Materials and Methods: We retrospectively reviewed 12 feet of nine patients diagnosed as symptomatic bunionette and treated with diaphyseal oblique osteotomy. All patients were female and the average age at the time of surgery was 48 years. We checked the foot standing anteroposterior, oblique, and lateral images pre- and post-operatively. We measured the fourth intermetatarsal angle and fifth metatarsophalangeal angle and evaluated the clinical results using the American Orthopaedic Foot and Ankle Society (AOFAS) lesser metatarsophalangeal-interphalangeal (MTP-IP) scale preoperatively and six months postoperatively. Results: Of the nine patients, hallux valgus was combined with symptomatic bunionette in seven feet of five patients. In all of our cases, the average AOFAS lesser MTP-IP scale showed improvement after surgery. Painful callosity around the fifth metatarsophalangeal joint disappeared after surgery in all of our cases. The fourth intermetatarsal angle improved from 12.7 o to 3.1 o and the fifth metatarsophalangeal angle improved from 16.6 o to 2.3 o. Conclusion: Diaphyseal oblique osteotomy of the fifth metatarsal appears to be a safe and satisfactory surgical procedure for treatment of symptomatic bunionette. Conflict of interest: None. 론 하는경우제 5중족골외과의돌출부위에잦은마찰이발생하여소건막류가발생할수있다. 또한제 5중족골이짧거나아령모양 (dumbbell shape) 을하고있는경우소건막류가발생할수있으며, 제 4중족골외측에부골이있어제 5중족골을외측으로전위시키거나횡중족골간인대와무지내전근의불완전발달로인해제 5 중족골을충분히견인하지못하는경우에도소건막류가발생할수있다. 5,6) 한편제 5중족골의과운동성, 제 4, 5중족골간각의증가및외측으로구부러진제 5중족골도소건막류발생의중요한원인으로보고되고있다. 7,8) 소건막류는일반적으로보존적치료를먼저시도한다. 볼이넓은신발을신고, 변형에맞게신발을교정해서신도록하며, 통증이동반된점액낭염에대해서는비스테로이드성소염제등을투약하기도한다. 이러한보존적치료에반응하지않는경우수술적치료를시행하게되는데, 수술적치료법으로는제 5중족골두외측절제술, 3,9) 제 5중족골원위절골술, 10-12) 제 5 Copyright c2014 The Korean Foot and Ankle Society. 이 하여 고정한 환자들에 대한 임상적 및 방사선적 치료 결과를 제시 후 이동형 수술용 엑스레이(C-arm fluoroscopy)로 확인하며 절골 하고자 한다. 원위 골편이 이상적인 위치로 오도록 교정한 후, 3개의 1.1 mm K강선(Kirschner wire)을 삽입하였다. 이후 3개의 K-강선 중, 가운데 대상 및 방법 K-강선을 제거한 후, 2.

근위부 골편의 원위 외측 및 원위부 골편의 근위 외측 부위 소건막류에 대해 제 5중족골 간부 사형 절골술을 시행하고, 6개월 의 돌출된 뼈를 미세 전동톱을 이용하여 다듬은 후, 제 5중족 족지 이상 경과 관찰한 9명의 환자 12예를 대상으로 후향적 분석을 하였 관절의 외측 관절막을 겹치게 하여 봉합하였다. 수술 후 2주째 봉 다.

(C) Bony union was achieved and the correction of the bunionette deformity was well maintained 6 months postoperatively. 술 후 2.3±0.9도로 호전되었다(Fig. 3). 고 찰 소건막류에 대한 현대적 개념은 1949년 Davies5)에 의해 처음 기 술되었으며, 소건막류는 제 5중족골의 벌어짐에 의해 발생한다고 발표되었다. Schoenhaus 등21)은 243예의 방사선 영상을 분석한 결 과 제 4, 5중족골 간 각의 평균은 8도였으며, Fallat과 Buckholz19)의 연구에 의하면 제 4, 5중족골 간 각은 3도에서 11도 사이의 범위를 가지며 평균 6.5도였다. Preop: preoperative, Postop: postoperative, IMA: intermetatarsal angle, MTP: metatarsophalangeal. 정도를 평가하는 또 다른 척도인 제 5중족-족지 관절각은 제 5중족 골의 해부학적 축과 제 5족지의 해부학적 축이 이루는 각으로 정의 되며, Steel 등22)의 정상 성인 족부의 방사선적 계측에 대한 연구에 의하면 이 척도는 1도에서 21도 사이의 범위를 가지며 90%의 환자 결 과 에서 14도 이하로 나타났다. Coughlin14)은 30예의 증상이 있는 소 건막류 환자 20명에서 제 5중족-족지 관절각은 평균 16도였으며, 12예 중 5예에 대해서는 소건막류에 대한 수술만 시행하였으며, 5도에서 30도 사이의 범위를 갖는다고 발표하였다. 제 5중족골 7예에 대해서는 무지 외반증에 대한 근위 중족골 절골술과 소건막 두의 평균 폭은 13 mm로 알려져 있다. 중족골두가 돌출될 경우 소 류에 대한 수술을 동시에 시행하였다. 임상적으로 AOFAS 소족지 건막류 증상이 발생할 수 있는데, Fallat과 Buckholz19)와 Gerbert 등 중족족지-족지 간 점수는 소건막류에 대한 수술만 시행한 5예에서 23) 는 술 전 평균 66점에서 술 후 평균 93점으로 호전되었으며, 무지 에 의해 제 5중족골두가 돌출될 수 있다고 주장하였다. 은 제 5중족골의 외측 결절의 외측 회전이 동반된 족부 회내 변형 외반증과 소건막류에 대한 수술을 동시에 진행한 7예의 환자에서 이번 연구에서 9명의 환자 12예 중 5명의 환자 7예에 대해서는 는 술 전 평균 61점에서 술 후 평균 89점으로 호전되었다. 전례에 동통성 소건막류와 무지 외반증이 모두 있어 이에 대한 수술적 치 서 제 5중족골두 주변의 피부 각화증은 소실되었으며, 피부 각화 료를 동시에 시행하였다. 벌어진 형태의 발이나 앞이 좁은 신발 증이 재발한 예는 없었다. 수술 후 창상 감염, 피부 괴사, 불유합 및 을 장기간 착용하는 것은 무지 외반증과 소건막류 발생의 공통 원 부정 유합과 같은 합병증은 관찰되지 않았다. 방사선적으로 제 4, 인이기 때문에 소건막류와 무지 외반증이 같이 동반될 수 있다. 5중족골 간 각은 술 전 12.7±1.0도에서 술 후 6개월째 3.1±1.2도 Nestor 등24)의 연구에 의하면 소건막류 환자군에서의 무지 외반증 로 호전되었으며, 제 5중족-족지 관절각은 술 전 16.6±5.8도에서 의 유병률은 23%였으며, 정상 대조군에서의 무지 외반증 유병률은

4 22 Vol. 18 No. 1, March % 로나타났다. 따라서소건막류환자에서무지외반증변형및이에의한증상유무에대해평가해볼필요가있다. Fallat 25) 은소건막류를방사선영상을통해 4가지유형으로분류하였는데제 1형은제 5중족골두가크거나외측면이돌출된경우이며, 제 2형은제 5중족골원위부가외측으로휘어진경우, 제 3형은제 4, 5중족골간각이증가한경우이며, 제 4형은위에기술한 3 가지형태중 2가지이상의분류가혼합된형태로정의하였다. 소건막류형태에따른수술적치료법에대한연구에의하면제 1형은제 5중족골두외측절제술이나원위갈매기형절골술을시행하고, 제 2형이나제 3형인경우에는제 5중족골간부혹은근위절골술이도움이된다고제시하였다. 26) Cohen과 Nicholson 27) 은제 1형에는단순골절제술이나원위갈매기형절골술을, 제 2형은원위갈매기형절골술을, 그리고제 3형에서는제 5중족골간부사형절골술을시행할것을추천하였다. 본연구의대상환자는제 2형이 2 예, 제 3형이 10예였으며, 제 5중족골간부사형절골술을통해소건막류변형을효과적으로교정할수있었다. 간부사형절골술은원위절골술에비해교정력이높고중족골두의혈행을방해하지않는장점이있지만지연유합과불유합은상대적으로더흔히발생한다는문제점이있다. 14,23) 하지만제 5중족골두족저부의피부각화증이동반된경우간부사형절골술을시행할때절골방향을외측에서내측으로갈수록족배부를향하도록하여원위부가족배부로전이되도록한다면, 굳은살을효과적으로치료할수있다. 16) Coughlin 3,14) 은절골방향이근위족배부에서시작하여원위족저부로향하는간부사형절골술을시행하고, 금속나사나 K-강선을이용하여고정한후좋은수술적치료결과를보고하였다. 저자는 Coughlin의방법으로간부사형절골술을시행하였고 C-arm으로교정위치를확인하며 3개의 1.1 mm K- 강선을삽입한후 K-강선을하나씩제거하며그자리에 2.3 mm 금속나사를삽입하였다. K-강선을삽입하여피질골을뚫은후그자리에금속나사삽입을위한천공 (drilling) 을하게되면드릴이미끄러지는현상을막을수있고, 천공도중뼈가깨지는문제도예방할수있다. 또한 K-강선으로만고정을한다면 K-강선의위치가이동할수있고절골부위가벌어질수있기때문에보다견고한고정을위해금속나사를이용하여고정하였다. 또한저자가사용한금속나사는나사머리부분이작고납작하여주변연부조직을자극하지않기때문에내고정물제거술을시행하지않아도되는장점이있었다. 한편 London 등, 15) Ahn 등 16) 은절골면이원위족배부에서근위족저부로향하는사형절골술을시행하였으며, 이수술법은체중부하스트레스에좀더안정적인장점이있다고제시하였다. 본연구의한계점으로증례의수가적고, 최소추시기간이 6개월로비교적짧았다. 또한대조군이없으며, 단일수술법에의한환자군연구인점도이번연구의제한점으로볼수있다. 하지만제 5중족골간부사형절골술시행후 3개의금속나사를이용하여 고정하는수술법은동통이동반된제 2형및제 3형소건막류환자에대해비교적안전하고우수한치료결과를기대할수있는유용한수술법으로생각된다. 결 골간부사형절골술은동통이동반된제 2형및제 3형소건막류환자에대해시행할수있는수술법이며, 금속나사고정법은초기에견고한고정력을얻을수있고, 내고정물제거를위한추가적인수술이필요하지않다는장점이있다. 론 REFERENCES 1.
