Anti-D immunization is clinically important since it may cause clinical problems, such as severe hemolytic transfusion reactions and hemolytic disease of the newborn. DEL is an extremely weak D variant that cannot be detected by basic serologic typing and is typed as D-negative without the absorption-elution techniques and RHD genotyping. Of the DEL phenotype, RHD (c.1227g>a) allelic variant is the most common in Korea. The DEL phenotype has been considered to carry only a few D antigens to induce anti-d immunization, but a few cases have reported that this allelic variant is capable of inducing anti-d immunization in a D-negative recipient, for which it is clinical significant. Herein, we present a case of primary anti-d alloimmunization in a RhD negative patient after receiving RHD (c.1227g>a) DEL red cell transfusion identified by serological and molecular tests, including RHD genotyping. 2 Korean J Blood Transfus Vol. 27, No. 2, , Aug 재가증명되는 D항원이극소량으로존재하는유형이다. 2) 과거에는 DEL형적혈구는항원의수가적어 D 음성환자에게수혈해도항-D항체를생성하지않는다고생각되었으나 2) 최근에수혈후항-D항체가생긴증례가국내외로보고된바가있어 DEL형의정확한검출이강조되고있다. 1,3-5) 저자들은 D 음성환자에서수혈후항-D항체가관찰되어혈청학적검사및유전자분석을실시하였고 DEL형수혈에의한항-D항체형성임을확인한증례를경험하였기에보고하는바이다. 증례 54세의난소암을가진여자환자의 EDTA 검체가수혈전검사로의뢰되었다. 환자는수혈의뢰 2개월전농축적혈구 2단위를수혈받은과거력이있었으며이때수혈전시행한비예기항체선별검사는음성이었다. 1. 혈청학적검사 환자의혈액형은 B형 D 음성이었으며 RhCE 표현형검사에서 ce형으로나타났다. 수혈전검사로시행한비예기항체선별검사 (QWALYS-3, DIAGAST, Loos, France) 에서양성을보여시행한비예기항체동정검사 (Resolve Panel A, Ortho Clinical Diagnostics, Raritan, USA) LISS/Coombs 단계에서항-D 항체가동정되었다. 자가대조검사는음성이었으며직접항글로불린검사또한음성이었다. 환자는 2개월전수혈받은이후수혈을받은적이없었으며면역글로불린을투여받은과거력도없었다. 2. 3 윤정외 : DEL 형적혈구수혈에의한항 -D 항체발생 액수혈연구원에의뢰하였다. Luettringhaus 등 6) 과 Kim 등 7) 이보고한바를바탕으로, 한국인의 RHD 유전자분석에필요한 promoter, 엑손 4, 엑손 7, 엑손 10을표적으로하여 RHD 유전형검사를시행하였다. EDTA 전혈검체에서 MagNa Pure DNA isolation kit I (Roche Diagnostics GmbH, Mannheim, Germany) 를이용하여 DNA를추출한후, promoter 와엑손 7은 Wagner 등 8) 이보고한방법으로각각 re012, re011d 시발체와 ga71, ga72 시발체를이용하였고, 엑손 4와엑손 10은 Aubin 등 9) 이보고한방법으로각각 D8, D9 시발체와 D4, D5 시발체를이용하여 DNA를증폭하였다. PCR 조건은 95 o C 에서 10분, 40 주기의증폭싸이클 (95 o C에서 30초, 엑손 4와엑손 10의경우 50 o C에서 30초, promoter 와엑손 7의경우 57 o C에서 30초, 72 o C에서 30초 ) 후마지막으로 72 o C에서 7분반응시킨후 Lab Chip GX (Caliper Life Sciences, Hopkinton, USA) 을통해증폭산물을확인하였다. 검사결과첫번째헌혈자는 promoter, 엑손 4, 엑손 7, 엑손 10이모두음성으로 RHD 유전자의완전결손 (complete deletion) 임을확인하였고, 두번째헌혈자는엑손 4, 엑손 7이증폭되어 RHD 유전자가존재함을확인하였다 (Fig. 1). 두번째헌혈자검체는 Silvy 등 10) 이보고한방법으로엑손 9의직접염기서열분석을 추가로시행하였다. 증폭을위해사용한시발체는 forward 5 -tactgtcgttttgacacacaatatttc-3, reverse 5 - gttcctcctgcaatgctcct-3 를사용하였으며, 염기서열분석은 BigDye Terminator v3.1 Cycle Sequencing Kit (Applied Biosystems, Foster City, USA) 을사용하여 sequencing PCR을실시하고 3130xl Genetic Analyzer (Applied Biosystems) 로염기서열을자동분석한후 Mutation Surveyor (SoftGenetics, Pennsylvania, USA) v5.0.0 software로염기서열결과를판독하였다. 분석한결과 RHD (c.1227g>a) 변이가있는 DEL형임을확인할수있었다 (Fig. 2, Table 1). 고찰 RHD 유전자변이의유전적배경과그빈도는인종마다다양하다. D음성혈액형의경우유럽에서는 17% 이상인것으로알려져있지만, 11) 한국의경우 % 로매우낮은것으로보고되고있다. 7) 한국인에서혈청학적으로 D음성인사람들의 RHD 유전형은 RHD 유전자의전체소실에의한 D음성발생률이 74 75%, DEL형에의한발생률이 13 17%, RHD-CE-D hybrid에의한발생률이 9 10% 로보고되었다. 6,7) DEL 대립유전자는동아시아인에서혈청학적으로 D음성인사 Table 1. 4 Korean J Blood Transfus Vol. 27, No. 2, , Aug 람중 10 33% 로자주관찰되지만다른인종에서는보고된바는있으나드물다. 12) 본증례에서제시된 RHD (c.1227g>a) 대립유전자가있는 DEL형은서구에서는극히드물지만동양인에서 DEL형의대부분을차지해 Asia type DEL형이라고부르기도한다. 12) 실제로한국인을대상으로 DEL형으로나타난사람의유전형을분석한결과대부분 RHD (c.1227g>a) 대립유전자에서비롯된것으로보고되었다. 6,7) RHD 유전자 c.1227g>a 변이는침묵변이 (silent mutation) 이지만엑손 9번의마지막염기의치환으로인한 splicing 과정의이상으로 mrna 전사과정에서엑손 9번소실이유발되어매우낮은 D항원을발현한다. 7) Rh 혈액형군에서 D항원은면역원성이강해 Rh 혈액형중항체발생률이가장높은것으로알려져있다. 일반적으로적혈구표면의 D항원의수와항체발생률은비례하여약 D형에의한항체발생률은 D 양성에비하여낮다. 2) 따라서 D항원수가극소량으로존재하는 DEL의경우항-D 항체를생성하기어려울것으로여겨왔으나 DEL 형수혈에의한 D 음성자에게항-D항체가발생한사례가국내외로보고되고있다. 현재까지보고된증례들은 1경우를제외하고모두 RHD (c.1227g> A) DEL형에의한것으로이대립유전자가한국의 DEL형의대부분을차지한다는것을고려할때그임상적의미가크다. 1,3-5) 또한 Shao 13) 는 RHD (c.1227g>a) DEL형환자군은 D항원에노출되어도항-D항체를생성하지않는것을간접적으로증명한바있어 RHD (c.1227g>a) DEL 형은비록수는적지만완전한 D epitope를보유할것으로생각된다. 14) 따라서, Asia type DEL형혈액이 D 음성자에게항-D항체를유발할수있다는점과이들환자는 RhD 혈액을수혈받아도안전할수있다는임상적의미를고려해보았을때 D 음성과 DEL형의감별은의료기관수혈정책 을세우는데중요할수있겠다. DEL형혈액은혈청학적검사에서 D 음성으로분류되며흡착및용출법또는 RHD 유전자검사만으로확인가능하다. 흡착및용출법은검사과정이복잡하며정확한검사결과를얻을수없는경우가많아 RHD 유전자검사가더욱선호되고있다. 현재한국에서 DEL형의감별을위해 D 음성헌혈혈액에대한수혈정책이수립되지않아그구별이어려운실정이다. 일부연구에서는 RHD (c.1227g>a) DEL형에서 C항원의표현형이양성인비율이높다는점을이용하여 C, E, c, e항원의표현형으로 DEL형의추정할수있을것이라제시한바가있다. 15) RHD (c.1227g>a) 대립유전자가있는한국인을대상으로한연구에서도 C항원이양성인비율이높게나왔는데 42명중 1명을제외한 41명에서 RhC 양성이었으며 (97.6%) 그중 Cce형이가장흔한것으로 (36/42, 85.7%) 나타났다. 7) 본증례의 DEL형의헌혈자의경우에도 Cce 표현형을보여 DEL형의판정에 C, E, c, e 항원을통한추정이도움을줄것이라생각된다. 저자들은 RHD (c.1227g>a) DEL형수혈로인해 D 음성자에서항-D항체가발생할수있음을확인하였다. 본증례는한국에서 DEL형의빈도와항D-항체의임상적의미를생각해보았을때 D 음성과 DEL형감별의중요성을다시한번시사하였다. 요약 RhD 혈액형은 ABO 혈액형다음으로강한면역원성을가지며신생아용혈성질환, 심각한용혈수혈이상반응등을유발할수있어임상적으로중요한혈액형이다. DEL형은기본적인혈청학적검사에서는검출되지않으며, 흡착-해리시험없이는 D 음성으로판정되고 RHD 유전자분석에
