의한차이를보였다. Hajjeh 등 [24] 도 tropicalis 및 glabrata 칸디다혈증의경우각각 95% 및 96% 가성인에서발병한반면, albicans 와 parapsilosis는각각 87% 및 85% 로더적었다고보고하였다. Stamos와 Rowley[32] 는소아칸디다혈증원인균중 albicans (53%) 와 parapsilosis (24%) 가가장많았다고하였는데, 본연구에서도 albicans 칸디다혈증의경우다른칸디다혈증에비해소아에서더많이발생하였고특히미숙아에서발생하는비율이의의있게높았다. Meunier 등 [18] 은칸디다혈증환자 42% 에서혈액종양을비롯한악성종양이있었으며, Hajjeh 등 [24] 은 tropicalis (29%), glabrata (25%), albicans (23%) 및 parapsilosis (20%) 칸디다혈증순으로악성종양환자에서발생하는빈도가높은것으로보고하였다. 본연구에서는전체칸디다혈증환자중악성종양환자가 11% 로상대적으로적었는데, 이는병원마다환자구성분포의차이가있기때문으로생각된다. 본성적에서다른세종류의칸디다혈증에비해 tropicalis 칸디다혈증이혈액종양환자에서발생하는빈도가높아 Kao 등 [1] 및 Meunier 등 [18] 의성적과일치하였다. tropicalis 칸디다혈증은병독성이더높고사망률이높다는주장도일부있으나 [20, 23], 최근 tropicalis 칸디다혈증의기저질환과예후등임상적특성은병원마다다를수있다고보고되었다 [25]. 본연구에서 glabrata 칸디다혈증이위장관계질환과연관이된것은특이적인소견인데, 이는소화기관에상재하는균주가위장관수술등과관계하여혈류로유입되어발생되었으리라추측할수있다. 칸디다혈증발병과연관되는중요한위험인자로기저질환의존재, 항균제치료와중심정맥관유치는잘알려져있는데 [19], 본연구에서도거의모든환자가기저질환을가지고있었고, 96% 환자가항균제치료를받았으며, 83% 에서진단당시중심정맥관을유치하고있었다. Viudes 등 [19] 은미국내 1개 3차병원의칸디다혈증환자를대상으로가능한위험인자를분석한결과, 중환자실입원 69%, 정맥양양요법 66%, 수술 37%, 백혈구감소증 12% 라고보고하였는데, 본연구도전체칸디다혈증환자중중환자실입원 50%, 정맥양양요법 61%, 수술 39%, 백혈구감소증 17% 등으로이와유사하였다. 구미에서칸디다혈증의원인균으로서 glabrata의출현과증가는 krusei와함께백혈구감소증환자에서진균감염을예방하기위한 fluconazole 사용증가로인한결과로알려져있는데 [16], 본성적에서 glabrata 칸디다혈증은다른칸디다혈증에비해진단당시백혈구감소증을보이는환자수가유의하게많았고이전에항진균제사용력이있는경향이있었다. Abi-Said 등 [16] 은칸디다혈증의원인균중 parapsilosis만이카테터관련칸디다혈증과연관되었으며, glabrata는중심정맥관사용과가장관련이없다고하였는데, 본연구에서전체칸디다혈증중중심정맥관관련칸디다혈증으로확진된예는 parapsilosis가가장높았고 (P=0.012), glabrata 칸디다혈증에서빈도가가장낮았다 (P<0.001). parapsilosis는원래인체의손등에상재하는비병원균인데, 최근혈관카테터사용과연관되어칸디다혈증의주원인균이되고있다 [26]. 특히바이오필름을생산하는칸디다균의능력은정맥영양요법을받고있는환자에서카테터관련칸디다혈증발생과연관이있음이최근보고되었다 [27]. 본병원의 parapsilosis 칸디다혈증의경우중심정맥관관련성

5 칸디다혈증의원인균종과임상적특성분석 321 이의의있게높았으며, 칸디다혈증이발생하기이전에항생제치료를받은빈도가낮았다 (P<0.001). 본성적에서칸디다혈증의회복률은 albicans 71%, parapsilosis 90%, tropicalis 72% 및 glabrata 59% 로서 parapsilosis 칸디다혈증에서유의하게높았고 (P=0.004), glabrata에서유의하게낮았다 (P=0.030). parapsilosis 칸디다혈증은대부분카테터관련칸디다혈증으로서다른칸디다혈증에비해예후가좋은것으로이미잘알려져있다 [26-30]. 본연구에서는 glabrata 칸디다혈증의회복률이 59% 로서다른칸디다혈증에비해가장낮았는데, Viscoli 등 [31] 및 Virdes 등 [19] 은 glabrata 칸디다혈증의예후가다른칸디다혈증에비해좋지않다고보고한반면, Pappas 등 [30] 은차이가없다고보고한바있다. 본성적에서 glabrata 칸디다혈증의회복률이다른칸디다혈증에비해낮은이유는진단당시백혈구감소증을보이는예가다른칸디다혈증보다유의하게많았고, 칸디다혈증이발생한이후에항진균제치료를받은환자의비율이 41% 로가장낮은것과연관이있으리라생각된다. 일반적으로보고된칸디다혈증환자의전체사망률은 45-80% 이며, 이중칸디다혈증에의한사망률은약 30% 정도로알려져있다 [19, 28, 29]. 나쁜예후를시사하는인자로는악성종양, 스테로이드치료, parapsilosis가아닌다른원인균, 백혈구감소증등이알려져있다 [19, 30]. 본성적에서칸디다혈증이회복된것으로판단된경우는전체의 75% 로서다른보고보다예후가더좋았는데, 이는본병원칸디다혈증의주원인균이 parapsilosis이며, 악성종양환자와백혈구감소증환자비율이낮고, 또카테터관련칸디다혈증으로진단된예가많기때문으로생각된다 [26-29]. 이상전남대학교병원에서최근 7년간발생한칸디다혈증의임상적특성을원인균별로분석해본결과, albicans와 parapsilosis 칸디다혈증의경우다른칸디다혈증에비해미숙아에서발생하는비율이높았고, glabrata 칸디다혈증은백혈구감소증과연관된반면, 중심정맥관과의관련성이가장적었다. 또한 parapsilosis 칸디다혈증중중심정맥관관련칸디다혈증으로확진된예가많고좋은예후를보임을알수있었다. 이러한성적은다른보고와비교할때, 지역과병원간에칸디다혈증의원인균종은다소차이가있으나칸디다균종별로칸디다혈증발병의선행인자, 중심정맥관관련유무및예후등은유사함을확인시켜주었다. 요약서론 : 최근 Candida albicans 이외에다양한칸디다균종에의한칸디다혈증의발생빈도가증가하고있다. 저자들은전남대학교병원에서발생한네가지주요칸디다균종 ( albicans, tropicalis, parapsilosis 및 glabrata) 에의한칸디다혈증의임상및검사실적특성을원인균별로분석하여보았다. 방법 : 1996년부터 2002년 12월까지전남대학교병원혈액배양 에서칸디다가분리된환자중병력조사가가능했던 157예를대상으로하였다. 157예의원인균주는 albicans (48명), parapsilosis (48명), tropicalis (32명) 및 glabrata (29명) 이었다. 각칸디다균종에의한칸디다혈증의임상및검사실적특성은다른 3종칸디다에의한칸디다혈증의특성과비교하여분석하였다. 결과 : 전체칸디다혈증의 77% 가성인에서발생하였고, albicans와 parapsilosis 칸디다혈증의경우미숙아에서발생하는경우가각각 15% 로다른균종 (0%) 에비해높았다 (P=0.002). glabrata 칸디다혈증은백혈구감소증과연관성 (41%) 이높았으며 (P<0.001), 중심정맥관관련칸디다혈증으로진단된예 (14%) 가적었다 (P<0.001). parapsilosis 칸디다혈증중중심정맥관관련칸디다혈증으로확진된예는 56% 로서다른칸디다혈증에비해유의하게많았으며 (P=0.012), 칸디다혈증이회복된경우도 90% 로서가장많았다 (P=0.004). 결론 : 본연구로서칸디다혈증은원인칸디다균종에따라환자의연령, 기저질환, 카테터관련성및예후등의특성이서로다를수있음을확인할수있었다. 참고문헌 1.
