The purpose of this study is to evaluate the incidence of hallux interphalangeal angle in the normal and hallux valgus feet and also the statistical correlations of HIA with HVA and IMA in the 2 groups. Materials and Methods: The study is base on the standing foot AP radiographs of the 100 normal feet (HVA<12 and IMA<9 ) and 100 hallux valgus feet (HVA>25 and IMA>12 ). We measured the hallux valgus angle, intermetatarsal angle and hallux interphalangeal angle, where we defined the hallux valgus interphalangeus when the HIA was more than 10. We evaluated the incidence of the hallux valgus interphalangeus in the normal and hallux valgus groups and the correlations of the HIA with HVA and IMA. Results: In normal feet group, hallux valgus interphalangeus comprised 82%, while there were only 20% of HVI in hallux valgus group. Among 200 total feet, there was negative correlations between the HVA and HIA as well as IMA and HIA statistically (p<0.01). Conclusion: There was lower incidence of hallux valgus interphalangeus in the hallux valgus group compared to the normal feet group. 2 김유진 정홍근 방승철 골의통증등을특징으로하고있는복합변형을보이는질환 5,8,9) 으로서, 치료로는보존적치료와수술적치료가있으나수술적치료가원칙으로제시되고있다. 수술적치료에는원위연부조직재건술, 중족골절골술, 중족-족지관절유합술, 중족-족지관절성혈술등무지의외반, 제1 중족골의내반정도및중족-족지관절의상합성여부에따라다양한수술적방법들이보고되고있다. 이러한수술적방법결정에있어무지외반각및제1-2 중족골간각등의방사선학적계측치가중요한판단기준중하나로사용되고있다. 그러나만족스러운무지외반증교정수술에있어서는동반가능한무지지간외반증 (hallux valgus interphalangeus) 을고려할필요가있으며, 따라서무지외반각과제1-2 중족골간각이외에무지지간각 (hallux interphalangeal angle) 의측정도중요한사항이다. 이에저자들은한국인의정상족과무지외반족에서의무지지간외반증의발생빈도를조사하고, 상기두집단에서무지지간각과무지외반각및제1-2 중족골간각과의통계학적연관성을분석하고자하였다. 대상및방법 1. 연구대상 20세이상성인의무지외반증소견이없는정상족부 100 족과무지외반증변형을동반한족부 100 족을대상으로하였으며체중부하하에족부의전후방단순방사선촬영을시행하였다. 연구대상군의선정에있어족부골격에변화를줄수있는류마티스관절염, 뇌성마비, 말초신경질환, 선천성족부기형, 골절및탈구의경우는연구대상에서제외하였다. 무지외반각이 12 이하이며제1-2 중족골간각이 9 이하인경우를선별하여정상족부군에분류하였으며, 정상족과의중첩을줄이기위해 Coughlin 3) 의무지외반증중증도분류상중등도변형인무지외반각이 25 이상과제1-2 중족골간각이 12 이상인경우를무지외반증군으로분류하여각군에 100족씩을할당하였다. 무지지간외반증은 Gentili 등 4) 에의하여 10 이상인경우를무지지간외반증으로정의하였다. 정상군의평균연령은 49.2 세 (21-79 세 ), 무지외반증군의평균연령은 52.0 세 (21-81 세 ) 였다. 연령분포는정상군의경우남자가 32명, 여자가 29명이었으며, 무지외반증군의경우는남자가 10명, 여자가 65명이었다. 양측족부를측정한경우는정상군의경우 39명이었으며무지외반증군의경우 27명이었다. 2. 연구방법무지외반각은미국족부정형외과학회연구위원회 13) 에서정의한내용을사용하여제1 중족골과근위지골의골간부에서내외측피질골의중간점을연결하여정중을종으로잇는선사이의각도로, 제1-2 중족골간각은 Hardy와 Clapham 6) 이정의한제1 중족골과제2 중족골의골간부에서내외측피질골의중간점을연결하여종으로잇는선이이루는각으로측정하였으며, 무지지간각은 Sorto 등 14) 이정의한원위지골의내측과외측접선이만나는점에서이를이등분하는선과근위지골을이등분하는선이만나는각으로하였다 (Fig. 1A-C). 체중부하전후면족부방사선사진은 1993 년미국족부정형외과학회의기준 11) 에따라기립상태에서방사선관구를 1 m 높이에서일정하게두고방사선조사의방향은족부에대해수직선으로부터후방 20 각도로촬영하였다. 무지외반각, 제1-2 중족골간각과무지지간각은체중부하전후면방사선사진상에서각도기와자를이용하여측정하였다. 세가지종류의각도에대해서 1명의계측자가총 5회에걸쳐측정하여측정자간및측정자내측정상의오차가최소화되도록하였다. 정상군과무지외반증군에서의무지지간외반증의빈도를측정비교하였고, 두군을합친전체군에서무지지간각의무지외반각및제1-2 중족골간각과의통계학적연관성을분석하고자하였다. 3. 통계학적분석방법자료의통계학적처리를위하여 SPSS 12.0 을사용하였으며, 정상군과무지외반증군에서의무지지간외반증의빈도를비교분석하는데 T-test 를시행하였다. 무지외반각과무지지간각그리고제1-2 중족골간각과무지지간각의상호연관성에대한분석에는 Pearson 상관분석을시행하였다. 결과 100 족의정상군에서무지외반각은평균 8.6±4.6 이고제1-2 중족골간각은평균 6.7±1.9 이었으며, 무지외반증군에서는제1-2 중족골간각은평균 15.6±2.5 이었고, 무지외반각은평균 37.6±9.0 로조사되었다. 정상군에서의무지지간각은평균 13.2±4.1 인반면무지외반증군에서의무지지간각은평균 7.0±8.2 이었으며, 이러한두군간의차이는통계학적으로유의한차이를보였다 (p<0.01)

(Table 1). 또한 100 예의정상족군중 82% 에서무지지간외반증을보인반면, 100 예의무지외반족군에서는 20% 에서만이무지지간외반증이동반되었다 (Fig. 2). 200 족전체를대상으로한분석에서무지외반각과제 1-2 중족졸간각은통계학적으로유의한양의상관관계를보인반면 (p<0.01), 무지지간각은무지외반각과제1-2 중족골간각모두에서통계학적으로유의한음의상관관계를보였다 (p<0.01)(fig. 3). 고 찰 무지외반증은제1 중족족지관절에서무지가외측으로과잉변형되고제1 중족골은내측으로기울어짐으로써제2 중족골과멀어지게되며제1 중족골두의내측비대, 관절낭 Tabl e 1. Mean Val ues (in degrees) from the Radiologic Measurement Angles measured Normal group Hallux valgus group HVA* 8.6± ±9.0 IMA 6.7± ±2.5 HIA 13.2± ±8.2 *HVA, hallux valgus angle; IMA, intermetatarsal angle; HIA, hallux interphalangeal angle. 내측의이완및외측의수축, 무지의회내전, 다른족지의변형등여러가지이차적인변형이동반되는복합질환이다 5,8,9). 이의치료로는보존적방법과수술적방법으로대변할수있으며치료의목표는동통의제거와변형의교정및이들의진행을막고제1 중족지간관절의정상적인기능을보존또는향상시키는데있다. 치료방법을결정하는데에는관절의퇴행성변화유무나제1 중족골의내전정도및중족골의상대적길이, 무지지간관절의변형여부, 원위중족관절면의각도및상합성여부, 제1 중족-설상관절의각도및과운동성유무, 편평족여부, 아킬레스건단축유무등많은점을고려해야한다. 이러한여러수술결정에있어전족부의방사선학적계측치를기준으로하게되는데전족부의단순방사선사진은체중부하상태에서촬영하는것이일반적이며, 무지외반각, 제1-2 중족골간각, 원위중족골관절각과함께관절면의일치성및내측융기의크기등을잘관찰하여야한다. 그리고이러한방사선적측정값은전족부의정상과병적인구조를이해하는데많은정보를준다고보고되어있다 7,9,12,15). 이외에도무지외반증환자의적절한수술적교정을위해관심을가져야할또다른요소로무지지간각이있으며, 현재정상인과무지외반증환자에서무지외반각과제1-2 중족골간각의관계에대해여러보고가있지만, 이들과무지지간각의관계에대해

Sorto 등 15) 은무지외반각과무지지간각은음의상관관계를보인다고하였으며, 25 이상의무지외반각을갖는군에서는무지 지간각이평균 8.9, 8 이하의무지외반각을갖는군에서는평균 15.7 의무지지간각을보인다고발표하였다. 본연구결과도이와비교하여볼때대부분일치하는결과를보였다 년 Daw 는무지원위지골의외측편향을무지지간외반증이라고처음으로명명하였으며 14), Barnett 2) 은무지지간외반증은무지의원위지골과근위지골의종적이등분선이이루는각의관계라고하였으며, 이각은근위지골두관절면의기울어진정도 (obliquity) 와원위지골자체의비대칭적인형성에기인한다고하였다. Sorto 등 14) 은이두가지요소이외에도관절편향성 (joint deviation) 이라고명명되는세번째요소가있다고하였으며, 무지지간외반증은이세가지요소에의해결정되어진다고하였다. Barnett 2) 과 Wilkinson 16) 은무지지간외반증은다른영장류에서볼수없는인간에서만나타나는특징이라고하였으며, Barnett 2) 은이를 2족보행을하는인간의독특한특징이라고하였다. Sorto 등 14) 은제1 중족-족지관절의안정성의증가는낮은무지외반각과높은무지지간외반증을보이며, 성장기동안에신발에의한압력으로인해원위지골관절성장판의외측저성장이발생하여무지지간외반증이심해진다고하였다. 반면제1 중족족지관절의불안정은높은외반각과낮은무지지간각과연관이있다고하였다. 무지외반증수술시, 무지의내측돌기절제술과근위지골의쇄기절골술즉, Akin 술식 1) 을자주병행하게되는데이러한것에기인하여본저자들은무지지간외반증은무지외반증에서더빈번할것이라는생각하였는데, 실제 ( A ) ( B ) Figure 3. 5 무지외반증에서무지지간외반증의빈도는더낮은것으로조사되었다. 이는무지외반증에서무지의회내변형과무지지간각의연관성도관여하였을것이라생각된다. 본연구에서는무지지간각과무지외반각, 제1-2 중족골간각사이의상관관계만을조사하였는데, 향후시행되어질연구에서는무지지골관절의상합성 (congruity) 과근위지골두관절면의기울어짐정도, 원위지골의비대칭성과무지지골관절의관절편향성 (deviation) 등의무지지골외반증의기여인자에대한조사와무지의회내변형이무지지간각에미치는영향등에대한연구가필요할것으로사료된다. 결 론 무지지간외반증이무지외반증족에비해정상족에서더빈번하였고, 무지지간각이무지외반각, 제1-2 중족골간각과음의상관관계를보이는것이확인되었다. 이는무지외반증수술에있어서참고자료로서도움이될것으로사료된다.
