Purpose: The aim of the present study was to evaluate the effect of urinary catheter on species distribution and susceptibilities of antifungals against clinical isolates of yeasts from catheter-associated urinary tract infection (CAUTI). Materials and Methods: A total 281 yeast isolates from catheterized urine in a medical and surgical ward were collected. Species identification and antifungal susceptibulity test to amphotericin B, fluconazole, voriconazole and flucytosine were performed by VITEK 2 system (biomérieux Inc. Hazelwood, MO, USA). Results: The most frequent species was Candida tropicalis (48.8%), followed by C. albicans (24.6%), C. glabrata (15.7%) and Trichosporon asahii (5.0%). C. tropicalis and T. asahii were more frequently isolated in a surgical ward than medical ward (p<5). Decreased susceptibilities to amphotericin B were observed in C. albicans and T. asahii. All isolates except C. glabrata and C. krusei were susceptible to fluconazole and voriconazole. Conclusions: The results of this study suggest the possibility that urinary catheter may lead to influence on species distribution of yeast of CAUTI. There is an need for continuous surveillance of CAUTI by yeast for the control of CAUTI. 2 74 대한요로생식기감염학회지 : 제 6 권제 1 호 2011 년 4 월 서론요로감염은가장흔한병원-획득성감염으로, 모든병원-획득성감염의약 40% 를차지하고있으며특히집중치료실에서발생하는가장흔한병원-획득성감염이다. 1 요로감염의대부분은요도카테터를내재한환자에서발생하게되며, 무증상의카테터관련세균뇨는특히집중치료실에서내성세균의거대한저장소가되고있다. 2 또한카테터관련요로감염 (catheter-associated urinary tract infection, CAUTI) 은병원-획득성세균혈증의주요원인으로알려져있다. 3 병원-획득성요로감염의원인균은지역이나보고자에따라약간의차이는있지만, 전체적으로 Escherichia coli (E. coli) 가가장많고 Pseudomonas aeruginosa, Enterococcus spp., Klebsiella pneumoniae, Enterobacter spp., Proteus mirabilis 및 Candida spp. 등의순으로알려져있다. 1,4 국내요로감염의원인균에서도 E. coli가가장많았고 Enterococcus spp., Klebsiella spp., P. aeruginosa, coagulase 음성 Staphylococci 등이주로분리되었다. 5,6 또한지난 20년간요로를포함한각종임상검체에서기회감염진균병원체의분리가증가하고있으며, 이중가장흔한병원성진균은 Candida spp. 이었다. Candida spp. 를포함한진균감염의증가는광범위항생제나면역억제제의사용, 요도카테터삽입및혈액암증가등에기인한것으로조사되었다. 7 그리고진균에의한요로감염의빈도증가도광범위항생제투여, 요로유치카테터, 당뇨, 면역억제제투여, 장기입원, 고령, 여성, 정맥내카테터, 수술력등의위험인자와관련이있으며특히광범위항생제투여나지속적요로유치카테터삽입환자에서요로감염의위험이높은것으로보고되었다. 8,9 요로카테터를삽입한환자의절반정도에서 5일이상요로카테터를유지할경우세균뇨나칸디다뇨가발생하는것으로알려져있다. 또한 CAUTI의발생경로는관외의경로로카테터삽입시초기에직접균이접종되거나카테터삽입이후에회음부에있던균이카테터의외부표면을통하 여상행하거나또는폐쇄식배출 (closed drainage) 의기능상실이나소변수집낭의오염으로인한관내경로로설명되기도한다. 10 이에저자들은요로유치카테터가소변에서분리되는효모에미치는영향을평가하고자, 내과와외과진료의모든영역에서지속적으로증가하고있는진균에의한요로감염중특히도뇨검체에서분리된효모의균종분포와항진균제감수성양상을분석하고자하였다. 대상및방법 2009년 1월 1일부터 2010년 7월 31일사이에중앙대학교용산병원에입원중인환자의소변배양검사중도뇨검체에서분리된효모를대상으로하였다. 유치도관을통하여채취한소변은즉시진단검사의학과검사실로보내어혈액한천과 MacConkey 한천배지에 calibrated loop를사용하여 01 ml씩접종하고 37 에서 18 24시간배양하여집락수를측정하였다. 효모양진균이 10 4 CFUs/mL 이상자랐을경우 Sabouraud dextrose agar에계대배양하여사람혼주혈청을사용한발아관시험또는 VITEK 2 ID-YST 시스템 (biomérieux Inc. Hazelwood, MO, USA) 을이용하여제조사의지침에따라균종동정을시행하였다. 항진균제감수성시험은 VITEK-2 시스템의 VITEK AST-YS01 (biomerieux) 카드를이용하여시행하였으며, amphotericin B, fluconazole, voriconazole과 flucytosine의 4가지약제를시험하였다. 즉균농도를 2.0 McFarland 탁도로맞추고, 균부유액의희석과카드로의주입, 트랜스퍼튜브밀봉, 배양, 판독등의전과정은 VITEK-2 장비에서자동으로수행되었다. 각항진균제감수성검사결과는반응이끝난후 VITEK-2의 Advanced Expert System software version VT2-R03.01에의해자동판독후출력되었으며, 최소억제농도 (minimum inhibitory concentration, MIC) 는 Clinical and Laboratory Standards Institute (CLSI) 의기준에따라판정하였다. 즉, amphotericin B에대한감수성과내성은 MIC가각각 1 µg/ml 이하와 2 µg/ml 이상으로판

Species distribution of yeast isolates from catheterized urine specimens in a medical and surgical ward Species (No. of isolates) C. tropicalis (137) C. albicans (69) C. glabrata (44) T. asahii (14) C. laurentii (7) C. famata (5) C. krusei (2) C. parapsilosis (2) Unidentified (1) Medical 79 (42.2) 56 (29.9) 36 (19.3) 5 (2.7) 4 (2.1) 3 (1.6) 2 (1.1) 2 (1.1) - No. of isolates (%) Surgical 58 (61.7) 13 (13.8) 8 (8.5) 9 (9.6) 3 (3.2) 2 (2.1) (1.1) Total (281) 187 (100) 94 (100) p-value 정하였다. 과 voriconazole에대해서는감수성은 MIC가각각 8 µg/ml 이하와 1 µg/ml 이하, 약용량의존감수성 (susceptible dose dependent, SDD) 은 MIC가 µg/ml와 2 µg/ml, 그리고내성은 MIC가 64 µg/ml 이상과 4 µg/ml 이상으로판정하였다. 의경우는 MIC가 4 µg/ml 이하면감수성, 8-16 µg/ml 사이면중간내성, 32 µg/ml 이상이면내성으로판정하였다. 한편동일환자에서 4주이내에동일균종이분리되어 fluconazole MIC가 2 희석배수이내인균주는동일균주로간주하여제외하였다. 통계분석은 Statcalc (Epi Info TM Version 3.5.1, Centers for Disease Control and Prevention, GA, USA) 을사용하여 Chi-square 검증을하였으며, p<5를통계적으로유의한것으로판정하였다. 결과 1. 도뇨검체에서분리된효모의균종분포도뇨검체에서총 281균주의효모양진균이분리되었고, 내과계병동에서 187균주, 외과계병동에서 94균주가각각분리되었다. 총 281균주의균종분포는 C. tropicalis (137균주, 48.8%) 가가장많았으며, 그다음으로 C. albicans (69균주, 24.6%), C. glabrata (69균주, 15.7%), Trichosporon asahii (14균 주, 5.0%) 의순으로분리되었다. 병동에따른효모의분리비율은 C. albicans와 C. glabrata가외과계병동에비해내과계병동에서통계적으로유의하게높았고 (p<5), 반면 C. tropicalis와 T. asahii 는내과계병동에비해외과계병동에서통계적으로유의하게높았다 (p<5) (Table 1). 2. 도뇨검체에서분리된효모의항진균제감수성총 281균주의효모에대한 amphotericin B, fluconazole, voriconazole과 flucytosine의 MIC는각각 µg/ml, - 64 μg/ml, - 8 µg/ml, µg/ml로분포하였으며, 균종별로차이를보였다. Amphotericin B에 100% 감수성인균종은 C. tropicalis, C. glabrata 및기타칸디다균종들이었으며 C. albicans 1균주와 T. asahii 1균주가 amphotericin B에내성을보였다. 과 voriconazole 의경우 C. glabrata와 C. krusei를제외한모든균종에서 100% 감수성을보였다. 에대해서는 T. asahii 5균주와 C. krusei 1균주가중간내성을보인경우를제외하고는다른모든균주에서 100% 감수성이었다 (Table 2). 고찰최근 20년사이에요로에서분리되는기회감염

Antifungal susceptibilities of 281 yeast isolates from catheterized urine specimens, determined by the VITEK AST-YS01 card Species Antifungal agent (No. of isolates) C. tropicalis (137) Amphotericin B C. albicans (69) Amphotericin B C. glabrata (44) Amphotericin B T. asahii (14) Amphotericin B Others (17) Amphotericin B Total (281) Amphotericin B MIC (µg/ml) % of MICs by category Range MIC 50 MIC 90 S SDD R Abbreviation: S, Susceptible; SDD, Dose dependent susceptible; R, Resistant; MIC, Minimum inhibitory concentration 진균병원체가증가하고있으며, 이러한진균에의한요로감염의빈도증가는주로감염에걸리기쉬운환자층의확대와함께요로의진균침범을촉진하거나감염가능성이있는시술들이증가하기때문으로생각되고있다. 요로감염을일으키는진균중카테터삽입또는요로폐쇄환자에서의상행감염에서가장흔한병원체는 Candida spp. 이고, 드물게 Trichosporon spp. Cryptococcus spp. 등이있다. 요로에서분리된 Candida spp. 의균종분포는대부분의연구에서 C. albicans를가장많은원인균으로보고하였으며, 8,9,11 일부연구에서는 C. tropicalis가가장흔히분리되 는것으로보고되기도하였다 저자들의연구에서는 CAUTI 환자의소변에서분리된효모중 C. tropicalis가 48.8% 로가장많았고, 그다음으로 C. albicans가 24.6%, C. glabrata가 15.7% 의순으로분리되었다. 이러한결과는소변에서분리된 Candida spp. 의균종분포에관한국내의이전보고와일치하고있으며, 14,15 소변을제외한다른부위에서는모두 C. albicans가가장흔히분리되는것으로보고되고있어검체종류에따른효모의균종분포차이라고생각한다. 저자들은효모의균종분포를내과계와외과계병동 CAUTI 환자의소변에서분리된경우를나누

C. albicans와 C. glabrata는외과계병동보다내과계병동에서많이분리되었고, C. tropicalis와 T. asahii는내과계병동보다외과계병동에서많이분리되었다. 지금까지 Candida spp. 에의한병원-획득성요로감염환자에서의위험인자분석에서항생제치료, 요로유치카테터, 동반감염, 긴재원기간, 운동제한, 고령, 발열, 중심정맥관삽입등이주위험인자로알려져있으며, 8,9 특히 Candida spp. 의균종에따른위험인자비교에서 C. tropicalis에서수술력이통계적으로유의한차이를보이는위험인자로보고되었다. 14 또한심장수술후비강과손에 C. tropicalis를보균하고있는의료인에의한수술부위감염의유행보고도있어, 수술과관련하여 C. tropicalis에의한감염이발생할수있음을시사하고있다. 16 그러므로요로감염과관련된위험인자들은수술과관련된외과계병동의도뇨검체에서 C. tropicalis의분리비율이높은저자들의연구결과를뒷받침하는내용으로생각하며, 위험인자들은소변에서분리되는진균의균종분포에도영향을줄수있을것으로생각한다. 본연구의도뇨검체에서 4번째로많이분리된 T. asahii는다른 Trichosporon spp. 가대부분표재성피부병변에서분리되고있는것에비해전신성진균감염에서좀더흔한것으로알려져있다. 17 과거에는 Trichosporon spp. 를거의대부분 T. beigelii로보고하였는데, 최근광범위한분류의재평가를통하여사람에서감염을일으킬수있는 Trichosporon spp. 로 T. asahii, T. asteroides, T. cutaneum, T. inkin, T. jirovecii, T. mucoides, T. ovoides 등의 7종류가관련되는것으로알려졌으며, 17 일부연구에서는소변에서분리된진균의 3% 를차지한다고보고하였다. 18 또한 20 여년전에 T. beigelii를 amphotericin B 에내성을갖는새로이출현한병원체로보고하였는데, 19 분류재평가후 T. asahii가특히 amphotericin B에내성이많은것으로확인되고있다. 17 저자들의연구결과에서도 T. asahii가도뇨검체에서분리된효모양진균의 5% 를차지하였고, 다른효모에비하여 amphotericin B에내성으로나타났다. 따라서본연구의결과를의미있게간주하고추후 T. asahii의빈도, 항진균제감수성양상및내성기 전, 카테터삽입등과같은위험인자에관한조사가필요할것으로생각한다. 진균중효모는임상검체에서분리빈도가높고증식이빠르며세균과비슷하게집락형성을하기때문에항진균제감수성검사에대한표준화된검사법이개발되어있고, 많은검사실에서시행하고있다. 물론아직까지효모에대한항진균제감수성검사가일상적검사로는권장되지않으나, 효모특히 Candida spp. 에의한중증감염의발생이점차증가하고있어생체외항진균제감수성검사의필요성이증가하고있다. 20 저자들의연구에서는최근자동화된장비를사용하여항진균제감수성검사를시행하는 VITEK-2 시스템의 VITEK AST-YS01 (biomerieux) 을이용하였다. VITEK-2 시스템을이용한항진균제감수성검사는재현성이좋고표준화된검사법인 CLSI M27법과도일치율이매우높은것으로보고되어있다. 21,22 도뇨검체에서분리된효모의 amphotericin B, fluconazole, voriconazole과 flucytosine에대한항진균제감수성결과는지금까지알려진균종별항진균제감수성결과와유사하였다. 즉 C. krusei와 C. glabrata가 azole계약제인 fluconazole과 voriconazole 에대한 MIC가증가되었고, T. asahii 1균주와 C. albicans 1균주에서 amphotericin B에내성을보였다. 이는지금까지보고된소변에서분리된효모의대부분이요로카테터를삽입한환자에서분리되었기때문으로생각되며, 효모의균종별로항진균제감수성양상을예측할수있을것으로생각한다. 그러나지속적으로진균병원체의분리빈도가증가하고있고일부균종에서항진균제에대한내성을보이고있으므로, 효모가분리되는임상검체중많은부분을차지하고있는도뇨검체를대상으로한효모의균종분포와항진균제감수성양상에대한지속적인감시가필요할것으로생각한다. 결론요로에서분리되는효모양진균의대부분은 Candida spp. 이고이중가장흔한균종은 C. albicans로알려져있다. 그러나 CAUTI 환자의도뇨검

6 78 대한요로생식기감염학회지 : 제 6 권제 1 호 2011 년 4 월 체에서분리된효모를대상으로한저자들의연구에서는 C. tropicalis가가장많이분리되었다. 특히외과계병동에서더욱많이분리되었으며, amphotericin B에내성이많은것으로알려진 T. asahii도 4번째로많이분리되었다. 그러므로지속적으로증가하고있는진균감염분야에서요로카테터가효모의균종분포에영향을줄가능성이있다. 따라서, 효모에의한병원-획득성요로감염을감소시키기위하여요로카테터삽입과관련한소변내효모의균종분포와항진균제감수성양상을감시하기위한지속적인연구가필요할것으로생각한다.
