Korea Thirteen infants were examined with high-resolution real time ultrasound following Rammstedt pyloromyotomy for hypertrophic pyloric stenosis and 6 patients were followed up to 3 months. Our results were as follows 1. In the preoperative study. all cases with hypertrophic pyloric stenosis demonstrated the pyloric muscle thickness to be 4mm or greater. the pyloric diameter to be 13mm or greater. the pyloric channel length to be 17mm or greater. the pyloric muscle volume to be 2.25cm J or greater and the pyloric muscle index to be 0.61 or greater. respectively. 2. Operative measurement of the pyloric muscle thickness was between 2 and 6mm. the pyloric diameter was 14 and 25 mm. the pyloric channellength was between 18 and 31mm. the pyloric musclè volume was 1.85 and 8. 18cm J and pyloric muscle index was 0.46 킹 ld showing g d correlation with ultrasonographic measurements. 3. Sequential sonograms showed that it took 3 months for the muscle hypertrophy to resolve in all of the six patients. And in the postoperative evaluation. pyloric muscle thickness and pyloric muscle index were valuable parameters. In conclusion. following longitudinal pyloromyotomy. the hypertrophied pylorus returns to normal by 3 months. 2 大韓放射線훌훌學會찮 : 第 27 卷第 l 號 1991 보고된바있다 (2). 통모양으로가정하여유문근부피를 πtl(d - T) 식에 유문근협착중의초음파진단은 1977 년 T ee le 가처음 의하여산출하였고 (T 유문근두께 L 유문근길이, 보고하였으며 (3) 이 진단방법은유문부의비후된근육충 D 유문경, 단위는 cm) 유문근부피를체중 (kg) 으로나 을직접관찰, 측정할수있는신속, 간면하고안전하다 누어유문근지표를얻었다. 는이유로 IHPS 의조기진단과수술후추적검사에유용하며상부위장관조영술을대치하고있다 (4, 5, 6). 결 과 저자들은 1988 년 4 월부터 1989 년 12월까지영동세브란스병원에서비후성유문협착증으로진단되어 Rammstedt 정상대조군과 IHPS 환아의초음파측정소견 (Table 1,2) 시술 (Longitudinal Pyloromyotomy) 을행한총 20 명의 환자중수술후 1 개월이상초음파로추적검사가 유문근두께는정상대조군에서 mm, IHPS 가능하였던 13 명의환자와수술후 3개월까지추적겁사가가능했던 6 명의환자를대상으로수술의성공여부, 합병증및유문근이언제정상치로돌아오는지를알아보기위하여본연구를시행하였다. 에서 mm( 평균 5.5 mm) 였고유문경은정상대조군에서 mm, IHPS 에서 mm( 평균 16.3 mm) 였으며유문관길이는정상대조군에서 mm, IHPS에서 mm( 평균 mm) 였 다. 대상및방법 유문근부피는정상대조군에서 cm 3 ( 평균 1988 년 4월부터 1989 년 12월까지 1 년 9개월에걸쳐영동세브란스병원에서 IHPS 로진단되어수술을행한 20 명의환자중수술후 l 개월이상초음파로추적검사가가능하였던 1 3예와수술후 3개월까지추적검사가가능했던 6예를대상으로하였고, 증상이없었던 12 예를정상대조군으로하였다. 수술전초음파를이용하여유문근의두께, 직경, 길이를측정하였고이것을이용하여유문근부피와유문근지표 (lndex) 를구하였으며, 수술시유문근측정소견과비교하였다. 또한수술후 1 개월 (13 예 ), 3개월 (6 예 ) 에유문근을관찰하여유문근의수술후변화를살펴보았다. 초음파검사는 Acuson 128, Aloka SSD-280 LS 초음파기와 5 MHz 및 7.5 MHz 선상탑촉자 (Linear Transducer) 를사용하였다. 한국인영유아에서 IHPS 의초음파진단기준은보고된바있으며 (4) Caver퉁에의한방법인 (5) 유문부위를원 0.22 cm 3 ), IHPS 에서 c m 3 ( 평균 4.19 cm 3 ) 였고유문근지표는정상대조군에서 ( 평균 0.06), IHPS에서 ( 평균 0.98) 이었다. IHPS에서의유문관의초음파측정소견과수술소견의비교 (Table 3) 유문근두께는초음파소견상 mm 였고수술소견상 2-6mm 로차이가 1 mm 미만이 12 예중 9예가해당되었고 (75 %), 유문경은초음파소견상 mm, 수술소견상 mm 로그차이가 2mm 미만인경우가 10예중 7 예였으며 (70%) 유문관길이는초음파상 mm, 수술소견상 mm 로차이가 2mm 미만인경우가 12 예중 7 예였다 (58%). 유문근부피는초음파계산상 cm\ 수술소견상 cm 3 으로차이가 2 cm 3 미만인경우가 10예중 8 예였으며 (80%) 유문근지표는초음파계산상 이었고수술소견상 으로그차이가 0.4 미 Table 1. Borderline case 에서는단순계측치보다유문근지표가더정확한진단기준이되었다 (Fig. 2). 또한유문근의개구가더자주관찰되었으며유문관 영유아비후성유문협착증은 1000 명출산에 3 명정도의발생빈도를보이는비교적흔한질환으로 (6) 생후 2 주 에서 8 주사이에호발하며남아에서 3 : 1 또는 5: 1 로 더우세하게나타나고 (4, 6, 7), 이질환의원인으로는유문 부횡문근의부피가증가하여유문관을압박하는것으 로간주하고있다 (8). 이질환의진단은임상적으로상복부에서올리브모양 의유문종괴가촉지되는경우진단할수있으며, 상부 위장조영술로는위유출의지연, 위확장, 위운동의증 가및유문관신장동의소견이관찰된다 (6, 9, 10) 년 Teele 퉁이최초로초음파검사를이용하여우 측상복부에특정적인고에코의중심부를갖는저에코 의유문근종괴를관찰한이후초음파소견으로 IHPS을진단할수있었고최근에는정확한진단방법으로인정되 고있다 (3, 11, 12) IHPS 를진단할수있는초음파상유문근계측치는여러보고에서다양하게제시하였는바유문근두께가 ~ ,' oj 91 oj 0' o 231m이 o 2 31mol mol 0 31mol o 2 3(mol Pyloric m. thickness Pyloric m. diameter pyloric m. length Pvloric m. volume Pyloric m. index Fig. 1. And then. musde volume and index were calculated. which were 2.25cm' and 0.61 and definite diagnosis could be made. b. 1 month after pyloromyotomy. the hypertrophied pyloric muscle returned to normal range of measurements. (muscle thickness=3.0mm. muscle diameter= 12.6mm. muscle length= 12.6mm. muscle volume= 1. 14cm'. and muscle index =0.25) 4mm 이상, 유문경이 10 mm 이상, 유문근길이가 15 mm 이상이면정상치에서벗어난것이며 (4, 12), 정상과 IHPS 환자에서계측치의중복되는범위가있음에도불구하고 (13-18), IHPS의진단은유문근두께가 4 mm 이상, 유문경이 15 mm 이상, 유문근길이가 18 mm 이상이면양성으로보고있다 (6). Caver 퉁은유문관을원통형으로가정하여유문근의단순측정치들은유문근의모양이변함에따라그값이달라지지만유문근부피는일정하며, 연령이증가됨에따라유문근부피도증가하게되므로그값을체중으로나눈유문근지수 (l ndex) 가가장정확한진단기준 ( ind ex 0.4 이상 ) 이된다고보고하였다 ( 히. Wollstein 동은수술후 2주이내에 IHPS 환자의유문근이좀더부드러워지며 25 일째는위장이정상크기로되고유문부는보다단단해진다하였고현미경소견상에는수술후 일후수술창상이완전히아물고수술한자리는함몰되며 25 일후에는점점줄어들어 6주에는미미한반흔만남기고치유된다하였다 {2). Sauerbrei 퉁도역시같은관찰결과를얻어 6 주이후유문근이정상으 하였다 {2이. 또한 Caver 동은유문근 lnd ex 를구하여그값이수술후 8주까지 IHPS 의최하계수이하로되며 12 주까지는완전정상수치로돌아온다고하였다 (21). 본보고에서는수술전정상대조군과 IHPS 의단순계측치상중복되는부분이전혀없었고, 따라서유문근부피와 lndex 를계산하였을때도 Cave r 퉁이보고한 IHP S 의진단기준치와정상대조군의상위한계치와는 차이가있다. 이것은본보고에서장상대조군으로설정한영유아의범위가다양하지못하였던데기인된것으로여겨진다. 본보고에서수술후 3개월때유문근이정상대조군의측정값범위로돌아온예는유문근 lnd ex 를기준 (0. 18) 으로하여 6 예중 4예가해당되었는데 (66.7 %), 여기에다른보고에의한 (Caver 둥 ) 진단기준을고려해볼때 (5, 21), 정상치와 IHPS 측정치의중복되는부분이있음에도불구하고공통된기준인유문근두께 4mm 미만, 직경 10mm 미만, 길이 15 mm 미만, 부피 1. 5 c m 3 미만, lndex 0.28 미만으로복귀하는것은유문근두께와유문근 lndex를기준으로하였을때만이정상범위로돌아온것 로돌아온다고하였다 (1 9). Baurchier 동은유문근이 4 주 을알수있었다. 까지빠르게그측정치가감소하며그이후완만하게정 따라서수술후유무근변화의추적검사에는단순측정 상범위로돌아가 6개월까지는완전히정상으로된다고 치로는유문근두께가유용하며환자의체중 ( 곧연령 ) 을 154 -
