The authors also gave the method a trial the survival of the strains according to temperature during transit from transport media. The following results were obtained. The following methods kept the culture alive for a week : tryptic soy agar with 15 % glycerol at -20 o C : 10% skim milk at -20 o C ; cystine tryptic agar at 35 0 C. Gonococci survived for more than 8 weeks in the one half strength dextrose starch agar and one half strength dextrose starch with VCN, but GC medium-1/2 could be preserved for average 4.4 weeks. It is recommended that preservation of gonococci in one half strength dextrose starch agar with VCN, which produces less frequent contamination is a practical to maintain cultures for teaching and quality control. The survival period of gonococci with Stuart s transport media at 35 0 C, room temperature and 4 oc were retained for 24 hours respectively. Stuart s transport with 1 % charcoal survived for more than 4 days at room temperature and 4 oc, but alive for 24 hours at 35 0 C -11-

2 1 서론 Neisseria gonorrhoeae 는성병중에서흔히발생 하는 1) 임질의원인균으로세계적인전염성질환이 며주로생식요도계에감염이많은호기성세균이 다 2) 이세끌은분리배양이복잡할뿐아니라오랫 동안보관하는것이매우까다봅다 3) 그러나이균 주의세균학적연구와역학적조사옥적으록임상검 체에서분리한균주의보존이필요하며, 교육용과 정도관리를위해서도매우중요하다. 셰균의보존방 법은동결건조 4) (lyophilization) 법이널리알려졌지 만장비구입이비싸고, 술식이복잡하여소규모실 험실에서는용이하지않으며액체질소 5) 에넣어동 결시키는방법역시쉬운일은아니다. 그러나 Vera 는 1948 년에 cystine tryptic aga~) 를 Difco (1971 년 ) 회사는 dextrose starch agar- 1! 2 7) 을각각 35 0 C 에 서임균의보존을시도하였다. 이방법들은특이한 기구와복잡한술식없이상품화된배지로쉽게준비 할수있다. La scolera 는 tryptic soy broth with 15% glycerol (TSB-G) 에임균을접종하여 C 에보존하면오랫동안생존하는것을보고하였다. 본실험실에서일반셰균은주로 10% skim milk 어 l 접종하여 C 에보존하고있다. 본연구에서 TSB-G 와 10% skim milk 는 C 에 CT A-D, DSA -1/2 빛 GC medium base -1/2 배지는 35 0 C 에서임균의생존기간을비교하였다. 임 률은산소분압이낮은조건에서는저온에생존이더 오래지속되는것으로알려졌다 9) 임상검체수송에 많이이용되는 Stuart s transport 배지는검체를면 봉으로채취하여설온보관으로많이이용되고있다. 그러나 Neisse 깨 a gonorγhoeae 는한천에포함된지 방산에의해서억제된다고하였으며 Klaesson 은지 방산을중화시키기위해서 charcoal 과같은흡착제 를첨가하는것을보고하였다 10) 또한본설험은 Neisseria gonorrhoeae 를임상검체에서분리배양하 기위해서수송배지로써사용되고잇는 Stuart s transport 배지와 Stuart s transport with charcoal 을 35 0 C, 실온빛 4 0 C 에보존하여생존을비교하였 다. 11. 실험재료및방법 TSB(Difco) 에 15 % glycerol 을넣은 TSB-G 와 10% skim milk 를나사마개시험관에.5ml 씩넣어 I 궐캄하였다. CT A (Difco) 에 o. 5 % dextrose 를넣은 CT A-D, DSA -1/2, DSA -1/2 with VCN 및 GC medium-1/2 은 75x100mm 나사마개유리시험관 에넣어준비하였다. DSA-1/2 성분은 proteose peptone N 0. 3 (Difco) 0.75 %, bacto dextrose. 1%, soluble starch (Difco) 0.5%, sodium chloride 0.25%, disodium phosphate 0.15%, gelatin 1 %, bacto agar 0.5%, 25 0 C 에 ph 7.3:t 0.2 이며상품화된 DSA -1/2 (Difco) 는 1, 000ml 증 류수에 32.5g 을녹혀사용했다. DSA - 1/2 with VCN 은 DSA-1/2 을멸늄후 50 0 C 에식혀서 1% VCN (BBL) 을넣었다. GC medium base - 1/2 은 이배지에 0.45% soluble starch, 1 % gelatin 빛. 1 % dextrose 를첨가하였다. Neisse 서 'a gonorrhoeae 는 1987 년 5- 월에전북 옥구군보건지소에서 Thayer Martin 배지를사용 해서분리한균주이며 chocolate 에 24 시간계대배양 하여사용하였다. TSB-G 와 10% skim milk 에 3 mm loop 를이용해서 chocolate agar 에배양된집 략을두껍게접종하여마개를꽉막고즉시 C ( 냉동고 ) 에넣었다. CT A-D, DSA -1/2, DSA -1/ 2 with VCN 맺 GC medium base-1/2 은증류수 5ml 에 chocolate agar 에서집락을채취하여 MacFarland nephelometer N.3 시험관의탁도 에맞추어멸균된 Pasteur pipette 으로균부유액을 3-4 방울씩떨어뜨려 3mm loop 로천자하여접 종하였다. 접종된배지는마개를막고 35 0 C 에보관 한후 gonococci 의생존을확인하기위해서 1-2 주마다계대배양하였다. TSB-G 와 10% skim milk 는냉동고에서꺼내어빨리녹인다음즉시배양 하였다. 계대배양은 chocolate agar 에 3mm loop 로접종하여 35 0 C 에 48 시간 candle jar 로배양후육 안적으로판단하여집략이보이면생존균주록판정 하였다 실험성적 1987 년 5- 월사이에전북옥구군보건지소에 서외래환자로부터분리한 Neisseria gono γrhoeae 주를이용하여기초실험으로 TSB-G, 10% skim milk 어 l 접종후 C 에보관한바 주모두각각 의배지에서 1 주간생존하였으며 CTA-D 역시 35 0 C 에서동일하였다. 그러나가장생존기간이좋은 배지는 DSA-l l2 이었으며 균주모두 4 주이상 보존되었다 (Table 1). 이결과로 DSA-1/2 에서더규명하기위해서 gonococci 보존배지로선택하였다. DSA-1/2, DSA -1/2 with VCN 및 GC medium -1/2 에 균 주를접종하였다. DSA-1/2 과 DSA -1/2 with VCN 은생존기간이각각 8 주 ( 평률 10 주 ) 이상이었 12

CTA with 0.5% dextrose DSA-1/2 ou U ζu으며 GC medium -1/2 은 4-5 주 ( 평균 4.4 주 ) 였 다 (Table 2). 임상겸체수송용배지인 Stuart s transport 배지 는 35 0 C 에서 gonococci 의생존기간이 1-4 일 ( 평 끌 1. 9 일 ), 실온에 1-4 일 ( 평균 2.2 일 ) 및 4 0 C 에 서는 1-3 일 ( 평균 2 일 ) 였으며 Stuart s transport with charcoal 은 35 0 C 에서 1 일실온에서 4 - 일 ( 평균 4.7 일 ), 4 oc 에서 4-7 일 ( 평균 5 일 ) 였 cf- (Table 3). 뿐아니라우리나라에서도 Chlamydia trachomatis 감염이 Neisse,ψ gonorγhoeae 보다도더많다는보 고가 1ψ) 관심의대상이되고있으나아시아일부빛 아프리카에서임균감염환자가많은것으로알려졌 다 18) 또한최초로 197 년에 PPNG (penicillinase producing Neisse 서 'a gonorrhoeae) 가분리되었고아

4 뿐아니라우리나라에서도 Chlamydia trachomatis 감염이 Neisse,ψ gonorγhoeae 보다도더많다는보 고가 1ψ) 관심의대상이되고있으나아시아일부빛 아프리카에서임균감염환자가많은것으로알려졌 다 18) 또한최초로 197 년에 PPNG (penicillinase producing Neisse 서 'a gonorrhoeae) 가분리되었고아 시아지역중필리핀이나싱가폴에서처음발생지역 으로추측하고있으며 19, 20) 국내에서는 1979 년에정 등 21) 에의해서 1 예가분리되었다. CDC 는 년사이에미국에서이균주의분리수는 2419 명 22), 싱가폴에서는 23) 1979 년에 32.1%, 국내에 서는 1982 년에 34.1%, 1983 년에 38.% 였으나 1984 년에는약간감소되었다 13) 그러나최등 24) 은특수 지역대상에서 1984 년 12 월 년 3 월사이에 gonococci 의분리율은 25.3% 였으나 PPNG 는 50. % 의매우높은치를보고하였다. PPNG 출현으로 임균에대한관심은더욱높이지고있으며이균주 에대한세균학적연구및역학조사목적으로보존 은매우중요하다하겠다. Neisse 깨 'a gonorrhoeae 의배양은 시간이적 당한것으로알려져있으나 25, 2, 27) chocolate agar 에 서는접종후 3TC candle jar 에배양시 48 시간까지 생존하였으며 TSB-G 와 10% skim milk 는 C 에서 1 주정도보존되는것은정등 12) 의보고와비 슷하였다. CTA 에서 gonococci 는 35 0 C 에서 10 일또 는그이상생존 ) 하는것으로알려졌으나이실험에 서는 CTA-D 를사용하여 1 주정도로짧았으며정 등 12) 은평균 2 주정도보존되는것으로보고하였 다. Gonococci 보존의가장좋은방법은 % 젖당 (lactose) 이포함된 tryptic soy broth 에서동결건조 (lyophilization) 시켜 4 0 C 에보관하는것이다 (Heckly, 191).28) 그러나소규모실험실에서는이 러한조건이어려우며실제적으로경제적이면서간 단히준비할수있는보존배지로서 DSA-1/2 이추 천된다. 이배지에서 35 0 C 에보관시평균 9.3 주로 Difco 회사 7) - 8 주보다는다소연장되었으나정 등 12) 의평균 9. 주와는비슷하였다 Neisseria gonorrhoeae 는냉온과자가용해에감수 성이있으므로종종계대배양이필요하다. 계대배양 시섬요 (fimbriae) 와같은중요한항원인자는상실 할수있으므로 18) 이런분야의연구를위해서는이 인자의보존도시도되어야겠다. DSA-1/2 은 35 0 C 에서 gonococci 보존시다른세군과곰망이가종종 오염되기도한다. 이러한문제점을해결하기위해서 는 Thayer-Martin 배지에첨가하는항생제즉 vancomycin, colistimethate 및 nystatin 을포함시 킨 DSA -1/2 with VCN 을 35 0 C 에서보존시평균 10.7 주가생존되었으며정등 12) 의평균 11.5 주와는 큰차이는없었다. 또한 GC medium -1/2은정등 12) 에의하면 ph 7.2에서 DSA~1/2과비슷한것 으로보고되었으나본실험에서는평균 4.4 주로써 생존기간이매우짧았다. Stuart s transport 배지에서온도에대한임균의 보존은잘안려지지않았다. 본실험에서는 35 0 C 에 평균 1. 9 일, 실온빛 4 0 C 에서는각각 2.2 일, 2 일 이었으며, 온도에따른큰차이는없었다. 이수송 배지에서임균의안전한수송을위해서는 24 시간이 내해야겠다 Ley 와 Mueller 는 194 년에일반한천 에들어있는지방산은 Neisseria 와같은균주에해 로운것으로보고하였다 1 이일반적으로국내종합병 원에서겸체채취시주로면봉을사용하고있기때 문에만약면봉에지방산이있다면 Neisse 서 a gonorrhoeae 의배양에영향이미칠것으로생각된 다. Klaesson 은이와같은지방산을중화시키기위 해서활성탄과같은흡착제훈합을발표하였다 10) 한편 Short 등은 9) gonococci 에대해서혐기성조건 에서는 3TC 보다 2TC 에더잘보존되는것으로보 고하였다. 저자들은 Stuart s transport 배지에 1% 활성탄 (activated charcoal) 을넣은배지에서온도 에따라생존기간을비교한바 35 0 C 에서는 1 일, 실 온에서는 4- 일 ( 평균 4.7 일 ), 그리고 4 0 C 에서 는 4-7 일 ( 평균 5 일 ) 이생존하였다. Neisseria gono γrhoeae 배양목적으로검체를수송하기위해서 는 Stuart s transport with charcoal 배지를사용하 면은실온빛 4 0 C 에서 4 일까지는안천하게보존될 수있다하겠다. V. 결론 Neisse 서 a gono γrhoeae 의보존을위해서경제적이 면서간단한방법을알기위해서몇가지배지를비 교하였다. 저자들은수송배지에서수송하는동안온 도에따라균주의생존을시도하여다음과같은결 론을얻었다. Tryptic soy broth with 15 % glycerol 과 10% skim milk 는 C 에 cystine tryptic agar 는 35 0 C 에보존시 1 주생존하였다. DSA-1/2 과 DSA -1/2 with VCN 8 주이상생존 하였지만 GC medium base -1/ 2 은평균 4.4 주보존 되었다. 오염된세균을억제시키고임균을보존할 수있는 DSA -1/2 with VCN 은교육용과정도관리 목적으로사용할수있는추천되는배지이다. Stuart s transport 배지에서 35 0 C, 실온빛 4 0 C 에 gonococci 의생존기간은각각 24 시간이었다. Stuart s transport with charcoal 에서실온빛 4 0 C -14-
