This neoplasm has a poor prognosis and it frequently presents at an advanced stage of disease; it is clinically characterized by increased serum levels of alpha-fetoprotein (AFP) in most cases. We experienced a case of hepatoid adenocarcinoma of the stomach with multiple liver metastases that mimicked germ cell tumor in a 43-year-old man. He had incidentally detected gastric adenocarcinoma via gastrofibroscopy and he underwent subtotal gastrectomy. The postoperative pathologic stage was II (T2N1M0). He complained of epigastric pain. The serum AFP was markedly increased (227,325 IU/mL) and abdominal CT revealed multiple liver masses. We considered these as extragonadal germ cell tumors that occurred in the liver because of the markedly elevated AFP, the short duration of follow-up and early stage of stomach cancer at the that time. To confirm the diagnosis, biopsy of liver mass was done and he was diagnosed with hepatoid adenocarcinoma.(korean J Med 73: , 2007) Key Words : Gastric cancer, Alpha-fetoproteins 서 론 간세포양 샘암종 (hepatoid adenocarcinoma)은 조직 학적으로는 샘암종 외에 간세포성 분화를 보이는 부분 이 동일 암종 내에 공존하면서, 높은 혈중 alpha fetoprotein (AFP)치를 보이는 암종으로 1985년 Ishikura 등 1) 에 의해 처음으로 명명되었다. 간세포성 분화를 보이는 샘암종은 위장관, 난소, 폐, 담낭, 자궁, 방광 등의 여러 부위에서 발생할 수 있음이 보고되고 있다 2-5). 위에 발생한 간세포양 샘암종은 진단 당시 진행된 경우가 많으며 높은 전이율과 불량한 예후를 보이고, 드물게 발견되어 임상적으로 간과하기 쉬워 진단에 있 어서 주의가 요망되는 질환이다 6). 이에 저자들은 위암 2기(T2N1M0)로 진단되어 근치 적 절제술을 시행 받은 후 5개월 만에 높은 AFP수치 (227,325 IU/mL), 간 내 다발성 종괴 및 복강 내 재발 Received : Accepted : Correspondence to : Ik-Joo Chung, M.D., Department of Internal Medicine Chonnam National University Hwasun Hospital, 160, Ilsimri, Hwasun-eup, Hwasun-gun , Korea ijchung@chonnam.ac.kr

Immunohistochemical staining for alpha-fetoprotein (AFP) showed diffuse positivity (B) ( 200) 병소를 보인 환자에서 간 조직 검사를 시행한 결과 간세 포양 샘암종으로 진단된 1예를 경험하였기에 문헌고찰 과 함께 보고하는 바이다. 증 례 환 자 : 박, 43세, 남자 주 소 : 상복부 통증 현병력 : 상기 환자는 내원 5개월 전 정기 검진에서 우연히 위 종괴가 발견되어 검사한 결과 위샘암 (T2N1M0) 2기로 진단받고 위 아전절제술 및 문합술을 시행받았다. 수술 후 시행한 복부 전산화 단층 촬영에서 도 잔존 종양 및 전이 병소는 보이지 않아 추적관찰 중 1일전부터 발생한 지속적인 상복부 통증을 호소하며 응 급실로 내원하였다. 과거력 : 특이 소견 없었다. 가족력 : 형이 위샘암으로 수술받은 병력이 있었다. 사회력 : 특이소견 없었다. 이학적 소견 : 내원 당시 혈압 160/90 mmhg, 맥박

Postoperative pathology of gastric lesion revealed moderately differentiated adenocarcinoma with round nest (A) (H&E, 200), and diffuse positivity on the immunohistochemical staining for alpha-fetoprotein (AFP)( 200) (B) 88/분, 호흡 20/분, 체온 36.0 였으며 신체 검진상 황달 성 공막과 우상복부에 미만성의 경한 압통을 보였으나 반발통은 호소하지 않았으며 촉지된 종괴는 없었다. 그 외에는 특이소견은 보이지 않았다. 검사 소견 : 내원 당시 말초혈액검사상 백혈구 수 4,700/mm 3 (정상치 : 4000~10,800), 혈색소 12.9 g/dl (정상치 : 12~18), 헤마토크리트 38.6% (정상치 : 37~ 52), 혈소판 수 175,000/mm 3 (정상치 : 130,000~450,000) 이었다. 혈청 생화학검사 상총단백질 7.1 g/dl (정상치 : 5.8~8.1), 알부민 4.1 g/dl (정상치 : 3.1~5.2), alkaline phosphatase 244 IU/L (정상치 : 39~117), AST 114 U/L (정상치 : 7~38), ALT 144 U/L (정상치 : 4~43), gamma-gtp 383 IU/L (정상치 : 7~49), 총 빌리루빈 9.0 mg/dl (정상치 :0.2~1.2), 직접 빌리루빈 5.6 mg/dl (정상치 : 0~0.4)이었으며 기타 전해질 수치는 정상이었 다. HBsAg 음성, anti-hbs 양성, anti-hcv는 음성이었 다. CEA는 ng/ml (정상치 : 0~3.4)로 증가되어 있었고, AFP은 227,325 IU/mL (정상치 : 0~5.8)로 크게 상승되어 있었으나 β-hcg는 정상범위였다. 위내시경 검사 : 위식도 점막 접합부에 발적과 백태 소견을 보여 역류성 식도염 이외에 수술 접합부위 재발 이나 다른 종괴는 관찰되지 않았다. 방사선학적 검사 : 우상복부 종괴와 AFP이 상승되 어 있어 복부 전산화 단층촬영을 시행하였다. 간에 최대 약 cm의 다발성 병변이 관찰되었으며, 복강동 맥주위로 약 cm의 림프절 종대가 있었다. 그리 고 복막을 따라 조영 증강되는 결절성 병변들이 관찰되 어 복강 내로 전이가 있음을 추정할 수 있었다(그림 1). AFP 상승에 대한 원인 검사 위해 시행한 골반 전산화 단층촬영에서는 생식선 주위로 종괴는 관찰되지 않았었 다. 임상 경과 : 위내시경과 복부 전산화 단층촬영에서 수술 부위에 위암의 재발 소견은 보이지 않고 다발성 간 병변, 복강 내로의 전이 소견 및 현저히 상승된 AFP 수 치를 동반할 수 있는 경우를 고려해 본 결과 생식세포종 양 혹은 가능성은 적지만 원발성을 감별하기 위해 경피 적 간생검을 시행하였다. 간조직검사 상에서 중등도의 분화를 보이는 샘암종의 소견을 보였으며, 간생검을 통 해 얻어진 조직(그림 2)과 위 아전절제술 당시 얻어진 위샘암 조직(그림 3)에 대해 AFP 면역조직화학 염색을 시행한 결과 두 조직 모두에서 세포질 내에 미만성으로 강하게 염색됨을 관찰할 수 있었고, 이전의 위암 조직과 간조직 모두 유사한 소견을 보여 위의 간세포양 샘암종 의 다발성 간전이 및 복강 내로의 전이를 진단할 수 있 었다. 따라서 항암화학요법을 고려하였으나 내원 당시 총빌리루빈의 증가 및 환자의 급격한 수행능력 감소로 환자는 보존적 치료를 위해 연고지 병원으로 전원되었다. 고 찰 간세포양 샘암종(hepatoid adenocarcinoma)은

4 - 대한내과학회지 : 제73권 제1호 통권 제563호 년 Ishikura등 1) 에 의해 간 이외의 장기에서 다량의 AFP 를 생산하며, 면역조직화학적으로 간세포성 분화를 보이 는 샘암종의 변이라 하여 명명되었다 1). AFP을 생산하는 종양은 전체 위암의 1.5~15%를 차지하고 있으며 7), Motoyama 등 8) 에 의하면 크게 3종류로 구분해 볼 수 있 다. 이는 간세포양 샘암종, 난황난낭종 유사종양(yolk sac tumor like tumor), 그리고 태아 위장관 종양형(fetal gastrointestinal type)의 세 가지로 나누어 볼 수 있으며 이 중에서 간세포양 샘암종이 가장 많이 발생하는 것으 로 알려져 있다. 간세포양 샘암종은 위에서 많이 발생하 며, 이에 대한 명확한 이유는 알려져 있지 않으나 현재 까지는 위와 간 모두가 배아의 원시적 전장(foregut)에 서 기원하는 장기이며, 태생학적으로 위와 간배아가 가 까운 위치에서 발생하는데서 그 원인을 찾고 있다 6, 9). 간세포양 샘암종의 조직학적인 특성에 대해 Kodama 등 10) 은 면역조직화학염색에 근거해 두 가지의 조직학적 소견을 보임을 보고하였다. 그 첫째는 커다란 다형성 혹 은 다핵거대세포가 충실형의 판상 혹은 둥우리(solid nest or sheets), 주형(trabe culartype)으로 배열되는 양 상을 보이는 수질형(medullary type)이며, 다른 한가지 는 잘 분화된 유두형 혹은 관상형이다. 이 두 가지는 한 종양 내에서 같이 보일 수 있으며 각각 다른 면역조직화 학염색을 보이는 것으로 기술하였다. Ishikura 등 11) 은 수 질형은 조직학적으로 원발성 간세포암과 유사하며 AFP 의 생산이 중요한 특징으로 보고하였으나 AFP를 분비 하지 않는 간세포성 분화성 암종이 보고되면서 12) AFP 의 생산이 필수적인 것은 아닌 것으로 알려졌다. 후에 Nagai 등 13) 은 AFP의 생산과 관계없이 조직학적으로 간 세포암과 유사한 충실형(solid) 또는 주형(trabecular)으 로 배열되는 다형성 세포로 구성된 종양을 간세포양 샘 암종의 진단기준으로 제시하였다. 본 증례에서는 샘암성 분화부위가 대부분을 차지하고 있었으며 간세포성 분화 부위는 거의 찾아볼 수 없었다. 이 종양의 특징으로는 수술 당시에 이미 병기가 높다 는 것과 높은 간 전이율을 보인다는 점이며, 5년 생존율 이 11.9%로 비간세포양 샘암종의 38.2%에 비해 예후가 극히 불량하다는 점을 들 수 있다 9). 국내에서도 서 등 6) 에 의하면 간세포양 샘암종으로 진단된 14예 중 8예가 사망하였으며 3년 생존율은 14.3%로 보고하였다. 특히 본 증례와 유사하게 수술당시 병기가 Ib였던 환자에서 술 후 12개월 만에 간 전이로 사망하는 경우처럼 급격히 진행하는 예가 있었다. 예후가 불량한 이유에 대해 여러 가지가 제시되고 있 다. 그 중 하나로 간세포양 샘암종은 정맥 침범이 많아 간 전이가 흔한 다는 것이다. 이것은 Aizawa 등 14) 의 동 물 실험에 의해 AFP를 생성하는 세포 중 간세포성 분화 를 보이지 않는 종양에 비해 간세포성 분화를 보이는 종 양이 간 전이를 훨씬 더 잘한다는 것이 증명되었다. 다 음으로 간세포양 샘암종은 AFP 뿐만 아니라 alpha-1 antitrypsin과 alpha-1 antichymotrypsin을 분비하며 이 것이 면역억제작용과 함께 protease 억제 작용을 하여 종양의 침습력(invasiveness)을 항진시킨다는 것이다. 그리고 AFP도 림프구의 전환을 억제하는 것으로 알려 져 있다 7). Koide 등 15) 은 AFP를 생산하는 위샘암이 높은 증식력, 약한 세포소멸능(apoptosis), 풍부한 신혈관생성 능을 보이는 특성을 가지고 있으며 항암요법에 반응이 좋지 않은 것으로 보고하였다. 이 종양의 치료에 관하여 Ishikura 등 1) 과 Chang 등 16) 의 보고에 의하면 이 종양은 예후가 좋지 않아서 일반적 인 위암의 치료방법으로 만족할만한 효과를 거둔 예가 없었다. 그러나 최근 간으로 전이된 예에서 S-1 또는 paclitaxel/cisplatin 항암화학요법을 시행하여 부분반응 이상을 보인 경우가 보고되었다 17, 18). 결론적으로, stage II의 비교적 낮은 병기의 위샘암 환자에서 처음 진단 당시에 조직 소견상 샘암성 분화가 대부분을 차지하고 있어 간세포양 샘암종을 조직학적으 로 생각하기 어려웠으나 수술 후 빠르게 진행하는 간과 복강내 전이와 함께 높은 혈청 AFP 근거 하에 간세포양 샘암종을 진단하게 된 경우로 위샘암 진단시 AFP 염색 을 통한 간세포양 샘암종을 선별하고 이러한 증례들을 대상으로 항암화학요법의 역할에 대한 연구가 필요할 것으로 생각된다. 요 약 저자들은 위암 2기(T2N1M0)로 근치적 수술을 시행 받은 후 추적 관찰 중 5개월 만에 높은 AFP수치(227,325 IU/mL)와 다수의 간 내 다발성 종괴 및 복강내 종양소 견이 관찰되어 시행한 간 조직 검사와 면역조직화학염 색을 통해 간세포양 샘암종으로 진단된 증례를 경험하 였기에 보고하는 바이다. 중심 단어 : 위암, 알파태아단백
