Nearly half of irregular antibodies with specificity are related to Rh antigens in Korea. Formation of alloantibody for red blood cells is considered variable according to Rh phenotype of patients. We therefore studied the significance of Rh phenotype in Korean irregular antibody positive patients. Methods: We performed retrospective reviews for the results of antibody identification tests performed from Jun to Nov in our university medical center. Rh phenotype, direct antiglobulin test, and antibody specificity were investigated. Rh phenotype was tested using RhD+ phenotype ID-card (DiaMed GmBH, Switzerland). Results: A total of 504 patients were included. Of 504 patients, 495 (98.2%) were RhD positive. The proportion of Rh phenotype differed significantly between irregular antibody positive patients and known RhD positive Korean population in CDe phenotype (59.0% vs 39.4%, P<0.0001) and CcDEe phenotype (22.6% vs 38.4%, P<0.0001), respectively. The percentage of other Rh phenotype was not different in two groups. Formation of anti-e antibody in E negative patients was significantly higher than that of anti-c formation in C negative patients (P<0.0001). Sixteen patients showed antibodies with specificity for their own Rh system antigens. Conclusion: A significant disproportion of Rh phenotype was observed between irregular antibody positive patients and RhD positive Korean population. There would be a difference of immunogenicity among C/c and E/e antigens. E antigen matching might be considered first for patient required chronic transfusion if additional RBC matching would be implemented. 2 김경희외 : 한국인비예기항체양성군의 Rh 표현형분포 서론 대상및방법 Rh 혈액형항원은 ABO 혈액형다음으로수혈의학분야에서가장중요한항원으로면역원성이강하고많은다형성을보인다. 현재까지 50여개이상의 Rh 혈액형항원이알려져있으며 D, C, c, E, e 항원이중요한항원이다. Rh 혈액형항원은인종또는지역에따라다양한분포를보인다. 백인중 RhD 음성은 16% 정도이나한국인중 RhD 음성은 0.1% 로뚜렷한차이가있다. 1,2) 한국인에서 Lewis 항원에대한항체를제외하고, 항-E 항체는가장흔히발견되는비예기항체이며, 특이성이동정된비예기항체의약절반이 Rh 항원과관련되어있다. 3,4) 하지만동종면역연구에서비예기항체의특이성은인종간의차이를보여스페인에서실시된연구에서는 Kell 항원에대한항체가가장흔하였다. 5) RhD 항원은수혈전검사로포함되어 RhD 항원이적합한수혈을실시하고있지만 C/c나 E/e 항원에대해서는수혈전검사를하지않는다. D, C, c, E, e 항원의항혈청반응에기초한 Rh 표현형은 RhD 양성인경우에는 9가지, RhD 음성인경우는 6가지가알려져있다. 한국인에서가장흔한 Rh 표현형은 CDe로써약 40% 정도를차지한다. 다음은 CcDEe 표현형이 34 38% 정도의빈도를보인다. 2,6) 한국인비예기항체양성환자에서 Rh 표현형의빈도에대해서는알려진바가없다. 그래서본연구에서는비예기항체양성환자군과기존에알려진 Rh 표현형빈도와의차이가있는지알아보고자하였다. 1. 대상 2004년 6월 21일부터 2013년 11월 30일까지동아대학교병원혈액은행으로비예기항체동정검사가의뢰된총 659검사, 617명을대상으로 Rh 표현형검사, 직접항글로불린검사, 비예기항체선별검사및동정검사의결과를조사하였다. 반복검사가실시되었던경우가 36명이었다. 그외에불충분한정보, Rh 표현형의결과가다른경우, 면역글로불린치료, 외국인, 신생아용혈성질환, Rh 표현형의검사결과가없었던경우를제외하여총 504명의환자가최종연구대상이되었다 (Fig. 1). 비예기항체동정검사결과는동정된중요한비예기항체와범혈구응집 (panagglutination), 특이성이동정되지않은경우등을조사하였다. 비예기항체양성환자군 504명의 Rh 표현형빈도를 RhD 양성 409개혈액검체의연구결과 6) 와비교하였다. 비예기항체양성환자군의 Rh 표현형에따라동정된비예기항체, 범혈구응집, 특이성이동정되지않은경우의빈도를분석하였다. Rh 표현형에따라항-E 항체및항-C 항체의빈도차이를조사하였고, E 항원음성인 Rh 표현형조합과 C 항원음성인 Rh 표현형조합에서각각항-E 항체와항-C 항체의빈도를구하였다. 환자의 Rh 표현형과비교해서자가 Rh 항원에대한 Rh 항체를가진환자가있는지도확인하였다. 2. 방법 1) Rh 표현형검사 Rh 표현형검사는 RhD+ phenotype ID-card (DiaMed GmBH, Switzerland) 을사용하였으며, 검사법은제조사가제시하는방법을따랐다

Study flow diagram. 2) 직접항글로불린검사 직접항글로불린검사는 DC-Screening I gel card (DiaMed GmBH, Switzerland) 를사용하여제조사의방법에따라시행하였다. 3) 비예기항체선별검사및동정검사 항체선별검사는 LISS/Coombs card (DiaMed GmBH, Switzerland) 와 ID-DiaCell I+II (DiaMed GmBH, Switzerland) 를이용하였고, 동정검사는 LISS/Coombs card와 NaCl/Enzyme card에동정혈구로는 ID-Dia Panel 1-11 (DiaMed, GmBH, Switzerland) 을이용하여제조사의방법을따랐다. 3. 통계 비예기항체양성환자군와기존에보고되었던 RhD 양성군과의 2,6) Rh 표현형빈도차이는 Chi-squared test를이용하였으며, 분석은 Medcalc 프로그램 (MedCalc Software ver12.6.1, Ostend, Belgium) 으로하였다. P value가 0.05 미만인경우 통계적으로유의한것으로간주하였다. 결과 1. 비예기항체양성환자와 RhD 양성군간의 Rh 표현형빈도차이 총 504명의연구대상환자중에서 9명이 RhD 음성이었고, 495명의 RhD 양성환자에서 Rh 표현형의빈도를조사한결과 CDe 표현형이 59.0% 로가장많았다. 그다음은 CcDEe 표현형으로 22.6% 를차지하였다. 빈도순서는 RhD 양성군과일치하였으나 RhD 양성군과비교해서 CDe 표현형은통계적으로의미있게높았으며 (P<0.0001), CcDEe 표현형은의미있게낮은빈도를보였다 (P< )(Table 1). 나머지 cde, CcDe, cdee 표현형등은비예기항체양성환자군과 RhD 양성군간의빈도차이는없었다

4 김경희외 : 한국인비예기항체양성군의 Rh 표현형분포 2. Rh 표현형에따른비예기항체특이성과특징 동정되는항체의특이성은 Rh 표현형에따라 명확하게구별되었으며, 항-E가가장높은빈도 (278/504, 55.2%) 로동정되었다. 표현형에따라예상할수있는 Rh 항체가대부분동정되었으며, CcDEe 표현형에서동정된항체의종류가가장 Table 1. CDe 표현형에서는항-E 항체가 80.8% (236/292) 의높은빈도로동정되었다. 그러나 cde 표현형에서항-C 항체가동정된빈도는 57.1% (24/42) 로낮아통계적으로의미있는빈도의차이를보였다 (P=0.0011)(Table 3). 항-E 항체의빈도는 CDe, CcDe 표현형간에차이가없었으며 (P=0.6), 항-C 항체의빈도도 cde, cdee 표현형간에차이가없었다 (P=0.35). E 항원음성 Rh 표현형을가진 331명중 80.4% 에서항-E 항체발생을보여, C 항원음성 Rh 표현형을가진환자 들의항 -C 항체발생률보다의미있게높았다 (P <0.0001)(Table 4). 3. Rh 표현형에따른범혈구응집 (panagglutination) 과특이성이동정되지않은항체의빈도 범혈구응집과특이성이동정되지않은항체의빈도는 CcDEe 표현형의경우각각 17.0%, 40.2% 로가장높았으며, CDe 표현형과비교해서통계적으로도의미있게높았다 (P<0.0001)(Table 5). Table 3. CcDEe 표현형인경우가가장많았으며, 직접항글로불린검사양성인경우가 14명 (87.5%) 이었다. 관련된질환은종양이 6명이었고, 자가면역성질환이 2명이었다 (Table 6). 고찰 본연구는 504명의비예기항체양성환자와 RhD 양성군의 Rh 표현형빈도의차이를알아보고그임상적의의를살펴보고자한것이다. Rh 혈액형항원은인종또는지역에따라다양한분포를보이며, 그분포를아는것이혈액수급및 관리에많은도움이된다. 그래서주로헌혈자를대상으로 Rh 항원의빈도에대한연구가이루어져있으며 7,8) 비예기항체양성환자를대상으로한 Rh 표현형연구의보고는드물다. 지중해빈혈이나겸상적혈구빈혈증등만성적으로수혈이필요한환자들을대상으로 Kell 항원과 Rh항원을일치시켜수혈하고자노력해왔으며, 동종면역을예방하는데효과적이라는보고들이있다. 9-11) 그러나겸상적혈구빈혈증환자에서 Rh 항원이일치된혈액을수혈했음에도높은빈도의적혈구동종면역이발생하였다는연구결과도있어, 12) Rh 항원의다형성이적혈구동종면역의감소에장애가되고있으며추가적인연구가필요한실정이다. 다행히한국인은 KEL*1 대립유전자보유비율이 0% 이어서국내 K 항원보유율이 0% 이므로서구처럼 Kell 항원에대한높은빈도의동종항체가발생할가능성은매우낮

7 Korean J Blood Transfus Vol. 25, No. 1, 60-68, Apr 다. 13) 그러므로만성적으로수혈이예상되는환자를대상으로 Rh 표현형검사를실시하여 RhD 이외에도 C/c, E/e 항원이적합한혈액을수혈하는정책에대한연구가요구된다. 비예기항체양성환자의 Rh 표현형빈도순서는기존보고되었던 RhD 양성군과차이가없었으나, CDe 표현형의빈도는의미있게높게나타나전체대상환자의약 60% 를차지하였다. 또한 CcDEe 표현형빈도는의미있게낮게나타났으며비예기항체의특이성도가장다양하게나타났다. CDe 표현형에서생성될수있는 Rh 계열동종항체가항-E 항체와항-c 항체임을감안하면, 본연구에서항-E 항체의빈도가가장높은것과일치하는소견이다. 항-E 항체는 2011년국내 31 개병원에서보고한비예기항체중에서항-Le a 항체를제외하고가장흔한동종항체이다. 3) 비예기항체양성환자에서항-C 항체나항-E 항체가생성가능한 cde, CcDe, cdee 표현형의빈도는 RhD 양성군과차이가나지않았다. 이를통해서특정 Rh 표현형을가진환자가 Rh 비예기항체발생의가능성이증가함을알수있다. 항-E 항체생성이가능한 E 항원음성 Rh 표현형조합의환자군에서항-E 항체발생률은 C 항원음성 Rh 표현형을가진환자보다통계적으로유의미하게높았다. 또한 RhD 양성군에서 C 항원양성인경우는대락 86% 정도로계산할수있는데 cde 표현형을가진환자의빈도는 RhD 양성군보다적게분포하고있어 C/c, E/e 항원간에도면역원성의차별을보이는것으로사료된다. 최근발표된겸상적혈구빈혈증환자의동종면역예방을위해적혈구항원이일치된수혈을위한여러방안중 C, E, K 항원만을제한적으로일치시키는방안도고려되었다. 14) 추가적인연구가필요하겠지만한국인에서는 C 항원보다 E 항원음성인환자인경우 Rh 계열동종면역의위험성 이증가함을알수있다. 본연구로토대로하면만성적으로수혈이예상되는환자들에게 ABO, RhD 이외의적혈구항원이일치된혈액을수혈하는정책을세운다면, E 항원음성혈액을우선적으로공급하는것이비용효율적일것이다. 자가 Rh 항원에대한 Rh 항체를가진환자가 16명 (3.2%) 에서관찰되었는데이는자가항체이지만유사특이성을가진항체 (Autoantibody with mimicking specificity) 와비슷한현상으로설명할수있다. 15,16) 온난자가항체를가진 71명의환자중 19명 (26.8%) 에서유사특이성을가진자가항체를보이며, 대부분이 Rh 계열의특이성을가진다는연구결과가있다. 15) 본연구에서도 14명 (87.5%) 의환자에서직접항글로불린검사양성으로자가항체임을추측해볼수있다. 이러한현상은검사의해석에혼란을야기하여주의를요하지만, 본연구를통해드물지않게나타나는것임을알수있겠다. 한국인의비예기항체양성환자에서 RhD 양성군과비교해서 CDe 표현형의빈도는의미있게높게나타났으며, E 항원음성인비예기항체양성환자군에서의항-E 항체발생률은 C 항원음성인경우의항-C 항체발생률보다높았다. C/c 및 E/e 항원간의면역원성에차별성이있음을추측할수있고, 향후만성적수혈이예상되는환자에게 ABO, RhD 이외의혈액형항원음성혈액을제공할때 E 항원음성혈액의우선적공급이고려되어야할것이다. 요약 배경 : Rh 항원은 ABO 혈액형다음으로수혈의학에서중요한혈액형항원이다. 한국인에서특이성을가진비예기항체의절반이 Rh 항원과관련되어있으며, Rh 표현형에따라적혈구동종항
