3 Abstract Purpose: Prostate specific antigen test is widely diffused as the main method of screening prostate cancer in Korea. And diffusion of ultrasound sonography may cause overdiagnosis of kidney cancer as well as thyroid cancer. The aims were to suggest epidemiological evidences about overdiagnosis of prostate and kidney cancer in Korean. Methods: The annual trends of national incidences and mortalities of prostate and kidney cancers offered by Statistics Korea were evaluated. Results: The increasing velocities of incidences in prostate and kidney cancers were 6 and 5 times higher than of mortality between 2000 and 2011, respectively. And the age group showing the highest incidence in prostate cancer was shifted from 85 years and older into years. Conclusions: These facts suspected the overdignosis of prostate and kidney cancers in Korean. The academic activities for creating the related evidences using nationwide cancer registry database should be encouraged to control of overdiagnosis. 4 1. 서론 암의조기발견을통한암사망을줄이려는암검진 (cancer screening) 은필연적으로 과진단 (overdiagnosis) 의문제를야기시킨다 [2]. 과진단이란 평생동안몰랐을암을검진으 로알아낸경우 로정의하고있는바 [2], 불필요한과치료 (overtreatment) 를유발한다는점 에서검진의위해 (harm) 로보고있다 [3]. 우리나라에서최근갑상선 [4] 과유방 [5] 에있어과 진단의우려가제기되었다. 과진단은암검진사업이도입되거나, 새로운암검진용검사가개발될때논란이생 긴다 [2]. 서구에서는갑상선암, 유방암, 전립선암, 신장암등에서과진단이있다는주장이있 어왔는데 [1], 혈중전립선특이항원 (Prostate Specific Antigen, 이하 PSA) 검사가보편화된 국내상황에서전립선암에대한과진단의여부를확인하는것은암관리정책상중요한근거 가된다. 또한전국민암발생통계에있어갑상선암의발생률이 1 위인것 [6] 은초음파검사기 기의보급및확산에따른과진단의결과라는주장이제기되었다 [4]. 그런데, 초음파기기는 목뿐만아니라, 복부의장기들에대하여도우연암 (incidental cancer) 을발견할기회를높 일수있다 [7]. 그렇다면신장암 (Kidney cancer) 에대하여도진료현장에서과진단이일어나 고있는것은아닐까라는의문도생기게된다. 이상의의문제기에따라, 한국인에있어전 립선암과신장암에대한과진단의역학적근거들을알아보고자한다.

5 2. 본론 가. 전립선암 Welch & Black [1] 이전립선암의과진단을주장한근거로제시한것은검진이확산 된직후부터발생률이크게증가하지만, 사망률변동은미약하다는것이다. 이는국내갑상 선암의과진단에대한주장근거로도활용되었다 [7]. Fig. 1 (A) 는통계청포탈 [8] 에서제공하 는전립선암의발생률과사망률을연도별로나타낸것이다 년도에비하여 2011 년의 사망률은십만명당 3.3 명 (= ) 증가한반면, 발생률은십만명당 20.4 명 (= ) 으로 6 배 (=20.4/3.3) 더증가한것은전립선암과진단의첫번째역학적근거가된다. 한편, Hilton et al. [9] 는 PSA 검사가확산된이후진단시연령이낮아짐을보고하였 다. 이에 Fig. 1(B) 는지난 10 년간한국남성에있어연령대별발생률을살펴본것이다. 발 생률이최고가되는연령군이 2001 년에는 85 세이상군, 년에는 세군, 2011 년도는 세군으로이동하는양상을보이고있다. 이것이과진단의두번째역학적근 거인것이다. 그러나진단시병기 (stage) 의분포에서변화가있는가를같이보아야만더 설득력있는근거가될것이다 [10]. 나. 신장암 전체신장암중우연암이차지하는정도가 1970 년초에약 10% 정도였지만, 그 이후 CT, MRI 등의다양한영상기기기개발되면서 1990 년대에서는약 60% 로증가하였다 [7]. 따라서대부분의신장암은다른목적으로영상의학검사를하는과정에서우연하게 (incidentally) 발견되는경우이다 [11]. 임상증상에따라의심하여알아낸경우 (symptomatic

6 tumors) 보다우연암은크기가더작고, 예후점수도더양호하며, 전이도더적게하는것 으로알려져있다 [12]. 따라서우연암을많이진단할수록신장암의발생률은증가하며, 사 망률은변화가없거나증가해도발생률처럼큰변동은보이지않을것으로예상할수있다. 이에따르면경제수준이높은선진국일수록신장암발생률이높지만사망률은낮은현상을 설명할수있다 [13]. 이처럼영상진단의료기기의보급이후우연암이많아지면서발생률과사망률간의 증가폭에서큰간격을보인다면, 이또한과진단이있다는역학적근거가된다 [1]. 미국의 경우 1971 년을기준으로 2010 년도사망률이 2 배증가한반면, 발생률은 5 배증가하였다 [14]. 특히발생률변동에있어연령이나출생코호트보다는시기 (period) 별로유의한변화 가있었다는보고 [15] 는보건의료환경변화에따른과진단결과임을지지하는소견이다. 통 계청포탈 [8] 에서제공하는신장암의 년도발생률과사망률추이를 Fig. 2(A) 로나 타내었다 년도를기준으로 2011 년도사망률이 1.5 배 (=1.7/1.1) 증가하고발생률은 2 배 (=6.0/3.0) 로나와별차이가없어보인다. 그렇지만사망률증가폭이십만명당 0.6 명 (= ), 발생률이 3.0 명 (= ) 으로써, 사망률에비하여발생률의증가속도가 5 배 (=3.0/0.6) 더높다. 다시말해서 Fig. 2 의곡선기울기가달라서최근으로올수록발생률 - 사 망률간격이더크게벌어지고있는것이다. 한편신장암은여자보다남자에서 2 배더높다는역학적특성이있어 [16], 여성의 발생률 - 사망률증가양상을살펴본것이 Fig. 2(B) 이다. 같은기간사망률에비하여발생률 증가속도에있어남자는 2.2 배 (=2/0.9), 여자는 6.0 배 (=1.8/0.3) 로나왔다. 목초음파로 알게되는갑상선암은여자가월등히높다는점 [6] 에서신장암여자에서큰간격을보이는 것이목초음파시행과연관된결과인가에대한추후연구가필요하겠다.

7 다. 식도암 국가암검진에있어가장수검율이높은검진법은 70.9% 를보인위암검진을위한 상부내시경검사 (upper endoscopy) 와유방암검진을위한유방촬영술이다 [17]. 앞서언급처 럼전립선암과신장암의과진단은검사의보급확대에따른것이라면, 암검진대상이되는 다른암들도과진단이생기느냐의의문이생긴다. 가장수검율이높은상부내시경검사는식 도를거쳐위로진행하는검사과정을고려할때, 식도암에대하여과진단이유발될가능성 은없는가? Fig 3. 은통계청포탈 [8] 에서제공하는식도암의발생률과사망률을연도별로나타 낸것이다 년도를기준으로 2011 년도까지발생률과사망률모두감소하거나변동이 없다. 이소견은상부내시경검사를시행함에도불구하고식도암에있어과진단이없음을알 려준다. 따라서진단을받은이후진행속도가지체 (indolent) 되는암이면서, 암검진사업이 확산되어기간차이바이어스 (length time bias) 가개입될때에과진단이일어나는것이다 [2,3].

8 3. 결론 Fig. 1(A) 과 Fig. 2(A) 에서과진단의역학적근거로제시한발생률 - 사망률괴리현상 에대하여는해석상주의를요한다. 그이유는전립선암에있어서검진보급률증가, 검사 법의민감도향상, 발생위험요인의변동등으로도설명할수있기때문이다 [19]. 신장암에 있어서도관련검사의시행률, 유전자검사같은새로운검사법의개발, 금연등의발생위 험요인의변동, 효과적인치료법개발, 의료서비스전달체계등으로도설명할수있기때문 이다 [16,18]. 이렇듯과진단을의심하는역학적근거들이있으면, 그크기를알아낼수있도록 해당암의자연사 (natural history) 연구가필요하다 [2,16]. 다시말해서검진의이득에비하 여위해가얼마나큰가를판단하기위해서과진단의규모를산출할필요가있는데, 이는 중앙암등록본부가관리하는암등록자료를활용하면가능하다 [2]. 또한과진단이있을경우 진단시암의크기가줄어들고, 병기 (stage) 가낮아지는현상 [9] 에대하여도중앙암등록본부 가관리하는전국민암등록자료에서해당근거들을생성할수있도록해야할것이다 [2]. 따라서개인정보보호강화로연구에제한이있는상황에서 [20], 중앙암등록본부는국가암통 계산출목적으로구축하는암등록자료를활용하여암관리정책에필요한유용한근거들을 주도적으로산출해주기를권고한다. 이외에도과진단을최소화하기위한보건의료인의다 각적인연구활동이요구된다 [2]. 감사의글 이논문은 2015 학년도제주대학교학술진흥연구비지원사업에의하여연구되었음
