And the rates of rotavirus gastroenteritis among children in industrialized and less developed countries are similar. Therefore rotavirus vaccine would have universal application as part of immunization program. And cost-effectiveness analyses have shown that rotavirus immunization program would be cost-effective from the perspective of society and the health care system. On the base of a number of effective clinical trial in infants worldwide, 2 recently developed vaccines, pentavalent human-bovine reassortant rotavirus vaccine(rotateq R)and human rotavirus vaccine(rotarixtm) were licensed in the United States and Korea, recently. These 2 vaccines are high efficacy, excellent tolerability, and no increased risk of intussusception in each clinical trials of more than 60,000 infants. Vaccination against rotavirus will substantially reduce rotavirus gastroenteritis associated morbidity and mortality. Key word : Rotavirus gastroenteritis, Rotavirus vaccine 서론 1) 로타바이러스는영유아설사의가장흔한원인으로 5 세이하에서호발하고, 사회환경이나개인위생의 개선으로발병을억제할수없어개발도상국은물론 교신저자 : 김창휘주소 : 경기도부천시원미구중동 1174, 순천향대학교부천병원 Tel : rosekwon@sch.ac.kr 접수일 : 2008년 10월 2 일 / 게재확정일 : 2008년 12월 20일 선진국에서도소아설사의가장흔한원인이되고있 으며유병률의차이가없다. 따라서로타바이러스위 장염을예방하기위해서는백신개발만이최선의방 법이다. 그동안여러가지백신이개발되었지만현재전세 계적으로 MSD의 5가사람-소로타바이러스재배열 백신 (Pentavalent human-bovine reassortant rotavirus vaccine, RotaTeq R) 과 GSK의 1가사람로타바이러

2 144 순천향의과학 : 제14권 2호 2008 스백신(Human rotavirus vaccine, RotarixTM) 이접 종되고있으며최근국내에서도이 성약화로타바이러스백신이접종되고있다. 원인 2가지경구용독 로타바이러스는 70nm의비피막형이중나선 RNA 바이러스로 Reoviridae 과에속한다. 로타바이러스의 전파경로는확실하게알려져있지않으나공기전파 또는인체접촉에의한감염이많은것으로생각하고 있다. 로타바이러스위장염은임상증상이콜레라와비슷 하여국내에서는가성콜레라로불리워오다가 1973 년호주의 Bishop 등 1) 이원인바이러스를처음발견 하여전자현미경으로보면바이러스의모양이수레바 퀴와비슷하다고하여로타바이러스위장염으로불리 우게되었다. 로타바이러스는내피막 (inner capsid), 중간피막 (middle capsid) 과외피막(outer capsid) 등 3개층의피막이 11개의 RNA 분절을둘러싸고있다. 내피막은 VP2, 중 간피막은 VP6, subgroup antigen 그리고외피막은 VP4(protease-clevaed protein, P protein) 와 VP 7(glycoprotein, G protein) 으로구성되어있다(Fig. 1). 로타바이러스의혈청형은 G형과 P형을같이표기한 다. G형은혈청형과유전자형이같기때문에 G1과 같이혈청형을숫자로표기한다. P형은혈청형과유전 자형이같지않기때문에혈청형은숫자로표기하고 유전자형은 [ ] 안에숫자로표기한다. 예를들어 P1A[8] 의 1A은 P 혈청형, [8] 은유전자형을의미한다. G형은적어도 15 가지, P형은적어도 26가지가보 고되고있으며, G1, G2, G4는대부분 P1A[8] 과, G2 는 P1B[4] 와결합되어있다. 사람에게서는 G4P1A[8] 이대부분을차지한다. 2) G1P1A[8], G2P1P[4], G3P1A[8] 과 2005년 4월부터 2006년 3월까지국내 8개병원에 입원한로타바이러스위장염환아의 639개검체검사 결과 G1P[8] 46.9%, G2P[4] 10.4%, 그리고 G3P[8] 이 15.7% 이었다. 반면 2002년 7월부터 2004년 6월까지전북정읍에 서발생한로타바이러스위장염환자에게서는 G9P[8] 39%, G1P[8] 24%, G3P[8] 17%, G2P[4] 13%, 기타가 7% 이었다. 3) 그리고 질병관리본부에서발표한 2005년 2007년국내로 타바이러스위장염환아의유전자형(Fig. 2) 은 2005년 에는 399례중 G3P[8] 27.1%, G2P[4] 22.6%, G4P[6] 15.5%, G1P[8] 이 13.0%, 2006년에는 280례중 G1P[8] 과 G3P[8] 각각 25.4%, G4P[6] 17.5%, G2P[4] 가 13.2% 이었으며, 2007년에는 491례중 G1P[8] 53.4%, G4P[6] 9.8%, G3P[8] 8.6%, G2P[8] 2.8%, 그리고 G2P[4] 가 2.0% 로발생시기와지역에따라많은차이 를보여주고있다. 4) 역학 Fig. 1. 4) 백신 로타바이러스백신에대한연구는 1970년대부터시 작되었으며, 백신개발의궁극적인목표는자연감염 과같은면역원성을얻어중등도또는심한로타바이 러스위장염을예방하는데있다. 로타바이러스에처음감염된 후얻은방어면역은 다음감염시혈청형에관계없이중등도또는심한로 타바이러스위장염을거의 100% 예방한다. 8) 또한로 타바이러스에반복감염되면다른혈청형에대한방어 력도생기게된다.

Genetic distribution of rotavirus in Korea, 2005~2007 명이사망한다. 6) 유럽연합국가에서는 5세이하소아중 700,000여명 이외래에내원하며 87,000 여명이입원하고, 200여명 이사망한다. 7) 우리나라의경우 2000년부터질병관리본부에서실 시하고있는설사환아의원인분석결과를보면바 이러스위장염의 60% 이상이로타바이러스이었으며, 2005년부터 2007년까지국내로타바이러스위장염의 주간별발생현황은 Fig. 3 과같다. 4) 백신 로타바이러스백신에대한연구는 1970년대부터시 작되었으며, 백신개발의궁극적인목표는자연감염 과같은면역원성을얻어중등도또는심한로타바이 러스위장염을예방하는데있다. 로타바이러스에처음감염된 후얻은방어면역은 다음감염시혈청형에관계없이중등도또는심한로 타바이러스위장염을거의 100% 예방한다. 8) 또한로 타바이러스에반복감염되면다른혈청형에대한방어 력도생기게된다. 혈청 IgA 항체가로타바이러스감염예방에중요한 역할을하는것으로알려져있다. 9) 1998년미국 Wyeth-Lederle 사에서개발한 4가원숭 이- 사람유전자재배열백신(Rhesus-human rotavirus reassortant tetravalent vaccine, RRV-TV, Rotashield R) 이미국 FDA의접종허가를받아 2, 4, 6개월에경구 접종하였으나접종후장겹침증(intussusception) 때문 에 1999년 10 월생산을자진중단하게되었다. 10) 최근에사람과소에서분리된로타바이러스를재배 열하여약독화시킨 5 가백신(MSD의 RotaTeq R) 과 사람에게서분리된로타바이러스 G1P1A [8] 를악독화 시켜만든 1 가백신(GSK의 RotarixTM) 이미국 FDA 와우리나라식품의약품안전청의접종허가를받아접 종되고있다. 현재국내에서접종되고있는경구용독성약화로 타바이러스백신은 1. 면역원성 Table 1 과같다. 1) 5가사람- 소로타바이러스재배열백신(Pentavalent human-bovine reassor-tant rotavirus vaccine, RotaTeq R) Fig. 3. The bovine-human reassortant rotavirus vaccine contains five different reassortant strains made by combing bovine strain C3 with human strains representing serotypes P1, G1, G2, G3, G 년 11개국에서생후 6 12 주영아 70,301 명 ( 접종군 34,035 명) 을대상으로한이중맹검위약대 조 3상연구결과 3번접종후로타바이러스위장염 에대한예방효과는 74.0% 이었고, 심한로타바이러스 위장염에대한예방효과는 바이러스위장염의진단기준은 98.0% 이었다. 12) 심한로타 Table 2와같이 Clark 점수체계(Clark scoring system) 를이용하여 24점 중 16점이상이면심한로타바이러스위장염으로진 단하였다. 13) G형청형에대한예방효과는 G1 75.0%, G2 63.4%, G3 55.6%, G4 48.1% 그리고 G9는 74.1% 이었으며, 연구기간동안 G1 상대적으로적었다. 이연구에참여한영아중 이외의혈청형에대한발생빈도가 5,673명을대상으로연구 한결과외래내원은 86% 가감소하였으며, 68,038명 을대상으로연구한결과에서는 2년간로타바이러스 위장염으로인한응급실내원 93.7%, 입원은 95.8% 가 감소하였으며, 원인에관계없이위장염으로인한입원 은 59% 가감소하였다. 접종후 대한예방효과는 에대한예방효과는 2번째유행시기에로타바이러스위장염에 62.6%, 심한로타바이러스위장염 88.0% 이었다. 12) 2) 1 가사람로타바이러스백신 (Human rotavirus vaccine, RotarixTM) 1가로타바이러스백신은 1988~1989 년미국 Cincinnati 에서로타바이러스유행시기에감염된영아에게서분 리된 G1P1A[8] 주로부터유래한 89~12로명명된바

The Clark Severity Scoring Scales for the Evaluation of Gastroenteritis Symptom Score Diarrhea No of stools/day Duration/days Vomiting No of emeses/day Duration/days Rectal temperature Temperature( ) Duration/days Behavioral symptoms/signs Irritable/less layful Lethargic/listless Convulsion Duration/days * Severe: GE episode with a score of > 16 points in 24-point Clark scale 이러스주를이용하여개발하였다. 2003년 8월 2004년 3월까지라틴아메리카와핀란 드등 11개국에서영아 63,225 명( 접종군 31,673 명) 을 대상으로한이중맹검위약대조 한로타바이러스위장염에예방효과는 대한예방효과는 85.0% 이었다. 14) 3상연구결과심 84.7%, 입원에 G1P[8] 에의한 Vesikari 척도(Vesikari scale, Table 3) 11 점이상의심한로타바이러스위장염 15) 에대한예 방효과는 90.8% 이었으며, G3P[8], G4P[8] 와 G9P[8] 에 의한심한로타바이러스위장염에대한예방효과는 86.9% 이었다. 반면 G2P[4] 에의한예방효과는 45.4% 이었으나이기간동안로타바이러스위장염의발생이 적어통계적으로유의성은없었다. 연구대상영아 63,225명중 14,237명에대하여접 종 2주후부터 24개월까지추적관찰한결과첫번째 유행시기에로타바이러스위장염에의한입원예방효 과는 81.5%, 2번째유행시기에는 81.5% 이었으며, 2년 간평균예방효과는 83.0% 이었다. 16) 최근유럽 6개국에서영아 3,994명을대상으로한 이중맹검위약대조연구결과첫번째유행시기에 로타바이러스위장염과심한로타바이러스위장염및 입원에대한예방효과는각각 87.1%, 95.8% 와 100% 이었으며, 2번째유행시기에는 71.9%, 85.6% 와 92.2% 이었다. G1, G3, G4, G9 및 G2에의한심한로타바 이러스위장염의예방효과는첫번째유행시기에 %, 2번째유행시기에는 % 이었으며, 2년간평균예방효과는 % 이었다. 임상 2상과 3상연구결과를메타분석해보면 G2 에대한예방효과는 2. 모유수유및동시접종 RotaTeqR과 81% 이었다. 18) RotarixTM 17) 모두모유수유를하여도 면역원성에영향을받지않으며, 개량 DTP, 주사용 폴리오, b 형헤모필루스인플루엔자, B형간염및폐 구균백신과동시접종이가능하다. 6,19)

7 김창휘 : 로타바이러스백신의최신지견 149 종후평균 100일까지관찰했을때접종군에서 9 명, 대조군에서 2) 16 명이발생하여차이가없었다. 14) 기타이상반응 RotaTeqR을접종한 11,722명을대상으로접종후 42 일간관찰한결과발열구토(15% vs. 14%), 설사 (24% vs. 21%), 인두염(7% vs. 6%), 중이염(25% vs. 13%), 기관지경련(1.1% vs. 0.7%), 그리고발열(43% vs. 43%) 이었다. 6) RotarixTM 를접종한후발열, 설사, 보챔, 식욕 부진, 기침과콧물등의이상반응은대조군과차이가 없었다. 19) Finland 임상연구에서도백신접종군과대 조군사이에발열, 설사, 구토등의이상반응발생의 차이가없었다. 21) 3) 백신바이러스의배설 RotaTeqR 을접종한후대변에서백신바이러스 를검사한결과 1차접종 360명중 32 명, 2차접종 249명중 0 명, 그리고 3차접종 385명중 1명에게서 백신바이러스가검출되었다. 백신바이러스는대부분1차접종 7 일이내에검출되었다. 6) 라틴아메리카에서 RotarixTM를접종한 2,155명을 대상으로한제 2상연구에서 1차접종후 7일에 35 44%, 2차접종후 7일에 11 21% 에서백신바이러스가검출되었다. 19) 지금까지언급한국내에서접종되고있는 2가지독 성약화로타바이러스경구용백신의특징은 Table 4 와같다. 22) 미국에서는 가지백신이접종되고있어 RotaTeq R(MSD) 과 RotarixTM(GSK) 년예방접종심의위원 회 (Advisory Commmittee on Immunization Prac- tices: ACIP) 에서 Table 5와같이접종방법을변경하 였으며, 교차접종도허용하였다. 23) 그러나대한소아과 학회감염위원회에서는교차접종에대한충분한연구 결과가나올때까지교차접종을허용하지않기로하였 다 년대한소아과학회의로타바이러스백신접종에대한지침은다음과같다. 24) 대한소아과학회의로타바이러스백신접종지침(2008 년) 1. 접종대상및시기로타바이러스감염의위험과질병의심한정도, 백신접종에따른효과와비용및위험등을보호자와상의한후에접종여부를결정한다. 접종을할경우에는다음과같은방법으로접종한다. 1) 5 가로타비이러스백신: 2, 4, 6개월 2) 1 가로타비이러스백신: 2, 4개월 * 1차접종은생후 6 주부터시작할수있으며, 생후 15 주이후에는접종을시작하지않는다. * 8개월 0 일(34주 6 일) 까지접종을완료한다 접종용량및방법 1) 5 가로타바이러스백신: 2 ml 경구투여 2) 1 가로타바이러스백신: 동결건조백신을같이들어있는용매 1 ml에녹여경구투여 이상반응 경미하다( 발열, 설사, 구토, 혈변등) 허가전임상연구에서장겹침증발생과무관해보이나향후허가후자료가더필요하다. 금기사항및주의사항 1) 백신성분에대한심한알레르기반응 2) 면역기능저하자 3) 급성위장염 4) 중증질환 5) 기존의만성위장관질환 6) 장겹침증
